1.Practice of proton and heavy ion radiotherapies for head and neck malignancies
Chinese Journal of Radiation Oncology 2016;25(5):427-431
Radiotherapy is an important therapeutic method in the management of head and neck cancer. However, since the head and neck region has many important functions of the human body, radiotherapy may cause serious sequelae.Proton and heavy ion radiotherapies have better radiophysical and biological properties compared with conventional proton beam and are beneficial to tumor control and protection of normal tissue. The advantages of proton and heavy ion radiotherapies have been demonstrated in soft tissue tumors (especially skull base chordoma and chondrosarcoma),malignant melanoma, and adenoid cystic carcinoma resistant to conventional protons in the head and neck region, and the safety of proton and heavy ion radiotherapies have been confirmed in reirradiation for recurrent tumors.

OBJECTIVE:To realize refined management of tablets i n the inpatient pharmacy ,and to ensure the medication safety of patients. METHODS :Based on intelligent pharmacy ,the dispensing and packaging process under the automated drug dispensing and packaging system (ADDPS)mode was optimized and modified ;PDA barcode scanning technology was applied in all links of taking ,dismantling and adding ,so as to realize the real-time tracking of batch number and inventory ,and improve the drug closed-loop management of tablets. The error rate ,staff consumption time and pharmacist/nurse satisfaction were compared before and after the process improvement. RESULTS :After the process improvement ,the dispensing error rate was decreased from 1.637‰ before improvement to 0.082‰(P<0.01);the staff consumption time decreased from (7.52±0.33)h to (5.11±0.24)h (P<0.01);the pharmacist/nurse satisfaction increased from 66.5% to 93.4%(P<0.01). CONCLUSIONS :Based on ADDPS ,the application of PDA barcode scanning technology standardizes the tablets management of inpatient pharmacy ,supplements and improves the drug closed-loop information ,realizes batch number tracking and inventory management ,reduces the occurrence of tablet dispensing errors ,improves the work efficiency and satisfaction of pharmacists ,and ensures the safety of clinical medication.

Objective To investigate the differences of anorectal manometry (ARM) parameters in different position (left lateral position and seated position) in patients with functional constipation (FC),and the value of rectoanal pressure gradient (RAPG) was assessed in seated position in the evaluation of anorectal motility disorder in patients with FC.Methods From March 2015 to July 2016,at Clinical Gastrointestinal Motility Center of Ningbo Yinzhou People's Hospital,66 consecutive patients with FC aged 18 to 75 who met Rome Ⅲ criteria were recruited. The questionnaires of patient assessment of constipation symptom (PAC-SYM)and patient assessment of constipation quality of life (PAC-QoL) were recorded.Patients randomly underwent ARM examination in left lateral or seated positions, and then followed by a balloon expulsion test (BET) in seated position. The differences of ARM parameters in different positions were compared. The correlation between ARM parameters and BET results, constipation symptoms and quality of life scores were analyzed.T-test,Spearman correlation analysis and Kappa coefficient were performed for statistical analysis. Results ARM parameters including rectal resting pressure, rectal defecation pressure and RAPG in seated position were both higher than those of left lateral position ((30.83 ±7.89) mmHg (1 mmHg =0.133 kPa) vs.(10.53 ± 3.94) mmHg,(78.86±22.25) mmHg vs.(54.92±21.26) mmHg,(17.53 ±27.40) mmHg vs.(-7.80 ±26.88) mmHg),and the differences were statistically significant (t =-21.10,-12.35 and-8.84,all P < 0.01).However,there was no significant difference in anal-related pressure parameters (P > 0.05).The RAPG in seated position was highly consistent with BET,with a maximum Kappa-value of 0.643,which was higher than the maxium Kappa-value of 0.349 in left lateral position. The optimal RAPG threshold of seated position was 10 mmHg,the sensitivity of RAPG in the prediction of BET was 85.71% and the specificity was 79.17%.According to the optimal RAPG threshold in seated position, the patients were divided into high RAPG group and low RAPG group. The frequency of weekly spontaneous defecation of high RAPG group was higher than that of low RAPG group (2.88 ±2.16 vs.1.66 ±0.96),and the difference was statistically significant (t=2.65,P=0.01).The satisfaction score of PAC-QoL questionnaire of high RAPG group was lower than that of low RAPG group (2.05 ±0.55 vs.2.83 ±0.78),and the difference was statistically significant (t =-4.72,P <0.01).Conclusion It may be more reasonable to perform ARM in the seated position in FC patients, especially for the RAPG in seated position is better correlated with BET results, constipation symptoms and quality of life scores which may have more clinical value in the evaluation of anorectal motility disorder in FC patients.

Journal of Medical Biomechanics 2024;39(2):305-311
Objective The reliability and validity of the SGGC-Net-based motion capture system(SGGC-Net system)and SIMI system for parsing walking gait were compared using a three-dimensional(3D)motion capture system(Vicon)with marker points as a reference standard. Methods Thirty healthy college students were recruited, and their gait characteristics while walking on a treadmill were analyzed.Kinematic data were collected using the Vicon system, and video data were collected synchronously using four cameras to obtain the right shoulder, elbow, hip, knee, and ankle joint angles.Reliability was assessed using intraclass correlation coefficients(ICCs)with 95%confidence intervals and standard error of measurement(SEM).Validity was assessed using multiple correlation coefficients(MCCs)and root mean square errors(RMSEs).Results The ICCs of the maximum and minimum 3D coordinate angles of the upper and lower limb joints of the SGGC-Net system ranged from 0.798-0.990 with an SEM of 0.04°-0.95°,and the ICCs of the SIMI system ranged from 0.650-0.967,with an SEM of 0.31°-1.24°.The ICCs of the SGGC-Net system were higher than those of the SIMI system for all joint angles except for the minimum hip and maximum knee angles.Compared to the joint angle curves derived from the SIMI system, the MCCs of the curves derived from the SGGC-Net system ranged from 0.945-0.996,with RMSEs of 1.44°-4.65°,and the multiple correlation coefficients of the SIMI system ranged from 0.815-0.986,with RMSEs of 2.56°-9.99°.The MCCs of the SGGC-Net system were greater than those of the SIMI system at all angles except for the ankle joint. The RMSEs of the SGGC-Net system were smaller than those of the SIMI system at all angles except for the ankle joints. Conclusions The SGGC-Net system has better reliability and validity than the SIMI system in most of the variables, and it has better repeatability and accuracy in analyzing walking gait.It can be applied to motion capture environments without marker points, such as technical analysis of athletes'movements and clinical gait analysis of special populations.

This article reports a patient with extensive high-pressure injection injury of the hand combined with deep chemical burn caused by high-pressure injection of industrial cement materials was diagnosed and treated in the Department of Hand Surgery, Xiaolan Hospital Affiliated to Southern Medical University in 2022. The nerves, tendons and blood vessels of the left hand were involved, and the ulnar skin of the left thumb was extensively necrosis, and a large number of extensive cement foreign bodies remained under the skin. Part of the cement was inserted into the joint capsule of the interphalangeal joint. After emergency surgical treatment, the patient was saved successfully, and the wound healed well without chemical poisoning and other related complications, which created conditions for the second stage of flap repair.
