1.Current development and clinical application of pancreatic stents

BACKGROUND:In recent years, the morbidity of chronic pancreatitis has been increasing dramaticaly with lifestyle changes. Fortunately, endoscopic pancreatic stenting for patients with chronic pancreatitis shows favorable clinical prospects.OBJECTIVE:To introduce the advantages and disadvantages of various stent materials and to review the development and clinical application of pancreatic stents.METHODS: With the key words of chronic pancreatitis, pancreatic stent in Chinese and English, respectively, a computer-based search was performed for articles published in CNKI and PubMed databases from January 2005 to November 2016. After the initial screening, articles related to endoscopic pancreatic stenting usedin vitro,in vivo or in clinic were included.RESULTS AND CONCLUSION: Endoscopic pancreatic stenting has become a recognized treatment for chronic pancreatitis in recent years. Therefore, the development and application of the materials of pancreatic duct stents appear to be particularly important. According to different materials, pancreatic duct stents can be classified into plastic stents, fully covered self-expandable metal stents, and biodegradable stents. The complications, including stent migration and secondary surgery, frequently appear in the clinical use of plastic pancreatic duct stents and self-expandable metal stents, although they have been widely used in clinical practice. Biodegradable pancreatic duct stents can be degraded completely in the pancreatic duct within the expected time, and thereby avoid several complications. Consequently, the exploitation and application of biodegradable pancreatic duct stents have become an issue of concern.
2.Primary report of single rib-cartilage recombination transplantation in Binder syndrome treatment.

OBJECTIVETo explore the surgical methods and advantages of single rib-cartilage recombination transplantation in Binder syndrome treatment.
METHODSFive patients were treated with single rib-cartilage recombination transplantation. We harvested only the seventh costal cartilage including partial costal bone of the same rib to correct all the deformities. The rib was divided into 3 parts: 1 I-shaped cartilage, 1 inverted-T-shaped cartilage, and 1 C-shaped implant (later divided into a C and a reversed-C implant) composed of rib bone and cartilage. The first 2 parts were constructed to an L-shaped implant for nasal dorsum augmentation, collumella support, and nasal base elevation. The C-shaped and reversed-C-shaped implants were placed on both sides of the pyriform aperture.
RESULTSAll patients were followed up for eighteen to thirty months. No apparent deviation on the nasal support was found.
CONCLUSIONSingle rib-cartilage recombination transplantation in Binder syndrome treatment has lots of advantages, such as less morbidity, rapid recovery, and good results.

3.Laws of acupoint selection of prescriptions for treatment of cervicogenic headache by acupuncture: a modern literature research.

OBJECTIVETo summarize laws of acupoint selection of prescriptions for treatment of cervicogenic headache by acupuncture in modern literature.
METHODSRandomized controlled trials (RCTs) involving acupuncture and moxibustion for treatment of cervicogenic headache were recruited from CBM (1978-2012), VIP (1989-2012), Wanfang Database (1998-2012), CNKI (1979-2012), PubMed (1966-2012), EMbase (1980-2012), and Cochrane Library (Volume 4, 2012). Hand recruitment was also auxiliarily used. The frequency and percentage of common acupoints, the distribution of acupoints along 14 meridians and across each part of the body, the application of specific acupoints, and features of using prescriptions for specific acupoints were statistically described.
RESULTSTotally 37 recruited papers included 42 acupoints and 159 times. Common acupoints covered Fengchi (GB20, 28 times), Jingjiaji (EX-B2, 21 times), Baihui (DU 20, 12 times), Tianzhu (BL9, 1 times), and Ashi point (11 times). Meridians along which acupoints were used mainly covered Foot-shaoyang Gallbladder Meridian, Foot-taiyang Bladder Meridian,and DU meridian. Acupoints were mainly needled from head, neck, and upper limbs. Eight confluence points and luo-connecting point were commonest used as specific acupoints. Acupuncture prescriptions were mostly composed of multiple acupoints. Filliform needle was mainly used in acupuncture methods, followed by electro-acupuncture needle.
CONCLUSIONSModern acupuncture treatment of cervicogenic headache focuses on local specific points and acupoints along meridians. Acupoints were mostly selected from head, neck, and upper limbs by syndrome typing of Chinese medicine.

4.Polymorphism in CD14 promoter region in Han population in Chongqing

Objective To investigate the distribution of CD14C ( 260)T single nucleotide polymorphism (SNP) in the Han population in Chongqing and to test the reliability with this random sample in genetic research. Methods Single round Touchdown PCR (TD PCR) and restriction fragment length polymorphism (RFLP) method were used for genotyping in 110 people from the Han population in Chongqing. Results Frequencies of TT, CT, and CC genotypes were 43 6%, 39 1%, and 17 3%, respectively. The sample distribution was accorded with Hardy Weinberg principle. Conclusion CD14C( 260)T SNP was found in the Han population in Chongqing, and the distribution of alleles was significantly different from that in white population. The sample from an H W equilibrium population can be used in the association study of the systematic inflammatory reaction in the Han population in Chongqing.
5.Hepatitis B virus reactivation in HBV DNA-negative patients with hepatocellular carcinoma after transarterial chemoembolization

Objective To evaluate risk factors of hepatitis B virus (HBV) reactivation after transarterial chemoembolization (TACE) in patients with HBV-related hepatocellular carcinoma (HCC) with undetectable HBV DNA levels.Methods From August 2012 to Sepemter 2015,all patients who underwent TACE for HBV-related HCC with HBV DNA level less than 103 copies/ml in the First Affiliated Hospital of Soochow University were enrolled into the study.These patients were followed-up for at least 6 months after TACE to assess HBV reactivation.Univariate and multivariate logistic regression analyses were done to evaluate the risk factors related to HBV reactivation in these patients.Results With predetermined inclusion and exclusion criteria,99 patients were enrolled into the study.Twenty-four patients (24.2%) developed HBV reactivation after TACE in the followed-up period.The level of transaminase was significantly higher in the HBV reactivation group than the non HBV reactivation group (P < 0.05).HBeAg positivity and tumors more than 3 in number were independent risk factors of HBV reactivation.Conclusion HCC patients with undetectable levels of HBV DNA had a significant risk of HBV reactivation after TACE.
6.Intra-operative implantation of I125 seeds to treat advanced pancreatic cancer

Objective To study the clinical efficacy of intra-operative implantation of I125 seeds to treat advanced pancreatic cancer.Methods We retrospectively analyzed the clinical data of 28 patients who underwent intra-operative implantation of I125 between January 2009 and June 2010 to treat advanced pancreatic cancer.Patients who received conservative treatment (n =136) at the same period were used as the control group.Results The mean tumor diameter was (5.7 ± 1.7) cm.The average number of implanted seeds were 50.There were no patients who suffered from pancreatic fistula or post-operative bleeding,and no patient died from the treatment.Twenty-five patients were regularly followed up,and the follow-up rate was 85.7%.The survival rates at one month,half a year and one year were 100%,70.8% and 26.9%,respectively.The average survival time was (9.9 ± 1.4) months.The survival data were significantly higher than that in the control group.Conclusion Intra-operative implantation of I125 seeds was an efficacious treatment for patients with advanced pancreatic cancer.
7.Treatment of lumbar degenerative disease with modified transforaminal lumbar interbody fusion

Objective To investigate the chnical effect of modified transforaminal lumbar interbody fusion (TLIF) on the treatment of lumbar degenerative disease. Methods Sixty-two patients with lumbar degenerative disease were treated by the modified TLIF from June 2007 to May 2009. The preoperative diagnosis was lumbar intervertebral disc herniation with spinal instability (28 cases), lumbar intervertebral disc herniation with lumbar stenosis (27 cases ), degenerative spondylohsthesis (7 cases ). Forty-eight cases were single-level and 14 cases were two-level. The patients were evaluated by observing the fusion rate and comparing the visual analog score( VAS ) and Japanese orthopaedics association (JOA) score of preoperation with those of postoperation. Results All the patients were followed up from 15 to 30 (22.77 ± 3.82)months,no nerve injury,leakage of cerebralspinal,infection,the broken of pedical screws and other complications. The fusion rate of segment was 96.8% at the follow-up after 1 year postoperatively. Judgement by JOA score,the rate of improvement was 93.5%(58/62),excellent in 34 cases,good in 24 cases,fair in 4 cases. The postoperative value of V AS and JOA score were higher than those of preoperation (P < 0.05 ), the values when follow-up of 3 months was performed had no statistic al difference with those of final follow-up (P>0.05). Conclusion The modified TLIF with fully decompression while reducing the accessing spinal canal complications have good clinical efficacy in treating lumbar degenerative disease.
8.Screening of proteins interacting with RSK4 in breast cancer by affinity purification and liquid chromatography/mass spectrometry

Background and purpose:As a tumor suppressor gene, ribosomal S6 kinase 4 (RSK4) plays important roles in inhibiting cell proliferation, migration and inducing cell apoptosis. However, the proteins interacting with RSK4 are still unknown. This study aimed to screen proteins interacting with RSK4 in breast cancer cell line MDA-MB-231 by lfag-tag affnity puriifcation and LC-MS/MS (liquid chromatography/mass spectrometry).Methods:The pcDNA3.1/EGFP-RSK4-Flag eukaryotic expression vector was constructed by inserting full lengthRSK4 gene into vector pcDNA3.1/EGFP-Flag. And then the recombinant plasmids were transferred into MDA-MB-231 cells. Real-timelfuorescent quantitative polymerase chain reaction (RTFQ-PCR) and Western blot were used to detect the expression of RSK4 in MDA-MB-231 cells. Affnity puriifcation and LC-MS/MS were applied to screen proteins interacting with RSK4, and the related action mechanism of RSK4 with its interacted proteins was detected based on bioinformatics gene ontology (GO) and ingenuity pathway analysis (IPA).Results:Twenty-four proteins, such as serine/threonine-pro-tein kinase 38 (STK38)/serine/threonine-protein kinase 38-like (STK38L), MOB kinase activator 2 (MOB2) and protein arginineN-methyltransferase 5 (PRMT5), were successfully identiifed by Flag-tag affnity puriifcation followed by LC-MS/MS analysis, which probably interacted with RSK4. Bioinformatics analysis of the identiifed proteins suggested the proteins interacting with RSK4 were involved in diverse biological pathways, such as apoptosis and cell migration. Conclusion:According to bioinformatics results of proteins interacting with RSK4 identiifed by affnity puriifcation and LC-MS/MS, biological networks of RSK4 are involved in apoptosis and migration in breast cancer cells.
9.Clinical significance of retinal binding protein and cystatin C in blood and urine of patients with chronic kidney disease

Objective To investigate the clinical significance of retinal binding protein (RBP) and Cystatin C in blood and urine in patients with chronic kidney disease (CKD).Methods One hundred and twenty-one patients with CKD in Nephrology Laboratory of The People's Hospital of Xinjiang Uygur Autonomous Region from Aug.2012 to Jan.2013 were served as the case group.Sixty healthy check-up people were considered as control group.Enzyme linked immunosorbent assay was used to detect RBP and Cystatin C in urine,and the immune turbidimetry testing was applied to detect RBP and Cystatin C in blood.The receiveroperating characteristic (ROC) curve and area under curve (AUC) were applied to evaluate the sensibility and diagnostic value of indexes in CKD.Results The positive rate of RBP,Cystatin C in urine and blood were 89.6%,92.6%,52.5%,59.5% respectively of 12 patients in case group,higher than those in control group (all value were 0,P <0.001).ROC curve showed that the sensitivity of urine RBP and Cystatin C were higher than that in blood of case group.AUC of RBP,Cystatin C level in urine and blood were 0.915,0.974,.655,and 0.623 respectively.And 95% confidence interval were 0.877-0.954,0.956-0.992,0.575-0.736,0.543-0.702 respectively,and the differences are statistically significant (P < 0.001).Conclusion The sensitivity of urine cystatin C and RBP are significantly higher than that in blood,which might be served as a diagnostic marker of CKD and can provide important basis for clinical diagnosis.
10.Syndrome inappropriate ADH secretion after allogeneic hematopoietic stem cell transplantation: a case report and literature review.

OBJECTIVETo study the syndrome of inappropriate ADH secretion (SIADH) after allogeneic hematopoietic stem cell transplantation (allo-HSCT) and the possible etiology.
METHODSThe clinical manifestation, laboratory examination, treatment and outcome of a patient with refractory anemia with excess blasts after allo-HSCT were presented.
RESULTSHyperacute graft-versus-host disease (GVHD) was developed in the patient after allo-HSCT followed by severe hyponatraemia (lowest serum sodium 103.7 mmol/L), natriuresis, hypo-osmolality of plasma, hyper-osmolality of urine, coma and twitch at day 17 after allo-HSCT. SIADH was diagnosed. The clinical condition was improved after restriction of water and administration of hypertonic saline, but SIADH was not controlled completely. Afterwards, graft failure was developed. Hyperacute GVHD and graft rejection occurred again after the second transplant. The patient died of secondary infection.
CONCLUSIONSIADH after allo-HSCT is a rare fatal acute complication of central nervous system. Numerous transplant-related causes are probably associated with the development of SIADH. Early accurate diagnosis and treatment promptly is of great importance.
