Objective To evaluate the formation of coronary collateral circulation with intracoronary Doppler ultrasound in pigs after acute occlusion and the influence of nitroglycerin and phentolamine on it.Methods The blood flow spectrun was detected in 14 pigs using intracoronary Doppler ultrasound before and after acute occlusion of the left circumflex artery (LCX) and its response to nitroglycerin and phentolamine.The average peak velocity (APV) and the diastolic systolic velocity ratio (DSVR) were recorded. Results The APV of the LCX decreased significantly at 10 min after acute occlusion (0.97?1.05 vs 25.78?4.36 cm/s,P0.05). Conclusion Intracoronary Doppler ultrasound can be used to evaluate coronary collateral circulation. The formation collateral circulation was insufficient after acute occlusion of coronary artery in pigs and nitroglycerin or phentolamine can improve the blood flow.

Objectives:The present study in vestigated the presence and significance of serum antibody against human myocardial mitochondria in patients with virus myocarditis (VMC) and dilated cardiomyopathy (DCM). 　　Methods:The human myocardial mitochondtia were used as antigen to detect serum autoantibody by immuno-dot blot in 29 VMC patients,24 DCM patients,33 patients with other cardiac (OCD) and 20 healthy blood donor (HBD).The antigen molecular weight was detected by Western blot. 　　Results:①Positive autoantibody against human myocardial mitochordria was found in 41.4% in VMC and 41.7% in DCM patients,which were much higher than 6.1% in OCD or 0% in HBD patients.②Cardiac troponin T was elevated in 43.8% of autoantibody-positive VMC and DCM patients,which was much higher than 12.0% in autoantibody-negative VMC and DCM patients (p＜0.05).③The antigen molecular weight of human myocardial mitochondria was 30KD. 　　Conclusions:①The presence of autoantibody against supports that concept that is responsible for the development of VMC and DCM.Autoantibody is one of the factors that give rise to cardiac injury.The antibody detection may serve as a diagnostic index for VMC and DCM.②The specific antigen is probably human myocardial adenine nucleotide translocator.

Objective To explore the improvement effect of rehabilitation nursing on joint swelling and pain after total knee arthroplasty.Methods According to the digital table,100 patients with total knee arthroplasty were randomly divided into observation group and control group,50 cases in each group.The control group after operation used routine nursing care,the observation group was given targeted rehabilitation nursing.They were compared with visual analogue scale (VAS),American Knee Society Score (AKSS),range of motion (ROM),length of hospital stay,joint swelling degree.Results Mter treatment,the VAS score of the observation group was lower than the control group[(2.43 ±0.51) points vs.(4.58 ± 0.81) points,t =15.882,P ＜ 0.05],AKSS scores and ROM were higher than those in the control group[(69.94 ± 14.23)points vs.(53.23± 12.91)points,(103.47 ± 14.84)° vs.(91.23 ±12.53) °,t =6.149,4.456,all P ＜ 0.05].The hospitalization time of the observation group was shorter than that of the control group[(14.23± 3.81)d vs.(17.82 ± 4.03) d,t =4.577,P ＜0.05].At 3d and 7d after operation,the swelling degree of the observation group was lower than those of the control group[(1.23 ±0.26)mm vs.(1.97 ±0.38)mm,(0.62 ± 0.21) mm vs.(1.38 ± 0.35) mm,t =11.364,13.166,all P ＜ 0.05].Conclusion The application of rehabilitation nursing after total knee joint surgery can effectively relieve joint swelling and pain,and it is worth to be popularized.

[Objective]To investigate the curative effect of intermittent pneumatic compression(IPC) in the treatment of deep venous thrombosis(DVT) after interventional thrombolysis.[Method]Fifty-two cases of DVT were treated from December 1998 to August 2007,and divided into 2 groups randomly.In the first group,all cases received low molecular heparin and warfarin after routine or interventional thrombolysis.In the second group,IPC was used on the basis of group one.Two groups were compared about cases for repeated thrombolysis,time point of symptom relief,reduction of swelling at 7 days after surgery,re-canalization of vein at 1 month after surgery.[Result]Three cases in the first group needed repeated thrombolysis while none was found in the second group.In the second group,quicker symptom relief,more obvious reduction of swelling,wider re-canalization of vein were observed compared with the first group.[Conclusion]IPC is proved effective in the comprehensive therapy of DVT.

For obtaining new structural compounds with unique resistance profiles or novel mechanisms of action on HIV-1 from natural products, anti-HIV-1 drug screening models were used in vitro. Norcantharidin (NCTD), a derivative from cantharidin, was found to have inhibitory activities on HIV-1(IIIB) p24 antigen in lymphocyte lines MT-4, CEM and H9. It inhibited HIV-1 strain 018a (sensitive to zidovudine) from replicating with EC50 (50% effective concentration) of 14.9 micromol L(-1) and also inhibited HIV-1 strain 018c (resistant to zidovudine) from replicating with EC50 of 20.2 micromol L(-1) in primary lymphocytes peripheral blood mononuclear cells (PBMC). Norcantharidin showed synergistic activity with zidovudine on HIV-1(IIIB) in MT-4 cells, the combination index was less than 0.3. But, it was not active on HIV-1 integrase, reverse transcriptase or protease in vitro. As the structure of norcantharidin is unique and different from that of all clinic drugs approved, it would be possible to obtain new and effective compounds against HIV-1 with low toxicities after modification of norcantharidin.

Objective To determine the lumbar anatomical structure parameters of the lumbar spine posterior column and its sample data of three-dimensional conformation,and based on these data to design the lumber laminar screw dynamic fixation system.Methods 20 human L3 ～ S1 bone specimens,the thickness of the lumber laminar region were measured to obtain the normal anatomical parameters of the lumbar laminar.And 20 healthy lumbar vertebrae L3-S1 for three-dimensional reconstruction were detected by CT scan,which could obtain the structure data of the normal lumbar posterior three-dimensional conformation,and to obtain conformational sample data of three-dimensional space of the posterior structure.Results Lumbar lamina medial 1/3 could serve as a spine laminar screw setting area by anatomical observations and measurements.Lamina thickness of L3-4 on average (6.6 ±0.9) mm,L5 ～ S1average (6.0 ± 0.6) mm.Lumber laminar screw length 6.5 ～ 8.0mm could meet the requirements of bilateral cortical fixation.Conclusion Lower lumbar spinal lamina area can serve as the region to fix the bilateral cortical bone screw,and the optimal length of the laminar screw is 6.5 ～ 8.0rmm.

Objective To establish the animal model of acute intracranial hypertension,investigate the changes of transeranial Doppler patterns in raised intraeranial pressure so that the necessary experimental evidence will be provided to clinical experience for monitoring intraeranial pressure (ICP) and cerebral perfusion pressure (CPP) using transeranial Doppler non-invasively. Methods Acute intraeranial hypertension was induced by inflating the balloon inserted into the epidural space. Blood flow velocity of basilar artery was measured with transcranial Doppler and intracranial pressure as well as cerebral perfusion pressure was measured. Results With progressive increase of intracranial pressure five characteristic flow patterns were observed, appearing in the following order: high resistance pattern, systolic flow, retrograde diastolic flow, very small systolic flow and zero flow. Conclusions Analysing patterns of TCD may be helpful for evaluating ICP and CPP qualitatively. The animal model is simple and strict so that it is worthy of being spreadcd.

Objective To study the effects about rhizoma drynariae poly (lactic-co-glycolic acid) (DR-PLGA) /Calcium phosphate cement (CPC) composite scaffold on treating rabbit femoral bone defect.Methods Eight Newzealand rabbits were randomly divided into experiment group(n =4) and control group (n =4).The preparation of rabbit femoral bone defect model,separately implanted DR-PLGA/CPC scaffold and PLGA/CPC scaffold.After 4,8 weeks,we took out the materials,observed with X-ray,gross anatomy,histology observation to evaluate the osteogenetic activity,the effect of accelerating the healing of bone defect.Resuits At the 4th week and 8th week after implantation,the effect of promoting fracture healing and osteogenic activity of the experiment group were greater than those in the control group.Conclusions DR-PLGA combined with CPC could induce new bone formation,promote the healing of rabbit femoral defect.

Objective:To evaluate the clinical effect of laryngofissure without tracheotomy.Method:One hundred and sixteen cases of laryngofissure without tracheotomy in our department were analyzed retrospectively.Result:Of 116 cases, 101 cases had the laryngeal carcinoma; 9 cases had laryngeal stenosis; 6 cases had benign tumor of larynx. All the incisions were healed by first intention; and postoperative duration of hospital stay were 7 to 20 days. Three cases recurred in 101 cases of laryngeal carcinoma (2.97%), and underwent reoperation,the 5 year survival rate was 100%. One case recurred in 9 cases of laryngeal stenosis, and underwent reoperarion of laryngoplasty with T-tube implanted. No recurrence in 6 cases of benign tumor of larynx.Conclusion:Laryngofissure without tracheotomy have the advantage of less trauma , fast rehabilitation and no destroying the normal respiratory functions. It is completely feasible so long as surgeons grasp the adaption of operation strictly and select suitable cases.
