1.Artificial femoral head replacement in treatment of advanced age patients with unstable intertrochanteric femoral fractures

ObjectiveTo evaluate the clinical effect of artificial femoral head replacement in treatment of unstable intertrochanteric femoral fractures.MethodsThe clinical data of 52 unstable intertrochanteric femoral fractures patients with artificial femoral head replacement were retrospectivly analyzed.Postoperative complications and postoperative weight-bearing time were recorded.The function of joints was evaluated with Harris score.ResultsAll patients were followed up for 6 to 24 months ,an average of 13.2 months.1 case was a lung infection after surgery,and 3 cases were deep vein thrombosis.Weight-bearing walking time averaged 28 days.According to Harris score:25 cases were rated as excellent, 18 good,7 fair,2 poor.A total of 82.7% of patients had their joint function returned to normal or near normal.ConclusionIt was an effective mothod to treat the unstable intertrochanteric fractures in elderly patients with hemiarthroplasty.
2.Synergistic effect of platelet-derived growth factor-BB and transforming growth factor-beta1, on expression of integrin beta3 in periodontal membrane of rat orthodontic tooth.

OBJECTIVETo investigate the synergistic effect of transforming growth factor-beta1 (TGF-beta1) and platelet-derived growth factor-BB (PDGF-BB) on the expression of integrin beta3, in periodontal membrane of rat orthodontic tooth.
METHODSAn orthodontic tooth movement model was established. Up to 32 experimental rats were randomly divided into four groups according to a random number table. The four groups were injected with 1% PBS, TGF-beta1 (5 ng), PDGF-BB (10 ng), and combined TGF-beta1 (5 ng) and PDGF-BB (10 ng) in the buccal submucosal, respectively. The volume injected in each group was 0.1 mL. The animals were then sacrificed on the 10th day. The left maxillary first molar and periodontal tissue were taken. Different expressions of integrin beta3 were detected in periodontal tissues through immunohistochemistry. Mean optical density (OD) values of the positive fields were examined. The data obtained were analyzed through ANOVA. The data followed normal distribution, and were compared via t-test.
RESULTSCompared with the control groups, the expression of integrin beta3 was higher in the experimental groupin tension sides (P < 0.01). Significant differences in tension sides between the single-injection groups and the combined group were observed (P < 0.01). Compared with the control groups, the expression of integrin beta3 was higher in the experimental group in compression sides (P < 0.05). In addition, there was no significant differences in compression sides between the single-injection groups and the combined group (P > 0.05).
CONCLUSIONIn terms of local regulatory factors, TGF-beta1 combined with PDGF-BB enhance the expression of integrin beta3 in the periodontal membrane and accelerate periodontal remodeling. The synergistic effect of the two growth factors is better than the single growth factor.

3.Contrast-induced nephropathy in the elderly and the correlated clinical risk factors

Objective To investigate the incidence of contrast-induced nephropathy (CIN) in the elderly undergoing coronary angiography and the correlated clinical risk factors.Methods 0.90% sodium chloride (1 ml · kg~-1 · h~-1 ) was administered 6 hours before and 12 hours after contrast media administration for 195 elderly patients,and the dose of sodium chloride was half when the patients were suffering from severe congestive heart failure.The levels of serum creatinine (Scr) and cystatin C were measured 3 days before and on the 2nd,5th day after the administration of contrast,respectively .Results(1) The incidence of CIN was 8.7% (17/195).The renal function of 11 cases recovered to baseline level 3 months after the use of contrast,2 cases had transformation to chronic renal failure but without undergoing maintenance hemodialysis,and 3 cases were dependent on maintenance hemodialysis,while 1 case developed multiple organ dysfunction until death.(2) There was no significant difference between pre-and post-contrast administration in the level of cystatin C (t=O.137,P= 0.891). But the concentration of Scr was reduced significantly on the 2nd day compared with the baseline (t=4.776,P = 0.000).Both the cystatin C and Scr recovered to the baseline on the 5th day.(3) There were no significant differences between the CIN and non-CIN group in gender,the dose of contrast,the baseline Scr,and the proportion of coronary heart disease.But there were significant differences in age,the baseline serum cystatin C,the proportions of diabetes mellitus,hypertension and congestive heart failure.(4) The clinical risk factors of CIN were the baseline Scr (OR = 1.039),the baseline serum cystatin C (OR=6.654),diabetes mellitus (OR=8.104) and congestive heart failure (OR=9.597) according to the analysis of logistic regression.ConclusionsWith the hydration and the use of low-osmolar nonionic contrast,it is safe to receive contrast examination for the elderly patients.The baseline serum levels of Scrand cystatin C,diabetes mellitus and congestive heart failure are the clinical risk factors of CIN.Compared with Scr,serum cystatin C is the more powerful predictor for the development of CIN.
4.Combined effects of platelet-derived growth factor-BB and insulin-like growth factor-I on the expression of integrin (beta 3 protein in periodontal ligament cells of rats during orthodontic tooth movement.

OBJECTIVETo investigate the combined effects of platelet-derived growth factor-BB (PDGF-BB) and insulin-like growth factor-I (IGF-I) on the expression of integrin (beta3 protein in the periodontal tissues of the compressing side of orthodontic tooth in rats.
METHODSEstablishing orthodontic movement model in Sprague-Dawley rats, which were injected with 10 ng PDGF-BB, 200 ng IGF-I alone or in combination in the buccal submucosal area of the orthodontic tooth. The injection was applied every other day. The experiment continued for ten days and then the rats were sacrificed. The expression of integrin (beta3 protein in periodontal ligament tissues of the compressing side was detected by immunohistochemical techniques.
RESULTSThe expression of integrin (beta3 protein in periodontal ligament tissues of the compressing side of each experimental group was significantly increased compared with that of the control group (P<0.01). Meanwhile the maximum mean optical density value of integrin (beta3 protein expression was attained in the combination group which showed a significant increase compared with the PDGF-BB group (P<0.05) and the IGF-I group (P<0.01).
CONCLUSIONThe combination of exogenous PDGF-BB and IGF- I in orthodontic tooth movement may enhance the expression of integrin (beta3 protein in periodontal ligament cells and PDGF-BB and IGF-I may have a synergistic effect during orthodontic tooth movement.

6.Further exploration on TCM properties of the foreign Chinese materia medica of Mirabilis Radix

Mirabilis jalapa L. is a plant of the family of Miraceae, native to tropical America and introduced to China during the Ming Dynasty. Its main medicinal part is the root. After sorting, it was found that the research on the medicinal properties of the Mirabilis Radixis incomplete, and the records of medicinal properties do not fully match the current application, and there is a situation of "the raw and processed for different treatments". This study further explored its medicinal properties. The Mirabilis Radix is bitter, pungent, and slight cold; belonging to the bladder, liver, and stomach meridians; it has the functions of clearing heat and dampness, detoxifying and reducing swelling, promoting blood circulation and regulating menstruation, dispelling wind and dampness, and nourishing yin. It was taken orally, with decoction, and adults should take 15-30 g, not in the form of original powder. People with spleen and stomach deficiency and cold should take it with caution, and pregnant women should not take it; for external use, an appropriate amount of fresh products should be applied and mashed. Processed Mirabilis Radix, recorded by Tujiazu medicine and Uyghur medicine, after boiling or steaming, the medicinal properties change and the nourishing effect is enhanced, but there is a lack of relevant research. Further exploration of the medicinal properties of the Mirabilis Radix can provide reference for clinical application, standard improvement, and product development.
