1.Insight on core human resource management of a hospital

Core human resource serves as the principal capital of carrier of knowledge and skills of a hospital,which is the most decisive factor of a hospital' s competitiveness and adaptability for its initiative and creativity.Their stability and development plays a key role in hospital development.This study analyzed the characteristics and capacity demand of such human resource,and proposed that their management should differ from the rest,recommending reforms in their job description,development and training,performance appraisal,remuneration design and incentive mechanism.
2.Clinical analysis of 32 cases of Ramsay Hunt syndrome.

OBJECTIVE:
To observe the clinical features and the treatment effect of 32 cases of Ramsay Hunt syndrome and explore the relationship between different clinical manifestations and the prognosis.
METHOD:
Using the integrated therapy including the medication and physiotherapy of ultrashort wave, the relationship between clinical manifestations and recovery time was analyzed.
RESULT:
Twenty two cases were cured, 10 cases showed improvement of the symptoms, and no patients complained ineffectiveness. The effective rate was 100%. The shortest recovery time was 13 days, and the longest recovery time was 1 year and 7 months.
CONCLUSION
The key to cure Ramsay Hunt syndrome is comprehensive treatment in time. The different clinical manifestations have something to do with the prognosis.

3.Analysis of deafness-related gene mutations in 100 non-syndromic hearing loss patients in Henan province.

OBJECTIVE:
To preliminarily determine the gene mutation frequency and the hotspots in Henan province, we analysed the deafness-related gene mutation in patients with non-syndromic hearing loss (NSHL).
METHOD:
Genomic DNA samples of 100 patients with NSHL in Henan province were extracted from peripheral blood after clinical history inquiry and clinical examination, Four common deafness genes GJB2, SLC26A4, mitochondrial 12SrRNA, and GJB3 were detected by Sanger sequencing method,and then data analysis were conducted.
RESULT:
Among 100 patients with NSHL. the gene mutation frequency was 44%. In these patients, 29 cases had GJB2 mutations, 13 cases had SLC26A4 gene mutations, and 3 cases had mitochondrial 12SrRNA mutations.
CONCLUSION
Among the patients with NSHL in Henan province, the most frequent mutation causing hereditary deafness was mutation in GJB2, followed by SLC26A4,and it will provide a theoretical basis to determine the etiology of deafness in Henan Province.

4.Effect of different delivery methods on the pregnancy outcome of premature pregnant women with ecarred uterus re-pregnancy after prior cesarean section

Objective:To explore the clinical effects and impacts of different delivery methods on pregnancy outcomes in preterm women with scarred uterus re-pregnancy after prior cesarean section.Methods:The clinical data of 431 pregnant women with scar uterus re-pregnancy and premature delivery admitted to Jiaxing Maternal and Child Health Hospital from January 2017 to December 2021 were retrospectively analyzed. According to the mode of delivery, 107 patients who were delivered through vagina were included in the study group and 324 patients undergoing cesarean section were selected as the control group. Amount of postpartum bleeding, incidence of uterine rupture, and hospital stay were compared between two groups. Incidence of neonatal asphyxia and 1-, 5-, and 10-minute Apgar scores were also compared between the two groups.Results:Compared with the control group, postpartum bleeding volume and hospital stay were significantly lower in the study group [(232.31 ± 73.82) mL vs. (354.27 ± 89.49) mL, (4.32 ± 0.61) days vs. (6.23 ± 0.94) days; t = 12.74, 19.69, P < 0.05). There was no significant difference in the incidence of uterine rupture ( P > 0.05), incidence of neonatal asphyxia ( P > 0.05), and 1-, 5-, and 10-minute Apgar scores ( P > 0.05) between the two groups. Conclusion:For premature delivery women with scarred uterus after prior cesarean section, both cesarean section and vaginal delivery can be chosen for successful delivery. However, vaginal delivery has less postpartum bleeding and shorter hospital stay, which is a feasible and safe delivery method. Vaginal delivery is encouraged for re-pregnancy after cesarean section.
5.Efficacy of magnesium sulfate plus aspirin in the treatment of pregnancy-induced hypertension and its effect on placental hypoxia-induced factor-1 α levels

Objective:To investigate the efficacy of magnesium sulfate plus aspirin in the treatment of pregnancy-induced hypertension and its effect on placental hypoxia-induced factor-1α (HIF-1α) levels.Methods:This is a case-control study. The clinical data were collected from 96 patients with gestational hypertension who were admitted to Jiaxing Maternity and Child Health Care Hospital between January 2019 and January 2023. These patients were divided into a control group and a study group, with 48 patients in each group, according to different treatment methods. The control group received magnesium sulfate treatment, whereas the study group was treated with magnesium sulfate combined with aspirin. Both groups were treated for 14 days. The clinical efficacy of the two groups was compared. Pre- and post-treatment blood pressure, placental HIF-1α levels, and endothelin levels were compared between the two groups. The incidence of adverse reactions and adverse pregnancy outcomes were also compared between the two groups.Results:The response rate in the study group was significantly higher compared with that in the control group, with 93.8% (45/48) versus 79.2% (38/48), respectively ( χ2 = 4.36, P < 0.05). Following treatment, both diastolic and systolic blood pressure levels in the study group were significantly lower than those in the control group ( t = 5.70, 7.33, both P < 0.05). Placental HIF-1α and endothelin levels in the study group were (41.03 ± 6.95) μg/L and (93.37 ± 8.29) ng/L, respectively, which were significantly lower than those in the control group [(82.97 ± 8.86) μg/L, (124.96 ± 9.32) ng/L, t = 17.55, 25.80, both P < 0.001]. The incidences of adverse reactions and unfavorable pregnancy outcomes in the study group were 12.5% (6/48) and 10.4% (5/48), respectively. These were significantly lower compared with those in the control group, which were 31.3% (15/48) and 27.1% (13/48), respectively ( χ2 = 4.94, 4.38, both P < 0.05). Conclusion:Magnesium sulfate combined with aspirin is highly effective in treating pregnancy-induced hypertension. This combined therapy can effectively lower patients' diastolic and systolic blood pressures, as well as placental HIF-1α and endothelin levels. It can obviously reduce the incidences of adverse reactions and unfavorable pregnancy outcomes.
6.Formulation Optimization and Quality Evaluation of Ferulic Acid Ligustrazine Solid Lipid Nanoparticles

OBJECTIVE:To optimize the formulation of ferulic acid ligustrazine (FATM) solid lipid nanoparticles (FATM-SLN),and conduct the quality evaluation. METHODS:Emulsification ultrasonic method was used to prepare FATM-SLN. Using particle size and entrapment efficiency as indexes,amount of glyceryl monostearate,egg yolk lecithin (PC),poloxamer 188 (P188),and sodium stearate as factors,single factor test and orthogonal test were used to optimize the formulation;and verifica-tion test was conducted. The appearance morphology,distribution of particle size,Zeta potential,stability and in vitro release de-gree of prepared FATM-SLN were investigated. RESULTS:The optimal formulation was as follows as FATM of 10 mg,glyceryl monostearate of 300 mg,PC of 200 mg,P188 of 200 mg,sodium stearate of 10 mg,and purified water of 20 mL. The prepared FATM-SLN showed spherical solid particles,appearance morphology was round,distribution of particle size was 40-800 nm,parti-cle size was 106.23 nm,polydispersity index was 0.254,Zeta potential was -34.8 mV,entrapment efficiency was 73.32%,drug loading was 1.20%;the appearance had no obvious changes within 10 d in 4 ℃(RSD<2%). The drug-release in 0.5-1 h was the fastest,the cumulative release degree reached to 60.47%;it tended to be stable after 8 h,the cumulative release degree reached to 93.46%,and drugs were basically released completely. CONCLUSIONS:FATM-SLN formulation is successfully optimized,and the prepared FATM-SLN has small particle size,high entrapment efficiency and good stability.
7.Effect of osthole on memory function of sleep deprivation mice

Objective To investigate the effect of Osthole on memory function of sleep deprivation(SD) mice. Methods Forty-eight male mice were randomly divided into 4 groups;normal control group(NC group ), large platform control group(TC group),sleep deprivation group(M group)and Osthole group(Ost group). The model of SD in mice was estabished by using improved multi platform method. The ability of learning and memory was tested by using Morris water maze test and pathological changes of hippocampal neurons in mice were observed by HE staining. The serum,hippcampus malondialdehyde(MDA)contents and superoxide dismutase(SOD)activity, so as the hippocampus No content,were detected. Results Compared with NC group and TC group,the escape la-tency of M group increased significiantly and the number of crossing platform decreased significantly. There were in-creased levels hippocampus tissue,serum MDA level,hippocampal SOD activity and NO content. After supplemen-tation of Osthole,the escape latency significantly shortened in mice. The number of crossing platform was increased while the contents of MDA both in hippocampus and serum were decreased,and the SOD activity in hippocampus re-turned to normal. However,the level of NO in hippocampus was not decreased. Conclusion Osthole can protect the memory function of SD mice by reducing the the damage of hippocampal neurons through antioxidant stress.
8.Construction and application of home self-sampling processes for cervical human papillomavirus detection

To construct and develop the home self-sampling processes of cervical human papillomavirus (HPV) detection and evaluate its application effect. An integrated HPV self-sampling detection platform is scheduled to include three terminals: a user terminal, a detection terminal and a medical terminal. It covers a wide range of functions of self-sampling kit acquisition of user, sample logistics tracking, inspection services, report query, medical consultation, health management, and follow-up tracking. A total of 8 053 users applied for self-sampling kits and all completed online user information registration from January to November 2020. The average age of users ranged from 17 to 84 with a median age of 42 years old. Registered users of the platform were distributed in Jiangsu, Jiangxi, Hebei, Shanxi, Shanghai, Ningxia, Anhui, Zhejiang, Inner Mongolia, Beijing and Xinjiang. 8 045 users completed self-sampling with a kit return rate of 99.9%. Six users lost the kits during the express delivery, and 2 users had the kits contaminated due to improper application; The amount of exfoliated cells collected from 8 045 cases in the sample kits were all within the endogenous internal standard of the nucleic acid kit, and the qualified rate of kits was 100%. The proportion of test report issued by the detection platform within 3 d accounts for 96.93% (7 799/8 054). Among the 763 positive users, 742 completed 6-month reexamination, with a reexamination rate of 97.25%. Unfortunately, 21 cases were lost to follow-up. Taken together, HPV home-based self-sampling is simple, convenient and efficient in use. It can expand the coverage of cervical cancer screening and may help promote HPV screening.

10.Analysis of influencing factors and acceptance on manual hyperinflation in patients with open sputum suction after tracheotomy

ObjectiveTo investigate the acceptance and influencing factors of manual hyperinflation (MIH) in patients with open sputum suction after tracheotomy, so as to provide reference for improving the acceptance of MIH for patients. MethodsBy convenience sampling, 140 patients with tracheotomy on open suction in Affiliated Hangzhou First People's Hospital, Zhejiang University School of Medicine from October 2016 to September 2018 were selected as participants in the study and the Self-Designed Questionnaire was used to study the acceptance towards MIH of the patients. Univariate analysis and Logistics regression analysis were conducted to analyze the influencing factors of the acceptance of patients on MIH. A total of 140 questionnaires were distributed, and 134 valid ones were collected. ResultsUnivariate analysis showed that there were statistical difference in age, gender, family support, duration of tracheotomy, whether the medical staff had explained the relevant knowledge of MIH, and whether the patients had experienced MIH on patients'acceptance of MIH (P< 0.05). The above 6 statistically significant factors were analyzed by multivariate Logistic regression and the results showed that age, gender, duration of tracheotomy, whether the patients had experienced MIH before were the influencing factors of the patients' acceptance on MIH(P< 0.05). ConclusionsAge, gender, duration of tracheotomy, whether or not the patents have experienced MIH are independent impact factors for the patients' acceptance of MIH and the above factors can be the cut-in points of explanation and education in the process of MIH,so as to improve the patients' acceptance of MIH and promote recovery.
