2.The influence of cold provocation on retinal light sensitivity of patients with primary open-angle glaucoma

Objective To evaluate the effects of cold provocation on optic dise blood flow and retinal light sensitivity of primary open-angle glaucoma (POAG) patients,and explore the relationship between the changes of optic disc blood flow and retinal light sensitivity. Methods A total 33 POAG patients (33 eyes) and 13 normal controls (13 eyes) were tested by using the Heidelberg retinal flowmetry (HRF) and Topcon automatic perimeter,and the optic disc blood flow and retinal light sensitivity of POAG patients and normal controls were measured at normal conditions and after cold provocation. Results The mean optic disc blood flow,volume and the mean retinal light sensitivity of POAG patients decreased significantly (P<0.05) after cold provocation.There was a linear and significant relationship between the decrease of mean optic disc blood flow and mean retinal light sensitivity of POAG patients (r=0.615,P<0.001).The optic disc blood flow of POAG patients with a history of migraine were more likely to reduce in response to cold provocation and reduced much more than those without such history (P<0.05). Conclusion Cold provocation can significantly reduce both the optic disc blood flow and retinal light sensitivity in POAG patients.A close correlation was found between the amount of mean optic disc blood flow decrease and the volume of mean retinal light sensitivity decline.
3.Study on tumor cells' multidrug resistance and its reversion by Chinese herbs

According to the literature at home and abroad, and combining with the results of authors' investigations, this paper mainly discusses the mechanism of tumor cells' MDR and its reversion by Chinese herbs.
4.Establishing a rat model of diabetic acromelic gangrene and intervention of Chinese compound

BACKGROUND:Diabetic acromelic gangrene(diabetic foot)has become one of the important causes for the disability and death in diabetes mellitus.OBJECTIVE:To establish model of diabetic foot in rat,and observe the interventional effect of Chinese compound on diabetic foot.DESIGN:A comparative observational experiment.SETTING:The First Affiliated Hospital of Clinical Medical College,Henan University of Science and Technology.MATERIALS:Fifty male Wistar rats of 8 weeks old,(200.0±16.3)g,were raised with common feed in separate cage at the room temperature of 18-22℃.and they were free to access of feed and water.Ten rats were randomly selected as the blank control group,and the other 40 were used for model establishment.METHODS:The experiments were carried out in the Animal Room of Henan University of Science and Technology from October 2001 to April 2002.①Grouping:The 40 rats were fasted for 6 hours,and then treated with intraperitoneal injection of streptozotocin(55.0 mg/kg),while the 10 rats in the blank control group were injected with isovolume sodium citrate buffer solution.20 models were successfully established,and they were randomly divided into model group(n=10)and treatment group(n=10).Rats in the treatment group were treated for 3 weeks with intragastric perfusion of Chinese compound(60 g/kg)at 9:00 every day after model establishment,and those in the model group were given intragastric perfusion of isovolume saline.At the end of the third week,the rats were all killed under anesthesia after fasted for 12 hours,blood samples were collected to determine the levels of fasting blood glucose,blood lipids and insulin.The daily amount of drinking water was recorded in each group during the experiment.②Scoring standards for acromelic gangrenes:The limbs rats with diabetic foot were scored by three grades,including dark skin,mild open focus on skin,and focus had invaded deep muscular tissue.The total score of each rat was calculated.The beta-cell function index (HBCI)was also evaluated.MAIN OUTCOME MEASURES:The changes of the amount of drinking water,body mass and levels of triglyceride,cholesterol,fasting blood glucose,blood lipids and insulin were observed before and after treatment.RESULTS:Totally 50 Wistar rats were used.20 of them were excluded due to unsuccessful model establishment,and the other 30 rats were involved in the final analysis of results.①The amount of drinking water was obviously higher in the model group and treatment group than in the blank control group during the experiment(P<0.01).As the treatmentlasted,the amount of drinking water was obviously decreased in the treatment group,but gradually increased in the model group.②After treatment,the body mass was obviously lower than that before treatment in the model group(P<0.01).but had a descending trend without obvious difference as compared with that before treatment in the treatment group(P>0.05).③Obvious acromelic gangrenes were obvious in both groups(P<0.01).The body mass in the treatment group was obviously higher than that in the model group(P<0.01),and the conditions of acromelic gangrene were obviously better than those in the model group(P<0.01).④Before treatment,the levels of fasting blood glucose in the treatment group and model group were obviously higher than that in the blank control group(P<0.01),while the levels of insulin and HBCl were obviously lower than those in the blank control group(P<0.01).After treatment,the levels of fasting blood glucose in the treatment group and model group were obviously higher than that in the blank control group(P<0.01),and it was obviously lower in the treatment group than in the model group(P<0.01),it was close to the normal value in the treatment group.⑤The levels of triglyceride and cholesterol before treatment were obviously higher in the treatment group and model group than in the blank control group(P<0.01).After treatment,the levels of triglyceride and cholesterol in the treatment group were obviously decreased(P<0.01), which were not obviously different from those in the blank control group (P>0.05), while those in the model group were increased continuously,and obviously higher than those in the blank control group(P<0.01).CONCLUSION:Increasing the serum level of insulin and decreasing the levels of blood glucose and blood lipids can prevent and treat the occurrence and development of diabetic foot to some extent.This model of diabetic foot is sensitive to drug,and can be used to investigate the pathogenesis of diabetic foot and evaluate the effect of drug therapy.
5.Inhibiting ERK1/2 pathway reduces brain edema and down-regulates matrix metalloproteinase-9 expression after subarachnoid hemorrhage in rats

Objective To study the effect of extracellular signal-regulated kinase1/2 (ERK1/2)inhibitor U0126 on matrix metalloproteinase-9(MMP-9)in brain tissue after subarachnoid hemorrhage(SAH)in rats and to investigate the action mechanisms of ERK1/2 and M M P-9 in blood-brain barrier(BBB)injury and brain edema after SAH.Methods Seventy-two male Sprague-Dawley rats were randomly divided into four groups:SAH model,sham operation,U0126 intervention,and vehicle groups.A SAH model was induced by injection of autologous blood into cisterna magna once.The dry-wet weight method was used to detected brain tissue water content in order to evaluate cerebral edema.BBB permeability was evaluated by the Evans blue extravasation method.The immunohistochemical method was used to detect the expression of MMP-9 and phosphorylated ERK1/2.Results The expression of phosphorylated ERK1/2 and MMP-9 was lower in the sham operation group.The expression of both was up regulated at 24 hours after SAH.The brain water content and Evans blue content also increased.U0126 treatment decreased the phosphorylation of ERK1/2 and the expression of MMP-9,improved the BBB permeability,and alleviated brain edema.Conclusions MMP-9 is involved in the pathophysiological processes of early BBB injury and brain edema aft er SAH.ERK1/2 pathway may play a vital role in the expression of MMP-9.U0126 may protect BBB and reduce brain edema after SAH by inhibiting the phosphorylation of ERK1/2.

7.Preliminary clinical report of computer assisted imaging guided TKA

Objective To analyze the differences of clinical results between CAS and routine NCAS localization in TKA and to evaluate the safety and reliability of CAS in TKA in order to take advantages and avoid disadvantages of this new technology.Methods From October of 2004 to October of 2005,18 cases(20 joints)of TKA in Group Ⅰ which were perform utilizing computer assisted imaging guidance system(CAS)and there were 7 males and 11 females with an average age of 67 years.From October of 2003 to October of 2004,17 cases(22 joints)of TKA in Group Ⅱ which were performed with conventional method previously(NCAS)and there were 5 males and 12 females with an average age of 65 years(56-78).The differences between two groups in clinical results,mechanical axes,soft-tissues balance,coincidence,blood loss,and operating time were statistically analyzed.Results All cases were followed for an average of 10 months(3-12).The mechanical axes error was 2-3 degrees in CAS group and 3-6 degrees in NCAS group.Soft-tissues balance angle was within 2 degree in CAS group and 2-4 degrees in NCAS group.The distance variable was 2-4 mm in CAS group and 5-7 mm in NCAS group.The blood loss was 550-700 ml in CAS group and 700-900 ml in NCAS group.The surgery time was 75-100 min in CAS group and 45-60 min in NCAS group.There were statistically significant differences between two groups.There were no major complications such as fat embolism occurred in both groups.Conclusion Utilizing computer assisted imaging guidance in TKA can obtain reliable and sound clinical results with better mechanical axes and soft-tissues balance.It is a safe and reliable procedure in TKA.
8.The anti-tumor immunoresponses by unreplicable recombinant vaccinia virus expressing HPV16 L1,L2,and E7 proteins

Objective:To evaluate the anti-tumor immune responses of nureplicable recombinant vaccinia virus-based vaccine expressing HPV16 L1 and L2E7 proteins.Methods:The anti-tumor effects of the vaccine were observed using mouse models of prophylactic,therapeutic and tumor excision.The cell-mediated immunity induced by the vaccine was assessed by enzyme-linked immunospot (ELISPOT) and cytotoxic T lymphocyte (CTL) assay.Results:The vaccine could protect 60% and 50% mice from the challenge of HPV16-positive tumor cells in prophylactic and therapeutic mouse model,respectively.It could protect from recurrence of the tumor in 70% mice of the excision model.The differences among the groups were significant.Strongly specific cell-mediated immunity was found using ELISPOT and CTL assay.Conclusion:The ideal anti-tumor immune responses can be induced by NTVJL1/L2E7 vaccine in mice and it can be considered to be one candidate vaccine of cervical cancer.
9.Evaluation of the effect of preemptive analgesia with continuous epidual anesthesia in total knee arthroplasty

[Objective]To observe the effect of preemptive analgesia with continuous epidual anesthesia in total knee arthroplasty.[Methods]Fifty-four patients who were treated in author's hospital were randomly assigned into two groups with 27 patients in each.One group was treated with 0.125% ropivacaine 2 ml/h through epidual catheter since 12 hours before the operation.The other group was given saline at the same rate.All the patients received general anaesthesia in the operation.The use of patient controlled analgesia(PCA) with epidual anesthesia was initiated 48 hours after operation.PCA was used as a supplement when the patients comldn't bear the pain.The following variations were compared between these two groups:visual analog score(VAS),the incidences of complication,deep vein thrombosis(DVT),the time of the ability to actively reach 90 degree knee flexion,the range of motion(ROM) and the chronic pain after operation.[Results]The following variations were statistically significantly lower in ropivacaine group than that in the saline group:VAS,DVT,the time to reach 90 degree knee flextion,while ROM 6 months and 1 year after operation were significant higher.There were no statistically significant differences between groups in the incidence of the side effects during the two days after the operation and the chronic pain for a longer time.[Conclusion]Preemptive analgesia with continuous epidual anesthesia can alleviate the early stage of the pain after total knee arthroplasty,decrease the incidence of the complication and increase the rang of motion without obvious side effects.
10.Clinical features of severe acute respiratory syndrome patients complicated with underlying diseases.
