Objective To probe into the usage of statins in the patients with coronary artery disease in elderly patients. Methods From Jun 1,2009 to Mar 31,2009 in our hospital, 323 elderly outpatients with coronary artery disease were included in the investigation. The data were statistically analyzed. Results 62.2% of the patients used statins. The statins usage rates (44. 8% ,61.3% ,71.3% ) among three age groups(60 -69 years old,70- 79 years old, and ＞ 80 years old)were significantly different. The rates of statins usage were significantly different between male and female groups. Conclusion The usage of statins as secondary prevention of coronary artery disease varies and there is urgent need to standardize the usage of statins according to the treatment guideline.

Objective To establish a HPLC method for determination of the content of ephedrine hydrochloride in Wenyang Tongbi Wan. Methods A DDS-C18 column (4.6 mm?150 mm, 5 ?m) was used. The mobile phase was methane-water (1∶1). The column temperature was 30 ℃. The flow rate was 1 mL/min. Results There was good linearity of ephedrine hydrochloride in the range of 1.308 6～3.918 0 ?g (r=0.999 3, n =5). The average recovery was 98.91% (RSD=0.68%, n =5). Conclusion This method is simple, accurate and repeatable, and can be used for quality control of Wenyang Tongbi Wan.

OBJECTIVE:To observe the other effects of cimetidine in the decompensated stage of chronic cardiopulmonary disease. METHODS: 72 patients with uncompensated chronic cardiopulmonary disease were assigned into therapy group and control group (n=36). Both groups were given routine treatment. Therapy group were additionally treated with cimetidine intravenously for 48 h. The changes of respiration frequency, arterial partial pressure of oxygen (PaO2), arterial partial pressure of carbon dioxide (PaCO2), intake of staple food and electrolytes in blood serum of 2 groups before and after treatment were determined. RESULTS:In therapy group, the respiration frequency, the level of PaCO2 and disorder of electrolytes in blood serum were decreased and the level of PaO2 and intake of staple food were increased. Objective To study the chemical constituents of the fruiting bodies of Hydnum repandum.Methods Separation and purification were performed on silica gel, Sephadex LH-20 and ODS CC. Their sturctures were established by 2D-NMR (1H-1HCOSY, HMQC, HMBC, and NOESY), MS, HR-MS spectra, and ORD data. Results Eleven compounds were obtained and identified as sarcodonin A ( Ⅰ ),scabronine B (Ⅱ), 3β-hydroxy-5α, 8α-epidioxyergosta-6, 22-dien (Ⅲ), (22E, 24R)-ergosta-7, 22-diene3β, 5α, 6β- triol (Ⅳ), (22E, 24R)-ergosta-7, 22-diene-3β-ol (Ⅴ), benzoic acid (Ⅵ), 4-hydroxylbenzaldehyde (Ⅶ), 4-monopropanoylbenzenediol (Ⅷ), ethyl-β-D-glucopyranoside (Ⅸ), thioacetic anhydride ( Ⅹ ), (2S, 2'R, 3S, 4R)-2-(2-hydroxyoctadecanoylamino) docosane-1, 3, 4-triol (Ⅺ). Conclusion All of the compounds are isolated from this fungus for the first time.

Objective To study the effect of fish oil on calcium oxalate kidney stone formation. Methods The efficacy of unprocessed fish oil in the treatment of 20 hypercakiuric patients with urinary stones was evaluated, after administration of fish oil at dosage of 3000mg daioy of one week. Results Excretion of urinary calcium reduced significantly from (461.37?80.67)mg/d to (312.63?40.57)mg/d.The Mg2+ reduced from (136. 41?56. 12) mg/d to (108.43?18.96) mg/d. The Mg2+ /Ca2+ increased significantly from (0.29?0.08) to ( 0. 36?0.11)-Uric acid remained unchanged. Conclusion Unprocessed fish oil is safe and effective for the reduction of urinary calcium and might be used for the prevention of calcium stone recurrence.

Objective To explore the effects of premature atrial contractions on hemodynamics combined with coronary heart disease and hypertension .Methods According to the advance rate ,62 patients with premature atrial contractions were invided into 3 groups :< 20% group(n= 18) ,20% - 40% group(n= 20) and > 40% group(n= 24) .All of the patients were performed imped‐ance cardiography .The hemodynamics indexes at different times were compared ,and the variation rates in hemodynamics of the three groups combined with basic diseases were analyzed .Results Stroke volume(SV) ,stroke index(SI) ,aortic compliance(AC) , stroke work(SW) ,stroke work index(SWI) ,and heather index(HI) were smallest when premature atrial contractions ,which were middle when the first sinus beats before premature atrial contractions ,and largest when the first sinus beats after premature atrial contractions(P< 0 .05) .The variation rates in hemodynamics were smallest in < 20% group ,which were middle in 20% - 40%group ,and largest in > 40% group(P< 0 .05) .The percentage of patients with basic heart diseases was lowest in < 20% group , which was middle in 20% - 40% group ,and highest in > 40% group(P< 0 .05) .Conclusion The advance rate is relatively large in patients with basic cardiovascular disease .Premature atrial contractions has certain effects on hemodynamics ,and the effects are positively correlated with advance rate .

Objective To investigate the effect of positive acceleration (+Gz) on monophasic action potential duration of 90%repolarization( MAPD90 ) and transmural dispersion of repolarization ( TDR) in ventricles of rabbits and to explore the cellular electrophysiologic mechanism of tachyarrhythmia induced by positive acceleration .Methods Twenty-four healthy, male New Zealand white rabbits were randomly and equally divided into control group and +Gz group.The +Gz group rabbits were given +8 Gz exposure, 1 min a time, 3 times a day,and a total of 7 days.The two groups were subjec-ted to Holter monitoring at the same time to observe the incidence of tachyarrhythmia .Using the monophasic action potential ( MAP) recording technology , the MAP of the left ventricle was recorded while MAPD 90 and TDR were measured .By using Burst stimulation method , the right ventricular anterior wall of the rabbits was stimulated , and the incidence of tachya-rrhythmia was observed .Results The Holter record showed that the incidence of tachyarrhythmias in +Gz group was 55%(6/11), but the control group did not have any case of tachyarrhythmias .Compared with the control group ,MAPD90 of en-docardial and epicardial cells was significantly decreased in the +Gz group, while MAPD90 of middle myocardial cells did not change significantly ,but TDR was increased obviously .Four rabbits in +Gz group suffered from tachyarrhythmias dur-ing Burst stimulation ,and the incidence of tachyarrhythmias was 40% ( 4/10 ) .Conclusion +Gz exposure can increase the incidence of tachyarrhythmias .The shortened MAPD90 of ventricular muscle cells and the increased TDR may be the cell electrophysiological mechanisms of tachyarrhythmias induced by +Gz.

Objective To investigate the distribution and evolvement of resistance profiles of the pathogens isolated from children diagnosed with urinary tract infection (UTI).Methods The pediatric patients who were diagnosed with UTI in Children's Hospital of Fudan University from January 2001 to December 2010 were enrolled in this retrospective study.The clean catch midstream urine samples were collected and the pathogens were isolated by culture.Bacterial susceptibility test was performed by following a protocol of the standardized Kirby Bauer (KB) method.The enumeration data were analyzed by chi square test and the tendency of drug resistant rate was analyzed by curvilinear regression.Results A total of 10 686 pathogen strains were isolated during the past 10years.The major pathogen was Escherichia coli which counted for 38％ -56％ of the isolates.However,the proportion of Enterococcus faecalis (0 - 17％,F =34.075,P =0.000) and Enterococcus faecium (3 ％ - 15 ％,F =7.139,P =0.028) increased rapidly.The resistant rates of Escherichia coli to cefazolin,cefuroxime sodium,cefotaxime and ceftazidime were 35.8％- 62.3％(F=215.735,P=0.000),33.1％ -57.0％(F=70.674,P=0.000),19.0％ -56.5％(F=52.355,P=0.000) and 2.1％-23.5％(F=16.807,P=0.003),respectively.The resistant rate of Escherichia coli to ciprofloxacin was 23.4％ - 33.8 ％ (F=0.989,P=0.349),while the resistant rate of Escherichia coli to amikacin and cefoperozone/sulbactam were both less than 7％.There was no Escherichia coli isolate showing resistant to imipenem.The resistant rates of Enterococcus faecium to ampicillin and ciprofloxacin were higher than 60.0％ and 50.0％,respectively,while the resistant rate of Enterococcus faecalis to these two antibiotics decreased from 40.0％ to 9.6 ％ (F =17.497,P =0.009) and 60.0％ to 21.6％ (F=12.826,P=0.009),respectively.The resistant rate of Enterococcus faecium to nitrofurantoin decreased from 32 ％ (in the year of 2002) to 9.4％ (in the year of 2010,F=34.075,P=0.000) and the resistant rate of Enterococcus faecalis decreased from 9.2％ (in the year of 2005) to 1.4％ (in the year of 2010,F=7.139,P=0.028).The resistant rates to erythromycin were both higher than 75.0％ in both Enterococcus in recent 10 years.There was no Enterococcus isolates showing resistant to vancomycin.Conclusions Escherichia coli,Enterococcus faecalis and Enterococcus faecium are the most common pathogens associated with UTI in children.The proportions of Enterococcus faecalis and Enterococcus faecium among the pathogens of UTI are increasing.The antimicrobial activity of ampicillin to Enterococcus faecalis keeps at a high level.However,there are few drugs can be used for treating Enterococcus faecium infection.

Objective To assess the prevalence of Campylobacter in the children with acute bacterial diarrhea in Shanghai.Methods Epidemiological survey.Totally 6 641 children with acute bacterial infectious diarrhea from outpatients and inpatients in Children′s Hospital of Fudan University were submitted to the investigation during January 2011 to December 2012.The Campylobacter was isolated from stool samples collected from subjects in micro aerobic environment and identified by both multi-PCR and matrix-assisted laser desorption ionization-time-of-flight mass spectrometry ( MALDI-TOF MS ) .Antimicrobial susceptibility tests were assayed by disk-diffusion method according to EUCAST standard.The isolates molecular typing was done by PFGE.SPSS16.0 was used to analyze the results.Results A total number of 6 641 subjects were enrolled, among them, 305 patients were infected with Campylobacter.The prevalence rate was 4.6%(305/6 641).Among the infected patients, 240 patients were infected with Campylobacter jejuni and 65 patients were infected with Campylobacter coli, the infectious rates of these two pathogenic bacteria were 3.6%and 1.0%, respectively.The peak infectious rate in patients older than 1 year of age was 6.2%(209/3 385) which was higher than that in children under 1 year of age (2.9%, 96/3 256),χ2 =35.98,P<0.001.The infectious rate in winter and spring (6.8%, 138/2 040) was higher than that in the other seasons ( 3.6%, 167/4 601 ) ,χ2 =28.59, P <0.001.Antimicrobial susceptibility test results showed that 91.5%( 279/305 ) isolates were resistant to ciprofloxacin and 11.8%( 36/305 ) isolates were resistant to erythromycin.A total of 9 genotypes of Campylobacter were found by PFGE cluster analysis.The similarity were ranged from 65.1%-100.0%for type A, 67.6%-100.0%for type B, 61.7%-100.0%for type C, 59.0%-100.0%for type D, 71.4%for type F, 80.0%for type H, 54.4%-90.9%for type I, and only one strain was classified as type E and G.Conclusions Campylobacter is a major pathogenic bacteria associated with acute bacterial infectious diarrhea in children especially in children older than 1 year of age in Shanghai.The prevalent pattern of this pathogen was sporadic and the sharp peak was in winter and spring.The isolates are highly resistant to ciprofloxacin but still sensitive to erythromycin.
