1.Influences of Radix salvia miltiorrhiza preconditioning on the expression of phosphorylated eukaryotic initiation factor 2α and caspase12 in hepatic ischemia reperfusion in rats

Objective To investigate the influences of Radix salvia miltiorrhiza (RSM) preconditioning on the expressions of phosphorylated eukaryotic initiation factor 2α (p-eIF-2ot) and caspase12 in hepatic ischemia reperfusion in rats.Methods Eighty Wistar rats were randomly divided into the control group (5 rats),sham operation group (25 rats),ischemia reperfusion (IR) group (25 rats) and RSM pretreated group (25 rats).Rats in the sham operation group,IR group and RSM pretreated group were subdivided into 5 groups at different time intervals (0,3,12,24 and 72 hours).Mter midline laparotomy,all structures in the hepatic portal were clamped for 45 minutes followed by different periods of reperfusion.Rats in the control group did not receive any treatment; rats in the sham operation group only received anatomy of the hepatic portal without clamping; rats in the RSM pretreated group received RSM by intravenous injection 30 minutes before ischemia at a dose of 6 ml/kg.Rats in the sham operation group and the IR group received a dose of normal saline as RSM pretreated group.The protein expressions of p-eIF-2α and caspase12 in the hepatic tissues of each group were detected by the Western blot,and the pathological changes of hepatic tissues in each group were detected by hematoxylin and eosin staining.All data were analyzed using the analysis of variance,LSD test or Games-Howell method.Results The relative expression of p-eIF-2α in the control group was 0.296 ± 0.038,and the relative expressions of p-eIF-2α in the sham operation group at different time points were 0.304 ± 0.048,0.298 ± 0.038,0.272 ± 0.042,0.266 ± 0.076 and 0.296 ± 0.043,with no significant difference between the 2 groups (P > 0.05).The relative expression of p-eIF-2α in the IR group at 0 hour was 0.310 ± 0.034,which had no significant difference compared with the control group and the sham operation group (F =0.15,P >0.05).The relative expressions of p-eIF-2α in the IR group at 3,12,24 hours were 0.386 ± 0.021,0.710 ± 0.034,0.474 ± 0.017,which had no significant difference compared with the control group and the sham operation group (F =11.90,211.52,25.15,P < 0.05).The relative expression of p-eIF-2α in the IR group at 72 hours was 0.336 ± 0.043,which was back to the level of the control group and the sham operation group (F =1.57,P > 0.05).The relative expressions of p-eIF-2α in the RSM pretreated group at different time intervals were 0.278 ± 0.044,0.800 ± 0.079,1.056 ± 0.125,0.736 ±0.087 and 0.442 ± 0.047,which were significantly lower than the group at 3,12,24,72 hours (P < 0.05).The relative expression of caspase12 in the control group was 0.983 ± 0.003,and the relative expressions of caspase12 in the sham operation group at different time intervals were 0.974 ± 0.004,0.983 ± 0.005,0.985 ± 0.003,0.981 ± 0.004 and 0.978 ± 0.004,with no significant difference between the 2 groups (P > 0.05).The relative expression of caspase12 in the IR group at 0 hour was increased to 1.018 ± 0.076,while it had no significant difference compared with the control group and the sham operation group (F =1.43,P > 0.05).The relative expressions of caspase12 in the IR group began to rise at 3 hours (2.056 ±0.067),and peaked at the 12 hours (2.804 ± 0.050),still at a relative higher level at 24 hours (1.882 ± 0.037),and began to decrease at 72 hours (1.282 ± 0.066),it had significant difference compared with the control group and the sham operation group (F=1290.69,6490.84,2898.71,103.61,P < 0.05).The relative expressions of caspase12 in the RSM pretreated group at different time intervals were 0.998 ± 0.056,1.442 ± 0.066,1.990 ± 0.068,1.364 ± 0.056and 0.962 ±0.054.The relative expressions of caspase12 in the RSM pretreated group at 3,12,24,72 hours were significantly lower than the IR group (P < 0.05).The results of pathomorphological examination showed that the hepatic lobules were integrated and the nucleuses were large and round in the control group an d the sham operation group; deformation of the hepatic lobule,smaller cell volume,nuclear condensation and necrosis were observed in the IR group; cell swelling and slight spotty necrosis were detected in the RSM pretreated group.Conclusions RSM could protect liver from damages during IR through up-regulating the expression of p-eIF-2α,reducing the apoptosis induced by endoplasmic reticulum stress.
2.Research for early growth response gene-1 in ischemia-reperfusion injury

Early growth response gene-1 is an immediate early response gene,a transcription factor which includ three zinc finger-like domains.Recentl studies demonstrate that early growth response gene-1 is involved in regulating cell cycle,proliferation and differentiation,and plays a critical role in injuring and repairing.Early growth response gene- 1 induces Target genes,and promotes extracellular environmental signal( such as oxygen deficit,oxidative stress,mechanical injury and cytokine)link with the complex biological responses.Intensive study to early growth respnse gene-1 will contribute to further illustrate the mechanism of the ischemia-reperfusion injury.This article summarizes from protein structure,signal transduction,biological functions and the relationship with ischemia- reperfusion injury.
3.Treatment of 96 Cases of Prolapse of Lumbar Intervertebral Disc of Posterior Protrusion Type by the Comprehensive Therapy

s: Ninety-six cases of prolapse of lumbar intervertebral disc of posterior protrusion type were divided randomly into observation group including 50 cases and control group including 46 cases.Patients in observation group were treated with combined use of acupuncture, traction and Tuina, and patients in control group were only treated with acupuncture. After treatment, the significant difference was found between the observation group and control group in therapeutic affect. The comprehensive therapy is definitely effective and simple in the treatment of prolapse of lumbar intervertebral disc of posterior protrusion type, which has guiding significance to the clinical practice.
4.Comparison of Angioseal closure device with manual compression

Objective To assess the safety and efficiency of Angioseal device in patients undergoing percutaneous femoral artery puncture in brain angiograph or interventional therapy. Methods A prospective trial was carried out in 128 patients undergoing brain angiograph and interventional therapy,in which 93undergoing brain angiography and 35 interventionsl therapy. All patients were divided into pure compression group by manual compression (83 cases) and vascular blocking group by Angioseal device (45 cases)according to different hemostatic measures. Results In angiography, hemostasis time of vascular blocking group and pure compression group was (1.8 ±0.8) min and(20.2 ±9.4) min (P<0.01 ),ambulation time was (3.8 ± 0.8) h and (19.4 ± 2.2) h (P < 0.01). In interventional therapy,hemostasis time of vascular blocking group and pure compression group was (2.0 ± 1.1) min and (24.5 ± 10.3) min (P < 0.01), ambulation time was (4.3 ± 1.5) h and (24.3 ± 3.2) h (P< 0.01). Pure compression group had higher complication rate (such as bleeding,hematoma,skin ulceration and blister) than vascular blocking group (P<0.01).Conclusion Angioseal closure device is safe, efficient and easy to use.
5.The role of electroencephalogram examination in the prognosis evaluation of patients with severe craniocerebral injury

ObjectiveTo investigate the role of electroencephalogram (EEG) examination in the prognosis evaluation of patients with severe craniocerebral injury.MethodsFifty-seven patients with severe craniocerebral injury were selected as study subjects and their clinical data were analyzed retrospectively.All the patients got EEG examination,and the relationships between EEG grading and Glasgow coma scale (GCS) score,the prognosis of outcome were analyzed.ResultsThe EEG of 57 patients within 24 h after hospitalization were all abnormal.There was significantly negative correlation between EEG grading and GCS score (r =-0.742,P <0.05).EEG grading was significantly positively correlated with the prognosis of outcome (r =0.730,P< 0.05).ConclusionsEEG examination and EEG grading can evaluate the prognosis of patients with severe craniocerebral injury.It provides reference for clinical treatments.
6.Modulatory effect of Rho kinase on the cerebral vasospasm following subarachnoid hemorrhage

Objective To investigate the dynamical changes of Rho kinase in the cerebrospinal fluid (CSF) and its relationship with cerebral vascular spasm CVS. Methods CSF were collected on the ist, 3rd, 7th, 10th and 14th day after subarachnoid hemorrhage. The expression of Rho-kinase mRNA in CSF was determined by RT-PCR. The expression of endothelin-1 in CSF was determined by radioimmuno-assay. TCD was used to measure the velocity of the cerebral artery. Results The levels of ET-1 and Rho-kinase mRNA in CSF were re-markably increased on the 3rd day, and reached at the peak on the 7th day after subarachnoid hemorrhage, which were significantly higher than those without CVS. Conclusion There is a positive correlation between the level of Rho-kinase mRNA and ET-1 in CSF. Rho-kinase may participate in the development of CVS.
7.Clinical research on the effects of Chinese herb fumigation and washing combined with electromyographic biofeedback on stroke patients with muscle cramp on limbs

Objective We tried to observe the effects of Chinese herb fumigation and washing combined with electromyographic biofeedback on stroke patients with muscle cramp on limbs.Methods 84 patients who had muscle cramp on limbs were selected and divided into two groups randomly,namely the intervention group and the control group and both groups had 42 stroke patients.The intervention group was treated with routine treatment,Chinese herbal fumigation and washing and electromyographic biofeedback treatment.The control group received only routine treatment and electromyographic biofeedback treatment.We had compared and analyzed the Ashworth Spasticity Scale and the Daily Life Abihty Score (Barthel index) of the two groups at the time of the pre therapy,two weeks after the treatment and at the end of the treatment(after four weeks),respectively.Results There were no significant differences between the two groups in Barthel index,however,there were significant differences in Ashworth Spasticity Scale scores between the two groups four weeks after treatment,t=-3.84,P < 0.05.Conclusions The effect of Chinese herb fumigation and washing combining with the electromyo-graphic biofeedback treatment is better than usage of the electromyographic biofeedback treatment alone on stroke patients with muscle cramp on limbs.
8.Clinical study of minimally invasive punctural evacuation in the treatment of basal ganglion region cerebral hemorrhage

ObjectiveTo study the clinical effect of YL-1 type hematoma puncture needle in the treatment of basal ganglion region cerebral hemorrhage.MethodsSixty-two patients with hypertensive basal ganglion region cerebral hemorrhage were treated by YL-1 type hematoma puncture needle from January 2007 to May 2011 (minimally invasive punctural evacuation group),of which,60 patients were treated by conservative treatment(conservative treatment group) as control,compared two groups of neural function defect score,hematoma clearance rate on admission,after 3 weeks treatment,and quality of life after 6 months.ResultsNeural function defect score on admission of minimally invasive punctural evacuation group was (23.6 ± 18.4) scores,while (23.4 ± 17.8) scores in conservative treatment group,the difference was not statistically significant(P > 0.05).After 3 weeks' treatment,neural function defect score and hematoma clearance rate of minimally invasive punctural evacuation group was superior to conservative treatment group [ (14.6 ± 12.4) scores vs.(20.1 ± 18.4) scores,(92.3 ± 5.4)% vs.(79.5 ± 13.8)% ] (P <0.05 ).After 6 months' treatment,the good rate of quality of life in minimally invasive punctural evacuation group was 81.7%(49/60),which was significantly increased compared with that of conservative treatment group [67.2% (39/58)] (P < 0.05).ConclusionsThe minimally invasive punctural evacuation in the treatment of basal ganglion region cerebral hemorrhage has small invasion,better prognosis,effective and fast decompression of intracranial hematoma,reducing disability rates,improvement of the quality of life,which could be a beneficial complement for traditional therapies.
9.Effect of Taichi footwork assisted routine rehabilitation training on the influence of limb balance disorder of patients with hemiparesis after stroke in the recovery period

Objective:To evaluate the effect of Taichi footwork assisted routine rehabilitation training on the influence of limb balance disorder in patients with hemiparesis after strokein the recovery period.Methods:A total of 60 patients with hemiparesis after stroke in our hospital from March 2017 to May 2018, who met the selection criteria, were divided into 2 groups by random number table method, 30 in each group. The conventional rehabilitation group took the conventional rehabilitation training method, and the Taichi footwork group took Taichi footwork training method based on conventional rehabilitation method. Both groups were treated for 4 weeks. The Berg Balance Scale (BBS) was used to evaluate the patient's balance ability, the Holden Walking Aability Classification (HWAC) was used to evaluate the lower limb walking ability, and the Fugl-Meyer Motor Function Rating Scale (FMS) was used to assess the limb function.Results:After the treatment, the BBS score (44.4 ± 7.9 vs. 38.7 ± 9.9, t=4.885), the HWAC grade (3.6 ± 1.0 grade vs. 2.7 ± 1.0 grade, t=5.152) and FMS score (12.3 ± 1.4 vs. 10.4 ± 1.4, t=4.785) of the Taichi footwork group were significantly higher than those of the conventional rehabilitation group ( Ps<0.01). Conclusions:Taichi footwork assisted conventional rehabilitation training can improve the walking ability of patients with hemiparesis after stroke in the recovery period, which helps the patientsregain limb function.
