1.The treatment and diagnosis of smooth muscle tumors of the duodenum

Objective To investigate the diagnostic and theraputic problems in patients with smooth muscle tumors of the duodenum.Method The clinical data from 7 cases smooth muscle tumors were analyzed retrospectively which were treated surgically and confirmed pathologically in our hospital from 1987 to 2001.Results Of all the cases,4 had a leiomyoma in the duodenum and 3 leiomysarcoma.This disease is lack of pathognomonic sign and difficult for early diagnosis.All the cases were treated surgically and got good results.Conclusions Upper gastrointestinal bleeding is common in patients with this disease.Upper gastrointestinal tract examination and gastrduodenoscopy and CT scan are the main methods for diagnosis.Operation is the best choice for treatment.
2.Fat necrosis of beast :a report of 50 cases

Objective To investigate the etiology, pathology, diagnosis and management of patients with fat necrosis of breast (FNB). Method The clinical data of 50 cases of FNB confirmed pathologically in our hospital from 1966 to 2001 were analyzed retrospectively. Results All the patients in this series were females with the average age of 32.5 years and 56% of the patients over 30 years. The main clinical feature was breast lump and some of the patients had a trauma history. 48 patients were treated surgically and other 2 got an accurate diagnosis by biopsy and were treated by non operative therapy. Conclusions FNB is easily misdiagnosed and difficult to be differentiated from breast cancer, thus the surgery or needle biopsy is recommended.
3.The progress of study on human CD5+B cells

CD5+ B cells are derived from early stage precursors independently on bone marrow. It is an important subtype of B cells. Abnormalities of number and function of CD5+ B cells are found in several kinds of autoimmune diseases and B lymphocyte malignancies. Recently,the abnormalities are also found in AIDS,hepatitis, schizophrenia and the regeneration of liver cells, which indicates that CD5+ B cell may have extensively clinical significance.
4.Application of Trihexyphenidyl in Dystonia in Children with Cerebral Palsy (review)

Dystonia is common in children with cerebral palsy. It can cause motor dysfunction, and is frequently associated with altered speech articulation, abnormal swallowing, and excessive drooling. Although trihexyphenidyl (Artane) is an anticholinergic agent with a long history of use in the management of dystonia in adults, information is limited regarding the use of trihexyphenidyl in children. This study reviewed the clinical experience of the use of trihexyphenidyl in children with cerebral palsy for dystonia, with special emphasis on benefits and tolerability.

6.Clinical Study on Operation for Rupture of Aortic Sinus Aneurysm

Objective To summarize the ruptured types, operational ways and therapy effect for rupture of aortic sinus aneurysm.Methods The operational cured 55 cases with aneurysm of aortic sinus were retrospectively analysed.Results There were 46 cases of right aortic sinus aneurysm, 45 cases among them rupture was peretrated into right ventricle, one rupture was peretrated into right atrium;there were null aneurysm of aortic sinus, 5 cases among them ruptured into right atrium, 2 cases ruptured into right ventricle; 2 cases of left aneurysm of aortic sinus, ruptured into left ventricular. There were 24 cases with ventricular septal defect, 32 cases with aortic valvular imcompetence, 6 cases with other diseases, 8 cases complicated with infective endocarditis. All cases underwent operational treatment, one patient who had severe left heart failure and huge left ventricle before operation died from refractory arrhythemia after operation, another who had severe infective endocarditis,and patients with cardic function degree Ⅳ were died from low cardic output syndrome, others were better.Conclusions The right aneurysm of aortic sinus were the most types, and it always ruptured into right ventricle; It suggests that it is important for aneurysm of aortic sinus should be operated as early as possible in order to avoid losing the chance of operation, particular in the patients complicated with infective endocarditis .
7.The Effect of Mesenchymal Stem Cells Transplantation on Angiogenesis and Secretion of Cytokines After Rat Myocardial Infarction

Objective To investigate the effect of mesenchymal stem cells transplatation on angiogenesis and secretion of cytokines after rat myocardial infarction. Methods Harvested rat mesenchymal stem cells were cultured and labeled in vitro. And at the same time, myocardial infarct model was set up by liquid nitrogen cryoinjuring the left ventricular free wall. 4 weeks later, 2?10 6 mesenchymal stem cells or cold D-Hanks liquid were injected into several different points of infarcted area. At 1, 2 and 4 weeks after mesenchymal stem cells transplatation, specimens were acquired from infarcted area. Then cell morphology and capillary density were measured, and vascular endothelial growth factor(VEGF) and transforming growth factor ?(TGF-?) expression levels were assayed by RT-PCR. Results Mesenchymal stem cells were alive at 1, 2 and 4 weeks after transplantation and had a trend toward differentiation and maturation in vivo. The number of capillary vessels in per infarcted area (0 1 mm 2) in experiment group increased compared with control group. Objective To investigate the effect of mesenchymal stem cells transplantation on heart function and the expression of Ca 2+ -ATPase (ERCA) mRNA after rat myocardial infarction. Methods Wistar inbred rat mesenchymal stem cells were cultivated, proliferated and labeled in vitro. Myocardial infarct models were set up by liquid nitrogen cryoinjuring the free wall of left ventricle. 4 weeks later, 2?10 6 mesenchymal stem cells or cold D-Hanks solution were injected into several different points of infarcted myocardial area of experiment group and control group rats. In addition, 5 Wistar inbred rats served as normal group only got thoracotomy twice. 4 weeks after transplantation, all rats received cardiac function test and specimens from infarcted area were got for the expression assay of SERCA2 mRNA by RT-PCR. Results Mesenchymal stem cells were alive at 1, 2 and 4 weeks after transplantation and had a trend toward differentiation and maturation in vivo. Traumatic dynamic measurement was applied to assay heart function by Buxco system, which showed that SLVP and +dp/dt were the highest in normal group, next in experiment group, and the lowest in control group, which LVEDP was the lowest in normal group, next in experiment group, and the highest in control group. 9.Experience in diagnosis and treatment of digestive tract bleeding with gastrointestinal vascular malformation

Objective To summarize the experience of diagnosis and management of bleeding with GIVM.Method The clinical data of 26 patients admitted from 1989 to 1999 were analyzed retrospectively.Results GIVM with bleeding were revealed by endoscopies (42%),by ECT (55%),by selective arteriography (83%),by barium radiography (0%),and by exploratory laparotomy(100%).1 case was cured by conservative treatment.Bleeding intestinal segment was resected in 14 cases.4 cases were cured by interventional therapy.The others were cured by sclerosing agent injection,or by suture ligation of the dilated vessel or arterial gastroduodenalis.Conclusions Endoscopy is the first choice in the bleeding from GIVM.ECT may beused later if endoscopy is useless.Selective arteriography is the most valuable method in GIVM diagnosis.If necessary, exploratory laparotomy and intraoperative endoscopies may be used.Resecting bleeding segment is the most effective treatment.Other methods such as interventional therapy and suture ligation of dilated vessel are also effective.
10.The Dynamic-enhanced MR Angiography of the Liver : A Comparision Study with DSA and CTAP

Objective To assess the role of dynamic-enhanced MR angiography (MRA) by comparing with DSA and CT arterial portography (CTAP).Methods Sixteen patients with liver diseases (mainly HCC) were included in the study. All patients underwent dynamic MRI of the liver using SENSE, digital subtraction angiography (DSA) and CTAP within two-week. MRA was reconstructed from raw data. The arterial phase of the modified MRA was compared with DSA for the evaluation of hepatic arteries and the portal phase compared with CTAP. In dynamic MRI, a fixed dosage (20 ml) of contrast medium and scan timing were used. Results The main branches and variations of the hepatic arterial system were well displayed on the dynamic-enhanced MRA, although the distant intrahepatic branches were showed poorly. Portal veins on MRV were showed as well as or even superiorly to CTAP. In addition, the inferior vena cava (IVC) was well demonstrated on MRV in most cases. MRV revealed 1 vascular variation which was not found on DSA and CTAP.Conclusion The dynamic-enhanced MRA using SENSE was a effective modality in demonstrating the hepatic vascular system. It may be a first choice modality in preoperative evaluation of orthotopic liver transplantation.
