1.Interventional micro-injury operation in treatment of emergent severe hypertensive intracerebral hemorrhage

30 ml) hospitalized in Southwest hospital from Oct 2002 to Aug 2005 were subjected in this study.They were equally divided into 3 groups(34 patients for each group),groupⅠreceiving non-surgical treatment,groupⅡgetting conventional surgical treatment,and group Ⅲ operated by interventional micro-injury operation.Results The fatality rate was 61.7%,55.8% and 20.6% respectively in the 3 groups.Their score of activities of daily living(ADL) were investigated during the 6 months' close follow-up,ADL_(1-3) in groupⅢ was much higher than in the other groups.Conclusion Interventional micro-injury operation can obviously reduce the fatality rate of severe hypertensive intracerebral hemorrhage and promote the ADL after operation.
2.Effect of nutritional intervention combined with aerobic exercises on serum adiponectin of nonalcoholic fatty liver patients

ObjectiveTo study the effect of nutritional intervention combined with aerobic exercises on serum adiponectin and insulin resistance in nonalcoholic fatty liver (NAFL) patients.Methods32 patients confirmed as NAFL between April 2008 and August 2009 were enrolled into NAFL group and 31 health people into control group.NAFL group received nutritional intervention and followed aerobic exercise program.The changes of body mass index (BMI),serum adiponectin,fasting blood glucose (FBG),fasting insulin (FINS) and serum lipids were recorded before and after interventions.Insulin resistance index (HOMA-IR) was calculated.Inter-group difference,changes after intervention,and correlation between serum adiponectin and the other indexes were analyzed.ResultsCompared with the control group,BMI [ (29.84 ±3.16) kg/m2 vs.(22.22 ± 1.81 ) kg/m2,P =0.000],total cholesterol (TC) [ (5.09 ±0.86) mmol/L vs.(4.38 ±0.85) mmol/L,P =0.002],triglyceride (TG) [ (1.99±0.86) mmol/Lvs.(1.10±0.38) mmol/L,P=0.000],FBG[ (6.02±0.63) mmol/Lvs.(5.02±0.71) mmol/L,P=0.000],FINS[ (13.73±3.24) mU/Lvs.(9.55±1.99) mU/L,P=0.000],HOMA-IR [ (3.74±1.21) mmol/L vs.(2.08 ±0.27) mmol/L,P=0.000] in the NAFL group were significantly higher,while serum adiponectin in the NAFL group was significantly lower [ (4.89 ± 1.81 ) ug/L vs.( 12.97 ± 3.15 ) ug/L,P =0.000 ].After nutritional intervention and aerobic exercises,BMI [ ( 26.65 ± 2.39 ) kg,/m2 vs.(29.84 ±3.16) kg/m2,P =0.000] 、FBG [ (5.61 ±0.47) mmol/L vs.(6.02 ±0.63) mmol/L,P =0.004 ],and FINS [ ( 11.12 ± 2.01 ) mU/L vs.( 13.73 ± 3.24) mU/L,P =0.000 ] in the NAFL group were decreased significantly,while serum adiponectin [ (6.54 ±2.12) ug/L vs.(4.89 ± 1.81 ) ug/L,P =0.001 ] was significantly higher than the level before intervention.Serum adiponectin was negatively correlated with BMI,FBG,FINS,HOMA-IR,and TG (P =0.004,0.000,0.000,0.000,and 0.010),and positively correlated with high density lipoprotein cholesterol (P =0.026),but not correlated with TC (P =0.101 ).ConclusionThe combination of nutritional intervention and aerobic exercises can improve the level of serum adiponectin,adjust the level of serum glucose and serum lipids,and alleviate insulin resistance in NAFL patients.
3.Relationship between serum adiponection and liver function and insulin level in patients with hepatogenous diabetes

Objective To study the relationship between serum adiponection and liver function and insulin level in patients with hepatogenous diabetes.Methods The research subjects included:28 patients with hepatog-enous diabetes who were admitted at Liaocheng People's Hospital (hepatogenous diabetes group);27 patients with simple type 2 diabetes without any complication (simple type 2 diabetes group),and 27 normal controb (control group).Blood adiponectin,fasting blood glucose (FBG),fasting insulins (FINS),alanine aminotransferase (ALT),aspartate transferase (AST),total bilirubin (TBIL) and albumin (ALB) were measured,and according-ly the insulin sensitivity index (ISI) was calculated.Results Compared with the control group.serum adiponec-tin levels and ISI levels were significantly lower in the other two groups (P<0.01),and the adiponectin levels were significantly higher in hepatogenous diabetes group than in simple type 2 diabetes group (P<0.05).The FBG and FINS levels in the hepatogenous diabetes group and the FBG levels in the simple type 2 diabetes group were significantly higher than those in control group (P<0.01).The FINS levels were significantly higher in the hepatogenous diabetes group than in simple diabetes group (P<0.05),while the levels of FBG and ISI were lower than that of simple type 2 diabetes group (P<0.05).Adiponectin levels were negatively correlated with ISI (r= 0.5265,P=0.000),and positively correlated with FBG (r=-0.6040,P=0.000),and were not correlated with liver functions Conclusion Insulin resistance and abnormal metabolism of adipenection exist in patients with hepatogenous diabetes and may together affect the occurrence and development of hepatogenous diabetes.
4.Surgical management of atrophic gallbladder

Objective To investigate the surgical management of atrophic gallbladder.Methods The clinical data of 85 patients with atrophic gallbladder surgically treated in our hospital from October 2002 to October 2005 were retrospectively analyzed.Results There were 3 cases of iatrogenic bile duct damage,2 of enteral damage,1of post operative remaining small gall bladder,1 of remain ingstonesinthe bile duct and 1 of gallstone ileum.Two 2 patients died after operation.Conclusion Atrophic cholecystitis is always accompanied by many complex clinical problems.Adoption of proper surgical methods can lower the incidence of postoperative complications.

Objective To analysis the genetic context of armA gene identified in different species .Methods BLASTN searches were used to analysis sequences harboring armA submitted to the GenBank database .Results armA gene was mostly found down-stream of ISCR1 element and was most commonly detected in Enterobacter spp ..Its flanking sequences were highly conserved in Enterobacter spp ..Conclusion The genetic context of armA gene reported in different species scattered in different geometrical lo-cations share some common characteristics .
6.Website information disclosure by 80 public hospitals in China and the analysis

Objective Studying information disclosure at China' s public hospitals and recommending on information disclosure policies.Methods Collecting and analyzing the websites of 80 public hospitals in five main Chinese cities (Beijing,Shanghai,etc.).Results Information disclosure by these hospitals is found to be identical,with a slight difference in information publicity.By percentage,the information disclosure of hospital profile,department navigation,specialists characteristics,hospital news,health education,and doctor-seeking guidance,is 100.0%,98.3%,95.0%,93.3%,90.0%and 88.3%respectively.The disclosure is found to be lack of general planning,objectivity,timeliness and sufficiency.ConclusionEstablishing the information disclosure system based on mandatory information disclosure,stipulating the content,approach and timing for public hospitals information disclosure.

8.Comparative study of cystic renal cell carcinoma by ultrasonography and CT

Objective To study the features and values of ultrasound and Helical computed tomography (CT) in diagnosis of cystic renal cell carcinoma. Methods Twenty patients were preoperatively examined by B-mode ultrasound, color Doppler ultrasound and helical CT. Results All the cases were confirmed by operation and pathology,including 12 cases on the left side and 8 cases on the right side. On B-mode ultrasound images, all lesions appeared as unilocular or multilocular cyst in 5 cases, intratumoral septum in 10 cases,intratumoral nodule in 4 cases,solid-like mass in 1 case. Color blood flow was showed within tumors in 18 patients,in which 17 cases showed the arterial spectrum with a mean resistance index of 0.66. On enhanced CT scans, all the lesions appeared as the early uneven enhancement(19 cases), septal enhancement(6 cases) and mural nodular enhancement(4 cases) respectively. The accuracy of conventional ultrasound combined with color Doppler ultrasound was 85%, while the accuracy of the enhanced CT was 80%.Conclusions Conventional ultrasound combined with color Doppler ultrasound and enhanced CT plays an important role in diagnosis of cystic renal cell carcinoma.
9.Clinical Effects of Acupuncture Combined with Herbal Medicine with the Functions of Detoxication and Purging Fu-organ on Patients with Constipation of Phlegm-heat and Bowel-repletion Type in Acute Cerebral Infraction

Objective To probe into the clinical effect of acupuncture combined with herbal medicine with the functions of detoxication and purging fu-organ on patients with constipation of phlegm-heat and bowel-repletion type in acute cerebral infarction.Methods The enrolled cases were randomly assigned to an observation group of 35 cases and a control grouD of 35 cases.Besides both groups were treated with routine neurological treatment and acupuncture with the flinchon of brain activating,the observation group was treated by acupuncture combined with herbal medicine with the functions of detoxication and purging fu-organ and the control group was treated with Dilute Glycerol.Such indexes of patients in both groups were observed 7 days and 21 days before and after the treatment as clinical effect of constipation,scores of related symptoms,and scores of traditional Chinese medical category and clinical effect.Results The effective rates of eonstipation 7 davs and 21 days after treatment were 93.7% and96.9% in the observation group,and 61.8% and79.4% in the control group,with significant differences between the two groups(P<0.01).After the treatment for 7 days and 21 days,the symptoms of constipation.dark tongue,yellow and greasy fur were improved significantly in the observation group than those in the control group(P<0.05);The observation group also showed a definite therapeutic effect than the control group on treating the svmptoms of upset minded,constipation,red or dark tongue,yellow and greasy fur(P<0.01).The scores of traditional Chinese medical category of the observation group were lower than that of the control group(P<0.01)after treatment for21 days.The total improvement rate was 97.0% in the observation group comparing 55.8%in the control group,showing significant difference(P<0.01).Conclusion The method of acupuncture combined with herbal medicine with the functions of detoxication and purging fu-organ has a definite therapeutic effect on treating constipation of phlegm-heat and bowel-repletion in acute cerebral infarction.It can also improve clinical effects of these patients,which is worth generalizing in the clinical practice.
10.Effects of ultrasound-guided paravertebral nerve block combined with general anesthesia on intraoperative stress response, hemodynamics and postoperative pain of patients undergoing laparoscopic cholecystectomy

Objective:To observe the effects of ultrasound-guided paravertebral nerve block combined with general anesthesia on intraoperative stress response, hemodynamics and postoperative pain of patients undergoing laparoscopic cholecystectomy.Methods:A total of 92 patients undergoing elective laparoscopic cholecystectomy in Wuhan Third Hospital between January 2017 and August 2019 wereselected. They were divided into the control group (63 cases) and the observation group (29 cases) by voluntary. The control group were given simple general intravenous anesthesia, and the observation group were given ultrasound-guided paravertebral nerve block combined with general intravenous anesthesia. Changes in hemodynamic parameters and concentrations of blood angiotensin-Ⅱ (AT-Ⅱ) and cortisol (Cor) in the two groups were observed. The cerebral state indexes (CSI) of both groups were recorded, and pain degree of patients was evaluated with the visual analogue scale (VAS).Results:The mean arterial pressure (MAP) and heart rate (HR) of the observation group were higher than those of the control group at 30 min after anesthesia, 30 min after operation, 3 hours after operation ( P<0.05), the stroke volume variation (SVV) was lower than that of the control group ( P<0.05); the AT-Ⅱ, Cor and CSI in the observation group were lower than those in the control group at each time point ( P<0.05); The dosage of narcotic drugs in the observation group was significantly lower than that in the control group ( P<0.05); The recovery time of spontaneous breathing, recovery time, extubation time and respiratory satisfaction time in the observation group were less than those in the control group ( P<0.05), and the VAS scores at 3 h, 6 h and 12 h after operation in the observation group were lower than those in the control group ( P<0.05). Conclusions:Ultrasound-guided paravertebral nerve block combined with general anesthesia can effectively reduce the patient's stress response during laparoscopic cholecystectomy, with good analgesic effect, which is helpful to maintain the hemodynamic stability of patients.
