BACKGROUND:Mesenchymal stem cels have the capacity of self-renewal and differentiation into certain lineage cels under appropriate conditions. But many mechanisms are unknown until now. OBJECTIVE:To clarify the role of miR-302 in the regulation of osteogenic and adipogenic differentiation of adipose-derived mesenchymal stem cels. METHODS:Chemicaly synthesized miR-302 specific mimics were transfected into adipose-derived mesenchymal stem cels as experimental group. miR-NC, miR-302b negative control mimics, was transfected into another cels as control group. By the experiments of alkaline phosphatase staining, alkaline phosphatase activity assay, alizarin red staining, oil red O staining and extraction test, the effect of miR-302 upregulation on the adipogenic and osteogenic differentiation of adipose-derived mesenchymal stem cels was analyzed and compared. Western blot assay was used to detect the expression of Runx2 and alkaline phosphatase after regulation of miR-302. RESULTS AND CONCLUSION: (1) Overexpression of miR-302 decreased the precipitate and activity of alkaline phosphatase significantly as compared with the control group (P < 0.05). (2) Overexpression of miR-302 inhibited the formation of mineral deposits and calcium nodules, and the number of calcium nodules in the experimental group was significantly lower than that in the control group (P < 0.05). (3) The number of cels positive for oil red O staining was significantly higher in the experimental group than the control group, which further showed the absorbance values of oil red O staining in the experimental group obtained in the extraction test were significantly increased (P < 0.05). (4) At 6 days of osteogenic induction, the expressions of Runx2 and alkaline phosphatase in the experimental group were decreased to different extents. These findings indicate that overexpression of miR-302 can suppress osteogenesis and accelerate adipocytes generation of adipose-derived mesenchymal stem cels. miR-302 plays a two-way regulatory role to balance the osteogenic and adipogenic differentiation of mesenchymal stem cels.

Objective To explore the status of infections of soil-transmitted nematodes of residents in Shuyang County,Jiang-su Province from 2011 to 2012. Methods Totally 2 140 residents of Machang Town and Longji Town in Shuyang County,Jiangsu Province were investigated,and their stool samples were collected and detected by Kato-Katz technique,and in addition,the chil-dren aged 3-12 years were examined by the cellophane anal swab method,in 2011 and 2012. Results In 2011,the total infec-tion rate of soil-transmitted nematodes was 1.06%(11/1 034),the infection rate of hookworm was 1.06%(11/1 034),the infec-tion rate of Ascaris lumbricoides was 0,the infection rate of Trichuris trichiura was 0,and the infection rate of Enterobius vermicu-laris was 0;In 2012,the total infection rate of soil-transmitted nematodes was 0.99%(11/1 106),the infection rate of hookworm was 0.27%(3/1 106),the infection rate of A. lumbricoides was 0.09(1/1 106),the infection rate of T. trichiura was 0,and the in-fection rate of E. vermicularis was 2.69%(7/223). Conclusion The infection rate of soil-transmitted nematodes of residents in Shuyang County is low,but the surveillance is still necessary.

Objective To research the forming technology of Chuansu Jiuxin dropping pills.Methods The pills were prepared by dropping method.The forming technology of the pills was optimized by single factor test and orthogonal experiment by using the amount of PEG4000 to PEG6000, ratio of drug-matrix ,dropping temperature and speed as factors and with pill weight RSD,the time limit of collapse and appearance quality of the pills as evaluation indexes.Results Using PEG 4000 and PEG 6000 as matrix with the ratio of 4:1 and dimethyl silicone oil(100 mPa· s)as refrigerant at 5~15℃,the optimized parameters for the forming technology were as follows:the ratio of herbal extract to matrix was 1:3,the dropping temperature was at 90 ℃ and the dropping speed was 35 drops per minute,dropping from 4 cm.Conclusion The optimized technology is sample and feasible,and can provide a reference for industrial production of Chuansu Jiuxin dropping pills.

Objective To investigate the correlation between the olasma concentrations of angiotesin Ⅱ,aldosterone and the parameters that reflected the reconstruction of left ventricular in chronic heart failur(CHF)patients.Methods 61 CHF pmiems and 20 normal control subjects were enrolled.plasma concentration of angiotesin Ⅱ,aldosterone were measured by specific radioimmunoassays.Two dimensional,M-mode and Doppler ultrasound recording were obtalned to determine left vantricular mass index(LVMI),left ventricular ejection fraction(LVEF)and maximal early transmitral flow velocity in diastole(VE)and maximal late transmittal flow velocity(VA).Results Baseline plasma concemrations of angiotesin Ⅱ,aldostemne were increased significantly in CHF patients as compared with normal thme of control group and were paralleled to the increase of NYHA cardiac functional classification of CHF(P＜0.01).Plasma concentrations of angiotesin Ⅱ,aldostemne were positively correlated with LVMI but negatively correlated with VE/VA(P＜0.01).Conclusion Plasma concentrations of angiotesin Ⅱ,aldosterone were increased signifcantly in CHF patients and might be involved in and regulated the reconstruction of left ventricular in CHF patients.

[Objective]To observe the effect of treating hallux valgus with modified Mitchell operation.[Method]A total of 18 cases(28 feet) of hallux valgus were treated with modified Mitchell operation from Apr.2000 to Oct.2003.The average preoperative hallux valgus angle(HVA) was 30.5 according to the X-ray images,while the average intermetatarso-phalangeal angle(IMA) was 13.5.The difference between the modified Mitchell operation and normal one was that only one transverse osteotomy need to be carried out in the distal metatarsal without lateral ridge left.The distal position of osteotomy was extended according to the sum of IMA and then moved to the metatarso side by 2~3 mm.Absorbable screws were used for fixation.[Result]All 28 feet were followed up for 10~32 months.It showed excellent result in 24 feet,good in 3,and bad in 1,with the total satisfactory rate of 92.9%.According to the postoperative weight-loading X-ray checking result,the average HVA was 15.5,with an average improving of 15;the average IMA was 8.5,with an average improving of 5.[Conclusion]The modified Mitchell operation can correct the metatarso-varus without destroying the biomechanical status.The loading function of great toe could be resumed by the removing of the first metatarso head.

OBJECTIVE To investigate the resistance and the disinfectant-resistant genes of chlorhexidine-sulfadiazine-resistant Escherichia coli isolates from hospitalized patients in the Lishui People's Hospital.METHODS Microdilute tests were performed to detect susceptibility to 19 kinds of antimicrobial agents in 60 E.coli strains,and the qacE△1 gene was detected by PCR.RESULTS The resistant rates to other antimicrobial agents arranged in 5.0-90.0%.The prevalence of ESBLs in isolates was 45.0%.qacE△1 Gene was 56.7%.CONCLUSIONS The 60 strains are multiple-drug-resistant.There are very high positive percentages of qacE△1 gene in E.coli isolates from the hospital.

Objective: To analyze the methods of Lishui Central Hospitals performance management, and evaluate the important role of performance management in the modern hospital management. Methods:Evaluate the hospital operation management data in recent 5 years, basing on the performance management measures of Lishui Central Hospital. Results: During 2007 to 2012, outpatients and emergency patients increased to 80.41%, discharges increased to 89.35%, operating table increased to 84.59%, total revenue increased to 121.61%, average length of stay decreased by 5.1 days, times cost of patients had been effectively controlled, and total assets increased by 149.21%. Conclusion: The reform of Central Hospital performance management has great significance in mobilizing the enthusiasm of workers and enhancing the level of hospital management, and will solve the difficulties of expensive medical treatment problems well.

Objective To investigate the prevention effect of Ezetimibe with loaded dose on hematologic parameters and re-infarction in patients undergoing PCI.Methods 240 cases of patients with non ST segment elevation myocardial infarction performed PCI from October 2012 to October 2014 were selected and randomly divided into two groups,the control group received conventional dose Ezetimibe 10 mg every time,qd,the study group received loading dose ezetimibe 20 mg every time, qd, both two groups treatment 30 days.The inflammatory factors, myocardial injury and vascular endothelial function were compared before, 24 h and 30 d after the operation, and patients were followed up for two years,record adverse reactions. Results Compared with before treatment,levels of serum inflammatory factor,myocardial injury and vascular endothelial function index in different time points had differences by single factor analysis of variance(P<0.05),and compared with the control group, levels of serum hs-CRP,TNF-α,IFN-γand IL-6 in the study group were lower, levels of serum CK-MB,cTnI and BNP were lower,the contents of ET in serum were lower,and the contents of NO were higher(P<0.05).The improvement degree in the study group of myocardial microcirculation perfusion after treatment was higher(P<0.05).No case shedding phenomenon occurred during the two year follow-up, and there was no statistical significance in the incidence of cardiovascular events between two groups.Conclusion Loading dose of ezetimibe can protect myocardium,and it can inhibit the inflammatory reaction and vascular endothelial injury caused by operation,it also can improve myocardial microcirculation.

Objective To assess the efficacy of radiofrequency catheter ablation ( RFCA) in premature ven-tricular contractions ( PVCs) by electrophysiological mapping.Methods 60 patients with symptomatic PVCs original from the right ventricular outflow tract underwent RFCA guided by pace mapping and activation mapping,ablation was performed by a catheter with temperature control.The patients were divided into 3 groups,20 cases in each group.The first group ( the combination group ) was the combination with pacing mapping and activation mapping underwent RFCA.The second group ( the activation mapping group) was guided by activation mapping underwent RFCA.The third group( the pacing mapping group) was guided by pacing mapping underwent RFCA.Successful ablation immedi-ate end was that PVCs disappeared and could not be induced by isoproterenol.The success rate,time and composition ratio of the three groups were observed.Results The immediate success rate of ablation of either the combination group or the activation mapping group was higher than that of the pacing mapping group.But the ablation time of the combination group was the shortest.There was no statistical difference between the baseline results of each test group. The cardiac arrhythmia originated from the septum was usual (45/60,75.0%),and the arrhythmia originated from the free wall (15/60,25.0%) was less.The success rate of PVCs ablation of right ventricle was high,and the total success rate was 91.7%(55/60).There was no significant difference in the success rate of the combined mapping and the activation mapping (χ2 =0.084,P>0.05),but were significantly higher than those in the pacing mapping group (χ2 =0.032,0.047,all P<0.05).The ablation time of the pacing mapping group and the combination group were significantly lower than that of the activation mapping group(all P<0.05).Conclusion The effect of the com-bination with pacing mapping and activation mapping is the best.Pacing mapping is better than activation mapping during RFCA of PVCs.

Objective To investigate the effects of diammonium glycyrrhizinate (DG) on the expression of Toll-like receptor 4 in rat' s kidney poisoned by paraquat (PQ).Methods The PQ (100 mg/kg) were administered via a gastric tube to make an animal model of acute kidney injury in rats.Then DG (50 mg/kg per 24 h) were administered immediately by intraperitoneal injection in the DG group.The ELISA assay was applied to measuring the IL-6 and IL-10 in kidneys and in blood.And the protein expressions of TLR-4, Myd88 and NF-κB P65 were examined by Immunohistochemistry.The gene expressions of TLR-4, Myd88 and NF-κB on kidneys were detected by RT-PCR.Results The levels of serum Cr, BUN , IL-6 and renal IL-6 in PQ group were significantly higher than those in NS group (F =266.014, 168.160, 63.279, 192.997, respectively, P ＜0.01).While, the levels of serum Cr, BUN , IL-6 and renal IL-6 in DG group were significantly lower than those in PQ group (F =266.014, 168.160, 63.279, 192.997, respectively, P ＜ 0.01).And the levels of serum and renal IL-10 in PQ group were higher than those in NS group (F =87.511, 79.473, respectively, P ＜ 0.01).But the levels of serum and renal IL-10 in DG group were lower than those in PQ group (F =87.511, 79.473, respectively, P ＜0.01).The results of PCR confirmed that the expressions of TLR-4 mRNA , myd88 mRNA and NF-κB mRNA in PQ group were higher than those in NS group (F =84.408, 60.683, 86.272, respectively, P ＜0.01), while, the expressions of TLR-4 mRNA , myd88 mRNA and NF-κB mRNA in DG group were lower than those in PQ group (F =84.408, 60.683, 86.272, respectively, P ＜ 0.01).The protein levels of TLR-4 , Myd88 and NF-κB P65 in PQ group were higher than those in NS group (F =79.139, 61.799,112.740, respectively, P ＜0.01).While the protein levels of TLR-4, Myd88 and NF-κB P65 in DG group were lower than those in PQ group (F =79.139, 61.799, 112.740, respectively, P ＜ 0.01).Conclusions Diammonium Glycyrrhizinate can attenuate rat's acute kidney injury caused by paraquat poisoning and decrease the expressions of TLR-4, Myd88 and NF-κB.
