OBJECTIVE:To investigate therapeutic efficacy of bifidobacteria triple viable bacteria combined with infliximab in the treatment of moderate and severe ulcerative colitis(UC),and its effects on immune function. METHODS:Seventy-two UC pa-tients were selected from gastroenterology department of the First Affiliated Hospital of Chongqing Medical University during Jun. 2009 to Jan. 2015. They was divided into single group and combination group according to therapy plan, with 36 cases in each group. Single group was given infliximab 5 mg/kg,ivgtt,on 0,2nd,6th,8th week. Combination group was additionally given bi-fidobacteria triple viable bacteria 420 mg,tid. Both groups were treated for consercutive 2 months. Clinical efficacies were com-pared between 2 groups as well as the levels of humoral immunity and T cell function. The occurrence of ADR was observed in 2 groups. RESULTS:Clinical response rate of combination group was 86.11%,which was significantly higher than 52.78% of single group, with statistical significance (P<0.05). Before treatment, there was no statistical significance in humoral immunity and T cell function between 2 groups(P>0.05). After treatment, the levels of CD4+ and CD4+/CD8+ were significantly increased in both 2 groups, while the levels of CD8+,Th1 and Th2 were significantly decreased;the combination group was significantly better than the control group, with statistical significance (P<0.05);there was no statistical significance in CD3+ between 2 groups before and after treatment(P>0.05). After treatment, the levels of IgA and IgG were increased significantly in 2 groups, and the level of C3 was decreased significantly;the combination group was significantly better than the control group, with statistical significance (P<0.05);there was no statistical significance in IgM content and C4 level between 2 groups before and after treatment (P>0.05). There was no statistical significance in the incidence of ADR between 2 groups(P>0.05). CONCLUSIONS:Bifidobacteria triple viable bacteria combined with infliximab shows significant therapeutic efficacy in the treatment of moderate and severe UC, and can significantly improve humoral immunity function and the recovery of T cell level, with good safety.

Objectives To observe the effects of glucose at different concentrations on the expression of PPAR? in the rat insulinoma INS-1 cells so as to illustrate the function of PPAR? in the cross-talk of lipid metabolism with glucose metabolism in pancreatic cells. Methods After cultured in RPMI1640 media containing 3, 11 and 20 mmol/L glucose respectively for 1 h, the cells were used to measure the content of intracellular triglyceride. The RNA and nuclear protein of INS-1 cells were extracted, and then the expression of PPAR? mRNA was detected by semi-quantity RT-PCR and the PPAR? protein was analyzed by Western blotting. Results With the increment of the concentrations of glucose, the content of intracellular triglyceride in the cultured cells was elevated, while the expressions of PPAR? mRNA and protein were both down-regulated. Conclusion Glucose promotes triglyceride deposition in INS-1 cells, and the down-regulation of PPAR? presumably contributes to the lipid deposition in INS-1 cells during hyperglycemic states.

Chinese Journal of Integrated Traditional and Western Medicine 2016;36(1):24-28
OBJECTIVETo observe the auxiliary efficacy and safety of Hebi Recipe (HR)in treating early rheumatoid arthritis (RA).
METHODSTotally 63 early RA patients with Gan-Pi disharmony were randomly assigned to the treatment group [32 cases, treated by HR (one dose per day, taken in two portions for 24 successive weeks) plus Methotrexate (MTX)] and the control group (31 cases, treated by MTX alone). The dosage of MTX was increased from 7.5 mg to 12.5 mg, once per week, 24 weeks as one course of treatment. Efficacy for Chinese medical syndromes, American College of Rheumatology 20 (ACR20) improvement rate, disease activity score in 28 joints (DAS28), laboratory related indices [ESR, rheumatoid factor (RF), C-reactive protein (CRP), anti-cyclic citrullinated peptide (CCP)], and related ultrasonic inspection items (synovium thickness, synovium blood flow classification, effusion of joint), and adverse reactions were observed.
RESULTSThe total effective rate (83.9%, 26/31 cases) and ACR20 improvement rate (74.2%, 23/31 cases) were higher in the treatment group than in the control group [60.7% (17/28 cases), 46.4% (13/28 cases); P < 0.05]. Compared with before treatment in the same group, DAS28 score, ESR, RF, CRP, CCP, synovium thickness, synovium blood flow classification, effusion of joint all decreased in the two groups after treatment (P < 0.01, P < 0.05). Compared with the control group after treatment, ACR20 improvement rate, DAS28 score, ESR, RF, CRP, CCP, synovium thickness, synovium blood flow classification, effusion of joint all decreased in the treatment group (P < 0.01, P < 0.05). Liver dysfunction occurred in 1 case of the treatment group. One leucopenia and 2 liver dysfunction occurred in the control group.
CONCLUSIONHR could effectively improve joints and systemic symptoms of early RA patients with Gan-Pi disharmony.
