1.Effect of Multiple Interventions on Therapy Efficacy to the Patients with Epilepsy

Objective To observe the effect of multiple interventions on secondary epilepsy patients.Methods 114 secondary epilepsy patients were randomly divided into the intervention group and control group with 57 cases in each group.The cases in the intervention group received multiple interventions including heath education,psychological treatment,safety nursing,overdose administration,early rehabilitation,discharge guide and so on besides usual nursing.While,the cases in the control group received usual nursing.The follow-up was performed for 6 months and the therapeutic efficacy of two groups was compared.Results After multiple interventions,the seizure frequency and trauma incidence rate decreased,compliance and quality of life increased in patients of the intervention group,and there was a significant difference compared with the control group(P<0.05).Conclusion Multiple interventions can decrease seizure frequency and increase compliance of treatment and quality of life of secondary epilepsy patients.

4.Lower deleterious diabetogenic effect of low concentration tacrolimus on Chinese Han people

Objective To elucidate the deleterious diabetogenic effect of tacrolimus on Chinese Han people.Methods According to patients' whole blood trough levels,the concentration of tacroli- mus was 5,15 and 25?g/L.Human islet cells cultured in vitro were exposed to various concentra- tions of tacrolimus for 72 h respectively,then stimulated by glucose at low (2.8 mmol/L) and high (16.7 mmol/L) levels.Glucose-stimulating insulin secretion during subsequent static incubation was measured using the ultrasensitive human insulin ELISA kit.The assessment of islet cell viability was studied with acridine orange (AO)-propidium iodide (PI).According to fasting plasma glucose,461 subjects were divided into normal glucosetolerance group (FPG<5.55 mmol/L),impaired fasting glu- cose group (5.55 mmol/L<FPG<6.88 mmol/L) and posttransplant diabetes mellitus group (FPG≥6.89 mmol/L).A retrospective review of 461 non-diabetic kidney recipients completing at least 12 months of follow-up was performed to determine risk factors,incidence,and characteristics of post- transplant diabetes mellitus (PTDM).Results In vitro,lower tacrolimus concentration had not dele- terious effect on insulin secretion and viability of islet;high tacrolimus impaired?-cell survival as well as insulin secretion.Of the 461 patients with no history of diabetes at transplantation over the study period of 12 months,123 received tacrolimus and 338 received cyclosporine A (CsA).For patients receiving tacrolimus the cumulative incidence of PTDM was 13.8%,compared with 7.7% for pa- tients receiving CsA,showing no significant difference between them.The reduced tacrolimus level within the therapeutic window resulted in an increase in better glucose metabolism.Older age,family history of diabetes,acute rejection episodes and pre-transplant impaired fasting glucose were identified as risk factors for PTDM.Conclusion The diabetogenic effect of TAC is dose-dependent,and in the majority is reversible.Low initial doses,more rapidly tapering and lower maintenance levels of tacroli- mus decrease the incidence of PTDM in patients receiving tacrolimus.
5.Prokaryotic expression of glutathione transferase gene from Taenia solium and its immunological investigations

In the present study, the prokaryotic expression of glutathione transferase (GST) gene from Taenia solium and its immunological properties were investigated by means of biological informatics methods. The GST gene from T.solium was screened from the cDNA plasmid library of the adult worms. This gene was cloned into prokaryotic expression plasmid pET28a(+) and then expressed in E.coli BL21(DE3) after double enzyme digestion, PCR identification and IPTG induction. The expressed product was identified by SDS-PAGE and the recombinant protein was purified through purification column of His-Ni~(2+) protein. Meanwhile, the immunoreactivity of the purified protein was analyzed by Western blot assay. In these ways, the recombinants were successfully constructed and the highly purified proteins were obtained. It was demonstrated that these proteins could be recognized by sera of patients infected with T.asitica and T.rhynchus saginatus. From these observations, it is evident that highly efficient expression of GST of Taenia solium with definite immunoreactivity can be demonstrated in the prokaryotic expression system.
6.Quantitative assessment of myocardial regional function of different coronary atherosclerosis severity using velocity vector imaging

Objective To assess isehemic myocardial regional function of different coronary atherosclerosis using velocity vector imaging(VVI).Methods Two-dimensional images of apical four-,two-chamber and long-axis view were obtained in 50 patients with coronary artery disease.All segments of the left ventricle were divided into 5 groups:group 0 with no coronary artery stenosis,group 1 with stenosis<50%,group 2 with stenosis 50%to 74%,group 3 with stenosis 75%to 99%and group 4 with a completely occluded coronary artery.The segmental myocardial systolic peak strain(ε),systolic strain rate(SRs),early diastolic strain rate(SRe),late diastolic strain rate(SRa)and their ratio(SRe/SRa)were analyzed with VVI offline software.Results①Compared with group 0,SRe and SRe/SRa of group 1,SRs,SRe,SRe/SRa of group 2~4,εof group 2 and group 4 decreased significantly(P<0.05).②In the comparison of four ischemic groups,ε,SRe,SRa of group 3 were markedly higher than those of group 2,while ε,SRs,SRa of group 2 were lower than those of group 1(P<0.05);for group 4,ε,SRs,SRe were lower than those of group 1,SRe was lower than that of group 2,and ε,SRs,SRe were lower than those of group 3(P<0.05).ConclusionsIn the process of coronary atherosclerosis,myocardial diastolic and systolic dysfunction occurred early and orderly,and has a bidirectional change:decreased firstly,then increased and decreased at last.
7.Correlation between HCV genotype and anti-HCV antibody level in hepatitis C patients

Objective To investigate the relationship between anti-HCV antibody level and hepatitis C virus genotype in the patients.Methods Total of 603 anti-HCV positive serum samples were collected during 2013 to 2014 by retrospective research method.HCV RNA were detected in anti-HCV positive samples by repeat test and the genotype were detected in HCV RNA positive samples.The distribution of anti-HCV level in different hepatitis C genotype patients was analyzed and the body's response to viral antibodies and viral genotype correlation with anti-HCV concentration interquartile range was explored.Rates among genotype groups were compared using chi-square test.Results Totally 412 of 603 (68.33%) samples were anti-HCV positive by double reagent screening.174(42.3%) samples were detected as HCV RNA positive.The distributions of different anti-HCV level in different genotype patients were 1a(n =8) 1/8,1/8,4/8,2/8;1b(n =112)25.9% (29/112),17.0% (19/112),25.9% (29/112),31.3% (35/112);2a(n =14)3/14,4/14,5/14,2/14;3a(n =11)3/11,6/11,2/11,0/11;3b(n =16)4/16,11/16,1/16,0/16;6a(n =8)2/8,2/8,1/8,3/8 with anti-HCV concentration interquartile range respectively.The anti-HCV concentration distribution was different in patients with different HCV genotypes.The anti-HCV concentration distribution in patients of 1 b,2a and 6a genotypes were evently,while anti-HCV level was relatively high in 1a (13.65) and relatively low in 3b (8.77).There were differences in different genotypes of antibody concentrations (x2 =35.2,P < 0.05).Conclusions There was correlation between anti-HCV level and HCV genotype.Because there were fewer cases in some genotypes,it was necessary to investigate more samples to corfirm the above results.
8.The correlation between copeptin and clinical severity and prognosis in acute cerebral infarction

Objective To explore the ability of copeptin to predict infarction severity and prognosis after 3 months in patients with acute cerebral infarction .Methods One hundred and thirty‐eight consecutive patients were included with an acute cerebral in‐farction and admitted to the hospital within 72 h .Plasma copeptin levels were measured by double antibody enzyme linked immu‐nosorbent assay and neurological impairment were evaluated by the national institutes of health stroke scale score (NIHSS) after hospitalization .The patients were divided into mild ,moderate and severe groups according to NIHSS score .At the same time ,60 healthy adults were selected as control group .Unfavorable outcome was defined as a modified rankin scale score>2 .The correlation of plasma copeptin levels with 3 month unfavorable outcome was analyzed .Results Upon admission ,plasma copeptin level in pa‐tients was statistically significantly higher than that in healthy controls (P<0 .01) ,the highest was found in severe subgroup (P<0 .05) .Multivariate logistic regression analysis revealed that copeptin was independently associated with unfavorable outcome within 3 months (OR=4 .403 ,95% CI:1 .120-8 .328 ,P<0 .05) .Conclusion Copeptin levels contribute to mirror the initial infarction se‐verity and even bear an association with poor outcome of patients with cerebral infarction within 3 months .
9.Optimization of Fermentation Conditions of Lipase Produced by Pseudomonas cepacia PCL-3

The fermentation conditions of alkaline lipase producing by Pseudomonas cepacia PCL-3 were optimized.Based on the analysis of single factorial experiments,dextrin was the most suitable carbon source,peptone and urea were the suitable compound nitrogen sources among the examined materials.Three significant factors(urea,inoculum and initial pH) were selected from the eight factors related to lipase production by Plaekett-Burman method,and were further optimized with response surface analysis.And then,steepest ascent procedures were applied to define the optimal response region of the three factors.The obtained optimal conditions were urea 0.15%,inoculum 3.05% and initial pH 8.38,under which conditions,the enzyme activity was improved from 25.37 U/ml to 48.88 U/ml,enhanced 1.93 folds.Starting from the flask conditions,the highest lipase activity of 47.69U/ml was achieved by batch fermentation in a 10 L fermentor after 52 h of the cultivation.
10.Reconstruction Algorithms and Realization of Freehand 3D Ultrasonic

Objective:Three-dimensional (3D) ultrasound (US) is increasingly being introduced in the clinic,both for diagnostics and image guidance.Although dedicated 3D US probes exist,because of its expensive cost,3D US can also be acquired with the still frequently used two-dimensional (2D) US probes.Methods:Obtaining 3D volumes with 2D US probes is a two-step process.First,a positioning sensor must be attached to the probe for 2D image matching;second,a reconstruction of a 3D volume can be performed into a regular voxel grid.Results:This paper presents a way to realize the 3D US in irled-based Image Guided Radiotherapy using a homemade 2D US.Conclusions:The experiments demonstrate a method of saving costs and having advantages in clinic application.
