1.Endemic situation of schistosomiasis in Jiangshan City from 2008 to 2018

Objective To analyze the endemic situation of schistosomiasis in Jiangshan City from 2008 to 2018, so as to provide evidence for formulating the future control strategy. Methods The data regarding Oncomelania snail survey and control, schistosomiasis examinations and environmental improvements were collected in Jiangshan City from 2008 to 2018. The changes in snail status and morbidity due to Schistosoma japonicum infections were analyzed to assess the effectiveness of comprehensive schistosomiasis control. Results From 2008 to 2018, a total of 61 220 person-times were examined for S. japonicum infections in Jiangshan City, and the overall seroprevalence of S. japonicum infections was 0.56% (343/61 220); however, no egg-positives were detected. Among 4 231 cattle screened for S. japonicum infections using blood tests in Jiangshan City during the period from 2008 to 2018, 12 were sero-positive, with sero-prevalence of 0.28% (12/4 231), and no egg-positives were identified. There were 422 snail habitats identified in Jiangshan City during the study period, covering an area of 46.915 hm2, and among the 31 686 snails dissected, no S. japonicum infections were detected. An area of 3 625.492 hm2 snail habitats were subjected to repeated snail control and 11 settings were given environmental improvements, covering snail habitats of 17.880 hm2 and historical snail habitats of 204.380 hm2. Conclusions Following the implementation of environmental improvements-based comprehensive schistosomiasis control, the endemic situation of schistosomiasis is stable in Jiangshan City; however, there is still a risk of snail importation and re-emerging schistosomiasis. Therefore, the monitoring of snails and morbidity due to S. japonicum infections should be intensified in the city.
2.Application of Multi-slice Spiral CT Angiography in Thoracic Vascular Emergency

Objective To evaluate the value of multi-slice spiral CT (MSCT) angiography in diagnosis of thoracic vascularemergency.Methods 25 cases suspected of thoracic vascular emergency were examined with MSCT. Two dimensional and three dimensionalimaging methods were used. Results Central pulmonary embolism 10 cases, aortic dissection 12 cases,aneurysm 2 cases and takayasuarteritis with stricture of kidney artery were found by two dimensional imaging(MPVR,CPR),and the extent, size and details of lesions couldbe showed well. The space relationship and calcifation of aorta were displayed in three dimensional imaging(SSD,VR,MIP). Therelationship between intraluminal embolism and the wall of pulmonary artery, as well as intimal flap and reentry of dissection could bevealuated by CTVE.Conclusion Associated with imaging post processing,MSCT angiography is an effective method in diagnosis of thoracic vascular emergency.
3.Effect of bundle hand hygiene intervention in controlling healthcare-asso-ciated infection in a primary comprehensive hospital

Objective To explore the effect of bundle hand hygiene intervention in controlling healthcare-associated infec-tion(HAI)in a primary comprehensive hospital,so as to improve hand hygiene compliance and correct rate,and reduce AHI rate. Methods In January-June 2014,bundle hand hygiene intervention among health care workers (HCWs)in a hospital was performed,hand hygiene knowledge awareness rate,hand hygiene compliance and correct rate,hand sanitizer usage and HAI rate before and after intervention were compared. Results After performing intervention for six months,the awareness rate of hand hygiene knowledge(concept,significance,indications,methods,sanitizer use)of HCWs improved com-pared with before intervention (P<0.05);hand hygiene compliance and correct rate were significantly higher than before inter-vention (77.92% vs 49.78% ;76.47% vs 37.72% )(P<0.05). Hand sanitizer usage increased from 2.14mL/bed-day to 4.63 mL/bed-day ,HAI rate decreased from 1.97% to 1.54% (P<0.05).Conclusion Bundle hand hygiene intervention can improve HCWs’knowledge awareness,compliance and execution rate of hand hygiene,and effectively reduce HAI rate.
4.Sleep disorders in patients w ith ischemic stroke:pathophysiological mechanisms and treatment

Sleep disorder is a common complication after is chemicstroke.If it is nottimely diagnosed and treated, it may affect the rehabilitation of stroke and recurrence. The manifestations of sleep disorders have various forms according to the different locations of infarction. For the treatment of sleep disorder after stroke, the method of the combination of drugs and physical therapy is used in clinical practice at present. In recent years, w ith the w idely use of the polysomnography, poststroke sleep disorders can be diagnosed clearly and carefuly. The treatment for sleep disorders after stroke may significantly improve the prognosis of patients.
5.Clopidogrel resistance and its countermeasures in the prevention of ischemic stroke

Clopidogrel can inhibit platelet aggregation and reduce the risk of recurrent stroke.However,it is a great difference in antiplatelet inhibition in different populations.This article reviews the mechanism of clopidogel resistance and its countermeasures in the prevention of ischemic stroke.
6.Antiplatelet therapies in ischemic stroke patients with diabetes mellitus or insulin resistance

Diabetes mellitus is an independent predictor of high platelet reactivity after antiplatelet therapy in patients with ischemic stroke,and the latter is closely related to the increased risk of recurrence of stroke.The mechanisms of high platelet reactivity in patients with diabetes or insulin resistance are associated with a variety of factors.Some circulating molecules can be used as markers for predicting the reactivity of platelets.Monitoring of platelet reactivity after treatment with new antiplatelet agents may provide basis for individualized antithrombotic therapy in ischemic stroke patients with diabetes or insulin resistance.
7.Surgical treatment for degenerative lumbar scoliosis associated with spinal stenosis.

OBJECTIVETo study surgical techniques for degenerative lumbar scoliosis associated with lumbar stenosis and evaluate their clinical significane.
METHODSThirty-two patients with degenerative lumbar scoliosis associated with spinal stenosis were treated by techniques of posterior lumbar interbody fusion or posterolateral fusion and pedicle screws. There were 18 male and 14 female with 56.8 years old on the average (ranging from 49 to 75 years). There were no evident change of lumberlordosis in 15 cases, and lumber lordosis were obvious loss associated with lumbar subluxation in 17 cases. The correcting, the improvement of back and leg pain, complications and followed-up results were analyzed retrospectively.
RESULTSThirty-two cases were followed-up for 6 to 39 months (the average time of 13 months). The average correction rate of scoliosis was 58.0% and the rate of pain relief was (80.2 +/- 5.8)%. There were two cases of dura sac laceration, two cases of nerve roots injury and a case of pseudoarthritis. During followed-up, correction rate and height of disc spaces were not lost. Shift of interbody cages were no displaced; all the internal fixation got well fusion and the rate of fusion for the bone graft was 96.9%.
CONCLUSIONPosterior pedicle screws combined with interbody fusion or posterolateral fusion is a safe and effective surgical treatment for degenerative lumbar scoliosis associated with lumbar stenosis.

8.Study on the relationship between relieving energy crisis in myofascial trigger points with An-Pressing manipulation and AMPK/PGC-1α pathway activation

Objective: To explore the mechanism of An-Pressing manipulation in relieving energy crisis in chronic myofascial trigger points (MTrPs) by observing the effects of An-Pressing manipulation on adenosine triphosphate (ATP), adenosine 5'-monophosphate (AMP)-activated protein kinase (AMPK)/peroxisome proliferator-activated receptor γ coactivator 1α (PGC-1α) pathway and mitochondrial ultrastructure of skeletal muscle cells in MTrPs rats. Methods: Forty-eight male Sprague-Dawley rats were randomly divided into a blank group, a model group, a lidocaine group, and an An-Pressing manipulation group, with 12 rats in each group. The model group, lidocaine group and An-Pressing manipulation group were used to replicate the MTrPs rat model by blunt shock and centrifugal motion method. After modeling, the An-Pressing manipulation group was subjected to 7 times An-Pressing manipulation, once every other day; the lidocaine group was treated with 3 times of injection of lidocaine at the MTrPs, once every 6 d. The blank group and the model group were fed normally without intervention. After the intervention, local muscle tissue was taken to detect the content of ATP and the expression of AMPK, phosphorylated AMPK (phospho-AMPK), PGC-1α, and glucose transporter 4 (GluT4), and the ultrastructure of mitochondria was observed under an electron microscope. Results: Compared with the blank group, the ATP content in the model group was decreased (P<0.05), the protein expression levels of phospho-AMPK, PGC-1α, and GluT4 and the ratio of phospho-AMPK to AMPK were decreased (P<0.05); under the electron microscope, the number of mitochondria decreased, and they were deformed, small in volume, and had deformed cristae. Compared with the model group, the ATP contents in the An-Pressing manipulation group and the lidocaine group were increased (P<0.05), and the protein expression levels of phospho-AMPK, PGC-1α, and GluT4 and the ratio of phospho-AMPK to AMPK were increased (P<0.05); under the electron microscope, the number of mitochondria increased, the shape and size of the mitochondria were basically normal, and the cristae could be seen. Compared with the lidocaine group, phospho-AMPK and the ratio of phospho-AMPK to AMPK in the An-Pressing manipulation group were increased (P<0.05); under the electron microscope, the numbers of mitochondria were similar, and the shape and size of the mitochondria were basically normal without swelling, and the cristae could be observed. Conclusion: An-Pressing manipulation can increase the ATP content in MTrPs tissue, improve the expression levels of PGC-1α and GluT4 proteins and the ratio of phospho-AMPK to AMPK; its mechanism may relate to the activation of AMPK/PGC-1α signaling pathway to promote the repair of mitochondrial damages.
9.Clinical observation of Liu’s infantile tuina therapy for allergic rhinitis

Objective:To discuss the clinical efficacy of Liu’s infantile tuina therapy in treating kid’s allergic rhinitis (AR).
Methods:Sixty eligible AR kids were randomized into a tuina group and a Western medication group by their visiting sequence, 30 cases in each group. The tuina group was intervened by Liu’s infantile tuina therapy, once daily, 5 times as a treatment course, with a 2-day interval after a course; the control group was by orally taking Loratadine. The therapeutic efficacies were compared and analyzed after treatment for 4 successive weeks.
Results:After treatment, the symptoms such as itchy nose, sneezing, runny nose, and stuffy nose were significantly improved in both groups (P<0.05), and the improvements in the tuina group were more remarkable than those in the Western medication group (P<0.05). The total effective rate was 90.0% in the tuina group versus 73.3% in the Western medication group, and the difference was statistically significant (P<0.05).
Conclusion:Liu’s infantile tuina therapy can produce a better therapeutic efficacy in treating AR kids compared to oral administration of Loratadine.
10.A study of changes in volume and location of target areas and organs at risk in intensity-modulated radiotherapy for cervical cancer

Objective To investigate the changes in volume and spatial location of target areas and normal tissues before and during intensity?modulated radiotherapy (IMRT) for cervical cancer by quantitative means. Methods Forty patients with cervical cancer who were treated with IMRT were enrolled as subjects. Computed tomography ( CT) was performed before IMRT and during IMRT when a dose of 27 Gy ( 15 fractions) was reached. Clinicians delineated the target areas and organs at risk in the two groups of CT images. The target areas and organs at risk in one group of images were mapped to the other group of CT image by image registration using the Pinnacle treatment planning system. Volume changes in target areas and organs at risk were analyzed, and changes in the spatial location were evaluated by volume difference method and Dice similarity method. Comparison was made by paired t?test. Results There were significant differences in gross target volumes of primary tumor lesions ( GTV?T) and pelvic metastatic lymph nodes (GTV?N) before and during IMRT ( P= 0?? 000; P= 0?? 000). According to the evaluation by volume difference method, the average rate of change in GTV?T was (38.64±19?? 50)% with a range between 3?? 16%and 86?? 49%, while the average rate of change in GTV?N was (42.49± 25?? 68)% with a range between 2?? 79% and 87?? 42%. In the organs at risk, the bladder had the maximum rate of volume change, the average of which was (55.13±33?? 40)% with a range between 3?? 25% and 116?? 01%. According to the evaluation by Dice similarity method, the average Dice similarity coefficient for GTV?T was 0.50± 0?? 18 with a range between 0?? 10 and 0?? 85, while the average Dice similarity coefficient for GTV?N was 0.31±0?? 20 with a range between 0?? 00 and 0?? 71. The rectum had the minimum Dice coefficient in the organs at risk, the average of which was 0.57± 0?? 14 with a range between 0?? 18 and 0?? 76 . Conclusions For patients with cervical cancer to receive IMRT, since there are substantial changes in volume and spatial location of target areas and normal tissues before and during treatment, it is quite necessary to modify the treatment regimen in time in order to provide adequate doses for target areas and avoid overdose for organs at risk.
