Edema, as one of common clinical diseases, could be treated by taking medicines and adopting external therapies with traditional Chinese medicines (TCM). In recent years, there have been many clinical and basic studies concerning external therapies with TCM on edema Data showed that the external therapies are mostly composed of such purgating drugs as Rhei Radix et Rhizoma, Natrii Sulfas and Pharbitidis Semen, heat-clearing drug such as Phellodendri Chinensis Cortex and resuscitation-inducing drug such as Borneolum Syntheticum. The study showed that ingredients of external therapies did not pass through hilum and hepatic system, and thus avoided the first pass effect of livers. They enabled effective components of drugs to be rapidly absorbed through pores and skins, strengthened the effect against edema, shortened the treatment course, decreased side effects, and were convenient and inexpensive. External therapies with TCM could play unique advantages in inhibiting edema in the future clinical studies.

International Journal of Pediatrics 2021;48(4):226-230
Henoch-Sch?nlein purpura(HSP)is an IgA-associated leukocytoclastic small-vessel vasculitis, commonly occurred in children.Recent studies have shown that its pathogenesis is closely related to the disturbance of humoral immune response mediated by B cells.CD4 + T cells have the function of help B cells. Each subgroup of CD4 + T cells has unique functions of promoting or regulating immune inflammatory response. There are abnormal changes in the proportion of CD4 + T cells subsets in HSP patients, including the imbalance of Th1/Th2, increasing numbers of Th17 and Tfh cells, together with decreasing Treg cell numbers. The changes are closely related to the occurrence, development and the clinical phenotype of HSP.It is suggested that the detection of CD4 + T cells plays critical roles in guiding the evaluation of HSP, and is expected to provide a new idea for the clinical diagnosis and individualized treatment of HSP.

Objective There are still different news on the preemptive analgesia. We aimed to investigate the preemptive analgesic effect of rofecoxib on the tetrodotoxin-resistant (TTX-R) sodium current in dorsal root ganglion (DRG) neurons m a rat model of acute pain. Methods Seventy-five male SD rats weighing 100-140 g were randomly divided into five groups with 15 animals in each group: (A) sham operation + placebo group; (B) operation control group received placebo orally before operation; (C) preoperatine rofecoxib group received oral rofecoxib 10mg?kg-1 h before operation; (D) and (E) postoperatire, rofecoxib group Ⅰ and Ⅱ received oral rofecoxib 10mg?kg immediately (D) and 1 h (E) after operation. Acute pain was produced by an 1 cm long incision in the plantar surface of the right hindpaw through skin, fascia and muscle. Pain threshold to mechanical stimulation with von Frey filament was measured before (baseline) and 2, 6,12, 48, 96 h after operation. The animals were killed by cervical dislocation before and 24, 48, 96 h after incision. DRGs at L3.5 on the operated side were isolated and minced. DRG neurons were isolated by digestion with collagenase Ⅰ A and trypsin TTX-R Na-current density in DRG neurons were measured by using whole-cell patch clamp technique. Results There was no significant change in pain threshold to mechanical stimulation and Na-current density in DRG neurons during the experiment in group A, The withdrawal threshold to mechanical stimuli was significantly decreased after operation compared to the baseline in the four groups ( B, C, D, E). The decrease in pain threshold was significantly less in group C,D,E than that m group B at 2, 6, 12, 24, 48 h after operation and was the least in group C Current density (pA/pF) was significantly increased at 24 h and 48 h after operation compared to baseline in group B, C, D, E. The increase in current density was significantly less in group C, D, E than in group B and was the least in group C.Conclusion Rofecoxib given before operation has preemptive analgesic effect in a rat model of acute pain. Stronger inhibition of TTX-R Na-current m DRG neurons may contribute to the underlying mechanism.

Objective To detect the effect and mechanism of Cyr61 on the apoptosis of renal tissue caused by early stage of ischemic acute kidney injury (AKI). Methods 30 SD rats were randomized into 5 groups, including control group, AKI group, AKI+bicarbonate group, AKI+blank virus group, and AKI+over?expression Cyr61 virus group. After animal models were created for 2h, serum and renal tissue were collected from sacrificed animals. Expression level of TNF?α was determined by ELISA. HE staining was used to observe the histologic changes of renal tissues. The levels of NF?κB p65 and TNFR1 were measured by immunohistochemical method. RT?PCR and Western blotting assay were adopted to detect the mRNA and protein expression levels of NF?κB p65, TNFR1 and Caspase3. Results Compared with control group, AKI group, AKI+bicarbonate group, AKI+blank virus group, AKI+over?expression Cyr61 virus group had obvious kidney injury. The levels of TNF?α, the mRNA and protein expression levels of NF?κB p65, TNFR1 and caspase3 were markedly up?regulated. Over?expression of Cyr61 significantly attenuated the degree of pathological injury, numbers of apoptotic renal tubular epithelial cells and increased the degree of Scr. Although compared with other groups, the level of TNF?α in kidney tissue had no difference, there was obvious decreased protein level of NF?κB p65, while the increase of TNFR1 and Caspase3 protein was moderate. Conclusions During the early stage of AKI, over expression of Cyr61 could inhibit apoptosis, which may be related to the suppression of TNFR1 transcriptional expression and interference of TNF?αpathway. Its underlying mechanism therefore deserves further research.

Chinese Journal of Medical Aesthetics and Cosmetology 2009;15(1):42-45
Objective To observe the effect of transfer factor on immunity in patients with varruca plana and the clinical effect of treatment. Methods Sixty patients with varruca plana were randomly divided into a treatment group (treated with transfer factor) and a control group (treated with routine therapy). Before and after the treatment, T-lymphocyte subgroups in peripheral blood of all the patients were determined by flow cytometry and serum levels of interleukin-10 (IL-10)and in-terferon-gamma (INF-γ)were detected by ELISA. The same detections were done to the twenty healthy volunteers as healthy controls. Results Effective rates in the treatment group and control group were 76.67 % and 43. 33 %, respectively (x2=5. 63,P<0.05). Decrease of CD3+ cells, CD4+ cells, CD4+/CD8+ ratio and increase of CD8+ cells were found in varruca plana group as com-pared with the healthy controls(P<0.01 ). After the treatment of transfer factor, increase of CD3+ cells ,CD4+ cells (P<0.05), CD4+/CD8+ ratio (P<0. 01)and decrease of CD8+ cells(P<0.01)were found in the treatment group as compared with those before treatment, while there were no significant difference in the control group before and after the treatment. Higher IL-10 and lower INF-γ in serum were found in varruca plana group as compared with the healthy controls (P<0.05). After the treat-ment of transfer factor, decrease of IL-10 and increase INF-γ in serum (P<0. 05)were found in the treatment group as compared with those before treatment, while there were no significant difference in control group before and after the treatment. Conclusion The results reveal that immunity is im-paired in patients with varruca plana. Transfer factor can enhance the immunity of the patients. Therefore, varruca plana patients treated with transfer factor receive better effects.
