1.Association of the genotypes and serum levels of CD31 with hepatocellular carcinoma

Objective To investigate the relationship between single nucleotide polymorphisms (SNPs),serum CD31 and HCC.Methods We analyzed three single nucleotide polymorphisms of CD31 gene Leu125Val,Asn563Ser and Gly670Arg in 190 HCC patients and 210 age and sex matched controls in a Chinese population,using a polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) strategy,and the serum level of CD31 was determined by enzyme-linked immunosorbent assay (ELISA).Results The distributions of CD31 gene Asn563Ser and Gly670Arg polymorphisms were not significantly different between HCC and control group (x2 =0.491,P > 0.05),but the CD31 gene Leu125Val polymorphism was significantly different (x2 =10.988,P < 0.05).The relative risk suffering from HCC of Val allele was 1.583 times of the Leu allele carriers(OR =1.583,95% CI,1.197-2.093,P =0.001) ; Serum level of CD31 Val allele carriers was significantly higher than that of no carriers(x2 =10.408,P < 0.05).Consistent with the results of the genotyping analyses,CD31 gene Leu125Val,Asn563Ser and Gly670Arg polymorphisms showed strong linkage disequilibrium,the Val-Ser-Arg haplotype was associated with a significantly increased risk of HCC as compared with the controls (OR =1.496,95%CI:1.095-2.046,P =0.011).Conclusions CD31 gene Leu125Val polymorphism and its Val-Ser-Arg haplotype were associated with HCC,Val allele is an important genetic susceptibility gene for HCC.CD31 Val allele carriers may subject to higher risk of HCC with enhanced CD31 expression.
2.The lung CT findings in patients with primary Sj(o)gren's syndrome and clinical significance

Objective To investigate the lung CT findings in patients with primary Sj(o)gren's syndrome (pSS) and clinical significance.Methods Images of lung CT scans of 68 patients with pSS were retrospectively observed.The distribution characteristics of lung lesions were observed.CT visual score were used to quantitatively analyze lung lesions.Binary classification unconditional logistic regression analysis was used to investigate the relationship between pulmonary arterial hypertension (PAH) and anti-SSB seropositivity in Patients with pSS.Results The 68 cases of patients with pSS,PAH in 13 (19%),anti-SSB seropositivity in 25(37%),CT shows normal in 13 (19%),interlobular septal thickening,ground-glass opacity and intralobular interstitial thickening were frequent findings.The lesion distribution was in the peripheral and lower lung fields.Interlobular septal thickening and cysts were associated with PAH in Patients with pSS (OR=1.780,2.513,P<0.05),cysts was significantly associated with anti-SSB seropositivity (OR =10.530,P<0.05).Conclusion Lung CT can quantitatively analyze the degree of pSS lung damage,and provide helps for clinical treatment and prognosis.

Objective: To study the fasting blood glucose level and associated factors relationship in physical examination population in Beijing. Methods: Totally 38 462 individuals who received physical examination from April 2006 through December 2007 in Peking Union Medical College Hospital physical examination center were enrolled in our study.We divided them into 8 groups according to the age,and the level of fasting blood glucose was tested.The relationship between BMI,age,sex and abnormity of blood glucose were analysed. Results: The incidences of overweight and obesity were 36.4% and 13.5%,respectively.The incidences of impaired fasting glucose and diabetes mellitus were 3.2% and 3.8%,respectively.The rate of abnormal glucose level increased with age and BMI.Compared to male,female persons had lower fasting blood glucose level and lower rate of abnormality(P =0.000).Compared to normal and thin population,overweight and obesity population had higher rate of fasting glucose abnormality(P

Objective To investigate the value of 18 MHz high-frequency linear array ultrasound probe in the diagnosis of thyroid anterior capsular invasion,and compare it with the conventional high-frequency ultrasound probe.Methods Seventy-three nodules in 68 patients in Peking Union Medical College Hospital from December 2015 to March 2016 underwent conventional and 18 MHz high-frequency probes examination before operation and were compared with pathological results.The accuracy,sensitivity and specificity of the 18 MHz high-frequency linear array probe and the conventional high-frequency probe were determined.The consistency between the gold standard and the diagnosis by using two different frequency probes was measured using Kappa statistics.Additionally,diagnostic accuracy of different frequency probes was further evaluated according to the area under the ROC curve.Results The diagnostic consistency test of the total sample of 73 nodules:capsular abutment as the diagnostic criterion,the diagnostic consistency of 18MHz high-frequency probe was good (Kappa=0.803,P < 0.01).The specificity,positive predictive value and accuracy were superior to the conventional high-frequency probe (90.7% vs 69.8%,87.1%vs 68.3%,90.4% vs 79.5%).The area under the ROC curve was 0.903,higher than that of the conventional high-frequency probe (0.816),which demonstrated that the former had better diagnostic accuracy.If the disruption of the perithyroidal echogenic line as another diagnostic criterion,the diagnostic consistency of the 18 MHz and conventional high-frequency probe was general,the Kappa value were 0.677 and 0.518.The sensitivity,positive predictive value,negative predictive value and accuracy of conventional high-frequency probe were inferior to the 18 MHz high-frequency probe (53.3% vs 70.0%,74.5% vs 82.0%,88.9% vs 91.3%,78.1% vs 84.9%).Conclusions The 18 MHz high frequency probe is a feasible tool for accurate prediction of the distance between tumor and thyroid anterior capsular and anterior capsular invasion,and it is helpful for the diagnosis of the preoperative staging and the prognosis of PTC.
5.Analysis of factors associated with T-tube sinus tract formation after common bile duct exploration and T-tube drainage

Objective To explore the risk factors affecting T-tube sinus tract formation after common bile duct exploration and T-tube drainage by spiral computed tomography (SCT)examination.Methods The clinical data of 465 patients undergoing common bile duct exploration and T-tube drainage at the Affiliated Hospital of Youjiang Medical College for Nationalities from May 2011 to December 2013 were retrospectively analyzed.The residual stones and biliary stricture were detected by T-tube cholangiography,and the T-tube sinus tract formation in all the patients was detected by SCT examination at postoperative week 2.The factors affecting sinus tract formation were analyzed,including gender,age,albumin (Alb),C-reactive protein,alanine transaminase (ALT),total bilirubin (TBil),hemoglobin (Hb),surgical method,effusion around T tube,reoperation,diabetes.Univariate analysis was done using the chi-square test.Multivariate analysis was done using the Logistic regression.Results T-tubes of 465 patients were clear without residual stones.T-tube in the 397 patients was removed when the sinus tract formation was confirmed by CT examination at postoperative week 2.T-tubes in other patients were removed when the sinus tract formation was detected by CT reexamination at postoperative week 4.In univariate analysis,Alb,surgery method,effusion around T-tube and diabetes were important factors affecting T-tube sinus tract formation (x2 =50.750,7.671,19.022,15.373,P < 0.05).Alb < 30 g/L,laparoscopic surgery,effusion around T-tube and diabetes were independent risk factors affecting T-tube sinus tract formation in multivariate analysis [Odds ratio =1.135,0.493,0.262,0.363; 95% confidence interval:1.061-1.214,0.280-0.865,0.104-0.658,0.156-0.843,P < 0.05].Conclusions The T-tube removal is determined according to the sinus tract formation by CT examination at week 2 after common bile duct exploration and T-tube drainage.Alb < 30 g/L,laparoscopic surgery,effusion around T-tube and diabetes are independent risk factors affecting T-tube sinus tract formation.
6.Changes of hemodynamics during heterotopic partial liver transplantation in pigs

Objective To study the changes of hemodynamics during heterotopic partial liver transplantation (HPLT) in pigs. Methods 10 young pigs after intravenous anesthesia were divided into two groups: donor group (5 pigs) and recipient group (5 pigs). In recipient pigs, a catheter inserted to right cervical artery was used to monitor artery bood pressue, and Swasn-Ganz catheter inserted via right cervical vein was used to monitor hemodynamics during operation.Results There were significant changes in CO, SV and SVR (P<0.01 or P<0.05); although decreasing in MAP and increasing in HR occurred during operation, but no significance(P>0.05) was found. There were no significant changes in PCWP and PAP during HPLT (P>0.05). Conclusions There is significant hemodynamic fluctuation during HPLT. It is very important that to monitor hemodynamics, prevent and manage hemodynamics disturbance durig HPLT
7.Value of sonographic score in the diagnosis of salivary gland involvement in patients with Sj(o)gren's syndrome

Objective To evaluate the diagnostic value of sonographic score in the diagnosis of salivary gland involvement in patients with Sj(o)gren's syndrome(SS). Methods One hundred and three cases (44 cases of SS group and 59 cases of control group) were involved in the study. Parotid and submandibular glands of all the cases were examined by a doctor unawaring of the clinical information. All the off-line images were scored by two doctors seperately. The best threshold and the according diagnostic efficiency were determined by statistical analysis. Results The Kappa coefficient between the two doctors was 0.80.The parotid score,submandibular score and total score of SS group were significantly higher than those of the control group (5.79 ± 2.40 vs 0.46 ± 0.97,5.93 ± 1.58 vs 1.32 ± 1.84,11.64 ± 3.27 vs 1.78 ± 2.33,respectively). According to the ROC curve for the parotid score, submandibular score and total score, the area under the curve were 0.98,0.95,0.99, respectively. The best diagnostic threshold for total score was 8 and under this threshold, the diagnostic sensitivity, specificity, positive and negative predictive values were 93%, 97%, 95%, 95%, respectively. Conclusions The sonographic score including both parotid and submandibular glands is a reliable method with high reproductivity and diagnostic accuracy in the diagnosis of SS salivary gland involvement.

9.Effect of tiotropium bromide on expression of CD(8) (+)CD (25) (+)FoxP (3) (+) regulatory T cells in patients with stable chronic obstructive pulmonary disease.

The expression of CD(8) (+)CD(25) (+)FoxP(3) (+) regulatory T cells (CD(8) (+)Tregs) in the peripheral blood of patients with stable chronic obstructive pulmonary disease (COPD), and the effect of muscarinic cholinergic receptor antagonist tiotropium bromide on the expression of CD(8) (+)Tregs were investigated. Twenty-three patients with moderate to severe stable COPD were enrolled in this study. All patients inhaled tiotropium bromide (18 μg daily) for 3 months. Before and after inhalation of tiotropium bromide, peripheral blood samples were collected from the patients, and T cells were labeled by three-color labeled monoclonal antibodies. Flow cytometry was used to detect the quantity and percentage of CD(8) (+)T cells, CD(8) (+)CD(25) (+)T cells, CD(8) (+)Tregs, CD(4) (+)T cells, CD(4) (+)CD(25) (+)T cells and CD(4) (+)CD(25) (+)FoxP(3) (+) regulatory T cells (CD(4) (+)Tregs) respectively. The percentage of CD(4) (+)T cells was increased from (27.82±2.18)% to (35.53±1.3)% (t=3.20, P=0.004) in the peripheral blood of patients with stable COPD after inhalation of tiotropium bromide for 3 months, that of CD(4) (+)CD(25) (+)T cells was decreased from (10.03 ±1.42)% to (4.21 ±0.65)% (t=3.78, P=0.001), and that of CD(8) (+)Tregs was increased from (8.41 ±1.68)% to (21.34 ±4.20)% (t=2.72, P=0.013). At baseline, CD(8) (+)T cells, CD(8) (+)CD(25) (+)T cells and CD(4) (+)Tregs were detectable in the peripheral blood, but no significant changes were observed after treatment. Linear correlation analysis revealed that the difference before and after treatment in CD(4) (+)T cells and CD(4) (+)CD(25) (+)T cells was negatively correlated with the ratio of change in CD(8) (+)Tregs before and after treatment (r=-0.61, P=0.013; r=-0.72, P=0.001 respectively). In the peripheral blood of patients with stable COPD, there was the expression of CD(8) (+)Tregs and CD(4) (+)Tregs. Muscarinic receptor antagonist, tiotropium bromide, can promote the amplification of CD(4) (+)T cells, inhibit the expression of CD(25) (+)T cells, and enhance the expression of CD(8) (+)Tregs. CD(8) (+)Tregs and CD(4) (+)Tregs can be used as new indicators to understand the immune status of patients. They are helpful in judging the treatment efficacy and disease immunophenotype.
10.Study of Psychosocial Skills Training Module for Schizophrenic Outpatients

Objective To develop psychosocial skills training module for schizophrenic outpatients.Methods Structured psychosocial skills training program, which were made according to community rehabilitation plan for schizophrenia, featured convenience and well operability. Subjects were 96 of 204 schizorenic outpatients in remission who were randomly assigned to receive psychosocial skills training combined medicine(observation group) or medicine only(control group) and followed up by 24 months. The changes during treatment were evaluated by positive and negative syndrome scale(PANSS), Social Disability Screening Schedule(SDSS) and patients' cognition function were also evaluated longitudinally. Results Relapse rate of control group (37%) is higer than observation group (17.7%, P<0.01). After Last Observation Carried Forward(LOCF), control group showed higer scores than observation group on positive syndrome (8.34±2.04 vs 7.66±0.97), negative syndrome(9.15±1.97 vs 813±1.15), total score (37.64±5.58 vs 34.79±3.64) and Social disability (2.81±1.90 vs 2.07±1.47) (P<0.01); observation group also showed a significant progress on cognition function(P<0.05). Conclusion This trial demonstrated the efficacy of the easy-operating structured psychosocial skill training grogram in preventing relapse and improving social adoptive function for schizophrenia.
