2.Surgical treatment of coronal shear fracture of the capitellum humeri of adults

[Objective]To evaluate the clinical result of surgical treatment of coronal shear fracture of the capitellum humeri of adults.[Method]From January 2001 to July 2005,6 cases of 2 males and 4 females with coronal fractures capitulum humeri were chosen open reduction and internal fixation utilizing screws.The fractures were terated under direct vision and were fixed with screws.The left arms in 4,right in 2,the age of the group were from 27 to 48 years old,with the average of 38 years.The operative time was from 3 days to 9 days after injury.All cases were fresh and close fractures.[Result]All operations were successful,no nerves and blood vessels injury was found,the incisions were all healed by first intention.The X-ray films showed that the fractures were anatomic reduction.The healing time was 7 weeks in average with 6 to 9 weeks range.All cases were followed up from 12 to 52 months with an average of 32 months,no postoperative complications such as myositis ossificans or avascular necrosis of capitulum humeri was found.According to BrobergMorrey Scale,there were 5 caese of excellent,1case of good.[Conclusion]The coronal shear fracture of the capitellum humeri of adults are suitable for opening reduced direct vision and fixing with screws,prompt treatment with anatomic reduction and internal fixation and early rehabilitation could lead to excellent or good outcomes.

Tyrosine phosphorylation is a key post-translational mechanism that regulates cellular processes and maintains homeostasis.Aberrant changes in tyrosine phosphorylation are often associated with disease states such as metabolicdisorders,cancer and cardiovascular disease.Protein tyrosine phosphatases (PTPs) are the enzyme family that regulates protein phosphorylation level of tyrosine residues in the cellular processes and signaling ways.So far,scientists have discovered 112 kinds of human PTPs.Among them,PTP1B is widely and clearly studied.Recently,as an enzyme that play a role in oncogenesis,PTP1B has been wildey studied by scientists.Here,we highlight the relationship between protein tyrosine phosphatase 1B and hematologic neoplasms.
4.Hospital medical equipment purchase demonstration based on cost-benefit analysis

Objective To establish an optimization model of hospital medical equipment purchase based on cost-benefit analysis to enhance equipment purchase demonstration.Methods An integer programming model for the types and amount of purchased hospital medical equipment was constructed with the cost benefit parameters based on market survey and the hospital annual purchase plan. LINGO optimization software was used to determine the optimal purchase scheme.Results The model acquired was applied to medical equipment purchase demonstration in some hospital, so that the purchase scheme was formed rapidly as desired.Conclusion The model may promote hospital medical equipment purchase demonstration and provide references for equipment selection.
5.Clinica study comparing HMVP and MVP regimens in the treatment of advanced non-small cell lung cancer

0.05). No significant differences were detected in the median time of remission, median survival time and 1-, 2-year survival rates between the groups. Moreover, no significant differences were detected in the grade Ⅲ~Ⅳ leukopenia, grade Ⅲ~Ⅳ thrombocytopenia, grade Ⅲ~Ⅳ nausea and vomiting and grade Ⅲ~Ⅳ constipation between the groups. Conclusions:The response rate of the MVP regimen is slightly lower than the HMVP regimen, but the HMVP regimen is not noticeably superior. It may increase the toxicity such as leukopenia, nausea/vomiting and constipation, as wellas being more expensive. In short, MVP regimen should be selected between the regimens in the chemotherapy of advanced NSCLC.
6.3-dimentional CT reconstruction in the diagnosis of upper urinary tract obstructive diseases (report of 37 cases)

Objective To study the diagnostic value of 3 dimentional CT reconstruction in upper urinary tract obstructive diseases. Methods 37 patients with upper urinary tract obstructive diseases underwent thin slice helical CT scan and 3 dimentional reconstruction of the urinary tract as well as 3 dimentional CT angiography when needed. Results All the cases have been definitely diagnosed including ureteral carcinoma in 8, bladder carcinoma in 2, ureteral calculi in 9, benign ureteral stricture in 7, congenital malformation in 7 and pelvic tumor in 4.All the patients except those with pelvic tumors underwent surgical intervention and the diagnosis were confirmed.The operative findings were in concord with the helical CT results. Conclusions 3 dimentional CT reconstruction of the urinary tract can provide distinct urinary tract images and helps determining the site and the cause of the obstruction. Compared with other methods, the procedure has the advantages of high resolution, rapid imaging and low invasiveness. It might become the most valuable means of detecting upper urinary tract diseases.
7.The application value of multislice CT in diagnosis of ureteral biology variation and diseases

Objective To discuss the application value of multislice CT (MSCT) in diagnosis of ureteral biology variation and diseases.Methods The MSCT data of 51 patients with suspected ureteral biology variation and diseases were retrospectively analyzed.The data of MSCT images included plain scan,renal cortical phase,renal parenchyma phase and excretion phase.The MSCT images of renal cortical phase,renal parenchyma phase and excretion phase were reconstructed respectively to gain the 3D images of renal arteries,veins and urinary passage.Results Among the 51 patients,1 patient with traumatic ureteral rupture,10 patients with ureteral biology variation diseases,14 patients with stone of ureter,2 patients with polyp of ureter,3 patients with non-specificity ureteritis,3 patients with tuberculosis of ureter,1 patient with endometriosis,17 patients with carcinoma of ureter.Satisfied 3D images of renal arteries,veins and urinary passage could be reconstructed.And the imaging diagnosis of 51 patients were all in accord with their clinical diagnosis or pathologic diagnosis.Conclusion MSCT can obtain high qualified images,and this technique should become the preferred inspection item of ureteral biology variation and diseases.
8.Analysis of protein tyrosine phosphatase 1B gene mutation in myeloproliferative neoplasms

Objective To analyse protein tyrosine phosphatase 1B (PTP1B) gene mutation in myeloproliferative neoplasms (MPN).Methods DNA sequencing technology was used to detect DNA sequences of PTP1B in MPN patients (n =84) and normal controls (n =37).Results For Exon1-6,Exon9 and Exon10,84 cases of MPN patients and 37 cases of control group were not detected mutation.For EXON 8,18 of 84 MPN patients had Exon8 C/T heterozygous mutation and 10 of 37 normal controls were detected C/T heterozygous mutation.There was no significant difference between MPN patients and normal controls (x2 =0.453,P =0.501).Exon7 was detected in 38 MPN patients and 2 cases of patients were found C/T heterozygous mutation,while in the control group,1 case with G/C heterozygous mutation.All of the cases were not detected homozygous mutation.Conclusion Using DNA sequencing technology to detect gene mutations of PTP1B,there is no significant difference between MPN patients and normal controls.
9.Analysis of etiological factors for non-ulcer dyspepsia in elderly patients presenting to the emergency department

Objective To identify etiological factors for non-ulcer dyspepsia (NUD) in elderly patients presenting to the emergency department.Methods A total of 97 elderly patients with NUD presenting to the emergency department from January 2011 to December 2013 were enrolled in the NUD group and 116 elderly people undergoing annual physical examinations served as the control group.Clinical data,including Helicobacter pylori (HP) infection,unhealthy living habits,anxiety,depression and regular intake,were collected and compared between the two groups.Results Compared with the control group,the NUP group showed a higher HP infection rate [70.1% (68/97)vs.54.3% (63/116),x2 =5.565,P<0.05],a higher incidence of unhealthy living habits (unhealthy dietary habits,smoking,drinking,and lack of regular exercise) (P<0.05),a higher incidence of suspect anxiety/anxiety and a higher incidence of regular intake [30.9% (30/97) vs 10.3% (12/116),x2=14.138,P<0.05].SerumpepsinogenⅠ(PGⅠ) [(178.7±13.6) μg/L vs.(125.2±10.5)μg/L,t=1.971,P<0.01] and serum pepsinogen Ⅱ(PG Ⅱ) [(28.8±5.3) μg/L vs.(14.7±3.8)μg/L,t=1.960,P<0.01] levels were also higher in the elderly NUD patients than in the control group.Conclusions HP infection,unhealthy living habits,anxiety and/or depression,and regular intake are closely associated with NUD in the elderly.Serum PGⅠ and PGⅡ could be used as markers in screening for malignant lesions in elderly patients with dyspepsia.
10.Clinical effect of sEMG-BFB training combined with swallowing training for dysphagic patients with cerebral infarction

Objective To investigate clinical effect of sEMG-BFB training combined with swallowing training for dysphagic patients with cerebral infarction,to provide a reference for life quality improvement of dysphagic patients.Methods 48 patients with cerebral infarction combined swallowing disorder were selected.According to the digital table,patients were divided into the control group and observation group,24 cases in each group.Patients in the control group were given training such as routine training and behavioral swallowing training,while patients in the observation group were given sEMG-BFB training based on training of the control group.FOIS score,Kubota's water drinking test grade,SSA score and SWAL-QOL score were compared between two groups.Results After treatment,FOIS score,SSA score and SWAL-QOL score of the observation group were (3.82 ± 1.04),(19.97 ± 8.17),(158.23 ± 10.27),which were significantly higher than those of the control group [(2.82 ± 1.21),(24.21 ± 6.31),(139.23±7.32),t =4.394,4.115,5.642,all P < 0.05] ; Kubota's water drinking test grade of the observation group was better than the control group,the difference was statistically significant (U =283.43,P < 0.05).Conclusion sEMG-BFB training combined with swallowing training is helpful for dysphagic patients with cerebral infarction to improve swallowing ability and life quality.
