1.ADVANCES AND CURRENT STATUS IN THE STUDY ON QUALITY OF LIFE AFTER OPERATION OF RECTAL CANCER

Objective To summarize the advances in the clinical application of the quality of life(QOL) assessment after surgery of rectal cancer. Methods The recent literatures on the advances in the studies on the QOL assessment following the treatment of rectal cancer were reviewed. Results After surgery of rectal cancer, the patients might suffer from short and/or long term of various complications affecting the physical, social and psychological aspects of their well-being. The QOL assessment included many aspects of the studies. Global QOL scores as measured by the specific questionnaires improved significantly after surgery. It had provided clinical evidences of therapeutic methods, operative treatment of choices and bowel reconstructed procedures for the patients with rectal cancer.Conclusion The QOL studies after surgery for rectal cancer are of increasing importance. Apart from the improvements of the survival rates and the recurrence-free period, it is essential to provide an acceptable QOL postoperatively. The QOL assessment for the guidance of clinical treatment should be discussed routinely for a patient with potentially curable rectal carcinoma so that the excellent outcome could be achieved.

Methods The mutagenicities of water samples collected from 7 representative sampling points in man-made lake in Wujiang River were detected with micronucleus test in vicia faba root tip cells in Oct. 2000 and Oct. 2001 in Jinhua, Zhejiang. 3.Influence factor of the prognosis of traumatic subretinal hemorrhage after vitreo-retinal surgery

Objective To evaluate the influence factor of the prognosis of traumatic subretinal hemorrhage after vitreo-retinal surgery. Methods The clinical data of 50 patients with traumatic subretinal hemorrhage who had undergone vitreo-retinal surgery were retrospectively analyzed. All patients had ocular traurnatie history and subretinal hemorrhage diagnosed by fundus and B-scan examination; the preoperative visual acuity was less than 0. 1. According to different conditions, the traumatic eyes were treated with vitreo-retinal surgery, combined with lensectomy, retinotomy or silicone oil tamponade, respectively. The period of follow-up after surgery was 2-53 months, and the average period was 7.27 months. The corrected visual acuity and retinal reattachment at the last follow-up were observed. The visual acuity ≥0. 1 was the standard of good prognosis; retina[ reattachment was observed by indirect ophthalmoscope and color fundus photography. The prognostic factors mainly included type of injury, open or closed injury, the disease course, preoperative visual acuity, retinal detachment, hemorrhagic choroidal detachment, vitreous hemorrhage, the sites of submacular hemorrhage, methods of surgery. The relationships between those prognostic factors and visual acuity outcome or retinal reattachment were analyzed by χ2 test and logistic regression analysis. Results About 46.0% patients had good prognosis of the visualacuity. In the eyes with preoperative visual acuity of no light perception to hand moving and finger counting to 0. 1, the rate of good visual aeuitywas 34. 2% and 83.3%, respectively; the difference between the two groups was significant(χ2=8. 860, P = 0.003). In the eyes with or without preoperative retinal detachment, the rate of good visual acuity was 37.5% and 80. 0%, respectively; the difference between the two groups was significant (χ2=4. 232,P=0. 040). In the eyes with subretinal hemorrhage involving the macular fovea or not, the rate of good visual acuity was 34.4% and 66.7%, respectively; the difference between the two groups wassignificant (χ2 =4. 836,P=0. 028). All the other prognostic factors had no obvious effect on the retinal reattachment after the surgery. Conclusion Preoperative visual acuity, retinal detachment and submacular retinal hemorrhage were the important influence factors associated with prognostic visual acuity of eyes with traumatic subretinal hemorrhage after vitreo-retinal surgery.

5.Recombinant AAV-IFNαinhibits HBV replication in vitro

Objective_To identify the anti-HBV efficiency of AAV-IFNαby infecting HepG2.2.15 cell line, and to develop a new strategy of curing hepatitis B.Methods_Firstly,recombinant vector was packaged into AAV by three plasmids co-transfection method;then AAV-IFNαinfected HepG2.2.15 cell line, and samples were collected on the 1st, 3rd,6th and 9th day;thirdly, The time-course of HBV DNA,HBsAg and HBeAg were detected by qPCR and ELISA.The proliferation of HepG2.2.15 cells detected by MTT.Results_Recombinant AAV-IFNαcan significantly inhibit HBV replication and expression in vitro because of the proliferation of HepG2.2.15 cells inhibited by AAV6-IFNα( P<0.05) .Conclusions_AAV-IFNαis a potential antiviral product for hepatitis B.
6.Study on the injury effects and mechanisms of circulating histones on the hepatocytes of patients with hepatitis B virus-related liver failure

Objective To investigate the injury effects and mechanisms of circulating histones on the hepatocytes in patients with hepatitis B virus (HBV )-related liver failure (HBV-LF) .Methods Serum samples from patients with HBV-LF were collected . The levels of serum histone H3 , histone H4 , prothrombin activity (PTA ) ,total bilirubin (TBil) ,creatinine (Cr) and international normalized ratio (INR) of the patients were measured .Model for end-stage liver disease (MELD) score was calculated in the patients .The serum levels of histones were compared between patients and the healthy volunteers . The correlation of histones with the MELD score and PTA was analyzed .The human liver cell line L-02 cells were cultured and treated with the serum of patients or L-02 cellular lysate supernatant preincubated with or without anti-histone H3 and H4 antibodies .The cellular morphology and rate of apoptosis were observed .Intracellular calcium ion concentration and Caspase-3 activity were detected in the cultured L-02 cells treated with histones .Mean of two independent samples was compared using t tests .Relationship between histones and the MELD score or PTA was conducted using Spearman correlation analysis .Results The levels of serum histones in the patients with HBV-LF were much higher than those in the healthy volunteers (H3 :[5 390 .3 ± 1 032 .0] μg/mL vs [42 .7 ± 12 .8] μg/mL , t = 32 .76 , P < 0 .01 ; H4 :[4 205 .1 ± 662 .3] μg/mL vs [40 .3 ± 14 .6] μg/mL ,t = 39 .74 , P< 0 .01) .In addition ,serum histones (H3/H4) levels in patients were negatively correlated with serum PTA (r= - 0 .325 ,P= 0 .038 and r =- 0 .572 ,P= 0 .028 ,respectively) ,but positively correlated with the MELD score (r= 0 .359 ,P= 0 .021 and r = 0 .568 , P = 0 .007 , respectively ) . Both serum of patients with HBV-LF and L-02 lysate supernatant were toxic to cultured L-02 cells .The injury effect was inhibited by anti-histone antibodies ([9 .3 ± 1 .5]% vs [14 .3 ± 0 .6]% , t = 4 .259 , P= 0 .02) .L-02 cells treated with calf thymus histone were cultured for 4 h . Cellular toxicity of histones resulted in Caspase-3 activation . The effect was inhibited by anti-histone antibodies ([3 .5 ± 0 .5]% vs [5 .2 ± 0 .6]% ,t= 4 .243 ,P= 0 .02) .Conclusion The elevated circulating histones in the patients with HBV-LF may aggravate the liver damage .
7.The combination ofKangfuxing liquid, ethacridine lactate and local oxygen therapy in the treatment of radiation-induced skin damage

Objective To investigate the efficiency ofKangfuxing liquid, ethacridine lactate in combination with local oxygen therapy for radiation-induced skin damage.Methods A total of 96 patients affected by radiation-induced skin damage were enrolled and randomly allocated to a treatment group and a control group by random number table method, with 48 patients in each group. All patients in the control group were treated with trolamine cream, and those in the treatment group were treated withKangfuxing liquid, ethacridine lactate in combination with local oxygen therapy. Skin dermatitis was scored utilizing Radiation Therapy Oncology Group (RTOG) toxicity scale.Results The total effective rate in the treatment group was significantly higher than that in the control group (97.9%vs.79.1%;χ2=10.119,P=0.002). The distribution of patients with RTOG grades 0 to 4 showed different degrees of improvement, and the number of patients with RTOG 0-4 grades changed from 0, 3, 9, 12, 24 before the treatment to 8, 12, 22, 6, 0 after the treatment in the treatment group, 0, 2, 10, 25, 11 to 2, 5, 18, 23, 0 in the control group. The percent of patients with RTOG grade 0 in the treatment group was significantly increased than that in the control group (16.7%vs. 4.2%;χ2=4.019, P=0.045) and the percent of patients with RTOG grade 3 significantly decreased (12.5%vs.47.9%;χ2=12.649, P<0.001). The percents of patients with dispirited spirit (31.3%vs. 58.3%;χ2=6.066,P=0.014), dry and chapped skin (20.8%vs. 58.3%;χ2=12.588,P=0.000) in the treatment group were all significantly lower than those in the control group.ConclusionKangfuxing Liquid, ethacridine lactate in combination with local oxygen therapy can improve radiation-induced skin damage.
8.Intracranial pressure monitoring in the management of traumatic brain injury

Intracranial hypertension remains the key biomarker of severe traumatic brain injury for neurosurgery doctors. The monitoring of intracranial pressure (ICP) provides the technical support of precision and effective treatment strategy. In this article, the authors analyze the methodology, timing, function and development trend of ICP monitoring. The developing process of ICP monitoring contains the efforts of exploring a safe and precise technique to reflect the pressure in an injured brain. The modern ICP monitoring technology provides sufficient information flow for the management of craniocerebral trauma. Neurosurgeons could follow the information in the value and trends of ICP monitoring and implement it into decision making throughout the whole process of patient management. With the advanced data collecting and analyzing system the clinician can look into the waveform and parameter generalized by ICP value, and can interpret to the pathophysiological profiling in brain. ICP monitoring could exert efficacy not only in reflecting the mechanism of brain injury but also in the directing the clinical practice.
9.Analysis and countermeasures of suspected medical devices adverse events of hospital

Objective:To sort out cases of suspected medical device adverse events in Subei hospital, analysis for the reasons of adverse events, and propose measures to ensure the safe clinical use of medical devices.Methods: From the National Adverse Drug Reaction Monitoring Centre database, 300 cases of suspicious medical device adverse events were selected from cities and provinces. The number of categories of adverse events involving medical devices and the main reasons resulting adverse events were analyzed by SPSS.Results: The main reasons resulting the medical device adverse events were quality-related issues of the product, biological incompatibility existing between the patient and the physical device, and improper operation or use.Conclusion: The enterprises of medical devices should product on the basis of GMP strictly to ensure the quality of product. The medical institutions should ask hypersensitivity of patients actively and operate on the basis of SOP strictly to improve the work of monitoring and reporting.

Faced with huge demand for building public health capacity in the post-Ebola era, Africa Union has planned to establish Africa Center for Disease Control and Prevention ( Africa CDC,ACDC), with headquarters in Addis Ababa, Ethiopia, and at least 5 regional collaborating centers in each region of the continent .The strategic objectives of ACDC focus on building a continental integrated public health system and strengthening needed public health capacity .The current study analyzed the background , plans, development layout and strategy of ACDC , and provided references for China-Africa cooperation in the areas of public health and for the formulation of China global strategies of biosecurity and disease prevention and control .
