Phthaloyl dichloride (1)was reacted with LiAlSeH2 to give benzo[c]selenophene-1;3-dione (2);which was treated with the Grignard reagents to generate hydroxyl compounds 3a-3h.These compounds were finally converted to target products 4a-4h by treatment with hydriodic acid.The structures of 4a-4h were confirmed by MS and 1 H NMR.Their inhibitory activity against adenosine diphosphate (ADP)-induced platelet aggregation was evaluated by Born′s turbidimetric assay;free radical scavenging activity was assayed by xanthine oxidasemethod and 1;10-phenanthroline spectrophotometric method.It was found that compound 4 f displayed more potent inhibi-tory effect on platelet aggregation than 3-n-butylphthalide and comparable hydroxyl free radical scavenging activity in vitro to that of edaravone.Therefore;compound 4 f might be the candidate for further investigation.
3.Clinical study on the relationship between circulating tumor cells and neoadjuvant therapy for breast cancer

Objective:To analyze the correlation between the level of circulating tumor cells(CTC)and the efficacy of neoadjuvant therapy in breast cancer.Methods:A total of 79 patients with breast cancer who underwent neoadjuvant therapy between October 2020 and December 2022 were included in this study.Venous blood samples were collected before and after treatment to measure the number of mononuclear cells and the presence of cytokeratin-19(CK-19)positive circulating tumor cells(CTCs).Results:The pre-treatment positive detection rate of CTC in patients was 26.6%(21/79), and it was found to be correlated with HER-2 expression, TNM staging, and KI-67 percentage.After neoadjuvant therapy, the positive rate of CTC decreased to 8.9%(7/79), which was lower than the rate before neoadjuvant therapy( χ2=16.185, P<0.001).Based on the CTC results measured before and after neoadjuvant therapy, the groups were classified into negative/negative, positive/positive, negative/positive, and positive/negative groups.In accordance with the RECIST standard, the predicted efficacy was 75.0%(36/48)in the CTC negative/negative group and 83.3%(20/24)in the positive/negative group.The treatment effectiveness rate was notably higher compared to the CTC positive/positive group, which had a rate of 25.0%(1/4), and the negative/positive group, which had a rate of 0.0%(0/3)( χ2=13.886, P=0.003).According to Miller Payne's standard for predicting efficacy, the treatment effectiveness rate in the CTC negative/negative group was 72.9%(35/48), while in the positive/negative group it was 79.2%(19/24).These rates were significantly higher than the rates in the CTC positive/positive group(50.0%, 2/4)and the negative/positive group(0.0%, 0/3).( χ2=9.043, P=0.029). Conclusion:There is a correlation between the level of circulating tumor cells(CTC)and the efficacy of neoadjuvant therapy.CTC can be considered as an evaluation indicator for assessing the efficacy of neoadjuvant therapy.
4.Information system of electronic blood-donation certificate in Zhejiang province

【Objective】 To introduce the construction of the electronic blood-donation certificate information system(EBDCIS) in Zhejiang province, including the construction foundation, system design, system function and preliminary development achieved. 【Methods】 A provincially unified EBDCIS, based on Internet and cloud computing technology relying on the Zhejiang blood cloud platform with Alipay and WeChat applet as service carriers, was developed in 2019, which was oriented towards " the Internet Plus blood donation service" . 【Results】 In addition to the realization of electronic blood-donation certificate, the system mainly improved the online service, including 6 categories as blood donation information query, blood donation navigation, facial recognition, satisfaction evaluation, blood use exemption and blood knowledge area, covering 21 functions. 【Conclusion】 The application of EBDCIS in Zhejiang province has provided better care and service for blood donors, and is conductive to establish one-stop blood donation service platform.
5.Effect of traditional Chinese medicine for replenishing qi, nourishing yin and activating blood on renal Notch/Hes1 signaling in rats with diabetic nephropathy.

OBJECTIVE:
To observe the effects of a traditional Chinese medicine (TCM) capsule for replenishing qi, nourishing yin and activating blood on Notch/Hes1 signaling pathway in the renal tissue and vascular endothelial CD34 and CD144 expressions in a rat model of diabetic nephropathy.
METHODS:
Rat models of early-stage diabetic nephropathy were established by left nephrectomy and high- fat and high- sugar feeding combined with intraperitoneal injection of STZ. The rats were randomized into model group, benazepril group, and high-, moderate-, and low-dose TCM capsule groups for corresponding treatments, with 6 normal rats as the control group. After 8 weeks of drug treatment, blood glucose and 24-h urinary albumin of the rats were measured, and the renal histopathology was observed with HE staining; Hes1 expression in the renal tissue was detected with immunohistochemical staining, and the renal expressions of CD34 and CD144 were detected using Western blotting.
RESULTS:
Compared with the normal control group, the rat models of diabetic nephropathy showed obvious abnormalities in 24- h urinary albumin and expressions of Hes1, CD34 and CD144d. The TCM capsule at both the high and moderate doses significantly reduced 24-h urinary albumin in the rats; the renal expressions of Hes1 and CD34 was significantly reduced in all the dose groups, and the expression of CD144 was significantly reduced in the high- dose group. Compared with benazepril group, the TCM capsule obviously reduced CD34 expression at all the 3 doses and lowered CD144 expression at the low dose. Histopathologically, the rats in the model group showed glomerular hypertrophy, increased mesenteric matrix, thickening and widening of the mesenteric membrane, and nodular hyperplasia. These pathologies were obviously alleviated by treatment with the TCM capsule at the high and moderate doses.
CONCLUSIONS
The Traditional Chinese medicine (TCM) capsule for replenishing qi, nourishing yin and activating blood can reduce Hes1, CD34 and CD144 in kidney tissue of model rats, play a protective role on kidney function and delay the development of DN.

AIM: To establish individualized drug therapy strategy for patients with rejection and hyperglycemia after liver transplantation. METHODS: Clinical pharmacist collaborated with the surgeons and participated in the diagnosis and treatment of rejection and hyperglycemia after liver transplantation. Taking together liver function, therapeutic drug monitoring, drug-drug interactions between tacrolimus and wuzhi capsule, individualized drug therapy was adapted to improve the prognosis. RESULTS: The patient recovered well and survived in good health till now. CONCLUSION: It is highly suggested that clinical pharmacists actively involved in treatment of more severe and difficult-to-treat disease and design the individualized dosing regimens. This will largely contribute in reduced adverse drug reaction, improved safety and effectiveness in drug use as well as the quality of life in the "post-transplantation era".
