1.Three-dimensional MR Spectroscopic Imaging of Normal Prostate in Chinese Male Adults: the Effects of Anatomy Zones and Age Groups

40 years, old group) without prostate diseases, underwent MRI and MRS examinations. The peripheral zone (PZ) and central gland (CG) of the prostate were distinctly demonstrated on MRI, the right and left side of PZ and CG were divided into bottom, middle and tip regions,totally 12 samples were obtained from each case. The levels of (Cho+Cre)/Cit ratios were calculated on the basis of the MRS metabolic map. Results ①The mean level of (Cho+Cre)/Cit ratios in PZs and CGs of different regions in young group and old group was not significantly different. Statistically significant variation of the (Cho+Cre)/Cit ratio in PZs and CGs in the two groups was detected. ②The mean level of (Cho+Cre)/Cit ratios of PZs between young group and old group was not significantly different. Only Statistically significant variation of (Cho+Cre)/Cit ratios in CGs between two groups was detected. Conclusion The normal levels of prostatic metabolites fluctuate dramatically among individuals. There is metabolic difference between the peripheral and central zone and that of young and old group.
2.Gait characteristics of children with spastic cerebral palsy assessed by dynamic plantar pressure measurement

BACKGROUND:Pressure sensitive instrumented shoes are fast and easily used tools to measure ground reaction forces. Currently researches about the utilities of these systems in assessment of gait in children with neurological diseases have been started to run.OBJECTIVE: To find the gait characteristics of dynamic plantar pressure in children with spastic cerebral palsy. DESIGN: A cross-sectional study.SETTINGS: Department of Rehabilitation, Songgang People's Hospital; Department of Rehabilitation, Shenzhen Children's Hospital; Department of Rehabilitation Medicine, the First Affiliated Hospital of Sun Yat-sen University. PARTICIPANTS: ① Patient group: Twenty children with spastic cerebral palsy were selected from Shenzhen Children's Hospital from May 2004 to April 2005, including 9 boys and 11 girls, aged 26-66 months old, and they all could walk for more than 10 m independently. ② Normal control group: 52 healthy children with normal walking ability were enrolled, including 28 boys.and 24 girls, aged 35-76 months old.METHODS: Ultraflex gait analysis system was used to perform continuous plantar pressure tests of both groups of children. Diagram and data of dynamic plantar pressures in gait were recorded and analyzed with a computer. MAIN OUTCOME MEASURES: ① 10 sequential gait cycles were chosen from the continuous stable steps to calculate the average data with the software; ② Differences of the characteristic parameters of plantar pressure curve between the two groups.RESULTS: ① The curves of plantar pressure to time of testees in the normal control group were of the wave shapes with two peaks and one valley. Half of the CP children could not perform the two-peak shape pressure curve in the gait analysis. One kind of abnormal plantar pressure curve style was a wave shape with only one peak. Another kind of abnormal plantar pressure curve style was a shape showing asymmetry between sides and variation in different steps even in the same side. Two-peak shapes of dynamic plantar pressure were recorded in 10 CP children. ② Characteristic parameters picked up from the curves of plantar pressure had been compared between the two groups. Significant difference have been found in ts of both sides and tz1,tz2,tz3 on the right side between the two groups. It was shown that the single-limb support time of both sides in the CP children was longer than that in healthy children [left: (0.879± 0.658), (0.472±0.051) s; right: (0.818±0.682), (0.478±0.050) s; P < 0.01]. The tz1 and tz3 on the right side in the CP children were longer and while the tz2 was shorter than those in the healthy children. CONCLUSION: ① Using dynamic plantar pressure gait analysis system, plantar pressure curves can be observed directly, and information about kinetic characters can be drawn straightly. ② Part of patients with spastic CP could not perform the two-peak shape pressure curves in gait analysis. In patient can be recorded of two-peak shape dynamic plantar pressure curves, ts of both sides are shorter than those in healthy children, tz1 and tz3 on the right side is longer and tz2 is shorter than those in healthy children.
3.A new dimeric lignan from Zanthoxylum simulans.

To study the chemical constituents from Zanthoxylum simulans and their anti-inflammatory activity. The constituents of Z. simulans were isolated and purified using various column chromatographies. Their chemical structures were elucidated using extensive spectroscopic methods. The compounds were assayed inhibitory activity against NO production in LPS stimulated RAW 264.7 cells. Four compounds were obtained from the ethanol extract of Z. simulans and determined to be isozanthpodocarpin B(1), kobusin (2), (+)-fargesin (3), and epieudesmin (4). Compound 1 exhibited NO production inhibitory effect with IC50 value of 14.49 µmol · L(-1). Compound 1 is a new dimeric lignan and may be serve as potential anti-inflammatory agent in the future.

5.Efficacy of dexmedetomidine mixed with ropivacaine for epidural anesthesia

Objective To evaluate the efficacy of dexmedetomidine mixed with ropivacaine for epidural anesthesia in the patients.Methods Forty-six patients, of ASA physical status Ⅰ or Ⅱ , aged 36-58 yr, weighing 48-77 kg, scheduled for elective hysterectomy under epidural anesthesia, were randomly divided into 2 groups (n =23 each) according to the random number table: normal saline + ropivacaine group (R group) and dexmedetomidine + ropivacaine group (DR group).Epidural anesthesia was performed at L2,3interspace.The catheter was inserted into the epidural space.A mixture of 0.75% ropivacaine 15 ml and dexmedetomidine 1 μg/kg was injected epidurally in DR group.A mixture of 0.75% ropivacaine 15 ml and normal sline 2 ml was injected epidurally in R group.The onset time of sensory block, maximum level of sensory block, and duration of sensory block were recorded.The modified Bromage scale, Ramsay sedation score and intraoperative traction reaction were assessed.The occurrence of dexmedetomidine-related cardiovascular events and respiratory depression, and epidural anesthesia-related adverse events within 2 h after operation was recorded.Results Compared with R group, the onset time of sensory block was significantly shortened, the duration of sensory block was prolonged, the maximum level of sensory block and Ramsay sedation score were increased, the degree of intraoperative traction reactions was lessened, and the incidence of shivering was decreased in DR group (P < 0.05).No patients developed dexmedetomidine-related adverse events.Conclusion Epidural dexmedetomidine 1 μg/kg significantly optimize the efficacy of ropivacaine when used for epidural anesthesia in the patients.

Objective To investigate the efficacy and safety of acute normovolemic hemodilu-tion for pregnant women with total placenta pravia and accreta.Methods Fifty-two pregnant women with placenta praevia and accreta were randomly divided into three groups.Groups A and B received ANH or AHH before the operation while group C received the normal treatment.The total blood loss,transfusion of allogeneic blood and preoperative and postoperative routine blood test were recor-ded;the infants’umblical artery blood samples were taken immediately after birth,and 1 min,5 min Apgar scores and the blood gas was analyzied.Results There were no significant difference in the general condition,operation duration,blood loss among the three groups;the transfusion of allogeneic blood in groups B and C were much more than group A (P <0.05);the postoperative hemoglobin and hematocrit and the count of plateletsin in group A were better than groups B and C (P <0.05),while there were no statistically difference between groups B and C;there were not only seldom difference in the Apgar scores of 1 min and 5 min,but also barely change in the blood gas analysis of umbilical ar-tery in new borns from the three groups.Conclusion ANH could play an important role in the pre-vention of postpartum hemorrhage without doing any harm to the mother and infant.
7.The influence of CYP2C19 gene polymorphism on the curative effect of clopidogrel in patients after receiving TIPS

Objective To evaluate the effect ofcytochrome P450 isoenzyme subfamily 2C19 (CYP2C19)gene polymorphism on the clopidogrel antiplatelet therapy in cirrhosis patients after receiving transjugular intrahepatic portosystemic shunt (TIPS).Methods The clinical data of 171 cirrhosis patients,who were treated with TIPS during the period from January 2013 to December 2014,were retrospectively analyzed.During operation both the portal vein and the elbow vein blood samples were collected and sent for CYP2C19 gene testing.After TIPS,clinical follow-up checkup was made once every 3 months.The gene detection results and clinical follow-up findings were comparatively analyzed.Results A total of 110 patients,who had not received blood transfusion before TIPS and who had regularly taken clopidogrel antiplatelet therapy after TIPS were enrolled in the study.The mean time to take clopidogrel was 192.4 days (31-517 days),and the gene detection results of portal vein and elbow vein were quite consistent.CYP2C19 genotype of *1/*1 was found in 49 patients (44.5%),CYP2C19 genotype of *1/*2 in 27 patients (24.6%),CYP2C19 genotype of *1/*3 in 18patients (16.4%),CYP2C19 genotype of *2/*2 in 11 patients (10.0%),CYP2C19 genotype of *2/*3 in 3patients (2.7%),and CYP2C 19 genotype of *3/*3 in 2 patients (1.8%).Following-up examinations showed that the incidence of shunt dysfunction in patients carrying slow metabolic gene was 87.5% (14/16),which was significantly higher than that in patients carrying moderate metabolic gene (20.0%,9/45;x2=22.9,P=0.006)as well as in patients carrying fast metabolic gene (8.2%,4/49;x2=37.91,P=O.O00 1).Multivariate analysis of Cox regression model indicated that CYP2C19 slow metabolic gene variation was an important predictive factor for shunt dysfunction (95%CI:1.80-9.03,P=O.O00 7).Conclusion CYP2C19 slow metabolic gene variation,including genotype of *2/*2,*2/*3 and *3/*3,is an important factor that can influence the efficacy of clopidogrel treatment after TIPS.Preoperative CYP2C19 gene detection results can provide useful information,which is very helpful in making an effective and reliable anti-platelet treatment plan for patients after TIPS.

9.Study on quality standard for Xiaozhi Tablets

AIM: To establish the quality standard for Xaozhi Tablets(Lasiosphaera seu Calvatia, Rhizoma Dioscoreae Cirrhosae, Radix Sophorae Tonkinensis, Concha Ostreae, etc.). METHODS: Rhizoma Dioscoreae Cirrhosae and Radix Sophorae Tonkinensis were identified by TLC. Matrine was determined by HPLC. RESULTS: Rhizoma Dioscoreae Cirrhosae and Radix Sophorae Tonkinensis could be detected by TLC. Matrine showed a good linear relationship in the range of 0.2040~2.040?g and the average recovery was 97.64%, RSD was 1.49%. CONCLUSION: The methods are simple, sensitive and accurate with good reproducibility. The methods may be used for the quality control of Xiaozhi Tablets.

Objective To quantitatively evaluate the metabolic changes of prostate adenocarcinoma using magnetic resonance spectroscopy (MRS).Methods Eighteen patients of prostate cancer proved by ultrasound guided systemic biopsy were enrolled in this study.All the puncture locations were marked and the corresponding (Choline+Creatine)/Citrate ratios were calculated on the basis of the MRS metabolic map.Results 204 samples were obtained and 106 of them were diagnosed as cancer.The average (Cho+Cre)/Cit ratio in the 106 positions was 2.53?1.02.The (Cho+Cre)/Cit ratio of cancer area was statistically higher than that of noncancerous area (F=8.64, P
