1.Protective Effect of Naoli Zhibao on Mice with Learning and Memory Impairment

Objective To observe the protective effect of Naoli Zhibao(NZ) on mice with learning and memory impairment.Methods (1) Thirty-two NIH mice were randomized into pseudo-operation group,model group,and low-and high-dose NZ groups(at the dose of 4 and 8 g?kg-1?d-1 respectively).Except the pseudo-operation group,the mice in other groups received the operation of cerebral ischemia/reperfusion.After treatment for 10 days,the learning and memory abilities of the mice were observed by step-down test,and the contents of malondialdehyde(MDA) and nitric oxide(NO) as well as the activities of superoxide dismustase(SOD) and glutathione peroxidase(GSH-Px) were detected.Meanwhile,the free amino acid content in mice cerebral cortex was measured with high performance liquid chromatography.(2) Forty NIH mice aged 9 months were randomized into normal control group,mimetic aging group,and low-and high-dose NZ groups(at the dose of 4 and 8 g?kg-1?d-1 respectively).The mice models of mimetic aging were induced by intraperitoneal injection of D-galactose 0.5mL for 6 weeks,and the mice in NZ groups were also given gastric gavage of NZ for 6 weeks.After treatment,the learning and memory abilities of the mice were observed by step-down test,and the contents of epinephrine(E),norepinephrine(NE) and tryptophan(TP) in mice cerebral cortex were measured.Results (1) NZ significantly improved the memory ability of mice with cerebral ischemia/reperfusion injury(P
2.Nutritional status investigation and analysis of 342 cases of elderly hospitalized patients

Objective To investigate the nutritional state in elderly hospitalized patients(≥70 years old)and provide clinical evi-dence for nutritional support in elderly people .Methods From June to December 2012 ,342 elderly hospitalized patients(≥70 years old)in Daping Hospital ,the Third Military Medical University were included .All of these patients were originally screened by resi-dent doctor ,and were revaluated by dietitians in the Department of Nutrition for the nutritional status .The revaluation was conduc-ted by Nutritional Risk Screening 2002(NRS 2002) ,which recommended by ESPEN ,and combined with the laboratory examina-tion .Results In 342 cases ,the nutritional risk rate was 96 .49% ,the incidence of malnutrition was 28 .79% ,and the incidence of a-nemia was 59 .24% .The incidence of low total serum protein ,low albumin ,and low prealbumin were 51 .40% 、93 .18% and 85 . 30% ,respectively .Conclusion Elderly hospitalized patients(≥70 years old)have high nutritional risk ,high incidence of malnutri-tion ,anemia ,and low blood albumin .Clinical nutritional support is necessary for bad nutritional status elderly patients .
3.Effects of Aurora A silence by RNA interference on the apoptosis and proliferation of glioma cells

Objective To investigate the inhibitory effect of RNA interference on the expression of Aurora A in U251 cells, and the influence on proliferation and apoptosis of U251 cells. Methods The siRNA specific for Aurora A was synthesized and transfected into U251 cells in vitro. Aurora A mRNA expression and protein content were detected by RT-PCR and Western blotting respectively. The cell proliferation and apoptosis were observed by methyl thiazolyl tetrazolium(MTT) and flow cytometry(FCM). Transmission electron microscope was used to observe the ultrastructural changes of U251 cells. Results After transfection, the expression level of Aurora A mRNA was significantly decreased(P<0.01), and the protein content of Aurora A was also obviously reduced. The inhibitory rate of cell proliferation reached up to 67.57% 72 hours after transfection, which was significantly higer than that of normal control group(P<0.01). The apoptosis rate of U251 cells was significantly increased from (3.69±0.87)% to (15.34±2.16)% (P<0.01). Under the transmission electron microscope, it was observed that the U251 cells showed typical morphologic changes of apoptosis after transfection, such as karyopyknosis, chromatin condensation and margination, intracytoplasmic vacuoles formed, and apoptotic bodies formed. Conclusion The expression of Aurora A gene can be inhibited by siRNA successfully, and it results in the suppression of cell growth and induce apoptosis of human glioma cells in vitro. Aurora A may become a new target for the gene therapy of gliomas.
4.Clinical study on postoperative anti-reflux function of vertical esophagogastic valve-plasty anastomosis for cardiac cancer

Objective To appraise the postoperative anti-reflux function of vertical esophagogastic valve-plasty anastomosis for cardiac cancer.Methods Forty patients with cardiac cancer were randomly divided into study group and surgery control group,with 20 patients in each group.The study group underwent vertical esopagogastric valve-plasty anastomosis,while the surgery control group underwent conventional esophagogastrostomy.Ten healthy volunteers were recruited as normal control group.A 24-hour esophageal pH monitoring and endoscopy check-up was carried out in all experimental subjects at 90 days postoperatively.Results All of the pH monitoring indexes in study and surgery control groups were higher than those in normal control group(P
5.Application of PBL in cardiovascular intervention training of continuing education

Objective To compare the application effect between problem-based learning (PBL) and traditional teaching in cardiovascular intervention . Methods 39 training physicians were divided randomly into the PBL group (n=20) and control group (n=19). The control group was trained with the tradi-tional teaching method while PBL group used PBL seven step method, namely they were trained through the process of clarifying unfamiliar terms—defining the problem—brainstorming—restructuring problem—defining learning goals—collecting information, personal learning, information sharing, and group discus-sion. After the end of the training, the two groups were tested by using the unified test questions and skills test, and the questionnaire survey of teaching satisfaction. SPSS 18.0 was used to do line t test or chi square test to the data of both groups. Results PBL group training physicians' cardiovascular intervention oper-ation [(30.07±1.67) vs. (28.54±1.98), P=0.036], their comprehensive analysis of clinical cases, [(34.47± 1.77) vs. (32.08 ±1.80), P=0.002], and the total score [(86.47 ±2.75) vs. (82.23 ±3.63), P=0.002], were better than the control group, and the difference was statistically significant. The survey results showed that the PBL group's evaluation on how the teaching methods stimulate the training physicians' interest in learning, enhance their ability of independent thinking and cultivate their teamwork ability, improve their language expression and clinical thinking and other aspects was higher than the control group (P<0.05).
Conclusion Compared with the traditional teaching, the application effect of PBL in the training of car-diovascular intervention can better exert training physicians' subjective initiative and improve the teaching effect.
6.Posterior short-segment instrumentation without fusion for treatment of thoracolumbar compression fractures

Objective To investigate the clinical effect of posterior short-segment pedicle screw reduction and fixation without fusion in treatment of thoracolumbar mono-segmental compression fractures.Methods Thirty cases of thoracolumbar mono-segmental compression fractures admitted from January 2009 to February 2010 were assigned to single posterior pedicle screw fixation (screw group,n =15) and posterior pedicle screw fixation with posterolateral fusion (fusion group,n =15) according to random number table.Clinical results in the two groups were assessed based on Cobb' s angle,anterior vertebral body height ratio (%) and Oswestry disability index (ODI) before operation,after operation,prior to the removal of implant and at the latest follow-up.Results All the cases were followed up for average 24 months.Both operation time and blood loss were less in screw group than in fusion group [(76.58 ±12.67) min vs (116.29 ± 17.45) min,P < 0.01 ; (287.54 ± 30.76) ml vs (480.34 ± 100.54) ml,P <0.01],whereas there were no statistical differences between the two groups in aspects of Cobb' s angle and anterior vertebral body height ratio before and after operation,prior to the removal of implant as well as at the latest follow-up.Moreover,no statistical difference of ODI was noted between the two groups prior to the removal of implant and at the latest follow-up.Conclusion Posterior pedicle screw fixation without bone grafting achieves similar effects with pedicle screw fixation with bone grafting.
7.Diagnostic value of endobronchial ultrasound-guided transbronchial needle aspiration in elderly patients with bronchogenic carcinoma

Objective To evaluate the diagnostic value and safety of endobronchial ultrasoundguided transbronchial needle aspiration (EBUS-TBNA) in elderly patients with bronchogenic carcinoma.Methods From August,2013 and July,2014,31 patients aged 65 years and over with space-occupying mass and enlarged mediastinal lymph nodes detected by CT or positron emission tomography-computed tomography (PET-CT) underwent EBUS-TBNA.Rapid onsite cytology evaluation was not performed.Results There were 26 males and 5 females in this study,aged 65-77 years (70.1 years old on average).In 31 patients,70 samples were obtained from lymph nodes (LNs) and 4 samples were obtained from intrapulmonary lesions.29 cases were diagnosed as lung cancer,and 2 cases had false-negative diagnoses.The sensitivity and specificity rates of EBUS-guided TBNA method were 93.6% and 100.0%,respectively.No major complications were observed in this series.Conclusions EBUS-TBNA is an safe and effective method in diagnosing bronchogenic carcinoma in elderly patients.
8.The analysis on short-term clinical efficacy of In-Space after decompressive laminectomy for treatment of degenerative lumbar spinal stenosis with vertebral instability

Objective To investigate the difference of short-term clinical efficacy between decompressive laminectomy into In-Space and simple decompressive laminectomy for treatment of lumbar spinal stenosis with vertebral instability.Methods Thirty-three patients with lumbar spinal stenosis with vertebral instability admired from May 2009 to July 2010,were divided into two groups by random number table.Group A of 16 cases was treated with laminectomy decompression and placement In-Space,group B of 17 cases was treated with laminectomy decompression.Lumbar anteroposterior,lateral and flexion-extension X-ray films,preoperatively,and the follow-up were used to measure anterior and posterior disc height,foraminal height,segmental lordotic angle at surgical level.Using Oswestry disability index (ODI) and the visual analogue scale (VAS) to evaluate the clinical efficacy.Results All patients were followed up for (13.20 ± 2.91 ) months (range 6 to 21 months).The anterior disc height after operation of group A was slightly decreased compared with the preoperative(P> 0.05 ),the posterior disc height at 1 day after operation and foraminal height after operation of group A were significantly increased compared with the preoperative (P< 0.05).The anterior and posterior disc height,foraminal height of group B at 1 day,1 month,3 months after operation were no significantly different compared with the preoperative (P > 0.05 ),at 6 months after operation and the end of follow-up were significantly decreased compared with the preoperative or 1 day after operation (P < 0.05 ).Activity of lumbar vertebra by preoperative 9.86° ± 1.90° decreased to the end of followup 5.60° ± 2.02°in group A,while activity of lumbar vertebra by preoperative 9.89° ± 2.00°increased to the end of follow-up 10.76° ± 3.14° in group B.At the end of follow-up,lumbar back pain VAS,ODI score [ (2.02 ± 1.98 ),( 20.18 ± 18.80) scores ] of group A were significantly lower than those of group B [ (4.15 ±2.36),(30.39 ± 16.62 ) scores ],the differences were statistically significant (P < 0.05 ).No patient suffered In-Space loosening,fracture and emerge.Conclusion The operation of In-Space can maintain spinal mobility and stability as well as avoiding lumbar vertebral instability,and its short-term efficacy is satisfactory.
