1.Evaluation of Dual-energy Digital Subtraction Chest Radiography in Detection of Small Pulmonary Nodules

Objective To investigate the value of dual-energy digital subtraction chest radiography in detection of small pulmonarynodules.Methods CT scanning,dual-energy digital subtraction chest radiographs and routine digital chest radiographs were obtainedrespectively from 27 consecutive oncology patients with pulmonary metastasis.The image quality of these two kinds of DR by quality control phantom of Kodak was compared. The images of dual-energy digital subtract radiographs and the routine digital radiography were analysedby two radiologists in biblind method and the detecting rate of pulmonary metastasis by both DR was compared based on the results CTscanning. Results The quality of dual-energy digital subtraction chest radiographs is equal to the routine one except the definition. Objective To construct the vector that expresses the fusion protein of HIV-Tat protein and red fluorescent protein(mCherry) in mammalian cells,and observe by fluorescence microscopy the intracellular transduction and localization of recombinant protein in cells,in order to obtain a useful tool for the study of the uptake mechanism and intracellular localization of HIV-TAT.Methods With the designed primer coding mCherry sequence,the mCherry gene was amplified by PCR with the vector pmCherry-C2 as template,and inserted into vector pET14b-His-TAT to construct the expression vector pET14b-His-TAT-mCherry.The constructed vector was then transformed into E.coli BL21(DE3),which had been identified by PCR and double digested with restriction endonuclease,followed by sequencing.After IPTG induction,the recombinant protein of His-TAT-mCherry was lyzed and analyzed with SDS-PAGE.Purified His-TAT-mCherry recombinant protein was added to Hela cells and the fluorescence was observed to evaluate the transduction efficiency.Results The results of identification by PCR,digestion with restriction endonuclease and sequencing indicated that the vector His-TAT-mCherry was correctly constructed.His-TAT-mCherry fusion protein was expressed in mammalian Hela cell line and purified successfully,and the fusion protein showed cellular transduction activity.It was found by fluorescence microscopy that the red fluorescence protein located mainly over the cytoplasm,and also the membrane to some extent.Conclusion The expression vector is successfully constructed for HIV-TAT labeled with mCherry sequence.Effective expression and purification of this fusion protein is achieved.It has been observed that the constructed vector may be expressed in mammalian Hela cell under active condition.Thus,it might be useful in the study of uptake mechanism and intracellular localization of HIV-TAT.
3.Characteristics of maxillary morphology in unilateral cleft lip and palate patients and skeletal class Ⅲ patients compared to normal subjects

Objective To investigate features of maxillary morphology in unilateral cleft lip and palate (UCLP) with maxillary retrusion and the dissimilarities of retruded maxilla.Methods Craniofacial measurements were done in 32 UCLP adult patients with maxillary retrusion (GC), 24 adult patients in class Ⅲ (GⅢ), and 32 healthy adults (GN).The CT slice data was reconstructed into a 3D video model and measured by Mimics 16.01.Results The maxillary volume (GM) and the volume composed of maxilla and maxillary sinuses (GT) were significantly smaller (P<0.05) in GC.The anterior and posterior parts of the maxillary length (A1-P3M⊥CP and P3M-P6M⊥CP) and overall maxillary length(A1-P6M⊥CP)at the dental level were all significantly reduced (P<0.05).There was no significant difference of the distances of A1⊥CP between the GC and GⅢ groups, while the P3M-CP and P6M-CP in the GⅢ group were significantly shorter (all P<0.05).The anterior and overall maxillary length at the dental level (A1-P3M⊥CP and A1-P6M⊥CP) in the GC group was significantly smaller than that in the GⅢ group (all P<0.05).Conclusions The decreased prominence of maxillary complex could be caused by the shortened maxillary length in UCLP patients;the posterior position of the maxilla is more obvious than that of GC group in class Ⅲ patients.
4.Impact of margin size on patients' long-term effect after nephron sparing surgery for early localized renal cell carcinoma

Objective To explore the safety and efficacy of small margin in nephron sparing surgery for early localized renal cell carcinoma (RCC). Methods A total of 325 cases of RCC with normal contralateral kidney and staged as Tla were retrospectivly studied.According to the margin size,125 cases were with surgical margin ≤ 5 mm (group ≤ 5 mm),102 cases with margin 6-9 mm (group 6-9 mm) and 98 cases with margin > 10 mm (group > 10 mm).The margin size and status was pathologically evaluated and clinical results including local recurrence,distant metastasis and overall survival rate were followed up and comparatively analyzed. Results None of the patients had positive surgical margins.The mean and median margin sizes were 2.2 and 2.0 mm for group ≤ 5 mm,6.7 and 6.0 mm for group 6-9 mm and 11.8 and 12.0 mm for group > 10 mm.The difference was statistically significant (P=0.025).The mean and median follow-up time for all the patients were 79 and 83 months (range 15-132 months),with 69 and 73 months (range,15-130 months) for group ≤ 5 mm,83 and 86 (range,17-132 months) for group 6-9mm and 82 and 82 (range 60-103 months) for group > 10 mm.Three patients in group ≤ 5 mm,5 in group 6-9 mm and 2 in group > 10 mm died of no-cancer related disease during follow-up.One patient in group ≤ 5 mm (0.74%) experienced ectopic recurrence in the same kidney and one in group 6-9 mm was detected local recurrence in situ (0.98%).No distant metastasis was detected in all the patients.The overall 5-year survival rate for patients in groups ≤ 5mm,6-9 mm and > 10 mm were 99.2%,99.0% and 98.0%,respectively.(Kaplan-Meier survival analysis,Log Rank,x2 =1.511,P=0.470). Conclusions Small margin in nephron sparing surgery is safe and effective in treating RCC with stage T1a,which provides excellent renal function preservation,favorable long-term progression-free survival rate,and is not associated with an increased risk of local recurrence.
5.Screening of Coronary Artery Calcification Using Digital Radiograhpy with Dual-energy Subtraction Chest:Compared with Multi-detector Row CT

Objective To evaluate the diagnostic accuracy of dual-energy subtraction chest digital radiography(DR) for detecting coronary artery calcification as compared with multi-detector row CT(MDCT).Methods 35 patients underwent dual-energy subtraction chest DR and ECG-gated MDCT for detection of coronary artery calcifications.The radiation dose of both DR and CT were noted respectively.Two senior and two junior radiologists reviewed the radiogram and assessed the calcifications in LAD,LCX and RCA totaling 105 vessels.ROC curve plotting were used for evaluation with CAC calculated from MDCT as the gold standard.Paired t test was calculated to compare the different radiation dose between DR and CT.Results 27.6% vessels(29/105) showed calcification on DR.The positive incidence in LAD was 48.5%(17/35),LCX 22.8%(8/35),and RCA 11.4%(4/35).The ROC area was 0.866,0.854,0.725,0.642 respectively,with a CAC score of more than 300.The average radiation dose was 0.469?0.22 mGy for DR,and 12.29?1.40 mGy for MDCT,showing significant differences.Conclusion Chest DR with dual energy subtraction can be used for screening of coronary artery calcification with relatively higher sensitivity for LAD.

7.Analysis of enteral nutrition tolerance in ICU patients and nursing strategy

Objective To explore tolerance-related factors of enteral nutrition(EN) by nasogastric (NG) tube in ICU patients, and to provide several nursing strategies for improving the patients' tolerance. Methods 83 patients using EN with NG tube in ICU were collected.Tolerance-related factors of EN were analyzed by multivariate Logistic regression analysis. Results Among the 83 patients, 36 patients (43.3%) could tolerate very well, 47 patients (56.6%) couldn't tolerate.After treatment 27 patients' tolerance was fair, the other was poor.Multivariate Logistic regression analysis showed that age, APACHE-Ⅱ score, the time of beginning EN, albumin level, drug administration by NG tube or not were the major factors affecting tolerance of NG tube feeding. Conclusions Patients' feeding with NG tube was well-tolerated in ICU.Besides the factors from patients as the severity of disease status and level of serum albumin,the time of beginning EN, drug administration by NG tube were significantly correlated with tolerance.
8.Study of thyroid dysfunction in the first half of pregnancy and gestational diabetes mellitus

Objective To explore the influence of thyroid dysfunction in the first half pregnancy (≤20 gestational weeks) on gestational diabetes. Methords By adopting the method of prospective intervention study, general data of pregnant-women in the care clinics of our hospital were collected. Thyroid function was detected and evaluated by ATA criteria. All cases were divided into two groups: normal thyroid function group and thyroid dysfunction group. The thyroid dysfunction group was intervened. 75 g OGTT were conducted in all cases in 24-28 gestational-weeks. GDM was diagnosed by IADPSG diagnostic criteria. GDM prevalence was compared between normal thyroid function group and abnormal thyroid function groups. Results 1 062 cases of pregnant women were collected. 857 cases of pregnant women were normal, while 14 cases were subclinical hyperthyroidism (1.32%), 164 cases were subclinical hypothyroidism (15.4%), 22 cases were hypothyroxinemia (2.07%). Compared with normal thyroid function, prevalence rate of GDM was the highest in hypothyroxinemia group (54.5% vs 27.8%). There was no significant difference between subclinical hypothyroidism group and normal thyroid function group. Conclusion Thyroid dysfunction in the first half pregnancy is associated with GDM , Screening and treatment of pregnant thyroid dysfunction should be attached.
9.Effect of nucleos (t)ide analog antiviral treatment on the pathological differentiation and prognosis of ;hepatitis B virus-related hepatocellular carcinoma

Objective To explore the effect of nucleos(t)ide analog (NA)antiviral treatment on the pathological differentiation of hepatitis B virus (HBV)-related hepatocellular carcinoma (HCC)and the prognostic factors of HCC.Methods Totally 127 patients with HBV-related HCC who were hospitalized and received partial hepatectomy in First Affiliated Hospital of Harbin Medical University from March 2007 to November 2013 were included in this study.Sixteen cases received antiviral treatment before operation and the remaining 111 cases had no history of NA treatment.The differences of histopathological grading were compared between the two groups.Twenty-nine patients received antiviral treatment for the first time after surgery,and the rest 82 patients did not.All these patients were followed up for survival and recurrence.Multivariate analysis was used to explore the prognostic factors for HCC.The categorical variables were analyzed byχ2 test or Fisher exact test.Survival rate was compared with Log-rank test. Univariate or multivariate Cox regression analysis was used to explore the related factors of survival. Results The proportions of well-,moderately- or poorly-differentiated HCC in patients with antiviral treatment before surgery were 18.75 %,68.75 % and 12.5 %,respectively.Whereas the proportions in those without treatment were 16.22%,66.67% and 17.11 %,respectively.There was no significant difference in histopathological grading of HCC between the two groups (χ2=0.224,P =0.885 ).The overall median survival time was 39 months.The 6-month,1-and 2-year survival rates were 91 .7%, 77.5 % and 59.3%,respectively.The 6-month,1- and 2-year survival rate of postoperative antiviral treatment were 96.3%,92.4% and 78.5 %,respectively,which were significantly higher than those of no antiviral treatment group (85 .9%,70.0% and 48.5 %,respectively;χ2= 6.967,P = 0.008 ). Univariate analysis showed that tumor number,size,portal vein transfer,AFP level,postoperative antiviral treatment,histopathological grading,TNM staging,BCLC staging,γ-GT and PTA were prognostic factors for postoperative HCC survival.Multivariate analysis showed that AFP level (HR=1 , 95 %CI :1 .0004—1 .002,P =0.004),postoperative antiviral treatment (HR =0.38,95 %CI :0.38—0.15 ,P =0.04)and BCLC stage (B vs A:HR=1 .55 ,95 %CI :0.76—3.18;C vs A:HR=3.63,95 %CI :1 .31 —10.09,P =0.04)were independent prognostic factors.Conclusions Preoperative antiviral treatment has no impact on the histopathological grading of HCC. BCLC stage, AFP level and postoperative antiviral treatment are independent prognostic factors for HBV-related HCC.

ObjectiveTo investigate the radiosensitizing effect of 3-(5'-hydroxy-2'-furyl)-1-benzyl indazole ( YC-1 ) on hypoxic human adenocarcinoma cell line A549.MethodsMTT assay was used to test the inhibitory effect of YC-1 on proliferation of A549 cells.Clonogenic assay was performed to determine the radiosensitizing effect of YC-1 on hopxic A549 cells.Single-hit multi-target model was used to plot survival curve and calculate sensitization enhancement ratio (SER).The cell cycle and apoptosis were measured by flow cytometry.ResultsThe proliferation of A549 cells was inhibited by YC-1 in a time-dose-dependent manner.In normoxic and hypoxic cells,the IC20 was 16.7 μmol/L and 39.2 μmol/L at 24 h,respectively.In the group of hypoxia plus YC-1,SERD0 and SERDq were 1.11 and 1.26,respectively.In hypoxia,YC-1combined with 2 Gy irradiation could induce cell apoptosis and prolong G2 + M phase arrest ( ( 30.17 ±1.21 )% ∶ ( 15.44 ±0.96) %,P =0.000; (21.56 ±0.47 )% ∶ (6.16 ±0.16)%,P =0.000).Concinsions YC-1 could enhance the radiosensitivity of hypoxic A549 cells.
