1.Analysis of Drug Resistance of 519 Strains of Fungus in Our Hospital from 2005 to 2006

OBJECTIVE:To study the feature of fungus infection and the drug susceptibility for the fungus in our hospital.METHODS:Fungi were cultured and isolated by routine procedure and identified by VITEK microbe automatic system.Drug susceptibility test was performed using Rosco paper disk diffusion and broth dilution method with NCCLS M27-A.RESULTS:519 strains of fungi were all Blastocystis,with Monilia albicans making up 70.3%.Mycostatin showed the highest susceptibility,followed by amphotericin B.Fluconazol and itraconazole showed poor susceptibility.CONCLUSION:The resistant strains to common antifungal drugs experienced an obvious increase in clinical practice;therefore,it is imperative to monitor the drug susceptibility of fungi.
2.Contrast-enhanced CT vs. X-ray examination in pest-operative breast cancer

Imaging examination results of 50 breast cancer patients undergoing surgical treatment between January 2006 and December 2007 were retrospectively analyzed. Our data showed that 32 patients (64%) had positive results in contrast-enhanced CT, and metastases to lung, mediastinal lymph node, axillary lymph node, or chest wall were found in 22 (44.0%), 11 (22.0%), 10 (20.0%), and 3 patients (6.0% ), respectively. Only 10 patients (20%) had abnormal image manifestation in X-ray, 8 of which showed metastases to lung. In summary, contrast-enhanced CT examinations may be more useful in the detection of metastases in post-operative follow up of breast cancer.
3.X-ray examination vs CT scan in identifying simple lumbar vertebral wedge following trauma in the elderly

This retrospective study was to evaluate the sensitivity and specificity of X-ray and CT in diagnose of simple lumbar vertebral wedge.Thirty—two elderly patients with CT-confirmed simple lumbar vertebral wedge were analyzed retrospectively.Following X-ray examination,23 patients were found to have compression fracture,and 9 patients were diagnosed with simple lumbar vertebral wedge.Thus,CT scan may be more sensitive and specific than X-ray examination in the diagnosis of simple lumbar vertebral wedge after trauma in the elderly.
4.Determination of VOCs in Air with Capillary Gas Chromatography

Objective To develop a method of determining benzene hydrocarbon and halogenated alkane hydrocarbon in the air of workplaces with the capillary gas chromatography of carbon disulphide desorption. Methods Benzene hydrocarbon and halogenated alkane hydrocarbon in the air of workplace were collected by active carbon sampling cuvette, then separated by hydrogen flames detector gas chromatography machine after carbon disulphide desorption. Benzene hydrocarbon and halogenated alkane hydrocarbon were determined quantitively by retention time and quantitatively by apex area. Results The linear ranges of benzene, toluene, p-xylene, m-xylene, o-xylene, ethyl benzene, styenl, chlorobenzene, acetone, carbontetrachloride, dichloromethanl, trichloromethane, 1, 2-dichloroethane, naphth alene were 0.019-81.600, 0.018-91.200, 0.018-88.800, 0.018-56.8, 0.011-92.000, 0.012-63.200, 0.018-93.200, 0.449-2298.400, 0.252-1287.000 and 0.076-390.000 mg/m3 respectively. The recovery rates were 88.4%-98.6% and RSD were 1.0%-6.0%. Conclusion This method can separate efficiently and determine accurately benzene hydrocarbon and halogenated alkane hydrocarbon in the air with a good precision. It is suitable for the determination of the toxicants in the air.
5.Determination of Kelthane and Pyrethroid Pesticide Residues in Water by Capillary Column Gas Chromatography

Objective To establish a method for simultaneous determination of kelthane and pyrethroid pesticide residues in water.Methods Kelthane and pyrethroid pesticide residues in the water were extracted by liquid-liquid,then analyzed with DB-1 capillary column.In the same time,they were determined with GC by controlling the temperature.Retention time of the peaks was used for qualitative analysis,while external standard method was used for quantitative analysis.Results The linear ranges of Kelthane,Bifenthrin,Fenpropanate,Lambda cyhalothrin,Permethrin,Beta cyfluthrin,Alphacypermethrin,Fenvalerate were 0.24-31.1,0.21-27.2,0.20-26.0,0.21-26.5,0.20-25.7,0.21-26.4,0.22-29.0 and 0.22-27.4 ?g/L,r≥0.999 0.The lowest determination limits were 0.20,0.33,0.30,0.13,0.36,0.26,0.33 and 0.30 ?g/L,the rate of recovery was 86.0%-111.7% and RSD was 3.8%-11.8%.Conclusion This method can separate efficiently and determine 8 kinds of pyrethroid pesticide residues in the water and only needs 23 minutes.It's simple,rapid and sensitive.
6.Bacteriology Study and Choice of Antibiotics for Open Wounds in Patients with Earthquake Injury in Our Hospital

OBJECTIVE:To study the bacteria strains and susceptible drugs in open wound in patients with earthquake injury. METHODS:Bacteriological test and the drug susceptibility test were conducted for earthquake injury patients with open wound and the results of bacterial culture and the drug susceptibility test were analyzed using Excel 2003. RESULTS:Of the 1 172 earthquake injury cases,the patients with trauma / wounds represented 93.9%,the wound had obvious infection in 132 cases; of the infected cases,the proportions of patients with open fractures,soft tissue injuries and traumatic brain injury accounted for 63%,28% and 9%,respectively. The positive rate of the bacterial culture was 69%,and gram-negative of bacteria which accounted for 85.6% took the lead,chiefly Acinetobacter baumanii,Enterobacter cloacae and Escherichia coli. The results showed bacterial drug resistance was on the high side. CONCLUSION:The open wounds in patients with earthquake injury are likely to be infected with gram negative organisms. Debridement and correct choice of anti-infective drugs are needed to reduce the risk of wound infection,
7.Dosimetric evaluation of RapidPlan model on different Radiotherapy devices

Objective To evaluate the dosimetric differences of one RapidPlan Model on different Radiotherapy devices.Methods A RapidPlan Model was built based on 30 reoptimization IMRT plans of cervical cancer patients on typeA LA.Dosimetric differences of automatic optimized IMRT plans using this model on 4 different type LAs,named respectivelyA,B,C andD,were compared with 12 test cervical cancer cases.These four LAs were well commissioned in the treatment planning system (TPS).Student t test was applied for statistical analysis on dosimetric differences.Results Dosimetric differences between A vs.B,C and D were observed on Dmean,HI,CI of PTV50 and PTV45,as well as on V50,V40,V30 of rectum and bladder.Significant dosimetric differences were observed between A and D (P<0.05).Conclusions Automatic planning with RapidPlan model may result in dosimetric differences on different Radiotherapy devices.These differences should be aware of with caution in its clinical application.
8.Effectiveness of ultrasound-guided botulinum toxin injection combined with orthosis training in the treatment of lower limb spasticity

Objective To observe the effect of ultrasound-guided injection of botulinum toxin type A (BTX-A) combined with orthosis training for the treatment of lower limb spasticity for children with cerebral palsy.Methods Fifty-four patients with spastic cerebral palsy were randomly divided into an observation group and a control group using a random number table.Patients in the control group were given conventional rehabilitation training,while those in the observation group were additionally given ultrasound-guided BTX-A injection,followed by daily knee-ankle-foot orthosis rehabilitation training 24 h after the injection.Before and 6 weeks after treatment the lower limb function of the 2 groups was evaluated using the modified Ashworth scale (MAS) and the gross motor function measure (GMFM).Adductor angles,popliteal angles and motor ranges of the ankle joint were also measured and compared.Results After 6 weeks the average MAS scores were lower than before treatment in both observation[(1.26±0.63) vs (3.07±0.68)] and control group [(2.56±0.71) vs (2.89±0.64).And in both groups the average GMFM scores,adductor angles and popliteal angles,as well as range of motion of the ankle were all much better than before treatment.The observation group,however,improved significantly more than the control group(P < 0.05).Conclusion Ultrasound-guided injection of BTX-A can effectively alleviate spasticity of lower extremity for children with cerebral palsy with accurate positioning,obvious curative effect and less adverse reactions.Knee-ankle-foot orthosis rehabilitation training after the injection can further alleviate spasticity and improve the motor function of the limbs.
9.The effect of different localizing techniques in guiding botulinum toxin injection in the treatment of lower limb spasticity in cerebral palsy children

Objective To observe the effect of free muscle localization and ultrasound-guided injection of botulinum toxin type A (BTX-A) combined with rehabilitation training on lower limb spasm of cerebral palsy children.Methods Sixty-one cerebral palsy children with lower limb spasticity were randomly divided into a manual localizing group (31 cases) and an ultrasound-guided group (30 cases).The children of both groups underwent BTX-A injection at adductors,hamstrings and calf triceps.The former group was injected with bare hands,while the latter was injected under the guidance of ultrasound.Both groups were administered with rehabilitation training from the 3rd day onwards after injection.Before and 12 weeks after the injection,the modified Ashworth scale (MAS) and gross motor function scale (GMFM) were used to evaluate the lower limb spasticity and function in 2 groups.Results Before the injection,there was no significant difference between the 2 groups in terms of MAS and GMFM scores (P > 0.05).However,12 weeks after the injection,the average MAS scores of the two groups decreased,and that of the ultrasound-guided injection group (1.43 ±0.50) was significantly lower than that of the manual localizing group (1.77 ± 0.56).After the injection,the average GMFM scores of both groups increased,and that of the ultrasound-guided injection group was significantly higher than the manual localizing group (67.10 ± 11.25).Conclusion Ultrasound-guided injection of BTX-A with intensive rehabilitation training can significantly reduce the lower limb spasticity in children with cerebral palsy,and increase their motion of joint and motor function,and improve their posture and gait.
10.Application of Ion Torrent PGMTM System in Detection of Fetal DNA in Maternal Plasma

Objective To explore the feasibility of detecting of Y-STR of fetal DNA in m aternal plasm a using Ion Torrent PGMTM System . Methods A total of 16 fetal DNA sam ples from m aternal plasm as (8 cases from 38 w eeks gestational age and 8 ones from 12 w eeks) w ere prepared and a m ultiplex assay w ith 7 STR loci (DYS390,DYS391,DYS393,DYS438,DYS437,DYS456,DYS635) w as designed for m ul-tiplex-PC R am plification. U sing Ion Torrent PGMTM System , the results of Y-STR sequences and capillary electrophoresis w ere obtained and com pared. Results Y-STR specific alleles w ere detected in the m ater-nal plasm a of all the pregnant w om en having m ale babies of second and third trim ester, w hich w ere higher than that detected by capillary electrophoresis. C onsistent Y-STR genotypes w ere observed betw een fetal DNA from m aternal plasm a and genom ic DNA from the new born babies. Conclusion B ased on Ion Torrent PGMTM System , the prenatal Y-STR detection m ethod m ay provide a high-sensitive and high-throughput choice for prenatal STR detection in forensic testing.
