2.Effects of heat-sensitive moxibustion plus Chinese medication on the expression of serum inflammatory indicators and microRNAs in patients with multiple myeloma after chemotherapy

Objective: To observe the effects of heat-sensitive moxibustion plus Chinese medication on serum inflammatory indicators, T-lymphocyte subsets, and serum microRNAs in patients with multiple myeloma after chemotherapy. Methods: Eighty-two patients with multiple myeloma who had terminated chemotherapy were divided into an observation group and a control group using the random number table method, with 41 cases in each group. The control group received symptomatic treatment of Western medicine plus oral Chinese medication Yi Shen Qiang Gu Tang (decoction for benefiting the kidney to strengthen bones), and the observation group received additional heat-sensitive moxibustion treatment. After the intervention, the following measures were observed, including clinical efficacy, symptom scores of traditional Chinese medicine (TCM), and tumor patient's quality of life (QOL) scores [including Karnofsky performance status (KPS) score, performance scale (PS) score, and tumor-specific QOL questionnaire score] alongside the serum inflammatory indicators [tumor necrosis factor-α (TNF-α), C-reactive protein (CRP), and interleukin-6 (IL-6)], and the relative expression levels of T-lymphocyte subsets (CD3+, CD4+, and CD8+) and serum microRNA-302 (miR-302), microRNA-140-5p (miR-140-5p), and microRNA-125a (miR-125a). Results: After treatment, the total effective rate was 85.4% in the observation group, higher than 68.3% in the control group (P<0.05). The observation group was superior to the control group in improving the serum inflammatory factor levels, TCM symptom scores, tumor patient's QOL scores, and the relative expression levels of T-lymphocyte subsets and serum miR-302, miR-140-5p, and miR-125a, and the between-group differences were statistically significant (P<0.05). Conclusion: Heat-sensitive moxibustion plus Chinese medication can lower the serum inflammatory factor levels, strengthen immune function, mitigate TCM symptoms, enhance QOL, and raise the expression levels of serum miR-125a, miR-140-5p, and miR-302 in multiple myeloma patients after chemotherapy.
3. HPLC fingerprint on wild germplasm resource of Ophiopogon japonicus in Sichuan and its chemical pattern recognition

Objective: To establish the method of chemical pattern recognition for wild germplasm resource of Ophiopogon japonicus in Sichuan Province. Methods: To establish chromatographic fingerprint of germplasm resource of O. japonicus in Sichuan by HPLC, according to the HPLC fingerprint, 26 wild germplasm resources of O. japonicus from different habitats in Sichuan were comprehensively compared by SPSS 17.0 software with principal component analysis and cluster analysis; and the correctness of "principal component - cluster analysis" was varified by the software of similarity evaluation. Results: Four principal components were extracted from 26 samples of O. japonicus and the samples were divided into four major categories; Compared with the direct clustering according to common peaks, the results of "principal component - cluster analysis" were more closer to the truth of similarity evaluation. Conclusion: The method of chemical pattern recognition is used for "principal component - cluster analysis" according to HPLC fingerprint of O. japonicus in Sichuan by SPSS 17.0 software. Its operation is simple and convenient, the result of statistics is reliable and able to provide a reference for the chemometrics classification and quality evaluation of O. japonicus.
4.Clinical study on Shutiao Decoction based on psycho-cardiacology model in treating coronary heart disease with anxiety and depression

Objective:To observe the clinical effect of Shutiao Decoction on patients with coronary heart disease and anxiety and depression. Methods:Sixty patients with stable coronary artery disease and anxiety and depression from January 2019 to December 2019 in Kunming Municipal Hospital of Traditional Chinese Medicine were selected. They were divided into a treatment group of 30 cases and a control group of 30 cases according to random number table method. The control group recieved Western medicine routine treatment combined with psychological intervention, the treatment group was treated with Shutiao Decoction on the basis of the control group, and the treatment course of both groups was 4 weeks. Before and after treatment, TCM syndrome score, Seattle Angina Scale (SAQ), Hamilton Anxiety Scale (HAMA) and Hamilton Depression Scale (HAMD) were evaluated. The adverse events were recorded during treatment and clinical efficacy was evaluated. Results:After treatment, the total effective rate in the treatment group was 90.0% (27/30), the control group was 73.3% (22/30), where the difference between the two groups was statistically significant ( Z=-2.515, P=0.012). After treatment, the TCM syndrome score (9.37 ± 6.47 vs. 18.37 ± 6.58; t=-5.341, P<0.01) of the treatment group was significantly lower than that of the control group. Among SAQ domains’ scores, the physical activity limitation, and the stable state of angina pectoris, angina attack in the treatment group were significantly higher than those of the control group ( t values were 2.294, 7.698, 4.677, 3.215, respectively, P<0.05 or P<0.01). The HAMA score of the treatment group was significantly lower than that of the control group (9.20 ± 5.26 vs. 12.17 ± 4.88; t=-2.266, P=0.027). The HAMD score in the treatment group was significantly lower than that of the control group (11.27 ± 5.83 vs. 15.03 ± 6.16; t=-2.433, P=0.018). During the treatment period, the incidence of adverse reactions was 3.33% (1/30) in the treatment group and 6.67% (2/30) in the control group. There was no statistically significant difference between the two groups ( χ2=0.351, P=0.554). Conclusion:Shutiao Decoction combined with western medicine routine treatment can improve patients’ TCM syndromes, relief angina pectoris, anxiety and depression.

7.Preparation and cell toxicity of a novel long-circulating paclitaxel-containing liposome

AIM: To prepare a novel long-circulating paclitaxel-containing liposomes, evaluate its quality and study its toxicity to gastric cancer cell line BGC823. METHODS: From May to November in 2006, the experiment was carried out in the Laboratory of Chemistry, Kaili College and the Laboratory of Physics and Chemistry Department, Southeast Guizhou Center for Disease Control and Prevention.①The novel long-circulating paclitaxel-containing liposomes was prepared by ultrasonic film method. Encapsulation efficiency and stability of the paclitaxel liposomes were measured by RP-HPLC.②The paclitaxel liposomes were dispersed in phosphate buffer at the pH value of 6.5, at 4 ℃ for 30 days, and the change of drug loads was observed.③The proliferation of BGC823 cell at logarithmic phase was tested by MTT method under the paclitaxel liposomes concentrations of 5, 10, 15, 20 mg/L. Meanwhile the paclitaxel free drug was taken as controls. RESULTS: ①Encapsulation efficiency of this paclitaxel liposomes was high to 97.6% detected by RP-HPLC.②When the paclitaxel liposomes were stored in refrigerator of 4 ℃, there was no flocculent precipitation during one month. Drug loaded in liposome after 3, 8, 15, 22, 30 days of preservation was respectively 97.3%, 96.2%, 95.6%, 94.9%, and 94.2%. The drug leakage of the paclitaxel liposomes was only 6% one month after dispersing in this buffer.③The result of in vitro cell toxicity showed that, under the drug concentration of 5, 10, 15, 20 mg/L, the inhibition rate to BGC823 cell of paclitaxel liposomes was 33.46%, 44.15%, 51.94%, 68.64% respectively, where those of free paclitaxel was 35.24%, 48.37%, 57.49%, 74.84%. CONCLUSION: ①This high-encapsulated and stable long-circulating paclitaxel-containing liposomes is expected to provide a more effective paclitaxel preparation.②The inhibition rate to BGC823 cell of the paclitaxel liposomes is low to that of free paclitaxel at the same drug concentration, indicating that it can present the slow release of liposomes.
8.Challenges and thoughts on precise hepatectomy

Precise hepatectomy is the application of minimally invasive concept in hepatic surgery. There are big challenges for surgeons to choose the proper approaches to achieve the aim of precise hepatectomy. How to choose therapeutic strategy, chemotherapy, radiotherapy, liver transplantation or radiofrequency ablation? How to choose the operation type, laparoscopic surgery, transabdominal surgery or Da Vinci robot-assisted surgery? How to choose the surgical instruments during hepatectomy? Although answers for these questions are various, the principle is unchangeable, which is providing minimal injury, less blood loss, fast recovery, little expense and good prognosis. The concept of precise hepatectomy includes precise judgment of liver function and careful preparation before operation, and also the elaborate nursing and multi-disciplinary cooperation during operation, as well as the fast track surgery after operation. Precise hepatectomy requires surgeons take the safety and effectiveness of the operations into account, and the conditions of hospitals, the skills and experiences of the surgeons should also be evaluated before operation. Finally, the economic condition of the patients should be considered and proper application of advanced equipments should be emphasized.
9.Determination of Butyltin Compounds in Sediments by Headspace Solid Phase Microextraction and Capillary Gas Chromatography with Flame Photometric Detector Using Quartz Surface-Induced Luminescence

Solid phase microextraction(SPME) can be effectively used to extract volatile and semivoltaile organic compounds from different environmental matrix. In this paper, we utilize headspace-SPME technique combined with capillary gas chromatography (CGC) and a laboratory modified flame photometric detector using quartz surfaceinduced luminescence (QSIL-FPD) for analyzing butyltin compounds in sediments. The detection limits of mono-,di-, and tri-butyltin compounds, based on the signal equal to 3 times the baseline noise are 16.9 ng/g, 1.58 ng/g adn 0.17 ng/g, respectively.
10.Update on the side effects of antithyroid drugs

Antithyroid drugs(ATD)is the main treatment for hyperthyroidism and its adverse reactions have been much concerned by physicians. Methimazole(MMI)and propylthiouracil(PTU)are the two common antitithyroid drugs used currently. Generally, the ATD are safe and effective, though their clinical adverse reactions are also relatively common. The toxic effects include liver damage and leukocytopenia, antineutrophil cytoplasmic antibody-associated pulmonary small-vessel vasculitis, hypoglycemia, allergic reactions, muscle impairment,and so on. They are usually reversible and disappear spontaneously when the drug is discontinued. However,the serious rare side effects can also occur and there may have potentially deadly threatening effects which need to be cautious for the clinicians. MMI is usually preferred over PTU because it has significantly fewer side effects. And unlike the dose-dependent side effects of MMI, there has no significant correlation between adverse reaction and drug dosage in using PTU. Moreover, PTU has more severe hepatotoxity than MMI, even fatal liver impairment and liver failure. The risk of liver damage from PTU is an important concern, particularly in children. For this reason, MMI is the first choice for treating children with hyperthyroidism.
