1.Pondering on the diagnosis and treatment criteria for syndromes of epilepsy in traditional Chinese medicine

International League Against Epilepsy announced the new International Classification of Epilepsy in 2001. One of the main objectives of the project is to adopt standard terms to describe seizure phenomena according to the terminological database. The project may profit academic communication and resource integration, and provide evidence for the individual treatment for epilepsy. Treatment based on syndrome differentiation is one of the characteristics of the treatment for epilepsy in traditional Chinese medicine (TCM). As far as the individuation on diagnosis and treatment is concerned, TCM and Western medicine present to reach the same goal by different routes gradually for the treatment of epilepsy. But for TCM, the diagnosis and treatment criteria for epilepsy are imperfect and the experts, opinions are not unified, so that the treatment based on syndrome differentiation becomes confused, hence the credit of treatment based on TCM is restricted accordingly. It is necessary to formulate advanced diagnosis and treatment criteria for syndromes of epilepsy in TCM.
2.The effect of topotecan on lung cancer cell cycle and the mechanism of topotecan-induced apoptosis

AIM To study the effect of Topotecan(TPT) on cell cycle and apoptosis of hunman lung cancer SPC A 1 cells and the role of Caspase 3 and bcl 2 involved in TPT induced apoptosis. METHODS Cells were incubated with TPT(5,10,15 and 20 mg?L -1 ) for 24 h. Apoptosis was detected by TUNEL staining, electronic microscope and DNA agarose gel electropheresis. Flow cytometer was used to analyse cell cycle changes and to detect apoptotic rates and bcl 2 expression of cells treated with TPT or TPT and 50 ?mol?L -1 Ac DEVD CHO(specific Caspase 3 inhibitor). RESULTS Apoptosis of cells treated with TPT(5 mg?L -1 ) was not detected, but apoptosis rates increased with the increase of TPT concentration and there was significant difference between each concentration group( P
3.Role of TPSA, F/T and PSAD in diagnosis of prostate cancer

Objective To investigate the diagnostic value of serum TPSA, the ratio of free PSA(FPSA)and TPSA(F/T) and PSAD in prostate cancer(PCa).Methods The serum TPSA and PSAD levels and the ratio of F/T of 467 patients with BPH,116 patients with PCa and 50 healthy men as normal control were determined, analyzed and compared. Results Serum TPSA of PCa (53.26±33.10) were significantly higher than that of BPH (8.12±9.70) (P <0.01), and normal control (1.51 ±1.17) (P <0.01); Serum PSAD of PCa[(1.59±1.46) ng·ml-1·cm-3] were significantly higher than that of BPH [(0.14±0.17) ng·ml-1·cm-3] (P <0.01),and normal control [(0.08±0.07) ng·ml-1·cm-3] (P <0.01); F/T of PCa (0.22±0.16) was significantly lower than that of BPH (0.27±0.15) and normal control (0.36±0.14) (P <0.01). When TPSA was limited specificity at 4 ~10 ng/ml, F/T of PCa (0.18±0.13) were significantly lower than that of BPH (0.27±0.14), (P <0.05), PSAD of Pca [(0.21 ±0.07) ng· ml -1· cm -3] were significantly higher than that of BPH [(0.11 ±0.06) ng· ml-1· cm-3], (P <0.001). When F/T was 0.16, PSAD used 0.15 as the critical value, the sensitivity, specificity of diagnostic value and positive prediction value were 81.6 %, 78.2 %, 96.1% and 53.8 %, 76.9 %, 97.9 %, respectively;And the diagnostic efficiency was the highest. ConclusionF/T and PSAD have valuable role as the diagnose marker of PCa. When PSA level between 4 ng/ml and 10 ng/ml, F/T ratio and PSAD are of significant value to differentiate PCa from BPH patients.
4.MR virtual endoscopy: a study of clinical applications in cholangiopancreatography

Objective To determine the added clinical applications of MR virtual endoscopy (MRVE) plus MRCP in cholangiopancreatography. Methods In 14 month, 44 patients with obstructive jaundice and symptomatic bile trace were assessed by MRVE in biliary tract.The 2D data of heavily T 2 weighted images with FSE(non breath hold, respiratory trigger) and SSFSE(breath hold, multi slice single shot fast spin echo) sequence were transferred to the computer workstation. Navigator software (GEMS) was used to display intraluminal views of the pancreaticobiliary tract with a surface rendering technique. Results Patients suffered from neoplasm ( n =18), bile stone (17), postoperative complications of cholecystectomy( n =3), and other disease of biliary tract( n =6). MRVE could clearly demonstrate the anatomical structures in biliary tract, extended pancreas tract,biliary meatus narrowing and obstruction, bile duct stones, duct branches, and the inner wall of pathological obstruction.The main limitation of virtual endoscopy was inability to evaluate mucosa and lack of histological diagnosis. Sensitivity for detection of bile duct dilatation and site of obstruction was: 2D FSE 91 7%, and SSFSE 97 9%. Conclusion MRVE provided noninvasively endoscope like display of biliary tract.Its display ability was a supplement to image biliary trace. MRVE effect of multi slice SSFSE sequence was better than that of fast spin echo.
5.Laparoscopic modified Swenson pull-through procedure for Hirschsprung's disease in children

Objective To introduce the experience of l ap aroscopic modified Swenson pull-through procedure for the treatment of Hirschspr ung's disease (HD). Methods A modified Swenson procedure was performed in 100 cases of HD, including 21 cases of short-segment phenotype, 72 cases of common phenotype, and 7 cases of long-segment phenotype. The patients w ere aged 8 months ~ 7 years, with a mean age of 1.5 years. A 0? or 30? laparos cope, with an inner diameter of 0.5 cm, was advanced from the umbilicular port, and operating instruments were introduced into the left and right lower quadaran t of the abdomen. The carbon dioxide was insufflated in the abdominal cavity at a pressure of 8~12 mmHg. Biopsies from the colonic seromuscular layer were made for pathological examinations. Laparoscopic dissection of blood vessels of the m esocolon was carried out closely near the colon, until the level 0.5~1 cm above the dentate line. The rectum and sigmoid was mobilized outside through the anus to complete rectal dissection. The proximal colon then was anastomosed to the re ctum by means of full-thickness suture and diagonal heart-shaped closure. Results All the cases were cured, without fatal cases. The operation time was 80~170 min (mean, 110 min), and the intraoperative blood loss was less than 10 ml. There were 5 cases of subcutaneous emphysema and 6 cases of urinary retention. On a follow-up in all the cases for 6~18 months, 5 cases of colitis, 2 cases of soiling, 1 case of anastomotic stricture, and 1 case of delayed anas tomotic leakage were observed. Conclusions Laparoscopic modifi ed Swenson pull-through procedure for Hirschsprung's disease is worthy of being recommended for its minimal invasion and simplicity of performance.

OBJECTIVE:To compare the quality of fungus and bacteria counting culture medium of drug microbial limit test among different manufacturers.METHODS:Culture medium for 7 kinds of nutrient agars and 5 kinds of sodium rose red bengal agars for sale or self-made were enrolled in study.The content of water and nitrogen,pH value were determined.The number of microcolonies which were recovered from 5 kinds of representative bacterial strains was calculated,and the size and number of microcolonies were compared.RESULTS:Physicochemical index of culture medium,the size and number of microcolonies were different among different manufacturers.CONCLUSION:The quality of culture medium is different among different manufacturers,which is associated with physicochemical index.
8.Safety and feasibility of Minocycline Hydrochloride Ointment and metronidazole membrane in the treatment of periodontal abscess

Objective To investigate the safety and feasibility of Minocycline Hydrochloride Ointment and metronidazole membrane in the treatment of periodontal abscess.Methods80 cases of patients with periodontal abscess in our hospital from May 2014 to May 2016 were selected, these patients were randomly divided into Minocycline Hydrochloride Ointment group (n=40) and metronidazole membrane group (n=40) two groups, the periodontal probing depth, gingival bleeding index, plaque index, attachment loss, clinical efficacy, the incidences of adverse reactions and recurrence of the two groups were statistically analyzed.ResultsThe periodontal probing depth, gingival bleeding index, plaque index, attachment loss of the Minocycline Hydrochloride Ointment group were significantly lower (P<0.05), the total treatment efficiency 95% (38/40) was significantly higher than the metronidazole membrane group 77.5% (31/40) (P<0.05), the recurrence rate 2.5% (1/40) was significantly lower than the metronidazole membrane group 15% (6/40) (P<0.05), but the difference of the incidences of adverse reactions 10% (4/40), 7.5% (3/40) between the two groups was not significant.ConclusionMinocycline Hydrochloride Ointment metronidazole membrane in the treatment of periodontal abscess has higher safety and feasibility than metronidazole membrane, so is worthy of promotion in the clinical.
9.Anesthetic effect of compound articaine on children's dental pulp without pain

Objective To analyze the anesthetic effect of Compound Articaine on Children's dental pulp without pain, and provide reference for clinical treatment.Methods124 patients with children with dental pulp disease in hospital from February 2015 to May 2016 were selected, patients were randomly divided into observation group and control group, every group with 62 cases.Control group patients were given Lidocaine anesthesia, observation group patients were taken trentment of compound articaine anesthesia, anesthetic effect of patients were compared.Changes in heart rate, blood pressure, adverse effects, and patient pain were recorded before and after anesthesia.ResultsThe total effective rate (96.8%) in the observation group was significantly higher than that in the control group (80.9%) (P<0.05).Observation group (79%) was significantly higher than the control group (1.6%,P<0.05), the observation group of severe pain rate (58.1%) was significantly lower than the control group (11.3%) (P<0.05),The heart rate and blood pressure in the observation group were no significant change, In the control group, the diastolic blood pressure (71.7±10.8) mmHg was significantly lower in the observation group (74.5±12.8) mmHg and the heart rate (80.2±8.8)/min was significantly higher than that in the observation group (76.2±8.3).Two groups of patients in the treatment of adverse reactions, including tachycardia, dizziness, headache, observation group of adverse reaction rate and the control group was not significantly different.ConclusionCompound articaine anesthesia was the implementation of children's dental painless treatment, can reduce the pain of patients, maintain the blood pressure and heart rate stable, has the use value.
10.Selective approach to the therapy of cystic lesions of the pancreas

Objective:To discuss the approach of treating the patients who suffered from cystic lesions of the pancreas. Methods:According to the retrospective analysis of all the 127 patients who were diagnosed as cystic lesions of the pancreas by the following methods such as ultrasonography ,CT,MRCP,PET,ERCP,EUS and FNA. Results:48 case of cystic lesions of the pancreas (38%) were selected an initial operation,and 79 of them (62%) selected follow-up visit. Nearly 40% of the patients who chose an initial operation were confirmed to suffer a malignant tumor by pathology. 9 patients(11%) of all the patients in the follow-up accept an operation because of their cystic lesions had a variation. According to the result of the pathology after the resection,3 patients of them had a canceration. The patients in the follow-up whose lesion diameter was smaller than 3cm,not existing a solid lesions,and without signs or symptoms,didn't have a risk in the transition to the malignant mutation. The transition rate was less than 4%,which was similar to the death risk of the patients undergoing resection. Conclusion:It is significant to adopt a careful radioactive follow-up to the surrounding parenchyma of all the patients who had not an initial resection.
