1.Catalpol protect diabetic vascular endothelial function by inhibiting NADPH oxidase.

The aim of the present study was to evaluate the protective effect of catalpol on vascular endothelial function in STZ-induced type 2 diabetes mellitus (T2DM) rats. 40 high-fat diet with STZ-induced diabetes rats were randomly divided into model group, catalpol low-dose, middle-dose and high-dose group (10, 50, 100 mg x kg(-1) x d(-1)), 10 normal Wistar rats were used as the normal group. The normal and model groups were given an equivalent amount of saline. All reagents were administered by oral gavage for 6 weeks. After 6 weeks, blood glucose and lipids were detected by an automatic biochemical analyzer. The endothelium-dependent vasodilation response of thoracic aortar was detected. The pathological changes of the thoracic aorta were observed by HE staining. Ser- um nitric oxide (NO), 8-iso prostaglandin F2α (8-iso-PGF2α) and superoxide dismutase (SOD) were detected by ELISA. Reactive oxygen species (ROS) level of thoracic aorta was detected by fluorescence method. The expression of Nox4 and p22phox mRNA and protein in aortic tissue were detected by RT-PCR and Western-blot respectively. After catalpol treatment, endothelial damage of thoracic aorta was attenuated significantly; ROS level of thoracic aorta and serum level of 8-iso-PGF2α were decreased significantly; serum NO and SOD levels were remarkably elevated; expression of Nox4, p22phox mRNA and protein in thoracic aorta were significantly reduced (P < 0.05). Therefore, catalpol has protective effect on endothelial of T2DM, its mechanism may be associated with the down-regulation of Nox4 and p22phox expression, inhibiting oxidative stress reaction response.

2.Multi-center Clinical Evaluation on Acupuncture Yanglingquan for Relieving Gastrocnemius Spasm

Objective To study the relative specificity of Yanglingquan on relieving spastic gastrocnemius. Methods According to evidence-based medicine/DME principle, the effect of Yanglingquan in relieving gastrocnemius spasm of old women was studied and evaluated by standard diagnostic and efficacy criteria in random blind multi-center clinical test, with Yanglingquan as the observation group and non-related acupoint Quchi as control group. The effect was investigated by muscle spasm rehabilitation assessment methods. Results There were significant differences in the grade integral of symptoms of both groups between before and after treatment (P 0.05). Conclusion The effect of Yanglingquan in relieving gastrocnemius spasm was better than Quchi, and gastrocnemius spasm isn’t relevant to the content of K+, Na+, Cl-, Ca2+ in blood.
3.Laparoscopic appendectomy: A report of 162 cases

Objective To summarize the experience of laparoscopic appendectomy(LA).Methods A retrospective analysis was carried out on clinical data of 162 cases of laparoscopic appendectomy in this hospital from August 2002 to April 2005.Results Laparoscopic operation was completed in all the cases but 1 case of intestinal tuberculosis.The operative time was 30~90 min(mean,42 min) and the intraoperative blood loss was 5~10 ml.No complications were noted.Conclusions Laparoscopic appendectomy has advantages of little invasion,less blood loss,rapid recovery,and fewer complications,being a safe and ideal surgical alternative.
4.Exploration of Role-experience Teaching Method in Respiratory Medicine Teaching

6.Quality standard study on Pteris multifida.

The quality control method and standard were established to control the quality of Pteris multifida in this paper. The tests of water content, total ash, acid-unsoluble ash and ethanol-soluble extractives of P. multifida were carried out according to the methods recorded in appendix of Chinese Pharmacopeia (2010 edition, volume 1) . The TLC method was established by using rhoifolin as references, and a mixture of CHCl3 -MeOH-HAc (6: 1: 1) as the developing solvent system on GF254 thin layer plate. The contents of rhoifolin was determined by HPLC on a Diamonsil C18 (4.6 mm x 250 mm, 5 μm) column, using acetonitrile-water (containing 0.15% formic acid) (16: 84) as mobile phase at a flow rate of 1.0 mL x min(-1). The column temperature was 30 degrees C and the detection wave-length was 350 nm. As a result, pterosin C 3-O-β-D-glucosidede and the other constituents were well separated on TLC detected under the UV light at 254 nm . The methodology validation for the assay of rhoifolin presented that it was in good linear correlation in the ranges of 0.025 5-5.1 μg with the regression equations of Y = 1 092.4X + 9.503 5 (r = 0.999 8), and the average recoveries were 100.3% (RSD 1.3%). The content range of rhoifolin from 16 different batches of Pteris multifida was 0.08-5.06 mg x g(-1). The water content, total ash, acid-unsoluble ash and ethanol-soluble extractives of 16 samples varied in the ranges of 7.35% - 12.96%, 6.90% - 16.33%, 2.07% -11.38% and 13.29% -23.87%, respectively. The suggesting limes in the quality standard for water content, total ash, acid-unsoluble ash, ethanol-soluble extractives and rhoifolin content were ≤ 12% , ≤ 15% , ≤ 8.5% , ≥ 14% and ≥ 0.040%, respectively. The result proved that the established quality of control method was specific and accurate, which can be used for the quality control of P. multifida.

7.Effects of removing laryngeal mask airway in anaesthetized state on emergence agitation after sevoflurane anaesthesia in children

Objective To investigate the effect of removing laryngeal mask airway (LMA) in anaesthetized state on emergence agitation after sevoflurane anaesthesia in children.Methods Ninety children with subumbilical operation,ASA Ⅰ or Ⅱ grade,were divided into three groups by random digits table with 30 cases each:ET-A group (endotracheal tube and extubation while awake),ET-D group (endotracheal tube and deep extubation) and LMA-D group (insertion of LMA and deep removal).The incidence of preoperative emotional bad,anaesthesia time,sevoflurane administration time,emergence agitation,the numbers of treated with propofol and/or fentanyl and postoperative anaesthesia care unit(PACU)retention time were measured.Results There was no significant difference in incidence of preoperative emotional bad,sevoflurane administration time and anaesthesia time among the three groups (P > 0.05).The incidence of emergence agitation,the numbers of treated with propofol and/or fentanyl and PACU retention time in LMA-D group were significantly lower than those in ET-A group [26.7%(8/30) vs.66.7%(20/30),8cases vs.20 cases and (23.9 ± 4.9) min vs.(32.9 ± 5.8) min,P < 0.01].Conclusion Insertion of LMA and removing LMA in anaesthetized state can decreased emergence agitation in children after sevoflurane anaesthesia,especially suitable for the children who do not suitable for drug treatment.
8.Gastrointestinal Perforation: The Value of the Periportal Free Air Sign on Multidetector Spiral Computed Tomography

Objective To evaluate the value of the periportal free air(PPFA) sign at computed tomography(CT) in distinguishing between upper and lower gastrointestinal(GI) tract perforation. Methods CT images of 62 patients with surgically proven GI tract perforation were retrospectively analyzed. 62 cases included upper and lower GI tract perforation in 35 cases and 27 cases,respective-ly. When there was free air in the periportal area,it was defined as positive periportal free air(PPFA) sign. The difference of PPFA sign in upper and lower GI tract perforation, and the sensitivity, specificity, positive predictive value, negative predictive value and ac-curacy of PPFA sign in diagnosing gastrointestinal perforation were analysed respectively. Results The PPFA sign was seen in 33 of 35(94%) patients with upper GI tract perforation,but only in 10 of 27 (37%) patients with lower GI tract perforation,there was sig-nificant difference between them(P<0.05). For diagnosis of super GI perforation with PPFA sign, the sensitivity, specificity, positive predictive value,negative predictive value and accuracy were 94% (33/35), 63%(17/27), 770%(33/43), 89%(17/19) and 81% (50/ 62), respectively. Conclusion The PPFA sign is a useful finding in distinguishing between upper and lower GI tract perforation.
9.Influence of hierarchical full-responsibility nursing system on health education effect of inpatients

Objective To improve the effects of health education(HE)and satisfaction degree on HE of inpatients by executing hierarchical full-responsibility nursing. Methods Fourteen wards were randomly chosen form the hospital and divided into the control group and the experimental group, each group having 7 wards. The control group carried out routine holistic nursing model, and the experimental group carried out a new hierarchical full-responsibility nursing system. Reforming nursing scheduling and diminishing the nursing unit to assure that the patients acquired the continous and stable nursing service when they were in hospital. The effect and the degree of satisfaction of HE were compared between two groups after 6 months. Results The effects of HE in the experimental group were significantly better than those in the control group, and the degree of satisfaction on HE in the experimental group was much higher than in the control group. Conclusions The hierarchical full-responsibility nursing system can give patients systematic and normalized HE. It can significantly improve the effects and the degree of satisfaction on HE.
10.The Application of Noninvasive Central Blood Pressure Detection in Blood Pressure Management and Vessel Function Assessment of Hypertension.

OBJECTIVESTo explore the significance of noninvasive central blood pressure detection in blood pressure management and vessel function assessment of hypertension.
METHODSA total of 348 subjects were recruited in the study and A-pulse CASP software were derived to measure the parameters of CASP and vessel function. The recruited subjects were divided into different groups for analysis according to age and disease.
RESULTS(1) CASP in hypertension group were both higher than those in normal group, while RAI and PRT lower. (2) Hypertensive patients of which brachial blood pressure were controlled have lower CASP and RAI than those above the level. (3) Hypertensive patients whose brachial blood pressure were controlled have significantly higher CASP than normal subjects.
CONCLUSIONSCentral blood pressure is more reflective of actural blood pressure and vessel function. Enhanced control of CASP in hypertensive patients contributes to improving vessel compliance.
