1.The influence of three level quality control nursing management model on the quality of nursing service in the department of general surgery

Objective To analyze effect of three level nursing quality control management model on the quality of the nursing service in the department of general surgery.Methods Analysis in the implementation of nurs-ing quality control before and after nursing quality and the improvement of the examination results of nurses situation in general surgery department of our hospital were compared between the 2012 -2013 without the implementation of three level quality control mode and 2013 -2014 of establishment of the three level nursing quality control manage-ment mode,which was set the nursing quality monitoring nurses,quality monitoring group leader and quality monito-ring nurses in three level nursing quality control system.Results After the implementation of quality control,the quality of basic nursing score[(94.02 ±2.01)points],ward management quality score[(94.13 ±3.04)points],first aid management quality score[(96.22 ±4.06)points]and disinfection and isolation,and the quality score[(95.13 ± 3.04)points]and quality control of basic nursing quality score[(8.121 ±3.31 )points],ward management quality score[(81.35 ±2.34)points],emergency management quality score [(91.42 ±4.36 )points]and disinfection [(81.74 ±3.44)points]degrees and isolation quality scores were significantly higher than those of before three level quality control nursing management model(t =33.07,33.32,8.06,29.17,all P <0.05).Nursing quality control of nursing personnel the technical operation of the average score [(96.35 ±1.03)points],theory test average score [(95.73 ±0.98)points],average and quality control of nursing personnel and technical operation [(89.42 ± 2.49)points],theory test average score[(90.36 ±1.79)points]were obviously higher.The results had statistical significance (t =25.72,26.32,all P <0.05).Conclusion Three level nursing quality control management model established in the general surgery department had remarkable effect on the quality of the nursing service,which is safe and reliable,and improving the quality of nursing.
2.Effect of continuous quality improvement nursing on grasping disease knowledge and satisfaction of patients with thyroid tumors

Objective To investigate the effect of continuous quality improvement nursing on grasping disease knowledge and satisfaction of patients with thyroid tumors.Methods 86 cases of patients with thyroid cancer were enrolled.They were devided into the observation group(42 cases)and the control group(46 cases)according to the date of admission.Patients in control group were treated with routine care,while patients in the observation group were treated with continuous quality improvement nursing on the basis of the control group.The effects of nursing in the two groups were compared and analysized.Results After intervention,the scores of cognitive thyroid tumors,daily preventive health care,thyroid cancer surgery and surgical considerations in the observation group were higher than those in the control group,theses differences were statistically significant between the two groups (t =9.806,10.271, 9.924,12.053,all P <0.05).The rate of satisfaction in the observation group was significantly higther than that in the control group(92.86%/75.00%),theses difference was statistically significant between the two groups (χ2 =9.025,P <0.05).Conclusion Continuous quality improvement nursing can effectively improve the diseases -related knowledge in patients with thyroid benign tumor,which contributes to the smooth operation and improves surgical results.
3.Cloning,expression and identification of SARS-CoV S1 gene in yeast p.methanolica

Objective To obtain high-yield and easy-purification severe acute respiratory syndrome coronavirus(SARS-CoV) S1 protein with biological activity and to study the activity of S1 protein and its antibody further.Methods SARS-CoV S1 gene was inserted into yeast expression vector pMET?A by ligation reaction.The recombinant plasmid was verified by enzyme digestion and sequencing,followed by being transformed into yeast host strain PMAD11 with electroporation.After induced with methanol,the S1 gene expression was verified with overlay assay and Western blotting.Results The positive clones of S1 gene into pMET?A were approved by restriction enzyme digestion and sequencing.The expression of S1 protein was confirmed subsequently by overlay assay and Western blotting.Conclusion SARS-CoV S1 gene has been cloned and expressed in yeast p.methanolica,which can provide experimental data for next study on the activity of this protein and its antibody during SARS-CoV infection.
5.Cloning and expression of human DcR3 molecule in COS-7 cells

Objective To clone ORF of DcR3 gene and insert it into eukaryotic expression vector and express it in COS-7 cells. Methods Encoding sequence of human DcR3 gene was cloned by PCR and sequenced. The sequenced ORF was cloned into eukaryotic expression vector pAAV-IRES-hrGFP to construct recombinant plasmid. COS-7 cells were transfected with recombinant plasmid by lipofectamine2000. Expression of recombinant DcR3 gene was verified by Western blotting and confocal microscopy. Results A 1 000-bp gene segment was obtained by PCR and inserted into pAAV-IRES-hrGFP to construct recombinant plasmid. The gene segment was proved to be encoding sequence of human DcR3 gene by sequencing. DcR3 expression in COS-7 cells was verified by Western blotting and confocal microscopy. Conclusion DcR3 gene was successfully cloned and expressed in COS-7 cells.
6.Sequential therapy of BTD and MPT regimen for the newly-diagnosed multiple myeloma patients no eligible for bone marrow transplantation

Objective To retrospectively analyze the outcomes and adverse effects of sequential therapy of BTD and MPT regimen for the newly-diagnosed multiple myeloma patients no eligible for high dose chemotherapy and stem cell transplantation. Methods Thirty-six patients were involved in this study and the patients were treated with tandem therapy of BTD and MPT regimen. The patients were treated with BTD regimen as induced therapy no less than 2 cycles. When the patients got PR or above PR,they were treated with MPT regimen as consolidation therapy which was no less than 2 cycles. Then,the patients who achieved PR or partial PR were received MPT chemotherapy regimen as consequent treatment. After that,low dose thalidomide was used as maintenance therapy. The outcomes and adverse effects were retrospectively evaluated. Results Thirty-six patients were treated with BTD regimen as induced therapy. The results were that 7 patients (19.4 %) achieved CR,8 (22.2 %) VGPR,14 (38.9 %) PR and the OR rate was 80.6 %. The patients (n=29) who achieved no less than PR was treated with MPT regimen as consequent therapy. The results were that four patients were in progression and the others were stable. Twenty-five patients were treated with low dose thalidomide as maintenance therapy. The median progression-free survival (PFS) did not reached yet until last follow-up (median follow-up time was 16.5 months). One-year overall survival rate was expected 86.0 % and 3-year expected overall survival rate was 77.0 %. The main regimen-associated toxicities included thrombocytopenia,peripheral neuropathy (PN),Herpes Zoster,gastrointestinal symptoms,anemia,neutropenia,constipation,fatigue,rash and so on. The incidence of grade 3 and 4 adverse events was low. Conclusion Sequential therapy of BTD and MPT regimen can be used as the front-line therapy for the newly-diagnosed multiple myeloma patients no eligible for high dose chemotherapy and stem cell transplantation.
8.Relationship Between Lysophosphatidic Acid Levels in Cerebrospinal Fluid and Cerebral Vasospasm After Subarachnoid Hemorrhage

Objective: To observe the characteristics of dynamic changes of lysophosphatidic acid (LPA) levels in cerebrospinal fluid (CSF) in patients with subarachnoid hemorrhage (SAH) and its relationship with cerebral vasospasm (CVS) and to explore the pathogenesis of CVS. Methods: Sixty-seven patients with SAH diagnozed by clinical and accessory examinations were selected. The LPA levels in CSF were measured at 24 hours, day 7,14, and 28 respectively after the onset of symptoms,and they were compared with a control group. The correlation between LPA levels and CVS on the time course was also observed at the same time. Results: Of the 67 patients with SAH, a total of 29 patients (43.3%) occurred CVS, the average time of occurrence was 6. 6 days. There was no significant difference between the LPA levels in CSF in patients with SAH and the control group at 24 hours after the onset of symptoms; they were significantly higher than the control group at day 7 (P <0. 001); they were significantly higher than the control group at day 14 (P < 0. 001), but they were significantly lower than those at day 7 (P < 0. 01); they decreased to baseline at day 28, and there was significant difference compared with the control group. There was no significant difference between the LPA levels in the CVS group and those in the non-CVS group at 24 hours, they were significantly higher than those in the non-CVS group at day 7 (P <0. 001), they were still significantly higher than those in the non-CVS group at day 14 (P <0. 01); and there was no significant difference between the 2 groups at day 28. Conclusions: The LPA levels in CSF in patients with SAH increased significantly from day 7 to day 14 after the onset of symptoms, and they had obvious association with CVS on the time course. The detection of the LPA levels in CSF may have important significance in predicting the occurrence of CVS.
9.Expression of a testis-specific gene 1700001022RIK in mice and its bioinformatic analysis.

OBJECTIVETo identify the expression characteristics of the 1700001022RIK (RIKEN cDNA 1700001022) gene in mice and explore its function by bioinformatic analysis.
METHODSUsing the expression profile of gene microarray, we detected the expression of a new testis-specific gene, 1700001022RIK, in mice. We analyzed its expression characteristics in the testis tissue and their changes in different developmental stages of the testis by RT-PCR, real-time RT-PCR, Western blot, and immunohistochemistry. We performed bioinformatic analysis using a bioinformatic software.
RESULTSThe 1700001022RIK gene was specifically expressed in the mouse testis in an age-dependent manner, most highly in the adult mice. The 1700001022RIK protein was mainly expressed in the spermatogonia, spermatocytes, and round spermatids of the adult mice. Bioinformatic analysis showed that the 1700001022RIK protein amino acid sequence had a high similarity in human and mice, which indicated that this gene was highly conserved in mammals.
CONCLUSION1700001022RIK is a testis-specific gene mainly expressed in the spermatogonia, spermatocytes, and round spermatids of seminiferous tubules, which might be involved in the regulation of spermatogenesis.

10.Applied Value of Monitoring Serum Hepcidin in Differential Diagnosis of Infection versus Tumor Fevers

The applied value of serum hepcidin in differential diagnosis of infection fevers versus tumor fevers was explored.A total of 432 fever patients were selected according to the domestic fever of unknown origin (FUO) diagnostic criteria from our hospital between June 2010 and November 2013.Venous blood samples were taken on the day 1,5,10 after admission.The infection group (98 cases) and the tumor group (50 cases) were set up based on the clinical and laboratory findings.ELISA was used to determine the serum hepcidin and IL-6 levels.SPSS 13.0 was used for statistical analysis.Hepcidin showed obvious descending trend on the 10th day in both the bacterial infection group (66 cases) and the virus infection group (32 cases),and the descending trend was similar to that of inflammatory indexes such as procalcitonin (PCT),hypersensitive C-reactive protein (h-CRP),erythrocyte sedimentation rate (ESR),white blood cell (WBC),and ferritin.Serum hepcidin showed no obvious differences in the tumor group on the day 1,5,10 after admission.In the infection groups,serum hepcidin was positively correlated with IL-6 (r=0.687,P=0.000) and CRP (r=0.487,P=0.026),but had a poor correlation with blood sedimentation,ferritin,PCT and WBC (P>0.05).Monitoring dynamic changes of hepcidin and related inflammatory factors in patients with fever is expected to be used for clinical identification of infection fever and tumor fever.
