1.Cost-Effectiveness Analysis of Domestic vs. Imported Acarbose Tablets for Type 2 Diabetes Mellitus

OBJECTIVE:To evaluate the cost-effectiveness of domestic versus imported acarbose tablets in the treatment of type 2 diabetic mellitus.METHODS:Based on the published medication data for type 2 diabetic mellitus,patients with type 2 diabetic mellitus were assigned to receive domestic acarbose tablets(Group A) or imported acarbose tablets(Group B).The cost-effectiveness of the two groups were compared applying the theory in pharmacoeconomics.RESULTS:Domestic and imported acarbose showed the same effectiveness for type 2 diabetic mellitus,while the costs in the two groups were significantly different.CONCLUSION:As compared with imported acarbose,the domestic acarbose has higher cost-effectiveness ratio.
2.Treatment of ureteral calculus with transurethral ureteroscopic pneumatic lithotripsy

Objective To investigate therapeutic effects of transurethral ureteroscopic pneumatic lithotripsy(TUPL).Methods A retrospective analysis was made on 320 cases of ureteral calculus treated with TUPL from September 2002 to August 2005.A Wolf F9.8 rigid ureteroscope and an EMS pneumatic ballistic lithotriptor were used.The probe was 0.8 mm or 1.0 mm in diameter.The pulse settings was 200~300 kPa.The fragmentation was conducted with single or multiple episode of treatment.Results The stone was pushed back to the renal pelvis in 6 cases and a conversion to extracorporeal shock wave lithotripsy(ESWL) was needed.The insertion of the ureteroscope was failed in 5 cases and a conversion to open surgery was required.The TUPL time was 2~10 min(mean,5.5 min) and the intraoperative blood loss was 3~15 ml(mean,7 ml).The stones were successfully fragmented on one session in 309 cases,the success rate being 96.6%(309/320).The length of postoperative hospital stay was 3~7 d(mean,4.5 d).There were 24 cases of mucosal injury of the ureter and 4 cases of false passage,all of which were cured with double-J catheter indwelling for 1 month.Follow-up examinations for 1~10 months(mean,4 months) showed stone clearance within 1 month and no ureteral stenosis or urinary infection.Conclusions Transurethral ureteroscopic pneumatic lithotripsy for the treatment of ureteral calculus is safe and effective.
3.Influence of hypothermia environment on treatment of limb gunshot fracture with internal fixation in sheep

[Objective]To compare the influence of hypothermia environment on healing of limb gunshot fracture in sheep with debridement and internal plate fixation at different time after wounded.[Method]Thirty sheep were divided into normal environment group(NE group) and hypothermia environment group(HE group) at random.They were kept in 26℃ and -25℃ environment respectively for 3 h,and then one side of hind limb was made fracture at middle part of the tibia by a 7.7 type handgun.The wound track was debrided at 6,12 and 24 h after wounded.The fracture was fixed by plate respectively.The healing of the wound track and fracture were observed and compared between groups.[Result]At 24 h after wounded,infection of the wound track occurred in 2 cases,and osteomylitis occurred in one by X-ray and in two by pathological examination in NE group.The healing of the wound track and fracture were excellent in HE group,and no osteomyelitis was observed by X-ray and pathological examination in HE group.The earlier the management started,the better the wound and fracture healed.[Conclusion]The hypothermia environment benefit the healing of the wound and the fracture after debrided and internal fixation.It is suggested that the management of gunshot fracture in hypothermia environment can make the time-limit of debridement and internal fixation prolonged.
4.Length polymorphism of cagA genotype in Helicobacter pylori isolates and its clinical significance

Objective To investigate the relationship of the distribution and heterogeneity of cagA gene of Chinese H. pylori isolates with Hp-associated gastroduodenal!diseases in China. Methods Pairs of oligonucleotide primers flanking a conservative 297bp segment and the variable segment of 3' region of cagA gene were designed according to the conserved cagA sequences, and the distribution of cagA gene in 82 Chinese H. pylori isolates and the heterogeneity of variable segment of cagA gene 3' region of 77 H. pylori strains were determined with PCR. Results 77(93.9%) of 82 Chinese Hp isolates were found to be cagA-positive by PCR amplification of the 297bp segment. The variable segment of 3' region of cagA genes was detected in 71(93.9%) of 77 Chinese H. pylori strains, including two without 297bp amplicon, with predominance of PCR products of about 825bp which were designated as type Ⅰ in China. Conclusion The prevalence of cagA is high in Chinese Hp isolates, and the cagA of type Ⅰ with the shortest variable 3' region predominates in China. Neither cagA nor the length of cagA 3' region is associated with the clinical outcome of Hp infection.
5.Other Effects of Cimetidine in the Decompensated Stage of Chronic Cardiopulmonary Disease

OBJECTIVE:To observe the other effects of cimetidine in the decompensated stage of chronic cardiopulmonary disease. METHODS: 72 patients with uncompensated chronic cardiopulmonary disease were assigned into therapy group and control group (n=36). Both groups were given routine treatment. Therapy group were additionally treated with cimetidine intravenously for 48 h. The changes of respiration frequency, arterial partial pressure of oxygen (PaO2), arterial partial pressure of carbon dioxide (PaCO2), intake of staple food and electrolytes in blood serum of 2 groups before and after treatment were determined. RESULTS:In therapy group, the respiration frequency, the level of PaCO2 and disorder of electrolytes in blood serum were decreased and the level of PaO2 and intake of staple food were increased. 7.Observation on the therapeutic Effects of Hepatic Fibrosis of Rats with Salvia Miltiorrhiza

Objective To investigate therapeutic effects of Salvia miltiorrhiza on hepatic fibrosis of rats induced by carbon tewachloridean. Methods Rat models of hepatic fibrosis were founded by intraperitoneal injection of carbon tetrachloride. Salvia miltiorrhiza were given to these rats. Normal group and control group were set for comparison at the same time. Serum levels of ALT, AST, HA, LN, and PC-Ⅲ were detected; HE and Masson staining were conducted in hepatic tissues to observe pathological variations. Results Salvia miltiorrhiza could decrease serum levels of ALT, AST, HA, LN,PC-Ⅲ obviously (P <0.01), compared with the control group; Salvia miltiorrhiza could obviously improve pathological variations compared with the control group. 8.Diagnostic value of N-terminal pro-brain natriuretic peptide and MB isoenzyme of creatine kinase for heart failure in pneumonia children

ObjectiveTo investigate the diagnostic value of N-terminal pro-brain natriuretic peptide (NT-proBNP) and MB isoenzyme of creatine kinase (CK-MB) for heart failure (HF) in pneumonia children.MethodsThe NT-proBNP and CK-MB were assayed in 132 pneumonia children with HF, 138 pneumonia children without HF and 62 healthy children were recruited into this study. A receiver operating characteristics (ROC) curve and a logistic regression model were employed to assess the diagnostic accuracy of NT-proBNP and CK-MB for HF in pneumonia children.ResultsPneumonia children with HF had higher blood NT-proBNP and CK-MB than those in pneumonia children without HF and healthy controls (P<0.01 for both). Pneumonia children with HF had higher blood NT-proBNP and CK-MB than the pneumonia children without HF. The area under curves (AUCs) of NT-proBNP and CK-MB for HF were 0.85 and 0.72, respectively. The AUC for their combinational usage was 0.87.ConclusionBoth NT-proBNP and CK-MB are effective markers as diagnostic adjuncts for HF in pneumonia children. Combination of NT-proBNP and CK-MB can improve the diagnostic accuracy for HF in pneumonia children.
9.Influence of targeted silence HOXA10 gene on leukemia NB4 cells

Objective To explore the effects of lentivirus-mediated RNA interference targeting HOXA10 gene on the proliferation, apoptosis and drug resistance of leukemic cell line NB4.Methods NB4 cells were divided into three groups: interference group, negative control group and untreated group.The infection efficiency of lentivirus for NB4 cells was detected by flow cytometry, and the expression of HOXA10 gene of NB4 cells at mRNA and protein level was detected by real-time PCR and Western blot.Cell survival was determined by MTF assay, and apoptosis and necrosis rates were detected by flow cytometry.Western blot was used to detect the influence of down-regulation HOXA10 gene on the multi-drug resistance-1 (MDR-1) protein.Results The ratio of GFP positive cells was up to 90 %.HOXA10 gene mRNA and protein levels were decreased in interference group compared with control group.The inhibition rate of interference group was (52.12±4.02) %, the apoptosis rate of interference group was (30.0±2.7) %, and their differences in the interference group and in control groups (negative control group and untreated group) were significant (P < 0.05).Western blot results showed that interfering HOXA10 gene significantly reduced the resistance gene MDR-1 expression level and reverse the drug-resistant of leukemia cells.Conclusions Lentivirns-shHOXA10 can steadily reduce the expression level of HOXA10, inhibit the leukemic cells proliferation, promote apoptosis and reverse drug-resistant.HOXA10 gene is expected to become a new target for reversing leukemia drug resistance.
10.Clinical effect of liraglutide and metformin hydrochioride on overweight diabetic patients due to poor glycemic control

Objective To investigate the effect of liraglutide and metformin hydrochioride on overweight diabetic patients because of poor glycemic control.Methods Forty-four overweight patients with poor glycemic control were randomly divided to the control group,the liraglutide group and the metformin hydrochioride group.Patients in control group were given diet and exercise control,in liraglutide group and the metformin hydrochioride group were given subcutaneous injection liraglutide,metformin hydrochioride oral treatment respectively for 12 weeks.Body mass index(BMI),fasting blood glucose (FBG),2-hour postprandial blood glucose (2 hPBG) and glycosylated hemoglobin (HbA1c) were measured.Results Compared with pretreatment,FBG,2 hPBG and HbA1c of patients in liraglutide group and metformin hydrochioride group were lower after treatment,and there was significant difference between the two group and the control group after treatment(P < 0.05).BMI of patients in liraglutide group was (24.61 ± 3.47) kg/m2,lower than of the control group((25.37 ± 4.70) kg/m2,P < 0.05).2 hPBG of patients in the liraglutide group was (7.13 ± 3.85)mtmol/L,lower than that of the metformin hydrochioride group ((8.03 ± 4.33) mtmol/L,P < 0.05).FBG level in metformin hydrochioride group ((6.31 ± 3.45) mmol/L) was lower than that of the liraglutide group ((6.98±2.97) mmol/L),but the difference was not statistically significant(P > 0.05).Conclusion Liraglutide and metformin hydrochioride treatment can effectively reduce weight and blood sugar of the overweight diabetics.The effect of liraglutide to reduce postprandial blood glucose and weight of the patient is more significant than of metformin hydrochioride.
