1.The role of Egr-1 in liver injury in acute pancreatitis in rat

Objective To evaluate the role of early growth response gene1 (Egr-1) in liver injury in taurocholate-induced acute pancreatitis rat model. Methods Twenty-four male rats were divided into 4 groups. Egr-1 immunohistochemistry staining and pancreatic pathologic scoring were assessed, and the serum levels of AST, LDH, TNF-? and IL-1 ? were measured. Egr-1 mRNA levels of primary hepatocyte culture stimulated with TNF-? and pretreated primary hepatocytes with PD98059 followed by TNF-? stimulation were also observed. Results Egr-1 expression of liver correlated with degree of liver injury in acute pancreatitis in rat, and high Egr-1 mRNA expression of primary hepatocyte was observed when hepatocyte injury occurred. After pretreated with PD98059, TNF-?-stimulated hepatocytes showed a lower level of Egr-1 mRNA compared with hepatocytes without pretreatment. Conclusions Egr-1 may play an important role in liver injury in acute pancreatitis, and this effect depended partly upon extracellular signal-regulated kinase 1/2 (ERK1/2) pathway.
2.Respiratory motion correction in positron emission tomography imaging using elastic registration based on sinogram data.

In the process of positron emission tomography (PET) data acquiring, respiratory motion reduces the quality of PET imaging. In this paper, we present a correction method using three level grids B-spline elastic method to correct denoised and reorganized sinograms for respiratory motion correction. Using GATE simulates NCAT respiratory motion model to generate raw data which are used in experiment, the experiment results showed a significantly improved respiratory image with higher quality of PET, and the motion blur and structural information were fixed. The results proved the method of this paper would be effective for the elastic registration.

3.Experience in dealing with iatrogenic bile duct injury due to laparoscocpic cholecystecomy

Objective To explore the diagnosis and treatment of iatrogenic bile duct injury during choledochocystectomy. Methods Clinical data of 24 cases of iatrogenic bile duct injury were analyzed retrospectively from 2005 to 2009. Results Eight patients underwent early repair of bile duct within 4 days after the injury. 14 patients with later recognized bile duct injury underwent selective operations, and two patients were discharged after their jaundice were relieved without any surgical treatment. Supporting T tubes were left in place in 22 patients for 8 to 14 months after operations, without any biliary obstruction found after one year and six months to five year follow-up. Conclusions Early bile duct injury within 4 days was easily treated, but later bile duct injury should have selective operation. The patients with jaundice could be diagnosed with PTCD and ERCP to observe the bile duct injury. During operation membrane-to-membrane wide hepatojejunostomy helps prevent later anastomotic stenosis. The left in drainage tube in anastomotic stoma for more than 8 months improves success rate in the process of injuried bile duct repair.
4.Effect of enhanced recovery after surgery on the surgical treatment of patients with radiation enteritis and intestinal obstruction

Objective:To investigate the effect of enhanced recovery after surgery (ERAS) on the surgical treatment of radiation enteritis with intestinal obstruction.Methods:A total of 80 patients with radiation enteritis and intestinal obstruction admitted to the Department of General Surgery, the Eighth Center of Chinese PLA General Hospital from June 2015 to December 2019 were selected and divided into observation group and control group according to the principle of baseline feature matching, with 40 cases in each group.Fourty cases in the control group received conventional surgical treatment combined with conventional rehabilitation intervention, while 40 cases in the observation group received conventional surgical treatment combined with ERAS intervention.According to hemoglobin (HGB), albumin (ALB), prealbumin (PA), transferrin (TRF), the nutritional status of patients in the two groups was compared before and after intervention.According to interleukin-2 (IL-2), interleukin-6 (IL-6), interleukin-8 (IL-8), hypersensitivity C-reactive protein (hs- CRP), tumor necrosis factor-α (TNF-α), the changes of inflammatory factors in the two groups was compared before and after intervention.The immunoglobulin (Ig) A, G and M of the two groups was compared before and after intervention.The postoperative recovery and the incidence of postoperative complications was observed in the two groups.Results:After the intervention, the level of HGB(125.56±11.18) g/L, ALB(42.46±3.95) g/L, PA(0.28±0.03) g/L and TRF(2.60±0.30) g/L in the observation group was higher than that in the control group (102.95±11.12), (36.28±4.25), (0.20±0.05), (2.09±0.27) g/L, respectively, and the differences between the two groups were significant (all P<0.001). After the intervention, the level of IL-2(3.69±0.79) ng/L, IL-6(15.79±7.17) ng/L, IL-8(6.24±1.25) ng/L, hs-CRP(12.51±2.34) ng/L, TNF-α(1.51±0.68) μg/L in the observation group was lower than that in the control group(7.26±1.23) ng/L, (23.82±6.95) ng/L, (9.13±1.71) ng/L, (17.63±2.27) ng/L, (2.02±0.81) μg/L, respectively, and the differences between the two groups were significant ( P<0.001, P<0.001, P<0.001, P<0.001, P=0.003). After the intervention, the level of IgA(1.92±0.63) g/L, IgG(11.36±1.26) g/L, IgM(2.01±0.57) g/L in the observation group was higher than that in the control group (1.62±0.49), (9.58±1.23), (1.60±0.47) g/L, respectively, and the differences between the two groups were significant ( P=0.020, <0.001, =0.001, respectively). In the observation group, the hospital stay(12.1±1.7) d, postoperative ambulation time (1.9±0.6) d and exhaust time (3.1±0.4) d was less than that in the control group(17.2±2.4) d, (2.8±1.0) d, (4.2±0.8) d, respectively, and there were significant differences between two groups(all P<0.001). The postoperative complication rate of 5.0%(2/40) in the observation group was significantly lower than 25.0%(10/40) in the control group (χ 2=6.275, P=0.012). Conclusion:The patients with radiation enteritis and intestinal obstruction treated by conventional surgery were given eras intervention, which improved the nutritional level, immune function and inflammatory stress reaction of the patients, improved the treatment effect of the patients, and shortened the hospitalization time.

Objective To investigate the effect of silicone oil on the corneal endothelium in experimental animal models,while the silicone oil in the anterior chamber of the experimental animals will be taken out so as to explore their corneal changes.Methods Forty healthy white rabbits were randomly divided into three groups : silicone oil in the anterior chamber group (SIG),silicone oil in and out of the anterior chamber group (SIOG) and control group(CG).The cornea of rabbit was investigated at different times by slit-lamp,corneal thickness instrument,endothelial cell count instrument,histology and electromicroscopy.The time of silicone oil in the anterior chamber without keratpathy and the pathological changes of cornea with taking out of the silicone oil were observed.Results The density of the corneal endothelium in SIOG and SIG decreased more obviously by endothelial cell count instrument from 8 weeks to 16 weeks compared with control group (P

Objective:Temperature keeping is vital in the long distance transportation of red blood cell suspensions in military affairs and emergency.This study aimed to develop a composite phase change material to be used for preserving blood in long time transporation without electricity.Methods: The n-tetradecane was adsorbed to the pore structure of silica aerogel through the capillarity and hydrophobicity of silica aerogel.The efficiency of the phase change material was detected by differential scanning calorimetry.Results: The specific surface area and the adsorption capacity were 863.59 m2/g and 9.16,respectively.The phase change temperature was 5.47℃ and phase change latent heat was 180J/g.Conclusion: The n-tetradecane /silica aerogel composite phase change material could be used for blood preservation.
7.Distribution of type-specific human papillomavirus and its characters on age in paraffin-embedded adenocarcinoma specimens

Objective Human Papillomavirus (HPV) is necessary to the development of cervical cancer.Until now,the distribution of specific HPV type has not been identified clearly yet.Besides,the relationship between age and the HPV type-specific distribution in cervical adenocarcinoma needs to be further investigated.Methods This study included 32 specimens of cervical adenocarcinoma out of a multicenter clinical study in China.The specimen blocks were sliced by sandwich method.DNA was amplified by SPF10-PCR method.DNA genotyping used LiPA method.All biopsy specimens had pathologic diagnosis.The prevalence of HPV type-specific distribution was described and the differences were analyzed statistically.Results 32 adenocarcinoma cases were included in this sttudy.HPV positive rate was 53.1% (17/32).HPV-18 was the most prevalent type,which took up 47.4 % (8/17) of the total HPV positive cases,followed by HPV-16,which took up 26.3 % (5/17) of the total HPV positive cases.Other HPV types detected were HPV-39,HPV-45 and HPV-66.One multi-infection case was found (HPV-18 and HPV-66 co-infection).The average ages of HPV-16 and HPV-18 were 48.6 and 40.6 years old respectively,which showed statistically difference (P =0.049).When the patients younger than 50 years old were compared with the patients equal or older than 50 years old as two age groups,HPV positive rate did not show significantly statistical difference (P =0.074) between the two age groups.The prevalence of HPV-18 manifested significant differences between the two age groups in both HPV positive adenocarcinoma cases and overall adenocarcinoma cases (P =0.029and P =0.003 respectively).The prevalence of HPV-16 did not show any significant statistical difference between the two age groups in either HPV positive adenocarcinoma cases or overall adenocarcinoma cases (P =0.6 and P =1 respectively).HPV-16 and/or HPV-18 infection (at least one positive) in HPV positive adenocarcinoma cases did not have statistical significance (P =0.052),while in overall adenocarcinoma cases,HPV-16 and/or HPV-18 infection presented significant statistical difference (P =0.005).Conclusion HPV-18 and HPV-16 were two most prevalent HPV types in cervical adenocarcinoma.HPV infection and HPV-18infection were more common in young women.The prevalence of HPV-16 was more common in older adenocarcinoma cases compared with HPV-18.Those findings will provide evidence for the evaluation of HPV vaccine in preventing cervix related diseases.
8.Endoscopic diagnosis and treatment of pancreas divisum with recurrent acute pancreatitis in children and teenagers

Objective To evaluate the safety and efficacy of endoscopic retrograde cholangiopancreatography (ERCP) for the diagnosis and treatment of pancreas divisum(PD) associated with recurrent acute pancreatitis(RAP) in children and teenagers. Methods Data of patients with symptomatic PD associated with RAP under 18 years old who were diagnosed and treated with ERCP from January 2011 to January 2015 were retrospectively analyzed. Patients with complete pancreas divisum underwent endoscopic minor sphincterotomy combined with dorsal duct stenting ( ESCS ) , and those with incomplete pancreas divisum underwent bi?papilla endoscopic sphincterotomy combined with dorsal duct stenting ( Bi?ESCS ) . ERCP?related data, complications were recorded. Long?term follow?up was conducted for procedure related complications. Results Nine patients with symptomatic PD associated with RAP were firstly diagnosed. A total of 18 ERCP procedures were performed in these patients, with success rate of 100. 0% ( 18/18) in the minor papilla cannulation, and mild ERCP?related complication rate of 11. 1% ( 2/18) . One was acute mild pancreatitis and the other was hyperamylasemia. During follow?up from 3 to 60 months, all patients had pain relief, among whom 7 were asymptomatic, with no onset of acute pancreatitis. Dorsal ducts of all patients were not obviously dilated in MRCP or CT scan. All patients gained weight and presented normal physical and intelligence development. Conclusion Symptomatic PD associated with RAP can be completely diagnosed with ERCP . ESCS and Bi?ESCS are safe and effective endoscopic treatment techniques for pediatric cases.
9.The effect of neurolytic celiac plexus block on liver inflammatory reaction after partial hepatectomy in rats

Objective To investigate the effect of neurolytic celiac plexus block (NCPB) on the inflammatory reaction of the remaining liver tissue and liver function after partial hepatectomy (PH) in rats.Methods Thirty male Sprague-Dawley rats of SPF were constructed as a PH model with deligation and ablation operated on their left and middle lobes,respectively.Then,they were randomly divided into two groups:NCPB group and control group.Twelve hours after the surgery,0.5% lidocaine was given in the NCPB group once a day,while 0.9% saline was given in the control group.Determination of liver function,generation of the remaining liver,and deposition of IL-β,TNF-α of the pathological section was respectively made on Day 1,3 and 7 after the surgery.Results On Day 1,3 and 7 after surgery,both the aspartate aminotransferase (AST) and the alanine aminotransferase (ALT) levels in NCPB group were,to different degrees,lower than those in the control group (P < 0.05).Strikingly,total bilirubin in NCPB group was lower than that in control group (P < 0.01) on Day 7,while the level of semm albumin was higher than that in control group (P < 0.01).There was no statistically significant difference on the generation of the remaining livers between NCPB and control groups.On Day 3 and 7,the deposition of IL-β,TNF-α in the pathological sections of NCPB group were lower than those in control group.Conclusion NCPB can not only effectively reduce the damage of liver function caused by PH surgery,but also improve the inflammatory reaction of the residual liver.
10.Changes in expression of GABAAα1 receptors in medial prefrontal cortex in a rat model of neuropathic pain

Objective To investigate the changes in the expression of GABAAα1, receptors in medial prefrontal cortex (mPFC) in a rat model of neuropathic pain. Methods Nine male Wistar rats weighing 200-210 g were randomly divided into 3 groups ( n = 3 each): control group (group C) , sham operation group (group S) and neuropathic pain group (group P). Neuropathic pain was induced by chronic constrictive injury. The right sciatic nerve was exposed and 4 loose ligatures were placed on the sciatic nerve at 1 mm intervals with 3-0 chromic catgut. In group S, the right sciatic nerve was exposed but not ligated. The thermal and mechanical pain threshold was measured at 1 d before and 1,4,7, 10 and 14 d after operation. The animals were then sacrificed and the mPFC was removed. The expression of GABAAα1, receptors in mPFC was determined by Western blot. Results Compared with C and S groups, thermal and mechanical pain threshold were significantly decreased and the expression of GABAAα1, receptors was up-regulated in group P ( P < 0.01) . There was no significant difference was in the thermal and mechanical pain threshold and expression of GABAAα1 receptors between C and S groups (P > 0.05). Conclusion Up-regulation of GABAAα1 receptor expression in mPFC may be involved in the development and maintenance of neuropathic pain in rats.
