1.Efficacy of Nutritioned Intervention with Nutritional Suspensions in Patients with Severe Stroke

Objective:To observe the efficacy of enteral nutritional suspensions in the patients with severe stroke. Methods:To-tally 823 cases of severe stroke patients were divided into the observation group and the control group according to the visiting se-quence. The 423 cases in the observation group were treated with enteral nutritional suspensions, while another 400 patients in the con-trol group were given the home-made meals with nasal feeding, and relative routine treatment was given the patients according to the disease type in the two groups. Before and after the treatment, serum total protein ( TB) , hemoglobin ( HB) , serum albumin ( ALB) and TG of the two groups were recorded, neurological deficit scores ( NIHSS) , Barthel index, muscle recovery and complications were also studied and compared. Results:After the treatment, TB, HB, ALB and TG of the two groups were significantly decreased ( P<0. 05), while the nutritional indicators in the observation group were significantly higher than those in the control group (P<0. 05). After the treatment, NIHSS score of the two groups was significantly decreased (P<0. 01), Barthel index was significantly increased (P<0. 01), and those in the observation group were better than those in the control group (P<0. 05). The muscle recovery of the observation group was significantly higher than that of the control group (P<0. 05). There was no significant difference in the compli-cations between the two groups(P>0. 05). Conclusion: Enteral nutritional suspensions can improve the nutritional status of the pa-tients with severe stroke with fewer complications and better safety, which are worthy of clinical application.
2.Analgesic effect of dexmedetomidine versus propofol on patients with coronary heart disease undergoing noncardiac surgery

Objective To compare the analgesic effect of dexmedetomidine and propofol on patients with coronary heart disease undergoing noncardiac surgery.Methods 86 patients in our hospital from Sep 2009 to Sep 2011 aged 55-76 years weighing 42-60 kg,who were scheduled for epidural anesthesia in routine,were randomly assigned to 2 groups:dexmedtomidine (Dex) group and propofol group (n=43,each).Visual analogue scale/score (VAS) analgesia score and ramsay sedation score were used to observe the analgesic effect after surgery.Serum concentration of cortisol,insulin and glucose were observed and compared between Dex and propofol group before and 4,24,48h after surgery.Patient satisfaction was surveyed.Results The analgesic effect evaluated by VAS and Ramsay scores was better in Dex group than in propofol group at different time after surgery (t=5.368,2.262,7.147,5.881,7.861,4.810,all P<0.05).Serum concentrations of cortisol,insulin and glucose were lower in Dex group than in propofol group before and 4,24,48 h after surgery (t=3.076,2.042,4.090,all P<0.05).The satisfaction rate was 93.0% (40/43) in Dex group.Conclusions Dexmedetomidine has a better analgesic effect than propofol,and it is safe and feasible for patients with coronary heart disease undergoing noncardiac surgery.
3.?-Lactamase Genes and Virulence Genes in Escherichia coli Isolated from Old Patients

OBJECTIVE To investigate the ?-lactamase genes and virulence gene in Escherichia coli isolated from the old patients.METHODS Twenty strains of E. coli were selected to test 15 kinds of ?-lactamase genes (TEM,SHV,CTX-M-1 group,CTX-M-2 group,CTX-M-9 group,OXA-1 group,OXA-2 group,OXA-10 group,PER,GES,VEB/CEF,CARB,DHA,ACT/MIR and LAT/CMY) and two virulence genes (CNF-1 and CNF-2).RESULTS Six ?-lactamase genes and one virulence gene have been found out from these 20 strains of E. coli. The results showed that there were 14,3,1,5,2,2 and 1 strain which carried TEM,SHV,CTX-M-1 group,OXA-1 group,DHA,ACT/MIR and CNF-1,respectively. These 20 E. coli strains contained at least 1 ?-lactamase gene and 7 E. coli strains were found containing more than 2 kinds of ?-lactamase at the same time.CONCLUSIONS The resistance of these 20 E. coli strains to ?-lactam antibiotics is closely relative to ?-lactamase.
4.CORRELATION BETWEEN SERUM hs-CRP LEVELS AND MR IMAGING IN ISCHEMIC CEREBROVASCULAR PATIENTS

Objective To analyze the correlation between serum hs -CRP levels and MR Imaging in ischemic cerebrovascu-lar patients.To explore the hs -CRP serum levels in ischemic cerebrovascular patients incidence of early diagnosis and prognosis e-valuation application value.Methods 300 cases of ischemic cerebrovascular disease as the research object were selected from May 2013 to December 2014.The disease is classified into acute cerebral infarction group 100 cases, acute phase of cerebral infarction group 140 cases and transient ischemic attack 60 cases.Among 100 cases of patients with acute cerebral infarction group, in accord-ance with the MR Imaging data, they can be subdivided into three groups.Group A: lacunar infarction group 28 cases; Group B:small area of infarction group 50 cases and group C: large area of infarction group 22 cases.Another 40 healthy physical examination were selected as control group.The hs -CRP serum levels were made correlation analysis with MR Imaging.Results The acute cerebral infarction and transient ischemic attack group serum hs -CRP level difference compared with the control group with statisti-cal significance (p<0.05); The SPSS Spearman correlation analysis result shows that the hs -CRP serum levels and MR Imaging data shown in the infarction area and the body has a direct correlation .Conclusion The ischemic cerebrovascular disease patients se-rum hs-CRP level with MR Imaging have shown increased infarct size and raise.Damage area is bigger, and the corresponding serum hs-CRP levels changes more violently.
5.Diagnosing and typing in adult congenital ureter cysts by ultrasonography

Objective To evaluate the diagnosing and typing value of ultrasonography in adult congenital ureter cysts.Methods Ultrasonography findings were depicted of 29 cases with adult congenital ureter cysts confirmed by surgical and pathological examinations. Results In 29 cases of congenital ureter cyst, 14 occurred in right lateral, 12 in left lateral, and 3 in bilateral, totally being 32 cysts. The largest size of the cyst in diameter was (5.2) cm, and the smallest was (0.4) cm. Twenty-eight cases was diagnosed correctly by ultrasonography,in which the accuracy was (96.6)%(28/29). According to the sonographic patterns of size, shape, periodical change,1 case hydronephrosis, surgical and pathological results, 32 adult ureter cysts were divided into four types: type Ⅰ, papillary(12); type Ⅱ, round or ellipse(9);type Ⅲ, semilunar(4);and type Ⅳ, oblate(7). Conclusions Ultrasonographic appearance of adult ureter cyst is specific, and ultrasonography is of great value.
6.Expression of GST fusion proteins of human cytochrome P 2B6 and preparation of anti-cytochrome P2B6 polyclonal antibody

AIM: To get large amounts of pure antigens to raise specific antibodies and to perform quantifications.METHODS: CYP2B6 (cytochrome P) cDNA fragments was ligated into BamHI restricted PGEX-3b to generate recombinants PGEX/2B6. We identified recombinants PGEX/2B6 by EcoRI digestion. The expression of fusion proteins were induced by adding isopropyl-thiogalactoside(IPTG). Several clones showed high-level expression of fusion proteins. Insoluble proteins was isolated from the bacteria and the fusion proteins was recovered and purified from a preparative (2mm) SDS-PAGE. The polyarcrylamide gel containing the fusion proteins glutathione S-transferase(GST-2B6) were used to immunize BALB/C mice from which polyclonal ascites fluid was prepared. The purified fusion proteins GST-1A1(GST fusion protein of CYP1A1 cDNA246~386aa expressed in this library ,purified by preparative SDS-PAGE), GST-2B6 were used to test the specificity of 2B6pAb. RESULTS:Fusion proteins constructed between GST and CYP2B6 was expressed in Escherichia coli DH5?. Mouse antibodies are raised against the fusion proteins GST-2B6. 2B6pAb was fond to be specific antibody.CONCLUSION:Recombinant PGEX/2B6 were constructed and purified fusion proteins GST-2B6, and specific 2B6pAb were obtained.
7.Probe melting curve analysis-based PCR assay for the rapid detection of non-deletionα-thalassemia(αT)

Objective To investigate the clinical value of the probe melting curve analysis‐based PCR assay for the detection of three types of αT .Methods A total of 149 blood and prenatal archival DNA samples (6 of which were amniotic fluid samples)with three knownαT genes ,which included 63 carriers with Hb CS ,22 cases with Hb QS ,43 individuals with Hb WS and 1 double heter‐ozygote with Hb CS and Hb WS) as well as 20 samples with normalα‐globin gene sequence that had been confirmed by RBD com‐bined with DNA sequencing were selected to test the specificity of probe melting curve analysis by blind analysis .Results The probe melting curve analysis accurately detected 100 of the DNA samples previously characterized by S RBD combined with DNA sequencing .Conclusion Probe melting curve analysis‐based PCR assay for the detection ofαT is featured with rapidity and accuracy and can be applied to clinical and prenatal diagnosis .
8.Expression of phenylalanine ammonia-lyase cDNA from rice in E. coli BL21DE3

AIM: To study the expression and its kinetics of rice phenylalanine ammonia-lyase gene encoding into E. coli as the basis of treatment for phenylketouria. METHODS: The phenylalanine ammonia-lyase-1-cDNA(rPAL-1-cDNA) from rice was recombined into E. coli high expression vector pET-28c and transformed into E. coli host strain BL21DE3. Engineering bacteria was then inducted by isopropyl-?-D-thiogalactoside (IPTG) for 1, 3, 5, 7 hours, in order to obtain high level expression. RESULTS: After induction, the expression level of fusion protein was 21.40%, 30.60%, 35.40%, 35.43% respectively. The fusion protein exhibited a band of 78 6 kD on SDS-PAGE analysis,but was not found in controls.The target protein was mainly existed in the form of inclusion body. CONCLUSION:Rice PAL gene expressing E. coli was established by gentic engineering technique.
9.Compatibility Studies of Effective Fractions in Shengmaisan on Cerebral Ischemia and Anoxia in Mice

Objective To explore the proper proportion of effective fractions in Shengmaisan(saponins of Radix ginseng,saponins of Radix ophiopogonis,Lignans of Fructus schisandrae)in different anoxia models.Methods Acute cerebral hypoxia was induced by sodium nitrite and decapitation in mice,and the orthogonal design was used in these two models to find the proper proportion of three effective fractions.The gasping time of the mice,which were decapitated was observed to compare the anti-anoxia effects of XZF with other clinical drugs,and cerebral ischemic reperfusion injury model of mice was also used to study the effect of XZF on related biochemical index.Results XZF(the proportion of saponins of Radix ginseng,saponins of Radix ophiopogonis,Lignans of Fructus schisandrae as 7 ∶ 2 ∶ 6)significantly prolonged the gasping time,and decreased brain nitrogen monoxidum(NO)content after reperfusion of the mice at dosages of 50 mg/kg and 150mg/kg.Meanwhile,XZF also reduced malondialdehyde(MDA)content and increased superoxide dismutase(SOD)activity at the higher dosage.Conclusion XZF obtained by experimental screening exerts a significant protective effect on cerebral ischemia injury in mice,which provide some pharmacological evidence for further development of new modern Chinese drug composed with effective fractions for cerebral vascular diseases.
10.An ERP Study on Visual Illusory Motion

Objective To explore the neural basis of the visual illusory motion using the event-related potential(ERP).Methods One hundred trials(each duration 2 000 ms)of a visual illusory motion figure,rippling wheat pattern made by Akiyoshi Kitaoka,and a visual static figure made of modified rippling wheat pattern were randomly presented with equal probability.Ten healthy right handed undergraduate students as subjects were asked to judge the stimulus whether motional or not.The EEG was recorded from 128 scalp sites using with electrodes mounted in HydroCel GSN cap(Electrical Geodesics Incorporated,Oregon,USA).Results 1)The C1 component of ERP could be evoked by both visual illusory motion pattern and static pattern in POz.The peak of C1 component was presented about 75 ms after each stimulus.C1 component evoked with visual illusory motion was negative,but it's static pattern to be positive.2)P100 and P200 components could be obviously evoked by both kinds of stimuli,but in O2 the amplitudes of P100 evoked by different stimuli were significant different and in T3 the amplitudes of P200 were significant different too.Conclusion The visual illusory motion is formed at the primary visual cortex.It may be related to the organized mode of perception.
