2.Changes of serum cortisol concentration in type 2 diabetic patients

Objective To assay serum cortisol concentration in type 2 diabetic patients and evaluate the relationship between serum cortisol concentration and insulin resistance. Methods Serum cortisol concentrations were measured at 8 am, 4 pm and during oral glucose tolerance test(OGTT) in 60 newly-onset type 2 diabetic patients and 40 healthy subjects using RIA. Results ① Sernm cortisol concentrations were elevated in obese type 2 diabetic patients at 8 am and 4 pm as compared with non-obese patients and normal controls. ② Changes of serum cortisol concentration during OGTT were different between obese type 2 diabetic and non-obese patients: serum cortisol concentrations were decreased after glucose administration in non-obese patients but not in obese patients. ③ In type 2 diabetic patients, serum cortisol concentrations were negatively correlated with insulin sensitivity index (r=-0.34, P<0.01). Conclusion There is some degree of abnormal in cortisol secretion in obese type 2 diabetic patients.
3.Electrophysiological classification of Guillain-Barr? syndrome: analysis of 100 cases

Objective To explore the electrophysiological and clinical features of various subtypes of Guillain Barr? syndrome (GBS). Methods The electrophysiological and clinical data of 100 cases with GBS admitted to Xijing Hospital from 1980 to 1999 were analyzed retrospectively. Correlations between varied subtypes and ages were examined by ? 2 test. Results Among the 100 patients with GBS, the demyelinating pattern was present in 51 patients, the axonal pattern in 25 patients, and 8 patients were inexcitable, 12 patients equivocal and 4 patients normal. The demyelinating pattern appeared as a major subtype not only in different age groups, but also in different test times after symptom onset. There was no statistically significant relationship between varied subtypes and ages. In the 100 patients, 32.0% suffered from a preceding upper respiratory infection, and 22.0% had a preceding gastrointestinal tract infection. The cases occurring in rural areas are almost in number equal to those in urban areas. That is, there was no a clear area distribution. Both demylinating and axonal GBS occurred throughout the year with a likely peak from July to September. Conclusion In the 100 patients with GBS admitted to Xijing Hospital, the demyelinating pattern was the major electrophysiological subtype. In addition, the electrophysiological and clinical features of various subtypes of GBS seemed to be different in some ways from those in the studies of both western countries and Li CY in northern China.
4.The effects of high relative molecular mass salivary mucin on the protection of tooth enamel against demineralization observed by electronic microscopy

Objective: To observe the effect of purified high rela ti ve molecular mass mucin(MG1) pellicle on the protection of human dental enamel against demineralization.Methods: MGI was extracted from human saliva and purified.MGI pellicle and whole saliva pellicle were formed on dental enamel samples in vitro ,Then The samples were treated by 34 mmol/L citric acid for 1 min.The demineralization of the samples was observed by scanning electronic microscopy(SEM). Results:After treatment by the acid the enamel surfaces covered by MGI pellicles or whol e saliva pellicle showed relatively smooth and normal enamel morphological feat ures, on the contrary the surface without pellicle showed large area of deminer alization and bee nest like appearance. Conclusion: The data indicate that the MG1 in enamel pellicle contribute to its protective effec ts against acidic attack on the enamel surface.
5.Cephalometric study of alveolar remodeling during incisor retraction

Objective:To investigate the remodeling of alveolar process following anterior teeth retraction. Methods: Cephalograms of pretreatment (T1), posttreatment (T2) and follow-up (T3) of 55 cases with four first premolars extracted were collected as the study samples. All the lateral head films were traced on the acetate. Palate best fit superimposition was used to transfer pretreatment SN to posttreatment and follow-up cephalograms and to evaluate the displacement of upper incisor teeth roots and the remodeling of alveolar bone. 7.Investigation on perioperative Antivirus Treatment with adeforir on Chronic Hepatitis B Patients with Colon Cancer

Objective To study the activity of hepatitis B virus and therapeutic efficacy of adefovir in chron-ic hepatitis B patients with colon cancer during operation period. Methods 50 chronic hepatitis B patients with co-lon cancer to be underwent colon resection were divided two groups at random, one group was given adcfovir (adefovir group), the other group not (control group), and therapeutic measure except antivirus is the same. HBV-DNA con-centration and liver functon of the patients were detected. Results Serum HBV-DNA concentration and ALT level of the patients was increased significantly at 1 week and 1 month after operation compared with that before operation in control group. Serum HBV-DNA concentration of the patients was decreased significantly at 1 week and 1 month after operation compared with that before operation in adefovir group. The ALT level of patients in adefovir group is signifi-cantly lower than that in control group. Conclusions Surgical procedure may result in HBV replication and antivirus treatment should be done perioperatively. Adefovir can inhibite replication of HBV and also improve the liver function of the chronic hepatitis B patients with colon cancer during operation.
8.Effect of continous subcutaneous insulin infusion and mutiple subcutaneous insulin infusion on type 2 diabetes mellitus during perioperation

Objective To study the effect of continous subcutaneous insulin infusion (CSII) and mutiple subcutaneous insulin infusion (MSII) on type 2 diabetes mellitus during perioperation. Methods One hundred and eighty surgical patients complicated with Type 2 diabetes were randomly divided into two groups,98 cases in the CSII group (treated with CSII of novolin R) and 82 cases in the MSII group (treated with MSII of novolin R and no-volin N). Blood glucose level,the time to reach normal blood glucose level, the average dosage of insulin, the inci-dence of hypoglycemic,infection rate of incisions and inpatient days were measured in two groups before and after treatment. Results The level of fasting blood glucose after treatment in the CSII group (4.8 mmol/L (SD: 1.6)) was significantly lower than that of the MSII group (6.4 mmol/L(SD :2.1)) (t = 7.74,P < 0.05), and 2-h glucose in the CSII group (7.6 mmol/L(SD :2.3)) was significantly lower than that of the MSII group (9.3 mmol/L(SD: 2.4)) (t = 7.72, P < 0.05). The time to reach normal blood glucose level in the CSII group (4.1 days (SD: 2.9)) was shorter than that of MSII group (6.9 days (SD :2.0)) (t=2.81, P < 0.05). The average dosage of insulin in the CSII group (40.7 U(SD: 10.3)) was lower than that of the MSII group (63.2 U (SD: 17.0)) (t=3.57, P <0.05). The incidence of hypoglycemic in the CSII group (3.05%) was lower than that of the MSII group (9.20%) (χ~2 = 4.92,P < 0.05). The infection rate of incisions in the CSII group (0.0%) was lower than that of the MSII group (10.9%) (χ~2 =4.18, P < 0.05). The inpatient days in the CSII group (15.3 days (SD :7.2)) was shorter than that of the MSII group (22.5 days (SD :9.7)) (t = 3.12, P < 0.05). Conclusions Compared to multiple sub-cutaneous insulin infusion, continuous subcutaneous insulin infusion is more effective in controlling blood glucose, hypoglycemic and incision infection, thus is recommend to perioperative patients complicated with type 2 diabetes mellitus.
9.Effects of dominant hand in contact with sternum on the quality of external chest compression on high-fidelity manikin

Objective To analyze the effects of dominant hand position on the quality of external chest compression for cardiopulmonary resuscitation (CPR) by the employment of the high-fidelity real-time feedback manikin system.Methods A total of 228 medical students of Wuhan University were enrolled in 2013 after standard CPR training courses.Participants were brought to a simulation scenario in which an adult happened to have heart arrest out of a hospital.Each studeut was asked to do five cycles of conventional CPR.In accordance with the dominant hand and the actual compressing hand position,the students were divided into dominant hand (DH) group and non-dominant hand (NH) group.Comparisons of chest compression rate,chest compression depth and chest recoil between two groups were carried out,respectively.The data were analyzed by the software of SPSS 13.0.Results There was no significant difference in overall chest compression rate between two groups (P > 0.05) while the frequency distributions of chest compression rate showed differences (P <0.01),and chest compression rates above 100 cycle per min in DH group were higher than that in NH group (97% vs.92%,P =0.002).There was significant differences in chest compression depth between two groups (DH 44±8 mm vs.NH 43±8mm,P=0.001).In NH group,the depth in cycle 5 (41 ±8) mm is obviously less than that in cycle 1 (44 ±7) mm,cycle 2 (43 ±7) mm and cycle 3 (43 ±8) mm.Totally,there was no differences in chest recoils between two groups [NH (98 ±8)% vs.DH (97 ± 10)%,P =0.13],but full chest recoils were seen more in NH group (85% vs.79%,P < 0.05).Conclusions The dominant hand position can improve the quality of CPR resulted from the higher compression rate,deeper compression depth as well as delayed fatigue.
10.Effect of alpha-melanocyte in rat models of renal ischemia-reperfusion injury

BACKGROUND: Kidney ischemia-reperfusion injury often combines with acute kidney and lung injury. The expression of cutin cel growth factor receptor (KGFR) and alpha sodiumchannel protein (α-ENaC) in kidney and lung after ischemia-reperfusion injury and the protective effect of α-melanocyte require further observation and research. OBJECTIVE:To explore the therapeutic effect of α-melanocyte on the expressions of KGFR and α-ENaC in rat models of ischemia-reperfusion injury. METHODS:A total of 30 healthy male Sprague-Dawley rats were randomly divided into control group, ischemia-reperfusion group and α-MSH group. Models of renal ischemia-reperfusion injury were established by 30-minute ligation of renal artery in the ischemia-reperfusion and α-melanocyte groups. Rats in the control group were only used to expose the renal artery, no ligation. Rats in the α-melanocyte group were intraperitonealy injected with α-melanocyte (0.25 mg/kg) at 30 minutes before model establishment. Rats in the ischemia-reperfusion group were injected with 4 mL of physiological saline. RESULTS AND CONCLUSION: Compared with control group, water content of kidney and lung increased significantly in rats of ischemia-reperfusion group and a-MSH group, while the levels of KGFR and α-ENaC of kidney and lung in rats were lower (P < 0.05). Compared with the ischemia-reperfusion group, water content of kidney and lung in rats of a-MSH group decreased significantly, while the levels of KGFR and α-ENaC of kidney and lung increased gradualy (P < 0.05). Moreover, edema was significantly lessened in the rat kidney and lung. Results confirmed that after renal ischemia-reperfusion injury, KGFR and α-ENaC expression was consistent to the kidney and lung injury. α-MSH could increase the protein and mRNA expression of KGFR and α-ENaC in kidney and lung of rats, reduce the kidney and lung injury, and exert a certain protective effect.
