1.Experimental study on lauromacrogol in sclerotherapy of endometrial cyst models

Objective To investigate the value of the lauromacrogol in sclerosis therapy of endometrial cyst models of SD rat.Methods 45 SD rats were randomly divided into lauromacrogol treatment group,dehydrated alcohol treatment group and physiological saline blank group after being built cysts models.Then corresponding medicine was injected into the cysts,and the changes of pathomorphology and VEGF expression of ectopia endomembrane were observed a week later.Results Microscope observation indicated that endometrial cyst glandular epithelium and gland were inactived by both lauromacrogol and dehydrated alcohol.The effective rate of lauromacrogol group was 87.5%,and that of dehydrated alcohol group was 85.0%.There was no significant difference between these two methods (P =1.000).The ectopia endomembrane glandular organs of physiological saline group were not destructed obviously.Besides,VEGF expression of lauromacrogol and dehydrated alcohol group were lower than physiological saline group (P =0.003 and 0.006).There was no significant difference between VEGF expression of lauromacrogol and dehydrated alcohol group (P =0.926).Conclusions Compared with dehydrated alcohol,lauromacrogol had the same therapeutic effect on sclerosis therapy of endometrial cyst models.
2.Kinetics of municipal sewage degradation in EGSB and UASB reactors at 10℃

Kinetics of municipal sewage degradation in Expanded Granular Sludge Bed (EGSB) and Up-flow Anaerobic Sludge Blanket (UASB) reactors at 10℃ were investigated via continuous experimental equipments. The results indicated that the whole reaction process can be simulated by the first-order dynamic equation model. Dynamic parameters such as k, Vmax and Ks of UASB in hydrolysis acidification stage were 1.08 d-1, 2.8 d-1 and 372 mg/L comparing to those of 1.18 d-1, 3.5 d-1 and 112 mg/L in the methanogenesis stage respectively. The EGSB's k, Vmax and Ks were 2.91 d-1, 14.3 d-1 and 470 mg/L in the hydrolysis acidification stage comparing to those of 1.68 d-1, 6.6 d-1 and 103 mg/L in the methanogenesis stage respectively. Comparison of k values of the two stages in UASB and EGSB indicates that hydrolysis acidification stage is the controlling step for the whole reaction process of UASB, while methanogenesis stage is the controlling step in EGSB. Compared with UASB, municipal sewage treatment by EGSB at 10 ℃ can reach the same effluent requirement with lower retention time due to its effluent recirculation.

AIM: To investigate the role of the nucleus tractus solitarius (NTS) in the regulation of paraventricular nucleus (PVN) AVP-ergic neurons on gastric ischemia- reperfusion injury (GI-RI). METHODS: Male SD rats were used in experiments. The celiac artery were clamped for 30 min and reperfused 1 h by removal of the clamp to obtain the ischemia-reperfusion state. The mechanism was analysed with nucleus electrical stimulation, electrolytic lesion and nucleus microinjection technique. AIM: To determine the effect of electrical stimulation of lateral hypothalamic area(LHA) on gastric ischemia-reperfusion injury(GI-RI) in rats and to analyse its possible neuroregulatory mechanisms. METHODS: The methods of electrical stimulation(ES), chemical stimulation, electrolytic lesion(EL) and denervation were used to investigate the effect of LHA ES on gastric mucosal injury in rats subjected to 30 min gastric ischemia followed by 60 min reperfusion and to analyse the role of dorsal vagal complex(DVC), vagus and sympathetic nerve in this effect. RESULTS: ①Electrical stimulation of LHA and microinjection of L-glutamic acid into LHA obviously aggravated GI-RI; ②Electrolytic lesion of the LHA attenuated the GI-RI;③DVC lesion eliminated the effect of electrical stimulation of LHA on GI-RI; ④Vagotomy or sympathectomy eliminated the effect of electrical stimulation of LHA on GI-RI too.CONCLUSION: These results indicate that the LHA is a specific area in the CNS for exerting aggravative effects on the GI-RI; the DVC, vagus and sympathetic nerve may be involved in regulatory effects of LHA on GI-RI.
5.Effects of jejunal infusion of amino acids on secretion of gastrointestinal hormone in dog model

Objective To investigate the effect of jejunal infusion of amino acids on secretion of gastrointestinal hormone in healthy dogs.Methods Six healthy adult dogs were treated with jejunal fistulas and femoral vein intubation.Twenty-four hours after the operation,solution of 8 different amino acid monomers (experimental group) or normal saline (control group) were infused into the jejunum of the dogs every 24hours.The levels of cholecystokinin (CCK),motilin,and gastrin in the peripheral plasma were measured using radioimmunoassay at the start of infusion (0 minute),and 30,60,90,and 120 minutes after infusion.Results Compared with the control group,the serum CCK level in the phenylalanine group was significantly higher 30 and 60 minutes after infusion [(1.25 ±0.19) ng/L vs.(0.66 ±0.14) ng/L,(1.23 ±0.12) ng/L vs.(0.80 ± 0.03) ng/L,both P < 0.01],while that in the tryptophan group was significantly higher 30 minutes after infusion [(1.08 ±0.26) ng/L vs.(0.66 ±0.14) rig/L,P <0.01].The other measurement results showed no statistically significant differences.Conclusions Jejunal infusion of phenylalanine or tryptophan may stimulate the secretion of gastrointestinal hormone to some extent.Aromatic amino acids (phenylalanine and tryptophan) is more potent in triggering the release of CCK than aliphatic (leucine,isoleucine,and methionine) and charged amino acids (aspartic acid,arginine,and glutamate).The mechanism may be related to the properties of the amino acids.
6.Diagnostic potential of Dickkopf-1 for heptocellular carcinoma using magnetic solid phase chemiluminescent immunoassay

Objective This is the first study to explore clinical application value of serum Dickkopf-1 (DKK-1) detection in diagnosis of heptocellular carcinoma (HCC) by magnetic solid phase chemiluminescent immunoassay.Methods The level of serum DKK-1 and AFP in 205 cases of HCC,40 cases of liver cirrhosis,and 200 cases of healthy control were quantitatively detected by Magnetic solid phase chemiluminescent immunoassay.The area under ROC curve,sensitivity and specificity of DKK-1 and AFP for diagnosing HCC were calculated.Results The serum level of DKK-1 in HCC group was significantly higher than those of the liver cirrhosis group and healthy control group (P<0.01).DKK-1 maintained diagnostic sensitivity for patients with HCC who were alpha-fetoprotein (AFP) negative (66.3%).ROC curves showed optimum diagnostic cut-off value was 2.4 ng/mL,area under curve (AUC) was 0.822 (95% CI:0.783-0.856),sensitivity 65.9%,and specificity 87.5%).Moreover,measurement of DKK1 and AFP together improved diagnostic accuracy for HCC versus all controls compared with either test alone [AUC 0.915,95%CI:0.886-0.940),sensitivity 81.5 %(P<0.05)].Conclusion Serum DKK-1 detection has an important clinical value for diagnosis of HCC,especially for HCC with AFP negative.The combined detection of serum DKK-1 and AFP can greatly increase sensitivity and accuracy for diagnosing HCC.
7.A case report of myelodysplastic syndrome treated with allogeneic transplantation of HLA-identical sibling using culture-expanded mesenchymal stem cells and hematopoietic stem cells originated from HBV infected donor

Objective To evaluate the safety, efficiency and feasibility of HLA-identical sibling using culture-expanded mesenchymal stem cells and hematopoietic stem cells in treatment for myelodysplastic syndrome (MDS). Also to investigate for valid preventive measures to avoid the infection of HBV originated from donor. Methods A 46-years-old male patient with myelodysplastic syndrome-refractory anemia (MDSRA) got a cotransplantation of culture-expanded mensenchymal stem cells (MSC) and hematopoietic stem cells (HSCs) from HLA-identical sibling donor (his sister) who was infected by hepatitis B virus (HBV). Some measures were applicated in order to avoid the recipient from getting a HBV infection. The antiviral therapy to the donor was began early at the time 1 month before transplant, and HBV vaccine inoculation was used 2 month before transplant. High titer of anti-hepatis B immunoglobulin was used 1 week before transplant and 1 month after transplant the use of prophylactic anti-hepatis B drug treatment was begun. A non-myeloablative preparative regimen included fludarabine monophosphate (Flu, 120 mg/m2), cyclophosphamide (Cy, 1200 mg/m2)and antithymocyte globulin (ATG, 15 mg/kg) was given to him before culture-expanded mesenchymal stem cell and allogeneic peripheral blood stem cell from his HLA-matched sister. Results The regimen was well tolerated, and hemopoiesis was reconstituted on day 10 after transplant, idiochromosome detected by fluorescent in situ hybridization on day 30 showed XY 47/300 and on day 90 it was 7/300. No evidence of HBV infection was detected on day 60 after transplant. Conclusion The clinical course of this patient indicate that HLA-identical sibling culture-expanded mesenchymal stem cell transplantation combined with non-myeloablative stem cell transplantation can be an effective and safe approach in treatment of MDS.
8.Treatment of complex bone nonunion with external skeletal fixation.

OBJECTIVESTo summarize the experience in the treatment of 112 cases of complex bone nonunion from 1982 to 1999 in our department and introduce the technique of external skeletal fixation.
METHODSThe two fragment ends of all cases were fixed under pressure with half-ring sulcated external skeletal fixator. Those cases complicated by bone defect or limb shortening were operated on with epiphysiotomy to restore the length of the limb in the period of compressive fixation or after the occurrence of bone union according to the condition of complicated infection and the length of the limb shortened.
RESULTSThe nonunion of the 112 cases was united eventually. The infection in 34 cases was eradicated. Bone union in cases without infection took 3 approximately 7 months (average 5.2 months) and in cases with infection took 5 approximately 11 months (average 5.5 months). The length of the limb in 11 cases with bone defect was restored in the same period of compressive external fixation and another 8 cases achieved after bone union. The length between the injured and healthy limbs was balanced.
CONCLUSIONSWhen external skeletal fixation is employed to treat those troublesome cases of bone nonunion, the pins for fixation are inserted in sites far from the lesions and the non-united fragment ends are exposed only in the area without scars. Consequently, there is little interference with the blood circulation and the osteogenic potency of the fragment ends. The sclerotic bone tissue is not excised, the marrow cavity is not chased to be open and the fragment ends are only moderately modified. As a result, the stability of fixation is increased and further shortening of the limb avoided. External skeletal fixation using small pins with cross penetration results in plastic fixation and promotes bone healing. Bone lengthening with epiphysiotomy can restore the balance of the limbs.

9.Clinical effects of self-expandable metallic stent insertion plus laparoscopy surgery for malignant colorectal obstruction in aged patients

Objective To investigate the clinical effects of the stent-laparoscopy approach in treatment of elderly patients with malignant colorectal obstruction.Methods Clinical data of 55 elderly hospitalized patients with colorectal cancer-induced bowel obstruction in Wenzhou Central Hospital from January 2009 to January 2014 were evaluated retrospectively.All patients were treated with expandable metallic stents for remission of bowel obstruction.Patients were divided to laparoscopy surgeryobservation group (n =14) receiving laparoscopic surgery after decompression and the laparotomy surgery-control group (n=41) receiving conventional open colorectal cancer operation.The intraoperative information,postoperative complications and long-term curative effect were compared between the laparoscopy surgery-observation vs laparotomy surgery-control group.Results During laparoscopy surgery period,two patients operated by laparoscopy surgery were converted to laparotomy surgery.Surgical time was significantly longer in the laparoscopy surgery-observation group than in laparotomy surgery-control group[(178 ± 33) min vs.(145 ± 31) min,t =3.384,P =0.001],and bowel function recovery and postoperative hospital stay were significantly shorter in the laparoscopy surgery-observation group than in laparotomy surgery-control group[3.1 ± 0.9) d vs.(4.3 ± 1.3) d and (7.1±1.3) d vs.(12.6±5.7) d,t=3.193 and t=2.911,P=0.002 and P=0.005].Intraoperative blood loss was less in the laparoscopy surgery-observation group than in the laparotomy surgery-control group[(63 ± 29) ml vs (86 ± 37) ml,t =2.11,P =0.04],with no significant differences in postoperative complications between the two groups (0.0 vs 9.76 %,x2 =1.47,P=0.225).Fifty-five patients were followed up for 2 years.All patients survived in the laparoscopy surgery-observation group,but one patient died from the cerebrovascular accident in the laparotomy surgery-control group.Conclusions The stent-laparoscopy approach to treat colorectal cancer (CRC) patients with acute colorectal obstruction is a safe,effective,recovery quick and minimally invasive option for elderly patients.Emergency surgery may be converted to a limited operation by this method.Laparoscopic radical surgery with one-stage anastomosis is feasible.
10.Study on the expression and clinical significance of MMP9 and MVD in the external auditory canal and middle ear squamous cell carcinoma.

OBJECTIVE:
To investigate the expression and clinical significance of MMP9 and MVD in the carcinogenesis of squamous cell carcinoma of external auditory canal and middle ear.
METHOD:
Immunohistochemical SP method was used to detect the expression of MMP9 and MVD proteins in 26 squamous cell carcinoma tissues of external auditory canal and middle ear and 20 normal external ear canal skin tissues.
RESULT:
The positive rate of MMP9 in squamous cell carcinoma tissues of external auditory canal and middle ear was 73.1% (19/26) lower than that in the normal external ear canal skin tissues 25.0% (5/20). The positive rates of CD34 were 33.58 +/- 3.04 and 22.50 +/- 5.22, respectively. The positive rates of MMP9 and CD34 were correlated with the histological grade and tumor grade, but had no relationship with age and sex. The positive rates between MMP9 and CD34 were related (r=0.42, P<0.05).
CONCLUSION
MMP9 may be involved in the carcinogenesis of squamous cell carcinoma of external auditory canal and middle ear, and may play an important role in the invasion and metastasis of squamous cell carcinoma of external auditory canal and middle ear. MMP9 and CD34 play a cooperative role in the process of squamous cell carcinoma of external auditory canal and middle ear.
