1.Discussion of standardized management of the traceability of implanted medical consumables

Objective:Discuss how the management of implantable medical consumables can be standard during the links of production, transportation, internal logistics of the hospital and the sector of using. Methods: In accordance with the relevant national laws and regulations, and long-term management practice and experience, and aiming at the specialty and high risk characteristics of implanting medical consumables, we apply information technique to regulate the standard operation of the whole medical supplies links (raw materials, processing of products, marketing, warehousing transportation, and the usage of the patients), therefore the roots can be traced back. The implanted medical supplies traceability standardized management needs to be explored. Results: Through the establishment of the traceability management process system of implanting medical supplies, the standard management mode of the medical supply technique will be attained. Even when the information is incomplete, we can retrieve and trace back quickly. Therefore, the goal of controlling the adverse events of medical implants, reducing medical risks can be achieved. Conclusion:the discussion above can be taken as a reference to the standardization of the management of implantable medical consumables.
2.Intervention with drug resistance related to metabolism of cancer cells:advances in research

The metabolic properties of cancer cells diverge significantly from those of normal cells. Energy production in cancer cells is abnormally dependent on aerobic glycolysis. In addition,cancer cells have other metabolic characteristics,such as increasing fatty acid synthesis and glutamine metabolism. Emerging evidences show that many key enzymes in dysregulated Warburg-like glucose metabolism,fatty acid synthesis and glutaminolysis are linked to drug resistance in cancer treatment. For example, lactate dehydrogenase A contributes to paclitaxel/trastuzuma resistance in breast cancer, fatty acid synthase is linked to docetaxel/trastuzumab/adriamycin resistance in breast cancer, and glutaminolysis is linked to cisplatin resistance in gastric cancer. Therefore,targeting cellular metabolism may improve the response to cancer therapeutics,and the combination of chemotherapeutic drugs with cellular metabolism inhibitors may overcome drug resistance in cancer therapy. This review discussed the relationship between dysregulated cellular metabolism and chemotherapy resistance, and the way in which targeting of metabolic enzymes can help overcome the resistance to cancer therapy or enhance the efficacy of common therapeutic agents.
3.Optimization of Lipase Production Conditions by Geotrichum candidum Y162 Using Single Factor-response Surface Methodology

The fermentation conditions of lipase production by Geotrichum candidum Y162 were optimized. Initially, the most suitable carbon olive oil, nitrogen source soybean flour and NH4Cl, salt BaCl2 and MgCl2 were selected according to single factorial experiments respectively. Based on the result, screening methodology Plackett-Burman design was used to evaluate the effects of twelve factors related to lipase production and three statistically significant factors olive oil, BaCl2 and NH4Cl were selected. The path of steepest ascent was used to approach the optimal region of lipase production subsequently. Then, the optimal combined concentration for maximum enzyme activity were further optimized by response surface methodology and determined as follows: olive oil 2.35%, BaCl2 0.36%,and NH4Cl 4.69%.The optimization of culture conditions of G.candidum Y162 led to a 2.25-fold increase in lipase production relative to initial result 14.16 U/ml, which indicate that single factor in combination with response surface methodology is an effective method for optimization of lipase production conditions by G.candidum Y162.
5.Advances in research on signal transduction mechanisms and their inhibitors for the proliferation of pulmonary artery smooth muscle cells

Pulmonary artery hypertension (PAH ) is a chronic progressive disease characterized by a persistent elevation of pul-monary vascular pressure,and the disease would limit the right ventricular function severely,fail the organ and even lead to death in the end.The histopathological change of PAH is fea-tured by the restructuring of pulmonary vessels,and the abnor-mal reproduction of pulmonary artery smooth muscle cells (PASMCs)in peripheral vessels is the major pathological basis of pulmonary vascular restructuring.This paper mainly reviews the research advances on signal transduction mechanisms and their inhibitors in promoting the proliferation of pulmonary artery smooth muscle cells.
6.Effects of bupivacaine on short-circuit currents in human alveolar epithelial cells

Aim Bupivacaine is a kind of long-acting amide local anesthetics.This paper aims to explore the effects of bupivacaine on the short-circuit currents in human alveolar epithelial monolayers and study the possible mechanisms.Methods Short-circuit currents were recorded by ussing-chamber setup.Amiloride-sensitive currents were defined as the difference be-tween the total current and the amiloride-resistant cur-rent.ERK1 /2 phosphorylation protein levels were ana-lyzed by Western blot at 0,1 5,30 and 60 min after administration of 1 00 μmol·L -1 bupivacaine.Results Bupivacaine could inhibit the short-circuit currents in H441 monolayers dose-dependently,which could be inhibited by amiloride.Western blot analysis showed that bupivacaine increased the level of ERK1 /2 phos-phorylation.Conclusion These data demonstrate that bupivacaine can reduce the alveolar ion transport by in-hibiting the amiloride-sensitive currents,possibly by the enhancement of ERK1 /2 phosphorylation. The effects of alveolar fluid clearance following application of bupivacaine should be considered clinically when the patient is complicated with lung injury.
7.Inhibiting role of polydatin to expression of intercellular adhesion molecule-1 induced by lipopolysaccharide on vascular endothelial cells

AIM and METHODS: To investigate expression of intercellular adhesion molecule-1(ICAM-1) on human umbilical vein endothelial cells (HUVEC) induced by lipopolysaccharide (LPS) , and inhibiting role of polydatin by cellular immune fluorescent staining and laser confocal microscope scanning technology. RESULTS: Compared with basic expression of ICAM-1 on HUVEC, the ICAM-1 expression was enhanced significantly after stimulated by LPS from 8 h to 36 h, dose-dependent relation appeared between expression of ICAM-1 and LPS. ICAM-1 expression on endothelial cells treated only by polydatin had no abvious change,but inducing role of LPS to expression of ICAM-1 was inhibited significantly by polydatin pretreating endothelial cells. CONCLUSION: The expression of ICAM-1 on endothelial cells can be promoted by LPS , and polydatin can inhibit LPS-induced ICAM-1 expression
8.Change of ICAM-1 expression in intestine tissue of mice induced by LPS and role of p38 MAPK in its expression

AIM: To study the change of intercellular adhesion molecule-1(ICAM-1) expression in intestine tissues of mice induced by LPS and regulatory effect of p38 mitogen-activated protein kinase (p38 MAPK) on ICAM-1 expression. METHODS: Protein and mRNA of ICAM-1 were measured using Western blotting and RT-PCR respectively in intestine tissue of BALB/c mice treated by lipopolysaccharide(LPS) or LPS plus SB203580, a specific inhibitor of p38 MAPK. RESULTS: Compared with control group, the expression of ICAM-1 protein and mRNA was increased significantly by LPS stimulation in dose- and time-dependent manner. ICAM-1 expression reached peak value at 12-36 h after LPS stimulation. 20.0 mg/kg of LPS could induce the maximum of ICAM-1 expression. Pretreatment of mice with SB203580 for 30 min could inhibit significantly LPS-induced expression of ICAM-1 protein and mRNA expression in mouse intestine tissues. CONCLUSIONS: These data highlight that LPS could up-regulate ICAM-1 protein and mRNA expression in intestine tissue of mice in dose- and time-dependent manner, and p38 MAPK signal pathway plays an important role in ICAM-1 expression induced by LPS. It suggests that inhibition of p38 MAPK might be a useful principle for the prevention and treatment of intestine damage of endotoxic shock.
9.Clinical research of intraoperative image-guidance in endoscopic nasocular operation

AIM:To evaluate the availability and our experience of intraoperative image-guidance in endoscopic nasocular operation.METHODS:Seven cases of endoscopic nasal surgery with intraoperative image-guidance were retropectively reviewed,including 3 cases of optic nerve injury;3 cases of foreign object of optic behind the eyeball;1 case of retrobulbar tumor(angeioma).RESULTS:The preoperative preparatory time would take 15 minutes,including coordination,head holder localization,conventional instrument registration.In our cases,the localization accuracy between 3-D image landmarks of navigation system and actual anatomical landmarks was less than 1.3mm.The optic nerve and other anatomical points could be orientated accurately in intraoperative procedures.No complication occurred.CONCLUSION:Nasal endoscope combined with imageguidance systems provides accurate anatomical localization of anterior skull base with enlarged operation field.It is possible for surgeons to observe important anatomical structures during endoscopic surgery.It could increase the effectiveness and decrease surgical complications,especially in complicated cases.
10.Effects of Hip Flection and Extension in Supine Position on Lumbar Traction for Prolapse of Lumbar Intervertebral Disc

Objective To compare the curative effects of lumbar traction in supine position with hip flection and extension for prolapse of lumbar intervertebral disc (PLID).Methods 84 PLID patients were all treated by lumbar traction in supine position. But, group A (42 cases) with hip flection and group B (42 cases) with hip extension. Before and after three weeks therapy, all patients were assessed by Fairbank JC index and Visual Analogue Scales (VAS) to observe the symptom and pain condition.Results The scores of Fairbank JC index and VAS scale of the patients in group A were significantly lower than those in the group B (P<0.001).Conclusion Lumbar traction in supine position with hip flection for the PLID can obviously promote the curative effects.
