1.Relationship between the expression of BRCA1 and TUBB3 in patients with advanced non-small-cell lung cancer and its efficacy to platinum-based chemotherapy

Objective To investigate the relationship between the expression of BRCA1,β-tubulin III (TUBB3) and their efficacies with platinum-based chemotherapy in patients with advanced non-small-cell lung cancer (NSCLC). Methods The expression levels of BRCA1 and TUBB3 were detected by immunohistochemistry. The relationship between BRCA1 and TUBB3 expressions and their efficacies were analyzed. Results The high expression rate of BRCA1 was 34.8%, and the efficacy of platinum-based chemotherapy in patients with BRCA1-lower expression is obviously better than that in patients with BRCA1-higher expression. There was a significant difference between these two groups (30.4%vs 67.4%, P=0.004). The effective rate of platinum-based chemotherapy in TUBB3-higher expression group and TUBB3-lower expression group were illustrated no significance (59.4% vs 50.0%, P=0.445). Conclusion Platinum-based chemotherapy is more suitable for the advanced NSCLC patients with lower expression of BRCA1. The expression level of BRCA1 could be used to predict the efficacy of platinum-based chemotherapy in advanced NSCLC patients.
2. Hemodynamic detection of posterior cerebral artery dysplasia with transcranial Doppler ultrasonography

Objectives: To analyze the hemodynamic characteristics of dysplastic posterior cerebral artery with transcranial Doppler ultrasonography (TCD) and to investigate the diagnostic value of TCD. Methods: Fifty-two patients with posterior cerebral artery P1 segment (PCA-P1) dysplasia screened by MRA and TCD were selected. The hemodynamic changes before and after the carotid compression test were observed, and compared with 52 healthy subjects without abnormal blood vessels (control group). Results: Circled digit oneThe blood flow velocity of 52 patients with PCA-P1 dysplasia on the dysplasia was 55 ± 8 cm/s on the dysplastic side, which was significantly lower than that of 60 ± 6 cm/s in the control group (P < 0.01). After compressing the ipsilateral common carotid arteries (CCAs) respectively, the velocity of PCA-P1 on the dysplastic sides was 192 ± 25 cm/s, and it was significantly higher than 96 ± 14 cm/s in the control group (P < 0.01). The ratio of velocity in patients with dysplasia before and after the compression test (after Vp/before Vp) was 3.52 ± 0.53, and it was significantly higher than 1.60 ± 0.17 in the control group (P < 0.01). Circled digit twoAfter compressing the ipsilateral CCAs respectively, the end-diastolic flow velocity and the ratio before compression (after Vp/before Vp) was 4.48 ± 1.28, and it was significantly higher than 1.61 ± 0.25 in the control group (P < 0.01). Circled digit threePCA-P1 pulsatility index (PI) on the dysplastic sides was 1.00 ± 0.22, and it was significantly higher than 0.78 ± 0.09 in the control group (P < 0.01). There was significant difference (P < 0.01). Circled digit fourOf the 52 patients, the flow spectrum of 28 patients showed high-resistance changes and the flow spectrum of 24 patients was normal. Before and after compressing the ipsilateral CCAs, the ratio of high-resistance spectrum group compared to that before compressing (after Vd/before Vd) was 5.3 ± 1.1, and it was significantly higher than 3.5 ± 0.6 in normal blood flow spectrum group. There was significant difference (P < 0.01). However, there was no significant difference between after VP/before VP and the normal blood flow spectrum group. ©Dysplasia in PCA-P1 was suspected in 39 cases by TCD, and 32 of them were confirmed with MRA. The positive coincidence rate was 82.1%, 17 of them showed high-resistance flow spectrum, and 16 had dysplasia in PCA-P1 confirmed by MRA. The positive coincidence rate was 94.1%. Conclusion: TCD in combination with blood flow spectrum shape may initially diagnose the dysplasia in PCA-P1 according to the changes of hemodynamic parameters of PCA-P1 before and after the CCA compression test.
3. Detection of cerebral hemodynamics of anterior cerebral artery and anterior communicating artery variations with transcranial Doppler ultrasonography

Objective: To analyze the hemodynamic characteristics in detecting of anatomic variations of anterior cerebral artery (ACA) and anterior communicating artery (ACoA) with transcranial Doppler (TCD). Methods: Sixty-two patients with anatomic variations of ACA or ACoA (patient group) and 48 healthy subjects without variation (control group) diagnosed by magnetic resonance imaging (MRA) and TCD were selected. The hemodynamic changes in extracranial and intracranial vessels in both groups were detected by TCD. Results: Circled digit oneForty-eight patients with unilateral hypogenesis of ACA-A1 in the patient group. The blood flow velocity of ACA-A1 on the hypogenetic sides was 61±16 cm/s, it was significantly lower than 86±15 cm/s in the control group; the blood flow velocity of ACA-A1 on the thick sides was 125±12 cm/s, and it was significantly higher than that in the control group (all P <0.01). The blood flow velocity on the thin sides was about 1/2 of that on the thick sides. Circled digit twoAfter pressing the contralateral CCA, the blood flow velocity of bilateral ACA-A1 increased to various degrees. The ratio of peak systolic blood flow velocity on the thin sides was 3.10±0.50, and it was significantly higher than 1.93± 0.24 in the control group. The ratio on the thick sides was 1.33 + 0.11. It was significantly lower than that in the control group. There was significant difference (P < 0.01). The ratio on the thin sides was more than two times of that on the thick sides. Circled digit threeNine patients with unilateral absence of ACA-A1, the blood flow velocity of contralateral ACA-A1 was 131±17 cm/s, and it was significantly higher than that of the control group and the ipsilateral MCA blood flow velocity. The blood flow velocity of the extracranial internal carotid artery in 57 patients on the thin or absence sides of ACA-A1 was 47±11 cm/s, and it was significantly lower than 60±13 cm/s on the contralateral sides (P < 0.01). Circled digit fourThe blood flow signals in 5 patients with the absence of ACoA were disappeared after compressing the ipsilateral CCA and ACA respectively. Conclusion: According to the hemodynamic changes before and after the bilateral ACA Queckenstedts test, and in combination with the assistant indices such as blood flow velocity of extracranial internal carotid artery, blood flow signal intensity, and blood flow distribution, TCD may primarily detect the significant variation of ACA or ACoA.
4. Unilateral hypogenesis of anterior cerebral artery: Transcranial doppler ultrasonography and analysis

Objectives: To analyze the hemodynamic characteristics in patients with unilateral hypogenesis of anterior cerebral artery (ACA) using transcranial Doppler ultrasonography (TCD) and to discuss the diagnostic value of it. Methods: Twenty-eight patients with unilateral hypogenesis of ACA were confirmed by TCD and magnetic resonance angiography (MRA). The characteristics of hemodynamic changes were observed in hypogenetic side and contralateral anterior cerebral artery A1 segment (ACA-A1) before and after compression of the common carotid artery. They were also compared with 28 normal controls. Results: The blood flow velocity of the hypogenetic side ACA-A1 (62 ± 17) cm/s of the 28 patients was significantly lower than that of the control group [(87 ± 15) cm/s; P < 0. 01]; the blood flow velocity of the contralateral ACA-A1 (123 ± 13) cm/s was significantly higher than that of the control group (P < 0.01). The average blood flow velocity of the hypogenetic side ACA-A1 was almost half of the contralateral ACA-A1. After compressing the common carotid arteries respectively, the blood flow velocity of the hypogenetic side ACA-A1 increased for several times (3.0.8 ± 0.51), and it was significantly higher than that of the control group (1.97 ± 0.25; P < 0.01); the blood flow velocity of the contralateral side ACA-A1 increased for several times (1.35 ± 0.11), and it was significantly lower than that of the control group (P < 0.01); the average blood flow velocity of the hypogenetic side ACA-A1 was more than 2 times of the contralateral ACA-A1. Conclusion: Accordin g to hemodynamic changes before and after compression of the common carotid artery, together with the signal strength of blood flow, and the distribution of blood flow, TCD may defect unilateral hypogenesis of ACA.
5.Pharmaceutical Care for One Patient with Bone Tuberculosis Complicated with Tubercular Meningitis

Objective:To explore the approaches and ideas for the pharmaceutical care performed by clinical pharmacists for the patients with bone tuberculosis and tubercular meningitis. Methods:Pharmacists participated in the clinical consultation for one patient with bone tuberculosis and tubercular meningitis. The methods and effect of clinical pharmacists were analyzed comprehensively in re-spect of the adjustment of anti-tuberculosis treatment scheme according to the disease condition, attention pald to the adverse reactions of anti-tuberculosis drugs, medical education for the patient and so on. Results: During the treatment, clinical pharmacists did have some actions to assist doctors in rational drug use, and the suggestions of clinical pharmacists in the consultation obtalned clinical rec-ognition, which significantly improved the effect of the medication. Conclusion:Clinical pharmacists can perform individual pharma-ceutical care for patients and help clinicians optimize drug therapy, which can improve the safety and efficacy of medication.
6.Research progress of epithelial mesenchymal transition mediated by FOX family in tumor metastasis

Epithelial mesenchymal transition ( EMT) is one of the important biological processes in tumor invasion and metastasis . However , due to the complexity of EMT signaling pathway and its unclear molecular mechanism , the treatment of EMT is still a worldwide problem .But many studies have proved that EMT is not an irreversible process .In recent years , the research of FOX gene family in EMT shows its important role in tumor metastasis . This review focuses on the FOX-mediated EMT process in many kinds of tumor , aiming to have a better understanding of EMT signaling network , and provide a new target for the effective pre-vention of EMT .
7.Relationship between factors of labour pain and delivery outcomes

Objective To evaluate factors associated with labor pain and delivery outcomes.Methods From Jul.to Dec.2009,111 normal singleton cephalic presentation pregnancies (including 5 elderly parturient) who delivered at the Department of Obstetrics and Gynecology,Second Affiliated Hospital,Zhejiang Chinese Medical University were enrolled in this study to evaluate the relationship between factors of labor pain and delivery outcomes.The labor pain of latent phase and active phase were scored by the visual analogue scale (VAS).Factors associated with pain included the age of parturient,the number of gravidity and parity,occupation,education profile,dwell location,etc.The questionnaire was designed by ourselves.Childbirth awareness,psychological preparation of delivery,emotional controllability,couple relationship,the relationship of parturient and mother-in-law,the relationship of parturient and parents,family economic status,use of sedative during the labor process and delivery outcomes were collected and analyzed.Results (1)Factors associated with pain:in the latent phase,the rate of moderate labour pain of 1/5 in women with more than 35 years old was statistically lower than 76.4% ( 81/106 ) in suitable age group (P<0.05 ).The women with a good understanding about delivery had a statistically lower rate of moderate pain of 64.7%( 44/68 ) than 88.4% ( 38/43 ) of those having a poor understanding( P<0.05 ).The women who had a better couple relationship had a significandy higher rate of moderate pain of 77.2% (78/101) than 4/10 of those who had a general couple relationship ( P<0.05 ).There was significant difference in rate of moderate pain between pluripara group ( 50.0%,11/22) and primipara group(79.8%,71/89;P<0.01 ).In the active phase,women with tense,scared or a poor emotion control expressed significantly severe labour pain(59.0%,36/61 ) than 35.6% ( 16/45 ) in well-prepared group.The rate of severe labour pain in good control of emotion group of 44.8% (43/96) was a statistically lower than 9/10 in poor control group.There was a statistically lower severe labour pain in women given by sedatives ( 29.2%,7/24 ) than 54.9% (45/82) in women without sedatives treatment (P < 0.05).(2) Delivery outcomes:in latent phase,the rates of fetal distress and cesarean section were 36.6% (30/82) and 39.0% (32/82) in moderate pain group,which were significantly higher than 13.8% (4/29) and 17.2% (5/29) in mild pain group.In active phase,the rate of fetal distress,cesarean section and postpartum hemorrhage were 36.5% ( 19/52),40.4% (21/52) and 13.5% (7/52) in severe pain group,which were significantly higher than [18.5% (10/54) ;20.4% (11/54) ;0] in moderate pain group (P<0.05).Conclusions Women with poor understanding of delivery,tense,scared,poor emotion control,young age and uniparous have severe labour pain.Sedative use could alleviate pain in active phase.Women with mild labour pain have good delivery outcomes.
8.Experimental Research on Jinqianduizuo Mixture Preventing and Treating Cholelithiasis

ObjectiveTo observe the preventative and therapeutical effect on bile pigment calculus of Jinqianduizuo Mixture which is a traditional Chinese medicine empirical formula and provide foundation for the therapy of cholelithiasis. MethodUsing the model of bile pigment calculus in guinea pigs. Observing the gall bladder’s shape,the rate of forming gallstone and the total bile. Then making biochemical determination of the bile and the blood serum. Result There are significant differences between model control group and three treated groups of Jinqianduizuo Mixture in the rate of forming gallstone,the bilious quantity,the contents of total bilirubin and total bile acids. While in the contents of direct bilirubin,total cholesterol,triglycerides and high-density lipoproteins,they have no significant difference. ConclusionThe results indicate that Jinqianduizuo Mixture can conspicuously cut down the gallstone’s incidence of guinea pigs and can obviously promote the bilious externalization in guinea pigs. It also can promote bile secretion distinctly,but have no evident effect on the ingredients of blood-lipid.
9.Effects of danshensuan on expression of CD11b, P-selectin, ICAM 1, VCAM-1 and E-slectin in vitro

AIM To study the action of danshengsuan (Dan) on expression of CD11b, P selectin, ICAM 1, VCAM 1 and E slectin in vitro. METHODS Adheshion molecule expression on the cell surface was measured by flow cytometry. RESULTS Treatment of neutrophils with N fomyl met leu phe(fMLP), resulted in a marked increase of CD11b expression. Dan inhibited the effects of fMLP in a concentration dependent manner. Dan did not inhibit the increase of P selectin expression of thrombin actived human blood platelets. Treatment of cultured human umbilical vein endothelial cells (HUVEC) with TNF ? resulted in an increase of ICAM 1, VCAM 1 and E slectin. Dan inhibited the increase of VCAM 1 and E slectin expression, not ICAM 1. CONCLUSION Dan inhibited expression of adhesion molecules CD11b, ICAM 1, VCAM 1 in human neutrophils and HUVEC.
10.Research on data acquisition of portable uroflowmeter and accuracy of measurement.

This paper presents the preliminary design of data acquisition system of a portable uroflowmeter. The system uses double-hole cantilever pressure sensor. The signal is transferred to ATmega644PA microprogrammed control unit (MCU), converted by A/D (analog to digital) convertor. Then the further data are processed and get the corresponding relationship of weight-time and two curves of urine flow and urinary flow rate. In the measurement accuracy of the device about urine flow, two factors about the placement and height of the data acquisition are analyzed to show the accuracy of the equipment through the Origin 8.0 data analysis software. The design is characterized by low cost and high speed of data collection, real-time, high accuracy.
