1.Function mechanism and research development of Skp2 in malignant tumors

Skp2 (S-phase kinase associated protein) is substrate recognition subunit of SCFSkp2 (Cul1-Rbx1-Skp1-F boxSkp2)complex,a kind of ubiquitin ligases (E3s). Skp2 involves in regulating many functions of cell,such as cell cycle progressing,cell proliferation and differentiation,as well as apoptosis.At present many studies indicated that Skp2 is closely related to tumorigenesis and prognosis.With deeper study of Skp2 in tumor, Skp2 has the potential to be a promising drug target and biomarker of prognosis in human cancer.
2.INFLUENCE OF GENERAL ANESTHESIA ON STRESS AND IMMUNE FUNCTIONS IN UPPER ABDOMINAL SURGERY

This study was set up to observe the changes in stress and immune functions in upper abdominal surgery after general anesthesia.In thirty elective upper abdominal operation patients,general anesthesia was induced with propofol,fentanyl,scoline and maintained with isoflane and atrumium. Catecholamines(CA),cortisol,interleukin-2(IL-2) and soluble interleukin-2 receptor(sIL-2R) were measured before anesthesia,after induction of anesthesia,at the end of operation,and on the first and third postoperative days.The results showed that epinephrine(E),norepinephrine(NE) concentration increased at the end of operation and on the first postoperative day(P<0.01),Cortisol concentration decreased after anesthesia induction(P<0.05),IL-2 concentration decreased at the end of operation(P<0.05),and sIL-2R lever did not change.The investigation suggested that general anesthesia with propofol,fentanyl and isoflane can suppress CA and cortisol increase resulting from induction of anesthesia. There was no change in IL-2 level.However it can not suppress the increase of stress hormones and decrease of IL-2 due to operation.The stress response correlate with immune functions.
3.Depth of counseling process:exploring of a three-dimension model

Objective:To test Jiang Guangrong's(2005)three-dimension model on the depth of counseling process.Methods:In this process-outcome research,28 sessions in a university counseling center were videotaped and coded in terms of the model.Structure Equation Model(SEM)was used to examine the relations between the coding and counselors and clients' evaluation of the session depth.Results:(1)Clients and counselors evaluated differently on the session depth,and clients' evaluation was more positive than counselors' [(4.95±1.12)vs.(4.23±0.94),P<0.05];(2)The model was significantly correlated with counselors' session depth evaluation(path coefficient = 0.55,P<0.01),but not with the clients';(3)The session curves describing each dimension changing over time had different patterns in sessions of different depth level,through which more about the counseling process could be known.Conclusion:The three-dimension model on the depth of counseling process has got some empirical evidences.
4.Clinical study on mechanical ventilation for acute left ventricular failure

Objective To research the clinical value of mechanical ventilation in the treatment of acute left ventricular failure.Methods Of 74 case with acute left ventricular failure,26 cases recevived routine treatment including nasal-catheter inhale of oxygen or veil inhale of oxygen and inotropic agent,diuretic,and dilation as well as sedative while the mechanical ventilation group were treated only with mechanical ventilation depending on the condition.The clinical indications,parameters of blood gas analysis and hemodynamies were compared before and after treatment.Results 18 cases from routine treatment group died(70.9%)and 5 died in mechanical ventilation group (10.4%).There was significant difference in heart rates (HR),central venous pressure (CVP),the mean arterial blood pressure (MAP) and some parameters of blood gas analysis before and after the treatment (P<0.05 or P
5.Clinical study of phacoemulsification combined with trabeculectomy for treatment of cataract and glaucoma

Objective To evaluate the effect of combination of phacoemulsification and trabeculectomy on patients with cataract and glaucoma.Methods 19 patients(23 eyes)with co-existing glaucoma and cataract were treated with triple procedure by means of scleral tunnel flap.The mean follow-up period was(15.8 ±3.5)months.Results Compared with preoperative visual acuity,16(64%)of 25 eyes had a best corrected visual acuity of ≥0.6 at the last follow-up(0.1~1.0)(P<0.01);The mean preoperative intraocular pressure(IOP)was(23.01 ±2.63)mmHg which decreased to(14.02 ±2.05)mmHg(P<0.05 ,P <0.01)after treatment.Conclusion The triple procedure is effective for treating patients with co-existing cataract and glaucoma,which could enhance the visual acuity,control IOP and reduce complications.
6.Clinical analysis of treating hernia with Lichtenstein's tension-free hernioplasty in 178 cases.

Objective To study the standard operating methods and skills of Lichtenstein's tenaion-free her-nioplasty for treating groin hernia and the methods of reducing the postoperative complication and recrudescence. Methods Clinical data of treating greinhemia in 178 eases at Department of General Surgery, Luan People's Hospi-tal were reviewed and analyzed. Results On average, each operation needs 50 minutes and the patients spend 4 days in hospital. After 2~3 weeks,the patients can restore the daily activity and manual labour. There were 9 cases of uri-nary retention after operation,5 cases of scrotum haematoma's accumulating liquid,7 cases of the cut fat deliques-cing, 14 cases of various pain and numbness in groin area and 1 case of hernia recrudescence. Conclusion Lichten-stein's tension-free hemioplasty is a perfect surgical operation method which has many advantages, such as minimal trauma,rapid recovery, simple operation sequence, and less postoperative complication and recrudescence. It is in keeping with anatomy physiology.
7.Clinical study of oral squamous cell carcinoma with clinically lymph node-negative

Objective:To investigate the clinical and pathological factors of metastasis in patients with oral squamous cell carcinoma (OSCC)and clinical lymph node-negative(cN0).Methods:85 patients of OSCC with cN0 treated in 2008-07 -2013-07 were inves-tigated retrospectively.Results:In the 85 patients 24(28.34%)showed occult metastasis.Patient's gender,lesion location had no im-pact to metastasis(P >0.05),the patient's age,tumor size,growth pattern and histopathologic grade showed significant influence on the metastasis of OSCC(P <0.05).Conclusion:The greater tumor volume,lower differentiation degree and young age are the factors of higher occult lymph node metastasis.Selective neck dissection treatments of OSCC is recommended.

9.Analgesic effect of processed Aconiti tuber in rats with neuropathic pain

Objective To investigated the analgesic effect of processed Aconiti tuber(PAT) in rats with a sciatic nerve chronic constriction injury(CCI) .Methods The study was conducted in two parts. CCI model was developed with four ligatures placed around the right sciatic nerve and tied without obstructing the blood supply of the nerve. In part Ⅰ, forty rats were randomly divided into five groups(n = 8) . In group Ⅰ normal saline 1 ml , In group Ⅱ-Ⅴ PAT 0.5 ,1, 2 ,or 4 g/kg was administered orally 14 days after CCI respectively. In part Ⅱ , thirty-two rats were randomly divided into four groups (n = 8) . In group Ⅰ oral normal saline 1 ml, in group Ⅱ oral PAT 2 g/kg, in group Ⅲ oral PAT 2 g/kg + intraperitoneal nor-binaltorphimine (nor-BNI) 2 mg/kg, in group Ⅳ oral PAT 2 g/kg + intrathecal nor-BNI 100 ?g was administered 14 days after CCI. As indicators of mechanical and heat hyperalgesia, the paw withdrawal pressure threshold (PWPT) in response to a graded pressure stimulus with a filament and the paw withdrawal latency (PWL) in response to radiant heat were measured in both hind paws before and after drugs administration.Results In part Ⅰ, in group Ⅲ-Ⅴ PWL was prolonged significantly and in group Ⅳ - Ⅴ PWPT were significantly increased compared with those in group I . There was no significant difference in PWL and PWPT between group Ⅰ and group Ⅱ . In part Ⅱ , in group Ⅲ, Ⅳ PWPT and PWL were significantly lower than those in group Ⅱ . There was no significantly difference in PWL and PWPT between group Ⅰ and group Ⅲ, Ⅳ . Conclusion In rat with neuropathic pain processed Aconiti tuber exerts analgesic effect via the spinal ?-opioid receptor activation.
10.Reasons for Clinical Shortage of Certain Drugs

OBJECTIVE: To draw the attention of our country and the concerned party on the shortage of certain drugs. METHODS: Based on the drug shortage condition in our hospital in recent years, we analyzed the reasons for drug shortage, put forward the solution and summarized the clinically indispensable categories. RESULTS & CONCLUSIONS: We should draw experiences from US by solving shortage problem of certain drugs through the co-efforts of government, manufactures and medical institutions to satisfy patients' needs of disease prevention and treatment.
