1.Establishing a rat model of diabetic acromelic gangrene and intervention of Chinese compound

BACKGROUND:Diabetic acromelic gangrene(diabetic foot)has become one of the important causes for the disability and death in diabetes mellitus.OBJECTIVE:To establish model of diabetic foot in rat,and observe the interventional effect of Chinese compound on diabetic foot.DESIGN:A comparative observational experiment.SETTING:The First Affiliated Hospital of Clinical Medical College,Henan University of Science and Technology.MATERIALS:Fifty male Wistar rats of 8 weeks old,(200.0±16.3)g,were raised with common feed in separate cage at the room temperature of 18-22℃.and they were free to access of feed and water.Ten rats were randomly selected as the blank control group,and the other 40 were used for model establishment.METHODS:The experiments were carried out in the Animal Room of Henan University of Science and Technology from October 2001 to April 2002.①Grouping:The 40 rats were fasted for 6 hours,and then treated with intraperitoneal injection of streptozotocin(55.0 mg/kg),while the 10 rats in the blank control group were injected with isovolume sodium citrate buffer solution.20 models were successfully established,and they were randomly divided into model group(n=10)and treatment group(n=10).Rats in the treatment group were treated for 3 weeks with intragastric perfusion of Chinese compound(60 g/kg)at 9:00 every day after model establishment,and those in the model group were given intragastric perfusion of isovolume saline.At the end of the third week,the rats were all killed under anesthesia after fasted for 12 hours,blood samples were collected to determine the levels of fasting blood glucose,blood lipids and insulin.The daily amount of drinking water was recorded in each group during the experiment.②Scoring standards for acromelic gangrenes:The limbs rats with diabetic foot were scored by three grades,including dark skin,mild open focus on skin,and focus had invaded deep muscular tissue.The total score of each rat was calculated.The beta-cell function index (HBCI)was also evaluated.MAIN OUTCOME MEASURES:The changes of the amount of drinking water,body mass and levels of triglyceride,cholesterol,fasting blood glucose,blood lipids and insulin were observed before and after treatment.RESULTS:Totally 50 Wistar rats were used.20 of them were excluded due to unsuccessful model establishment,and the other 30 rats were involved in the final analysis of results.①The amount of drinking water was obviously higher in the model group and treatment group than in the blank control group during the experiment(P<0.01).As the treatmentlasted,the amount of drinking water was obviously decreased in the treatment group,but gradually increased in the model group.②After treatment,the body mass was obviously lower than that before treatment in the model group(P<0.01).but had a descending trend without obvious difference as compared with that before treatment in the treatment group(P>0.05).③Obvious acromelic gangrenes were obvious in both groups(P<0.01).The body mass in the treatment group was obviously higher than that in the model group(P<0.01),and the conditions of acromelic gangrene were obviously better than those in the model group(P<0.01).④Before treatment,the levels of fasting blood glucose in the treatment group and model group were obviously higher than that in the blank control group(P<0.01),while the levels of insulin and HBCl were obviously lower than those in the blank control group(P<0.01).After treatment,the levels of fasting blood glucose in the treatment group and model group were obviously higher than that in the blank control group(P<0.01),and it was obviously lower in the treatment group than in the model group(P<0.01),it was close to the normal value in the treatment group.⑤The levels of triglyceride and cholesterol before treatment were obviously higher in the treatment group and model group than in the blank control group(P<0.01).After treatment,the levels of triglyceride and cholesterol in the treatment group were obviously decreased(P<0.01), which were not obviously different from those in the blank control group (P>0.05), while those in the model group were increased continuously,and obviously higher than those in the blank control group(P<0.01).CONCLUSION:Increasing the serum level of insulin and decreasing the levels of blood glucose and blood lipids can prevent and treat the occurrence and development of diabetic foot to some extent.This model of diabetic foot is sensitive to drug,and can be used to investigate the pathogenesis of diabetic foot and evaluate the effect of drug therapy.
2.Relationship between expression of ERCC1 and prognosis of smoking patients with advanced NSCLC

Objective To investigate the expression of ERCC1 in smoking patients with advanced non-small cell lung cancer(NSCLC) and its relationship with clinical features and prognosis.Methods The expression of ERCC1 was detected in 96 patients with advanced NSCLC by PCR assay.The clinicopathologic factors, treatment effect and survival time were observed.Results The expression of ERCC1 was related to smoking index(P=0.029), but it was not related to other clinicopathological factors.The patients with low expression of ERCC1 had better response rate and median survival time when compared to those with high expression patients.The difference was statistically significant(P=0.001,P<0.01).Conclusion ERCC1 expression is associated with smoking index and the patients with low expression of ERCC1 shows higher chemotherapy efficiency and longer median survival than patients with high expression, which indicates that detection of ERCC1 may be a useful parameter in evaluating the therapeutic effect and prognosis of smoking patients with advanced NSCLC.
3.An experimental study on repairing sciatic nerve defects of rats by human umbilical cord derived mesenchymal stem cells and acellular nerve basal lamina tube

Objective To explore the feasibility of using human umbilical cord derived mesenchymal stem cells as seed cells to repair sciatic nerve defects of rats by tissue engineering methods. Methods Mesenchymal stem cells from human umbilical cord were cultured and induced into neuron-liked cells,which were co-cultured with acellular basal lamina tube to construct tissue engineering nerve;models of sciatic nerve defects 10 mm in length were set up with thirty healthy adult SD rats and were divided randomly into 3 groups:tissue engineering nerve group (group A, compound of human umbilical cord derived mesenchymal stem cells and acellular basal lamina tube), pure acellular basal lamina tube group (group B), and autogenous nerve bridging group (group C). Evaluation of electrophysiological and histological results was carried out 10 weeks after operation. Results The engineering nerve group had good result in nerve regeneration which was close to the effect of autogenous nerve transfer group (group A), and much better than the effect of pure acellular basal lamina tube group. Conclusion Engineering nerves from human umbilical cord derived mesenchymal stem cells can effectively repair 10 mm defects of sciatic nerve.
4.Effects of simvastatin combined with aspirin on atherosclerosis and improvement of NMD in patients, HDL-C and LDL-C levels

Objective To investigate the effect of simvastatin and aspirin on atherosclerosis and its effect on non endothelium dependent diastolic function(NMD)and blood lipid in patients.Methods Retrospective analyzed a total of 95 the clinical data of patients with atherosclerosis treated in Tianjin Baodi Hospital from August 2014 to May 2016.The patients were divided into simvastatin group and combined treatment group according to their treatment methods,simvastatin group was given simvastatin,combined treatment group was given aspirin on the basis of simvastatin.After four weeks treatment,the difference of plaque quantity,NMD and blood lipid between the two groups were observed.Results After treatment,the size and number of plaques in the combined treatment group were smaller than those in the simvastatin group,the difference was statistically significant(P<0.05),the NMD level in the combined treatment group was significantly higher than that in the simvastatin group,the difference was statistically significant(P<0.05),the levels of IL-18,IL-6 and hs-CRP in the combined treatment group were lower than those in the simvastatin group,the difference was statistically significant(P<0.05),the levels of LDL-C,TG and TC in the combined treatment group were lower than those in the simvastatin group,HDL-C levels were higher than simvastatin group,the difference was statistically significant(P<0.05).Conclusion Simvastatin combined with aspirin can significantly improve the endothelium dependent diastolic function and lipid level in patients with atherosclerosis,and has good therapeutic effect.

6.Maxillary protraction and multiple loop edgewise arch wire technology in treatment of early mixed dentition skeletal class Ⅲ malocclusion

Objective To evaluate the orthodontic effect of surgery on mixed dentition period class Ⅲ malocclusion and to investigate the effective therapeutic method of the skeletal class Ⅲ malocclusion.Methods A total of 20 patients aged 8.5 to 9.1 years with early mixed dentition class Ⅲ skeletal malocclusion.The treatment was divided into two phases.During the first phase,the entire stuffy group was treated with a bonded maxillary expansion and protraction.During the second phase,the entire study group received multiple loop edgewise arch wire (MEAW) treatment until an ideal occlusion relationship achieved.Comparison of the pre-and post-orthodontic treatment cephalometric radiographs had been done after maxillary protraction and MEAW were applied for patients.Results In the 20 patients after orthodontic treatment,Class Ⅲ skeletal facial profile was improved,the retraction of anterior teeth was obvious,and there were significant changes in dentition; the profile of soft tissue and satisfactory occlusion relationship were significantly improved,as compared with the changes between pre-treatment and post-treatment.Conclusions Skeletal Ⅲ malocclusion in early mixed dentition can be successfully treated.The results can be satisfied with improved class Ⅲ skeletal and molar relationship,high-angle and profile of soft tissue.
7.BRCA1 microsatellite instability in endometrial carcinoma and its relationship with clinical pathology

Objective To study the occurrence of BRCA1 MSI in endometrial cancer and its relationship with clinic pathologic features; to explore the correlation between MSI and protein expression in BRCA1 gene. Methods Application of PCR-SSCP and DNA sequence analysis method was used to study D17S579 and D17S1349 in 49 sporadic endometrial cancer tissues, 20 cases with endometrial atypical hyperplasia and 28 cases with normal endometrial tissues. Results In the total samples of D17S579 and D17S1349, the three groups were significantly different: 34.69%(17/49) in the endometrial cancer group, 10%(2/20) in the endometrial atypical hyperplasia group and 7.14%(2/28) in the normal endometrium group (χ2= 11.208, P = 0.004). BRCA1 MSI positive rate related to the pathology grade and clinical stage, but no relationship was found in muscular infiltration depth, lymph node metastasis and histopathology types. In the endometrial cancer group, BRCA1 MSI positive rate and BRCA1 protein expression were in moderate correlated negatively (r = -0.779, P = 0.000). Conclusion BRCA1 MSI might play a role in the development of endometrial cancer, and low expression of BRCA1 protein. BRCA1 MSI might be associated with pathology grade and clinical stages in EC.

Objective To detect the differential expression genes(DEGs)between Barrettg esophagus(BE)and normal esophagus with oligomicroarray,and to explore the target genes related to the development of BE.Methods The total RNAs of matched BE and normal esophagus mucosa from saIne patient were isolated with one step Trizol method.Matched RNAs were qualified with 10g/L agarose gel electrophoresis.After tRNA purification,cRNAs were synthesized and labeled with fluorescence.which were tIlen hybridized with Agilent oligomicroarray containing 30,968 probes.The fluorescence intensity features were detected by Agilent scanner and quantified by software Feature Extraction.Results On average,2 biopsies by disposable jumbo biopsy forceps provided approximately 5μg RNA required for microarray.The total RNA,reverse transcription product and fluorescence labeled cRNA were all of high quality.Among 2-fold DEGs,there were 142 up-regulated genes and 284 down-regulated genes including 15 bel-2 related genes such as bel-2,MCL1,BAX,BIK and BCLAF1 Conclusion Microarray-based studies are feasible in endoscopically obtained tissues.The development of BE is a complicated process involving multi-genes,in which abnormal expression of bel-2 family related genes might be involved,but the exact mechanism needs further research.
10.Correlation of apolipoprotein M with severity of coronary artery stenosis in patients with coronary heart disease

One hundred and thirty seven pafients diagnosed by coronary angiography were recruited in the study and divided into coronary heart disease group(CAD)and control group.Relationship between serum apolipoprotein M levels and the severity of stenosis of coronary arteries was analyzed.In covariate analysis,serum apolipoprotein M levels were significantly lower in CAD group than those in control group adjusted for sex,age,body mass index(BMI)(P<0.05).By stepwise muhiple regression,serum levels of apolipoprotein M were correlated positively with BMI(r=0.65,P<0.01),and correlated negatively with total cholesterol(TC)(r=-0.53,P<0.01)and low density lipoproteins(LDL-C)(r=-0.42,P<0.01).Serum levels of apolipoprotein M were correlated negatively with the score of coronary stenosis(r=-0.62,P<0.01).The results suggest that apolipoprotein M might be related to the development of coronary artery disease.
