1.Protective effects of thalidomide on rats with acute necrotizing pancreatitis

Objective To investigate the protective effects of thalidomide on rats with acute necrotizing pancreatitis (ANP) and its mechanism. Methods Fifty four SD rats were randomly divided into the three groups; ANP group, thalidomide group and control group with 18 rats in each group. The model of ANP was induced by retrograde injection of 5% sodium taurocholate into the bili-pancreatic duct. Rats in thalidomide group received thalidomide 200 mg/kg body weight gastric lavage 1 h after ANP induction. The rats were sacrificed 3 h, 6 h, and 12 h after ANP induction, and the amount of intraperitoneal ascites was quantified. The serum levels of TNF-α, IL-6, IL-18 were measured by EUSA. The proportion of CD4 + T cell, CD8 + T cell in peripheral blood was determined by flow cytometry. The expression of TNF-α mRNA in pancreatic tissue were measured by RT-PCR. The expression of ICAM-1 protein in pancreatic tissue was measured by immunohistochemistry. Pancreatic tissue underwent pathologic examination. Results Six hours after surgery, the amount of ascite, serum levels of TNF-α, IL-6, IL-18, CD4 + T cell, CD,+ T cell, pancreatic TNF-α mRNA and ICAM-1 protein expression, pathologic score in control group was (1.03 ±0.31)ml,(57.17±11.29)pg/ml, (24.45 ±4.14)pg/ml, (64.23 ±21.85)pg/ml, (47.58 ±9.21)% , (40.88 ± 2.96)%, 0.07 ±0.02, 0.57 ±0.30, 0.67 ±0.81, respectively, and the corresponding values were (3.63 ± 0.38)ml, (107.54 ±33.05) pg/ml, (47.30 ± 11.40) pg/ml, (367.76 ± 108.43 ) pg/ml, (54.90 ± 7.15)%, (17.17 ±3.12)%, 0.65 ±0.26, 3.20 ±0.57, 11.50 ±1.87 in ANP group; and (1.45 ±0.53)ml, (80.60 ±20.48) pg/ml, (26.61 ±10.85) pg/ml, (321.82 ±85.20) pg/ml, (29.80 ±2.19)% , (15.52± 1.96)%, 0.35 ±0.23, 2.37 ±0.67, 8.00 ±3.03. Besides the value of CD8 + T cell was significantly decreased, all other values were significantly increased when compared with control group (P <0.05). Conclusions Thalidomide can decrease the release of inflammatory mediator, and reduce the pathological damage of pancreas of ANP rats by inhibiting TNF-α mRNA expression.
2.OVEREXPRESSION OF ZINC FINGER PROTEIN INHIBITS LPS-INDUCED EXPRESSION OF E-SELECTIN IN ENDOTHELIAL CELLS

Objective Previous studies have demonstrated that LPS can induce endothelial cell activation and the expression of E-selectin. In this study, we examined whether A20 gene could inhibit the expression of E-selectin in endothelial cells induced by LPS. Methods With the help of DOTAP, endothelial cells were transfected with pCDNA3.1 EHA20. The postive cell clones were selected with G418.The stable transfection and expression of A20 in the endothelial cells were determined by immunofluorescence analysis. The E-selectin expression was checked by immunofluorescence, Western blot and in situ hybridization. Results Abundant A20 stable expression in endothelial cells transfected with pCDNA3.1 EHA20 was confirmed by immunofluorescence analysis. 3.Control study on MRI of articular cartilage erosion and its histological expression

[Objective]To Study the sensitive sequence of MR for all stages of articular cartilage erosion and to evaluate the effect of MR on the correct diagnosis in the early stage of articular cartilage.[Method]One human fresh amputated knee joint and 4 normal pig knee joints were used to study and select the best sequence by T2WI,PDWI,GE,STIR,3D FS-FSPGR sequences and 6 pig oatcoarthritis(OA)model knee joints were studied by the selected best sequence,MRI was compared with correspont histological evidence to evaluate the effect of MR on the correct diagnosis of OA cartilage.[Result]The best results of tissue resolution and diagnosis rate were seen in 3D FS-FSPGR of all five sequences respectively 67.9% and 93.7%,of proteogly(PG)and collagen fiber(CF)mainly distributed in the deep cantilage.Dyeing of PG in diffent cantilage erosion was decreased while CF increasing.[Conclusion]3D FS-FSPGR is a favorable scan sequence to examine the OA changes of articular cartilage.MRI could show the pathological changes including early stage and have good veracity to the early pathological changes of OA cartilage.
4.Study of MR imaging sequence in detecting articular cartilage erosion:comparison with arthroscopy

[Objective]To compare the value of the MR imaging and arthroscopy for detecting articular cartilage erosion,and to evaluate the clinical effect of MR imaging on the correct diagnosis in the early stage of articular cartilage lesion.[Method]Twenty-six patients(27 knees)with persistent knee pain who were scheduled for arthroscopy underwent MR scanning,including 3D FS-FSPGR,FSE PDWI and FSE T1WI sequences.The results of each sequence were then compared with the arthroscopic findings.[Result]Using arthroscopic results as the standard of reference,the 3D FS-FSPGR images had the higher sensitivity(96.5%)and accuracy(95.0%)than the standard MR imaging,the 3D FS-FSPGR sequence was well consistent with the result of arthroscopic,Kappa value was higher(0.776)than the other sequences(P
5.Analysis on the Utilization of TCM Injections in Our Hospital During the Period of 2005~2008

OBJECTIVE: To analyze the utilization and tendency of TCM injections in our hospital, and to provide reference for clinical use of drugs. METHODS: Based on hospital database, the category, consumption sum and DDDs of TCM injections in our hospital during the period of 2005~2008 were analyzed statistically. RESULTS: The proportion of consumption sum of TCM injections in the total consumption sum from 2006 to 2008 were 13.06%, 9.33%, 9.97%, 9.08% respectively. The consumption sum of TCM injections was increased year by year. Stasis-removing agent for cardio-cerebral-vascular system took up the first place in the list of category of drugs, comsunption sum and DDDs. CONCLUSION: TCM injections are widely applied in our hospital. The utilization structure of TCM injections is the reasonable basically. But great importance should be attached to the safety and effectiveness of TCM injections and reasonable choice.

The sectional morphology of the human brachial artery, ulnar artery, radial artery, palmar digital artery of the thumb and the middle finger were observed under microscopy. Excepting the brachial artery, the intima of the other 4 arteries are thick, especially the ulnar artery. The histological variations must be noticed during the artery is anastomosed. The percentage of the adventitia of the whole wall thickness decreases and the elastic fibers become looser gradually in accordance with the size of the arteries. 7.Comprehensive prevention of deep vein thrombosis after total hip replacement

BACKGROUND:A high incidence of deep vein thrombosis after total hip replacement does great harm to patients,so it is necessary to look for a safe and effective prevention program after total hip replacement to reduce the harm of deep vein thrombosis.OBJECTIVE:To observe the influence of comprehensive prevention scheme on deep vein thrombosis after total hip replacement.METHODS:102 cases undergoing total hip replacement were randomly divided into observation group and control group with 51 cases in each group.Patients in the observation group received the comprehensive prevention program,which is composed of three interventions: sequential cross combined with low molecular heparin,intermittent pneumatic pressure therapy and continuous femoral nerve block analgesia.In the control group,patients received low-molecular-weight heparin for anticoagulation therapy by the doctor according to his clinical experience,or intermittent pneumatic pressure therapy.The occurrence and distribution of deep vein thrombosis were compared in patients of both groups.At the same time,platelet and hemoglobin were recorded.RESULTS AND CONCLUSION:In the observation group,three patients suffered from deep vein thrombosis with an incidence of 6%.In the control group,11 patients affected deep vein thrombosis,with an incidence of 22%.There were significant differences between the two groups (P< 0.05).No significant difference in constituent ratio of the distribution of deep vein thrombosis was detected in the distal vein and proximal vein in patients of both groups (P> 0.05).Hemoglobin value was lower at 1 day after surgery compared with that pre-operation in the two groups (P<0.05).Platelet value was lower at 1 day after surgery compared with that pre-operation in the two groups (P>0.05).No significant difference in hemoglobin and platelet values was detected between two groups at 1 and 6 days after surgery (P> 0.05).Results indicated that the comprehensive prevention program containing the sequential and cross combined with low molecular heparin,intermittent pneumatic pressure therapy and continuous femoral nerve block analgesia can significantly reduce the incidence of deep vein thrombosis,and no serious complications occurred,and the effects are affirmative and reliable.
8.Zinc finger protein gene A20 inhibits lipopolysaccharide-induced IL-8 expression of endothelial cells

Objective To investigate the effects of zinc finger protein gene A20 on the inhibition of lipopolysaccharide (LPS) induced interleukin 8 (IL 8) expression in endothelial cells. Methods Plasmid pcDNA3.1EHA20 was transfected into human umbilical vein endothelial cells (HUVECs) by DOTAP method. The positive cell clones were screened with G418. The stable transfection and expression of A20 in HUVECs were determined by immunofluorescent analysis. IL 8 expression was detected by sandwich ELISA with double monoclonal antibodies. AimTo establish a model of the injury of the vascular endothelial cell(VEC) in rats and observe the function of Chinese herbs of Li Qi Huo Xue(LQHX) will prevent and treat the VEC injury. Methods30 SD rats weredivided into the control, the model and LQHX groups. The model of the VEC injury was established. It tried to demonstrate the effect of LQHX for the coagula tion and fibrinolysis function of the VEC by the C EC count,t-PA, PAI activity, 6-keto-PGF1αcontent and PAgTmax. Results In LQHX group as compared with those of the medol group, The CEC count was redued obviously(P<0.01). t-PA activity was increased(P<0.01), so did the percentage of active t-PA(P<0.01), but PAI activity decreased (P<0.05), 6-keto-PGF1α content increased (P<0.01)α the PAgTmax decreasd (P <0.01) Conclution LQHX can enhance the anticoagulation and fibrinolysis activities. It is a more effective measure for the VEC protection.
10.The clinical and social factors associated with functional disturbance of acute hemiparetic stroke patients

Objective To investigate the prognostic factors of life and functional outcome of the first hemiparetic stroke patients. Methods One hundred and eighteen stroke subjects were registered prospectively. The Barthel index (BI) , Rankin scale (RS) , Mortricity index(MI) , Mini-mental state examination (MMSE) , Montgomery-Asberg depression scale (MADRS) and a scale of general state and risk factors were used to evaluate at the 48th hour, the 15th day and the 90th day after stroke. Results The patients' performance, as demonstrated by their scores with all the evaluation instruments, changed significantly at all the time points of evaluation after stroke (P <0.05). There was no significant difference between the performance at the 48th hour and the 15th day after stroke ( P > 0.05 ). But at the 90th day after stroke, the activity of daily living performance and the depression status recovered significantly (P < 0.01 ). Logistic regression analysis showed that, such factors as pneumonia, urinary incontinence within 48th hour and deep sensation disturbance might adversely influence patients' activity of daily living performance at the 90th day after stroke; the muscle strength of upper extremities at the 48th hour, and MMSE scores at the 15th day after stroke acted as the protective factors. Conclusions The stroke patients improved significantly with regard to their clinical and functional manifestations when evaluated 90 days after stroke onset. The main factors influencing the activity of daily living performance 90 days after stroke onset included deep sensation disturbance,pneumonia, urinary incontinence and muscles strength of upper extremities at 48th hour, and MMSE scores at the 15th days after onset.
