Objective To explore the relevance of the microRNA-192 (miR-192) levels and prognosis in patients with diabetic nephropathy (DN).Methods A total of 59 cases with duration of type 2 diabetes mellitus (T2DM) ≥10 years from January 2010 to July 2011 were retrospectively admitted.The miR-192 levels were determined after admission,and other relevant clinical data were collected.And the patients were followed-up for 24 months.According to the patient's prognosis,the patients were divided into DN group(20 cases) and non-DN group(39 cases),and the relevant information was analyzed in two groups.Results The miR-192 levels in DN group were lower than those in non-DN group[(4.47 ± 2.27) μ g/L vs.(7.28 ± 2.39) μ g/L],and there was significant difference (P ＜ 0.05).Multivariate Logistic regression analysis showed that hypertension history,smoking history,high glycosylated hemoglobin,low fasting C-peptide,low 24 h urine protein,low miR-192 level,high body mass index was the independent risk factors of prognosis of T2DM (P ＜ 0.05).The area under the receiver operating characteristic (ROC) curve was 0.697 in miR-192 levels individual predicting progression to DN stage,0.823 in six others without miR-192.After joining seven indicators forecast area under the curve increased significantly(0.856,95％ CI:0.832-0.904).The miR-192 levels 5.14 μ g/L was the best predictive value to the critical value of community,and the patients were divided into miR-192 ＜ 5.14 μ g/L group (28 cases) and miR-192 ≥ 5.14 μ g/L group (31 cases).The number of progression to DN within 24 months in miR-192 ＜ 5.14 μg/L group was more than that in miR-192 ≥5.14 μ g/L group (16/28 vs.4/31,P ＜ 0.05).Conclusions A reduced level of miR-192 is an independent risk factor that can lead to poor prognosis in patients with DN,and can predict adverse outcomes in patients with T2DM effectively.

Objective To study the pollution levels,characteristics,causes and related adverse effects of indoor volatile organic compounds(VOCs) on human health after decoration. Methods The concentration of indoor air formaldehyde and TVOC were detected,meanwhile the adverse health effects were investigated by questionnaires. Result The results showed that the houses in which formaldehyde concentration exceeded the national indoor air quality standard occupied 47.1%, and three months later, the rate still was 40%,the highest level was 6.56 times of the standard. While the rate of exceed the national standard of TVOC was 76% within one month and 0% three months later respectively. As far as the concerned subjective syndromes, only the complaint of disgusted smell was significantly different between those who lived in newly decorated rooms and those who lived in the houses which taken as the control. Conclusion Now the pollution level of VOCs caused by decoration is some lower than before,but the pollution of formaldehyde was still pervasive and serious. The causes of pollution lay in artificial board materials used for furniture and decoration. Moreover, the complaints of the discomfort of eyes and upper respiratory tract will increase as people moved in the newly decorated rooms.

Objective To evaluate the therapeutic effect of encircling scleral buckling surgery in rhegmatogenous retinal detachement (RRD) with vitrectomy. Methods The records of a series of 118 consecutive patients with RRD undergone encircling scleral buckling surgery and vitrectomy between 1995 1999 were reviewed. All of the patients underwent an onece vitrectomy, and they consisted of with encircling scleral buckling group and without encircling scleral buckling group. The difference of rate of retinal reattachment, and the relationship between the reattachment rate and the grade of PVR, position of retinal tear and the different intraocular tamponade substitute in both groups were analysed and compared. The average follow up period was 6.5 mons (1.5 mons 3 ys). Results The rate of retinal reattachment in this series of 118 cases was 81 4% (96/118) in which the encircling scleral buckling group accounted for 44 7% and the group without encircling scleral buckling accounted for 55.3%. There was no statistical difference of rate of retinal reattachment between both groups with PVR C, different intraocular tamponade and various positions of retinal tears, while the rate of retinal reattachment in cases of PVR D and retinal tears posterior to equator in group without encircling scleral buckling was significanthy higher than that in encircling scleral buckling group ( P

Objective To investigate the influence of nebulized Pulmieort respules inhalation after endoscopic sinus surgery (ESS)on the expression of epithelial Na~+ channel(ENaC)protein in nasal mucosa. Methods Forty-four patients with nasal polyps undergoing ESS were randomly divided into Pulmieort respules treatment group(n=21,nebulized Pulmieort respules inhalation for 10 d after ESS)and Rhinocort control group(n=23,Rhinoeort aqueous nasal spray for 10 d after ESS).All the patients were performed biopsy of membrane on the residual middle turbinate 14 d after ESS,eosinophils (Eos)and neutrophils(Neu)per hundred inflammation cells were counted under microscope during ESS and after ESS,and the expression of ENaC protein was detected by immunofluorescence assay. Results The percentages of Eos and Neu decreased in two groups after treatment,and the percentage of Neu in Pulmieort respules treatment group was significantly lower than that in Rhinoeort control group(P<0.05).The expression of ENaC protein after treatment in Pulmieort respules treatment group was significantly lower than that in Rhinoeort control group(P<0.01). Conclusion Application of pulmieort respules after ESS can decrease Neu infiltration and inhibit expression of ENaC protein,which can relieve acute inflammation and edema of nasal mucosa.

Objective: To investigate the alterations of bcl-2 gene family in the rat brain and the molecular mechanism of neuronal apoptosis follow ing traumatic brain injury (TBI). Methods: Male Sprague-Dawley rats were subjected to lateral fluid percussion brain injury(FPBI) of moderate severity. bax and bcl-xL mRNA and protein expression was detected by RT-PCR an d immunohistochemistry. In addition to morphological evidence of apoptosis, TUNE L histochemistry was used to identify DNA fragmentation in situ under both l ight and electron microscope, whereas characteristic internucleosomal DN A fragm entation of apoptosis was demonstrated by DNA gel electrophoresis. Resul ts: bcl-xL mRNA and protein decreased in the ipsilateral hemisphere t o the impact site as early as 6 h post-injury［(67.42±7.54)% and (85.85±5.72)% r espectively］. The decrease in bcl-xL mRNA and protein preceded apoptosis was observed 12 h post-injury. And this was the main cause of up-regulation of the ratio of bax to bcl-xL in the acute period(minutes-hours) followin g FPBI. bax mRNA and protein were observed to rise slowly, doubled 3 d post- injury, returned to sham level slowly. The delayed cell death (days-weeks) migh t associated with the up-regulation of pro-apoptotic gene bax. Conclusio n: The expression of bcl-xL and bax coincide with apoptosis following TBI. The reg ulation of bax and bcl-xL by TBI occur before transcription. The balance of bax/bcl-xL ratio determines the neurocytes to survive or die following FPBI.

Objective To investigate and analyze clinical researchers' knowledge,attitude and practice (KAP) regarding ethical review and its correlation with KAP,which could provide foundation information for developing ethical review education programs for clinical researchers in future.Methods The random sampling method was used to stratify for different group by professional titles.Total 277 clinical researchers in a top class hospital in Beijing were investigated using self-developed questionnaire.The collected data was analyzed by SPSS software.Results The average scores of clinical researchers' knowledge,attitude and practice about ethical review are 69.05,80.86,and 67.80 respectively.The correlation coefficient of knowledge and attitudes is 0.222 (P＜0.05),the correlation coefficient of knowledge and practice is 0.544 (P＜0.05),and the correlation coefficient of practice and attitudes is 0.351 (P＜0.05).Conclusions Clinical researchers' knowledge about ethical review were at a relatively low level.And their practice about ethical review were not good enough.But their attitude towards learning ethical review was relatively good.It has suggested that more education and training programs on ethical review should be implemented for clinical investigators to improve their ethical literacy.

OBJECTIVE:To establish a HPLC method for determining the contents of rhein and emodin in Jianfei Jiangzhi tablets.METHODS:Stationary phase was Waters Symmetry C 18 (250mm?4.6mm;5?m),mobile phase was menthol-0.01%H 3 PO 4 (90∶10),detection wavelength440nm,flow rate1.0ml/min and column temperature30℃.RESULTS:There was good linearity in range of0.108?g～0.468?g for rhein and in range of0.180?g～0.480?g for emodin.The average recoveries of rhein and emodin were99.7%and98.6%respectively;RSD were1.93%and2.19%respectively.CONCLUSION:This method is rapid and accurate.It can be applied to effective control of the quality of Jianfei Jiangzhi tablet.

OBJECTIVETo explore the effects of warm needling combined with Zhangmo decoction (see text) on endometrial receptivity in patients with clomiphene (CC)-induced ovulation.

METHODSOne hundred and sixty cases were randomly divided into a CC group (group A), a CC+ progynova group (group B), a CC+ Zhangmo decoction group (group C) and a CC+ Zhangmo decoction + warm needling group (group D), 40 cases in each one. In the Group A, CC alone was applied. In the group B, progynova was jointly used on the 8th day of menstrual cycle. In the Group C, Zhangmo decoction was jointly used on the 5th day of menstrual cycle. In the group D, based on treatment of the Zhangmo decoction, warm needling was applied at Guanyuan (CV 4), Zhongji (CV 3) and Zigong (EX-CA 1) etc. The endometrial thickness and type, pulsatility index (PI), resistance index (RI), ratio of S/D on day of human chorionic gonadotropin (HCG) and pregnancy rate were observed in fou groups.

RESULTSThe PI, RI and S/D in the group C and D were obviously lower than those in group A and B (all P < 0.01). The endometrial thickness was (7.7 +/- 1.49) mm in group B, (8.2 +/- 1.54) mm in group C and (8.9 +/- 1.51) mm in group D, which were significantly different from (6.4 5 +/- 1.26) mm in the group A (all P < 0.01) also there was a significant difference between group C and D (P < 0.05). The rate of endometrial type A was 65.0% in the group D, which was significantly higer than 27.5% in the group A, 32.5% in the group B and 35.0% in the group C (all P < 0.01). The pregnancy rate was 30.0% in the group D, which was obviously higher than 12.5% in the group A, 15.0% in the group B and 17.5% in the group C (P < 0.05). The endometrial thickness and rate of endometrial type A in the pregnant were obviously higher than those in the non-pregnant (both P < 0.01) while PI, RI and S/D was lower than those in the non-pregnant (P < 0.01, P < 0.05).

CONCLUSIONWarm needing combined with Zhangmo decoction could improve endometrial thickness, morphology and uterine spiral artery to improve pregnancy rate, which has superior effect to clomiphene, clomiphene combined with progynova and clomiphene combined with Zhangmo decoction.

Objective To study the effects of CD2-associated protein (CD2AP) on podocyte adhesion and extension ability and to explore its possible mechanism. Methods Conditionally immortalized murine podocyte cell line was cultured in RPMI 1640 medium at 33℃permissive conditions. The podocytes were transfected with CD2AP small interfering RNA (siRNA) and serambing sequences labeled with fluorescein were taken as control. The transfected podocytes were trypsinized and seed into collagen IV coated plates. The relative cell adhesion and cell area were examined 90 min later. Apoptotic rates of CD2AP siRNA transfected podoeytes and different PAN concentrations incubated podoeytes were detected by flow cytometer. The distribution of F-actin was observed under laser scanning confoeal microscope. Nephrin protein expression and its phosphorylation level were examined by immunofluorescence and Western blot. Results The relative ceil adhesion of CD2AP siRNA transfected podocytes was apparently lower than that of control group[(41.72±6.07)% vs (64.46±8.53)%, P<0.05]. The cell area analysis had the similar result. The apoptotic rate of CD2AP siRNA transfected podocytes was significantly higher than that of the controls [(5.73±0.61)% vs (3.26±0.45)%, P<0.05]. 100 mg/L PAN could markedly induce podocytes to apoptosis and impair cell adhesion ability (P<0.05). Nevertheless, no significant difference was found in cell body spreading (P>0.05). The distribution of F-actin in CD2AP depletion podocytes was apparently altered. The expression of nephrin protein and its phosphorylation level was conspicuously descended to some degree (P<0.05). Conclusions CD2AP depletion facilitates podocyte apoptosis and impairs cell adhesion function. Cytoskeleton confusion and nephrin signaling weakness caused by CD2AP depletion may he partly responsible for the decline of cell adhesion and spreading.
