1.Half-effective target effect-site concentration of sufentanil inhibiting urethroscope insertion response when combined with propofol by target-controlled infusion in elderly patients undergoing transurethral resection of prostate

Objective To determine the half-effective target effect-site concentration (EC50 ) of sufentanil inhibiting the urethroscope insertion response when combined with propofol by target-controlled infusion (TCI) in the elderly patients undergoing transurethral resection of prostate (TURP) . Methods Twenty-two ASA Ⅰ- Ⅲ patients, aged 65 -79 yr, weighing 47-81 kg, undergoing TURP, were enrolled in the study. Anesthesia was performed with TCI of propofol and sufentanil. The target plasma concentration of propofol was 4 μg/ml. The target effect-site concentration of sufentanil was determined by up-and-down sequential trial. The initial target effect-site concentration of sufentanil was 0.3 ng/ml and the ratio of the target concentrations between the two consecutive patients was 1.1. The ECW and 95% confidence interval of sufentanil required to inhibit the response to urethroscope insertion were calculated. Results The EC50 of sufentanil required to inhibit the urethroscope insertion response was 0.23 ng/ml (95% confidence interval 0.12-0.44 ng/ml). Conclusion When combined with propofol by TCI (target plasma concentration 4 μg/ml), the EC50 of sufentanil inhibiting the response to urethroscope insertion is 0,23 ng/ml in the elderly patients undergoing TURP.
2.Clinical Significance Interpretation of Positive Sputum Samples from One Patient with Acinetobacter Bau-manni Infection after Colon Cancer Surgery

Objective:To discuss the results interpretation and clinical significance of Acinetobacter Baumanni ( AB) positive spu-tum samples .Methods:The anti-infection treatment of one patient with lung infection after colon cancer surgery in ICU was analyzed , and the results interpretation and clinical significance of AB positive sputum samples were discussed .Results:Although the culture re-sults of sputum samples were positive , the quality of sputum samples was low and the credibility was poor .The possibility of multiple drug resistance AB ( XDR-AB) screened by antibiotics selective stress was small .Meanwhile, the clinical infection symptoms were mild, and the treatment with imipenem was effective .Although the patient had high risk factors for the colonization of Baumanni infec-tion, XDR-AB was not a pathogen .Conclusion:When respiratory samples are AB positive , the quality of samples should be evaluated by smear results firstly , especially the existence of white blood cell phagocytosis or accompanying should be paid attention to , and then the possibility of AB screened by antibiotics selective stress and high risk factors for colonization should be analyzed .Finally, combined with the clinical symptoms of patients and the treatment efficacy before drug sensitivity tests , whether XDR-AB is pathogenic bacteria should be judged , and then the corresponding anti-infection treatment plan should be determined .
3.Advances in online two-dimensional liquid chromatography for natural product screening

As a treasure resource of novel drug lead compounds, how to rapidly and high-efficiently screen and isolate active components from natural products is critical. Thanks to its high resolution, high automation and flexible integration, online two-dimensional liquid chromatography has great potential for screening active ingredients from complex matrices by integrating a highly specific bio-recognition process into a two-dimensional liquid chromatography system before, on or after the column separation. This review comprehensively summarized recent developments, applications and shortcomings of online two-dimensional liquid chromatography for natural product screening from different integration modes, including pre-column, on-column and post-column screening methods.
4.Intratracheal transplantation human umbilical cord blood mesenchymal stem cells attenuate bleomycin-induced pulmonary interstitial fibrosis in mice

Objective:To explore the effect of intratracheal transplantation human umbilical cord blood mesenchymal stem cells on pulmonary interstitial fibrosis by bleomycin in mice,compare the treatment in pulmonary fibrosis of intratracheal transplantation with tail vain injection human umbilical cord blood mesenchymal stem cells.Methods:The second generation of HUCBMSCs were cultured to the fourth generation.Sixty specific pathogen free male Kunming mice were randomly divided into 4 groups:negative control group ( Cont group) ,bleomycin group( BLM group) ,HUCBMSCs transplantation groupⅠ( MⅠgroup) and HUCBMSCs transplantation groupⅡ(MⅡ group),each group 15 mice.Pulmonary fibrosis models were induced by bleomycin via intratracheal perfusion in the latter three groups.Twenty four hours after model establishment,5-bromo-2-deoxyuridine( Brdu) marked HUCBMSCs were poured in trachea in MⅠgroup ,the same were injected into tail vein in MⅡ group.At the 7th,14th,28th day,5 mice in each group were executed re-spectively.The morphological changes of the lung tissues were observed by HE staining and Masson staining.The localization and distribution of human umbilical cord blood mesenchymal stem cells were determined by the method of immunohistochemistry.The hydroxyproline contents were measured by alkali hydrolysis assay.The protein levels of transforming growth factor β-1 ( TGF-β1 ) and smooth muscle alpha-actin(α-SMA) were detected by Western blot.Results:In the two mesenchymal stem cell transplantation groups, there were Brdu marked cells at the 7th,14th,28th days in lung tissue.The alveolitis and fibrosis in lung of the two mesenchymal stem cell transplantation groups were milder than which of the the bleomycin group(P<0.05).Hydroxyproline levels in the bleomycin groups at 3 time points showed a gradually rising trend,the highest level was at 28th day( P<0.01).The protein levels of TGF-β1 andα-SMA in the lung tissues ranked as follows:BLM group>MⅠ,MⅡgroup>Cont group(P<0.05),the difference between MⅠgroup and MⅡgroup was not statistically significant(P>0.05).Conclusion:The colonization of human umbilical cord blood mesenchymal stem cells can be seen in the damaged tissue via intratracheal transplantation which can alleviate pulmonary fibrosis in mice caused by bleomycin.
5.Effect of dexmedetomidine on apoptosis in myocardial cells in rats with severe scald

Objective To evaluate the effect of dexmedetomidine on apoptosis in myocardial cells in rats with severe scald.Methods Eighteen healthy male Sprague-Dawley rats,weighing 220-280 g,were randomly divided into 3 groups (n =6 each) using a random number table:control group (group C),scald group (group B)and scald + dexmedetomidine 30 μg/kg group (group D).Thirty percent of the total body surface was shaved and then exposed to 94 ℃ water for 12 s.Rats were resuscitated with isotonic saline according to Parkland formula immediately after burn.Sham burn was produced in C group.In group D,the rats received inraperitoneal injection of dexmedetomidine 30 μg/kg immediately after burn,and the equal volume of normal saline was injected in group B.The left ventricle was removed at 12 h after burn to observe the pathological changes of myocardial tissues with light microscope and to detect the apoptosis in myocardial cells (TUNEL assay) and expression of glucose-regulated protein 78 (GRP78) and C/EBP-homologous protein (CHOP) (using Western blot).The apoptosis index was calculated.Results Compared with group C,the apoptosis index was significantly increased and the expression of GRP78 and CHOP was up-regulated in B and D groups (P < 0.05).Compared with group B,the apoptosis index was significantly decreased and the expression of GRP78 and CHOP was down-regulated in group D (P < 0.05).The pathological changes were obvious in group B and were significantly attenuated in group D.Conclusion Dexmedetomidine can protect myocardium through inhibiting endoplasmic reticulum stress-mediated apoptosis in myocardial cells in rats with severe scald.
6.Effect of Liriope platyphylla total saponin on learning, memory and metabolites in aging mice induced by D-galactose

OBJECTIVE: To investigate the effects of Liriope platyphylla total saponin (LPTS) on learning, memory, neuromediators and metabolites in aging mice induced by D-galactose. METHODS: Ninety Kunming mice were randomly divided into nine groups: normal saline (NS)-treated group, untreated group, high- (100 mg/kg), medium- (50 mg/kg) and low-dose (10 mg/kg) LPTS-treated groups, Shuxuening-treated group, jiaogulanosidi-treated group, flunarizine-treated group and vitamin E-treated group. The Kunming mice in the NS-treated group were administered with NS by intraperitoneal injection, while the aging mice in the other eight groups were administered with D-galactose by intraperitoneal injection. At the same time, the aging mice in different groups were fed with corresponding drugs for 42 days, and the aging items of the mice in different groups were measured, respectively. RESULTS: LPTS could improve the memory of aging mice induced by D-galactose, promote its body weight, and increase the thymus and spleen indexes of the aging mice. LPTS could decrease the levels of MDA and lipofuscin, inhibit MAO activity and increase SOD activity and GSH-Px level. CONCLUSION: LPTS may improve the ability of learning and memory and delay aging.
7.Meta analysis on effect of heat moisture exchangers on airway humidification in mechanically ventilated patients

Objective To assess the effectiveness and safety of heat moisture exchangers (HMEs) on the artificial airway humid-ification in the mechanically ventilated patients .Methods The related literature in the Cochrane Library ,PubMed ,Embase ,CNKI , VIP ,WanFang database and CBM from the establishment to March 2012 were retrieved ,then included the randomized controlled trial(RCT) and quasi-RCT on the effect of HMEs for the artificial airway humidification in the mechanically ventilated patients were performed the meta analysis by using RevMan 5 .1 .2 software .Results 19 RCT were included(involving 2 960 patients) , which were the random parallel controlled trials .The meta analysis results showed that HMEs could not reduce the incidence rate of ventilator-associated pneumonia(VAP)(RR=0 .78 ,95% CI 0 .61-1 .01 ,P=0 .06) ,also could not decrease the hospitalization mor-tality(RR=0 .94 ,95% CI 0 .83-1 .08 ,P=0 .40) ,the length of ICU stay(MD= -0 .32 ,95% CI -3 .13 -2 .50 ,P=0 .82) and the occurrence rate of catheter occlusion (RR=0 .65 ,95% CI 0 .22 -1 .93 ,P= 0 .44) ,the differences between the two groups had no statistical significance .But 5 RCT reported that using HMEs could reduce the patient′s costs .Conclusion HMEs can not reduce the incidence rate of VAP ,mortality ,length of ICU stay ,occurrence rate of airway occlusion ,but can cut down the patient′s cost . Due to the risk of bias existing in all included RCT and the low quality of the obtained evidences ,more strictly designed and imple-mented RCTs are needed to further verify thse evidences .
8.Secreted expression of Bacillus pumilus xylanase gene in Pichia pastoris and study on enzymatic properties.

The endo-1,4-xylanase gene from Bacillus pumilus HB030 was cloned into the Pichia pastoris expression vector, pPIC9k, the recombinant plasmid was named pHBM220. The digested recombinant plasmid pHBM220 was transformed into Pichia pastoris KM71, GS115, SMD1168, respectively. The recombinant Pichia pastoris KM71 (pHBM220), GS115 (pHBM220), SMD1168 (pHBM220) secreted functional endo-1,4-xylanase, and the enzymatic activities reached 10.80IU/mL, 11.63IU/mL, 9.68IU/mL, respectively. The temperature and pH optimum for the recombinant xylanase were 60 degrees C and pH5.5, respectively.

10.A clinical observation on Shenmai injection for treatment of myocardial injury in patients after cardiopulmonary resuscitation

Objective To observe the curative effect of Shenmai injection on patients with myocardial injury after cardiopulmonary resuscitation (CPR) and to explore its possible mechanism.Methods Sixty-two patients with cardiac arrest and respiratory arrest admitted to Qingdao Hiser Hospital from January 2010 to December 2016 were enrolled, they were randomly divided into a conventional therapy control group (30 cases) and a Shenmai treatment group (32 cases) and both groups were also treated by conventional treatment. The patients in the two groups were given basic life support of CPR and its commonly used drugs simultaneously. In Shenmai treatment group, the patients were additionally infused intravenously with 100 mL of Shenmai injection once per day. The therapeutic effect was evaluated after treatment for 7 days. On the day before treatment and 1, 3 and 7 days after treatment, the patient's blood was collected to determine the levels of myocardial injury landmarks, serum creatine kinase isoenzyme (CK-MB), cardiac troponin T (cTnT) and N-terminal B-type natriuretic peptide (NT-proBNP), and the incidences of arrhythmia of ventricular tachycardia, ventricular fibrillation (VF) and atrioventricular block were observed in the two groups; the left ventricular ejection fraction (LVEF), stroke volume (SV) and cardiac output (CO) were measured at bedside by two-dimensional echocardiography for the patients of two groups.Results In conventional therapy control group, the levels of CK-MB, cTnT showed a temporary increase after 1 day of treatment, after 3 days of treatment, CK-MB and cTnT were significantly lower than those before treatment, and reached the lowest levels after 7 days of treatment; the level of NT-proBNP after treatment showed a continuous decrease, the levels of LVEF, SV, CO were persistently increased after treatment; in Shenmai treatment group, the levels of CK-MB, NT-proBNP were decreased continuously after treatment, cTnT was firstly increase and then decrease, and reached to the lowest revels after 7 days of treatment while the levels of LVEF, SV and CO were firstly decreased and then increased gradually, and reached to the highest levels after 7 days of treatment; compared with those of conventional therapy control group, the levels of CK-MB, cTnT, NT-proBNP in Shenmai treatment group were significantly lower after 3 and 7 days of treatment [3 days of treatment: CK-MB (U/L)was 51±1 vs. 82±3, cTnT (μg/L) was 2.5±0.3 vs. 3.9±0.2, NT-proBNP (ng/L) was 5 810±103 vs. 15 965±152;7 days of treatment: CK-MB (U/L) was 27±2 vs. 56±3, cTnT (μg/L) was 1.2±0.3 vs. 2.9±0.2, NT-proBNP (ng/L) was 2 834±123 vs. 4 832±76], while LVEF, SV and CO were significantly higher than those in conventional therapy control group [3 days of treatment: LVEF was 0.47±0.03 vs. 0.45±0.02, SV (mL) was 45±5 vs. 39±4, CO (L/min) was 3.7±0.2 vs. 3.6±0.2; 7 days of treatment: LVEF was 0.59±0.02 vs. 0.51±0.03, SV (mL) was 55±4 vs. 45±2, CO (L/min) was 5.3±0.3 vs. 4.6±0.4, all P < 0.05]. After CPR, arrhythmia developed in the patients of two groups, and compared with that before treatment, there was no statistical significant difference in the incidence of arrhythmia after 1 day of treatment in Shenmai treatment group (all P > 0.05); the incidence of arrhythmia was decreased significantly after 3 and 7 days of treatment compared with those before treatment, reached to the lowest level on the 7th day of treatment, and the degree of decrease of incidence of arrhythmia in Shenmai treatment group was more obvious than those of the conventional therapy control group [ventricular tachycardia: 9.4% (3/32) vs. 20.0% (6/30), VF: 9.4% (3/32) vs. 20.0 % (6/30), atrial ventricular block: 18.8% (6/32) vs. 36.7% (11/30), all P < 0.05]. Conclusions Shenmai injection has certain protective effect on injured myocardium in patients undergoing CPR, the mechanism is possibly related to reducing the levels of CK-MB,cTnT, NT-proBNP and further improving the LVEF, SV and CO.
