Objective To explore the function of stereotactic radiation therapy (SRT) and wholebrain radiation therapy (WBRT) in treatment of multiple brain metastasis tumor.Methods Fifty-two patients with multiple brain metastasis tumor were retrospectively analyzed.The patients were divided into simple stereotactic radiation therapy group (22 cases) and whole-brain radiation therapy + stereotactic radiation therapy group (30 cases) according to the radiation therapy method.The median overall survival time and distant brain tumor recurrence rate in 1 year etc.were compared between 2 groups.Results There were no statistical differences in the median overall survival time and fatality rate between simple stereotactic radiation therapy group and whole-brain radiation therapy + stereotactic radiation therapy group:13.0 months (95％ CI 8.05-27.84) vs.13.4 months (95％ CI 8.57-19.48),86.36％ (19/22) vs.86.67％ (26/30),P＞ 0.05.The distant brain tumor recurrence rate in 1 year in simple stereotactic radiation therapy group was significantly higher than that in whole-brain radiation therapy + stereotactic radiation therapy group:45.45％ (10/22) vs.20.00％ (6/30),and there was statistical difference (x2 =3,861,P =0.049).Conclusion Stereotactic radiation therapy and whole-brain radiotherapy can achieved similar median overall survival time.

Objective To investigate the therapy efficiency and complication prevention in patients undergoing radiofrequency current catheter ablation combined with multiple X-ray projecting angles.Methods Two hundred and six atrioventricular nodal reentrant tachycardia (AVNRT) patients were involved in this study. The distance between the tip of His and the ostium of coronary sinus were measured under different X-ray projecting angles respectively, including posterior anterior (PA), right anterior oblique 30° (RAO 30° ) and left anterior oblique 45° (LAO 45° ) in each patient, and mapping was proceeding under the corresponding X-ray projecting angles with the longest distance. The distance between ablating catheter and His was confirmed again before powering by the other two X-ray projecting angles. Results The longest distance between the tip of His and the ostium of coronary sinus were obtained in 66,75 and 65 patients under PA,RAO 30° and LAO 45° respectively(P＞ 0.05 ),and the data were (2.94 ± 0.87), (2.98 ± 0.76)and (2.91 ± 0.52) cm(P＞ 0.05). Transient first or second degree of atrioventricular blocking was observed in 4 cases among 22 high risk patients in which the powering sites were above the medium level of the distance between the tip of His and the ostium of coronary sinus under three kinds of X-ray projecting angles obviously.However,only 1 case was observed in other 184 patients(P＜ 0.01 ). All patients were cured successfully and no permanent atrioventricular blocking occurred. Conclusion Radiofrequency current catheter ablation combined with multiple X-ray projecting angles is helpful to increasing the successful rate and preventing complication in ablating therapy.

Objective To investigate the effects of angiotensin receptor blocker (ARB)losartan on the glomerular protein expression profile of spontaneous type 2 diabetic KKAy mice by two-dimensional differential gel eleetrophoresis and MALDI-TOF mass spectrometry.Methods 8-week-old spontaneous type 2 diabetic KKAy mice were randomly divided into losartan (10 mg·kg-1·d-1 given in drinking water) treatment group and non-treatment group.Eight-week-old C57BL/6 mice were used as normal control.The glomeruli were separated by magnetic bead perfusion through thoracic aorta at age of 20 weeks,then glomerular protein was extracted.The glomerular protein expression profile was investigated by CyDyes minimal fluorescence labelling,two-dimensional differential gel electrophoresis and MALDI-TOF mass spectrometry.Results KKAy mice developed higher body weight and blood glucose,higher urinary microalbumin creatinine ratio at age of 20 weeks than C57BL/6 mice at the same age (all P＜0.05).Losartan treatment markedly reduced urinary microalbumin creatinine ratio [(539.71 ±100.23)mg/g vs (728±177.19) mg/g],attenuated mesangial expansion and the thickening of glomerular basement membrane,but had no effect on the blood glucose.By DeCyder 2-D differential analysis software,62 protein spots of differential expression were found in glomeruli between losartan treatment and non-treatment KKAy mice at age of 20 weeks.Among them,41 proteins were identified by peptide mass fingerprinting.The expressions of 28 proteins were up-regulated by losartan treatment,including glycerokinase,sulfite oxidase,glycine amidinotransferase,adenosylhomocysteinase,etc.The expressions of 13proteins were down-regulaled by losartan treatment,including 3-mercaptopyruvate sulfurtransferase,ATP synthase subunit d,60 000 heat shock protein,stress-70 protein (alternative name 75 000glucose-regulated protein,GRP75),etc.Six differcntially expressed proteins were found in glomeruli between non-treatment KKAy mice and C57BL/6 mice,and the differential expressions were suppressed by losartan treatment,including dihydrolipoyllysine-residue acetyltransferase component of pyruvate dehydrogenase complex,succinyl-CoA ligase (GDP-forming) subunit beta,mitochondrial,ATP synthase subunit d,GRP75,nucleoside diphosphate-linked moiety X motif 19 and seleniumbinding protein 1.Conclusions Losartan significantly reduces the urinary protein excretion rate and renal pathological lesion of spontaneous type 2 diabetic KKAy mice,and suppresses the differential expression of mitochondrial ATP synthase subunit d,GRP75,selenium-binding protein 1,etc in glomeruli.Losartan may play a renoproteetive role by reducing glomerular mitochondrial reactive oxygen species genesis and inhibiting oxidative stress.

Objective To translate and revise the Individualized Care Scale-Patient Version(ICS-P) into Chinese,then to assess the reliability and validity of the Chinese version of the Individualized Care Scale-Patient Version (C-ICS-P).Methods Standard forward-back translation techniques were used in the translation of the ICS-P according to the Brislin translation model.Cross-cultural revision of the translated ICS-P was carried out through group discussion and pretesting.Totally 223 patients were recruited through convenience sampling method from a tertiary hospital in Changsha and investigated using general information questionnaire and the C-ICS-P,and its reliability and validity were assessed.Results The C-ICS-P contained two subscales,and both C-ICS-P-A and C-ICS-P-B contained 3 factors explaining 61.330％ and 65.263％ of the total variance.The dimensions of C-ICS-P-A were clinical characteristics (6 items),personal life characteristics (4 items) and participation willingness (5 items);the dimensions of C-ICS-P-B were clinical care (6 items),personal life care (4 items) and decisional control over care (5 items).The Cronbach's α coefficients of C-ICS-P-A and its dimensions were 0.897,and 0.730～0.774;the Cronbach's α coefficients of C-ICS-P-B and its dimensions were 0.909,and 0.688～0.754.Split-half reliability was 0.856 for C-ICS-P-A and 0.688～0.754 for its dimensions;split-half reliability was 0.889 for C-ICS-P-B and 0.750～0.758 for its dimensions.Analysis of content validity of the C-ICS-P indicated that I-CVI was at least 0.83,S-CVI was 0.943.Conclusion The reliability and validity of C-ICS-P are satisfactory and well meet the requirements of psychological measurement,indicating C-ICS-P is a reliable and valid instrument in the context of Chinese culture.

The relationship between glutathione S-transferases (GSTs) M1, T1 genotype and childhood acute lymphoblastic leukemia (ALL) was investigated. GSTM1 and GSTT1 genotypes in genomic DNA from 67 children with ALL and 146 healthy controls were analyzed by using the multiplex polymerase chain reaction (PCR). The frequencies of GSTM1, M1-T1 null genotypes in ALL children were significantly higher than in the healthy controls (76.12% versus 52.74%, OR=2.856, P<0.001; 50.74% versus 24.66%, OR=3.148, P<0.001, respectively). However, there was no significant relationship between GSTT1 null genotype and ALL of children (61.19% versus 49.32%, OR=1.621, P>0.05). It was suggested that GSTM1 null genotype might be a risk genotype of childhood ALL, while there as no correlation between GSTT1 null genotype and childhood ALL.

In order to investigate the levels of stem cell factor (SCF) and its receptor c-kit protein and mRNA in pediatric aplastic anemia (AA) and their relevance to the pathogenesis, immunocytochemical and in situ hybridization were utilized to detect the expression of SCF and its receptor c-kit gene protein and mRNA, respectively in 59 children with AA and 51 normal controls. The relationship between SCF and c-kit and the pathogenesis of AA was analyzed subsequently. The results showed that the positive rate of SCF protein and mRNA expression in children with AA was significantly lower than that in healthy controls (P < 0.05). However, there was no significant difference in the positive rate of c-kit protein and mRNA expression between children with AA and control group (P > 0.05). It was concluded that the expression of SCF is significantly decreased in children with AA, which may be closely associated with the pathogenesis of the AA. c-kit may be unrelated to the development of pediatric AA. Therefore, AA in children may have abnormalities at SCF/c-kit signal transduction levels.

ObjectiveTo observe the therapeutic effects of Shenxiong glucose injection in the patients with chronic kidney diseases.Methods Seventy-eight patients with chronic kidney disease were given intravenous glucose injection 200 ml,qd,for 2 weeks.The patients who were complicated with diabetes would be given insulin 2 U/100 ml.Before and after Shenxiong injection treatment,24 h urinary protein,blood urea nitrogen ( BUN),creatinine ( Cr),hemoglobin ( Hb),albumin ( ALB),hematocrit ( HCT),fiber fibrinogen (FIB),cholesterol (TC) and triglyceride (TG) were measured and compared.Results After 1 course of Shenxiong treatment,the serum BUN,Cr,FIB,24 h urinary protein,Hb and HCT ( [ 15.70 ± 3.62 ] mmol/L vs.[6.74 ± 1.56 ] mmol/L,[ 564 ± 65 ] μmol/Lvs.[ 189 ± 43 ] μmol/L,[ 0.08 ± 0.01 ] g/L vs.[ 0.04 ± 0.01 ] g/L,[7.96 ±3.45]g vs.[3.60 ± 1.92]g,[83.6 ±10.5]g/L vs.[79.5 ±8.7]g/L,[0.43 ±0.0] vs.[0.39 ±0.06 ] were decreased,and ALB ( 28.7 ± 8.6) vs.( 36.8 ± 6.2) was increased.The difference was statistically significant (t =3.1 1,2.98,3.04,2.82,2.02,2.23,P＜0.01 for all).TC and TG were not changed much after the treatment.ConclusionShenxiong injection can improve the kidney function,lower Fibrinogen and urine protein,which may effectively slow down the progress of chronic kidney disease and improve the life quality.
