1.The influence factors of maintenance dialysis patients with carotid plaque and its relationship with cardio-vascular disease

Objective To analyze the influence factors of maintenance dialysis patients with carotid plaque and explore its relationship with cardiovascular disease(CVD). Methods 52 patients on peritoneal dialy-sis and 46 patients on maintenance hemodialysis were involved in this study. Bilateral carotid intima-media thick-ness(IMT)and plaque,the related clinical indicators and CVD were recorded. According to the mode of dialysis, carotid plaque,CVD respectively,we divided our population into two groups,and statistical analyses were made between the two groups. Results Age,percentage of male,the number of hypertension and diabetes,blood uric acid ,blood glucose ,the incidence of cardiovascular disease are higher ,but the level of albumin is lower in the carotid plaque group. Binary Logistic Regression analysis showed that hypertension ,albumin and uric acid signifi-cantly influenced carotid plaque formation (P < 0.05). CVD is closely related to the carotid plaque (r = 0.415 , P = 0.000). Left and right IMT value are higher in the patients with CVD (P < 0.01);carotid plaque mainly distributed in the common carotid artery(CCA)and carotid artery bifurcation(BIF),and with the larger number in the patients with CVD(P<0.01). Conclusions Carotid plaque formation is closely related with the occurrence of CVD in maintenance dialysis patients. Hypertension ,albumin and uric acid are risk factors for the formation of carotid plaque,so dialysis patients in addition to maintain the blood pressure and improve nutrition,are also very important to control the level of uric acid.
2.Study on Extraction Technique for Xiangjiang Granula Essential Oil

OBJECTIVE: To study the optimum supercritical- fluid extraction technique for Xiangjiang Granula essential oil. METHODS: The orthogonal test was adopted to optimize the extraction process using the content of Ligustilide and the yield rate of essential oil as indicators, and 95% ethanol as co- solvents. The content of Ligustilide was determined by HPLC, using Phenomenex Luna C18( 250nm? 4. 6nm, 5? m) as column and methanol- 0. 5% glacial acetic acid( 30∶ 70) as mobile phase, with the detection wavelength set at 280 nm. RESULTS: The optimal extraction conditions were: temperature at 50℃ , pressure at 45MPa, extraction time for 3h, and 95% ethanol as co- solvents. The Ligustilide had a good linearty relationship between 5. 1~ 25. 5? g? mL- 1( r=0. 999 8) . CONCLUSIONS: This technique is easy, convenient and workable, and can provide theoretical support for production.
3.Survey on current status of humanistic care among medical students in clinical practice in obstetrics and gynecology department

Objective To explore the current status of humanistic care among medical students in clinical practice in obstetrics and gynecology department and its related influencing factors and to propose the corresponding countermeasures.Methods Scale of humanistic care quality was used in the survey for clinical practice students in obstetrics and gynecology department.ResultsThe total score rate of human care quality of medical students before clinical teaching were (84.12 ± 9.24)with a scoring rate of 72%.Significant differences were observed in the medical students with different medical professional(P <0.05).The total score of humanistic care quality of medical students after clinical teaching were (96.41 ± 1 1.53 ),significant higher than that of before training ( P < 0.05 ).ConclusionHumanistic care quality in medical students needs to be strengthen.Clinical training in obstetrics and gynecology department combined with humanities education has important practical significance to improve the quality of medical students.
4.Observation on clinical effect of electroacupuncture plus pricking-cupping bloodletting therapy for herpes zoster

Objective:To observe the clinical effect of electroacupuncture (EA) plus pricking-cupping bloodletting therapy for herpes zoster.
Methods:A total of 43 patients with herpes zoster were randomly divided into two groups, 23 cases in the treatment group and 20 cases in the control group. The treatment group was given EA plus pricking-cupping bloodletting therapy. The control group was given oral administration of Aciclovir Tablets, Diclofenac Sodium Dual Release Enteric-coated Capsules, Methylcobalamin Dispersible Tablets, and V itamin B12.
Results: The total effective rate was 87.0% in the treatment group and 70.0% in the control group, with a statistical significance in difference of the total effective rate between the two groups (P<0.01).
Conclusion:The clinical effect is better in the treatment of herpes zoster by EA plus pricking-cupping bloodletting therapy than by medications.
5.Different gene expressions in patients with severe preeclampsia

Objective To investigate gene expression profile in peripheral leucocytes of patients with severe preeclampsia (SPE) during 16-20 gestational weeks to see if there are different expression between normal pregnancy and SPE, and to provide the evidence for predicting the pathogenesis of preeclampsia in the future. Methods Eight hundred primipara who accepted pregnancy examination at the First Affiliated of Hospital SUN YAT-SEN University from August 2008 to December 2008 were selected into this study. The gestational age of all objects were confirmed as 16-20 weeks by ultrasonography. And they were followed up until delivered. Six patients developed severe preeclampsia (SPE group); and 40 pregnant women without any complications were chosen as the control. Human genome complementary DNA (cDNA) single-fluorescent chip were used to detect the different gene expression in peripheral leucocytes between normal pregnancy and SPE at 16-20 gestation weeks. Results There were different expressions in 983 genes between SPE group and control group, among which 719 genes were up-regulated and 264 genes were down-regulated in the SPE group. Up-regulating genes mainly involved in immunity, coagulation and fibrinolysis, signal transduction, cell adhesion, transcription and protein synthesis; and the expression of platelet and T cell activation antigen 1 (PTA1/CD226), bactericidal/permeability increasing protein (BPI), interleukin-8 (IL-8), protein kinase C (PKC), lymphocyte antigen 75 (LY-75), mucoprotein and EGFR pathway substrate 8 (EPS8) were significantly increased in SPE patients. Down-regulating genes mainly involved in apoptosis, calcium metabolism, lipid metabolism and cell transformation; and the expression of adrenomedullin (ADM), killer cell immunoglobulin-like receptors (KIR), vitamin D receptor (VDR), adipose differentiation-related protein (ADRP), parathyroid hormone (PTH) and mitogen-activated protein kinase kinase 4 (MKK4) were significantly decreased in SPE patients. Conclusions The gene expressions of peripheral leucocytes in pre-eclampsia patients were different from those of normal pregnant women during 16-20 gestational weeks. Gene CD226, BPI, IL-8, PKC, ADM, KIR and VDR might participate in the pathogenesis of SPE which should be further investigated.
6.Forensic analysis of 201 cases of knee injury in traffic accidents

Objective To discuss the characteristics of knee injury in traffic accidents so as to provide an objective basis for its forensic identification.Methods Retrospective analysis was carried out on 201 cases of knee injury in traffic accidents identified by our department from 2006 to 2009.Results According to the types of knee injury,all cases were divided into bone injury group,soft tissue injury group and bone combined soft tissue injury group.Analysis of the knee joint function in the three groups revealed that there were significant differences between bone injury or bone combined soft tissues injury groups and soft tissue injury group in weight bearing including the normal bearing rate and nonbearing rate( P < 0.05,respectively). ConclusionsIf patients' weight loading,walking functions as well as their gait conditions are introduced into the process of forensic identification for the knee injury in traffic accidents,the identification results will be more reasonable.
7.Influence of Induced before or after the Expected Date of Childbirth on Pregnancy Outcomes in Patients with Gestational Diabetes Mellitus

Objective]To investigate the influence of induced before or after the expected date of childbirth on maternal and neonatal outcome in patients with gestational diabetes mellitus which blood glucose well controlled.[Methods]We retrospectively analyzed 238 cases with mild gestational diabetes mellitus who had delivered in this hospital. Mild gestational diabetes mellitus cases were those who only need diet control,exercise therapy,no drug treatment. There were 120 cases who induced in 40 ~ 40+6 weeks was the research group,and 118 cases who induced in 39~39+6 weeks was the control group.[Results]The cesarean section rate be?tween the two groups was no difference(49.2%vs 55.9%,P=0.528);Postpartum hemorrhage rate of the two groups was no differ?ence(10.0%vs14.4%,P=0.299)Birth weight between the two groups was no difference(3.26 ± 0.33)kg vs(3.22 ± 0.34)kg,P =0.448). The incidences of the two groups including fetal distress,large for gestational age infants,neonatal pathological jaundice, neonatal hypoglycemia were no difference(P > 0.05).[Conclusion]Pregnant women with mild gestation diabetes mellitus induced before after 40 completed gestation weeks did not increase cesarean section rate. Pregnancy outcome between the two groups were no difference.It would be better to select the appropriate induction time according to the patient′s condition.
8.Radiosensitising effect of retinoblastoma gene (Rb94) on human lung adenocarcinoma cell line A549

Objective To construct an eukaryotic expression vector of retinoblastoma gene (pIRES-Rb94), and investigate the radiosensitising effect of Rb94 on lung adenocarcinoma cell line A549 and the mechanism. Methods Recombinant expression plasmid pIRES-Rb94 was constructed and then transfected into A549 cells using lipofectamine 2000. Steadily transfected cells were obtained using G418 se-lecting system. Cell counting method was used to produce the growth curve and the population doubling time was then calculated. The radiosensitivity of A549 cells was assessed by clonogenic assay. The expression of bTERT and Bcl-2 mRNA was detected by real-time RT-PCR. Cell cycle distribution was measured by flow cytometry. Results Steadily transfected cell line pIRES-Rb94-A549 was aquired. Compared with A549 cells, the population doubling time of pIRES-Rb94-A549 cells was increased from 31.5 h to 39.5 h (t=5.15, P<0.01). The expression of hTERT and Bcl-2 mRNA was both down-regulated (0.02:1.00, t= 18.99,P<0.01,0.01:1.00,t=13.73,P<0.01). The number of cells was increased in G2/M phases (35.91%:4.53%, t =36.78,P=0.00), whereas decreased in G0/G1 and S phases (47.02%:74.07%, t =11.71,P=0.00;17.07%:22.32%, t =2.30,P<0.05). The sensitizing enhancement ratio was 1.30. Conclusions Rb94 has marked radiosensitizing effect on A549 cells by G2/M phase blockage and down-regulation of hTERT and Bcl-2 mRNA expression. Rb94 may also inhibit the ability of cell proliferation by regulating cell cycle distribution.
9.Computed tomography pulmonary angiography with low dose contrast medium optimization research in dual-source CT

Objective To explore the feasibility of high-pitch CT pulmonary angiography (CTPA)with low dose contrast medium optimization in dual-source CT.Methods Eighty patients with high risk of pulmonary embolism who underwent CTPA on a dual-source CT were divided into two groups randomly:Group A,40 patients,50 mL contrast medium per patient,with conventional bolus tracing scanning;Group B,40 patients,contrast medium=body weight×0.3 mL/kg,with test bolus of small dose contrast agent. Pulmonary artery CT value,the differences of CT values in the paired PA and PV,SNR,CNR,radiation dose of two Groups were compared by two sample t-test.The positive rate of pulmonary embolism and the subjective indexes for assessing CT image quality were compared byχ2 test.Results There were no difference of the positive rate of PE between two Groups (P =0.655),and no sta-tistical difference for image quality scores between two Groups(P =0.1 74).The MPA CT value and the differences of CT values in the paired PA and PV in L-SⅢ of Group B were higher than those in Group A (P =0.001).However,no significant differences in CNR and SNR in MPA were observed between the two Groups(P =0.250,0.203,respectively).The ED of Group B was lower than that of Group A (P =0.001).Conclusion High-pitch CTPA in dual-source CT with lower dose contrast medium optimization can minimize the amount of contrast agent while meeting the clinical requirements.
10.A study on the tendency of genetic alteration of STR loci in human lung cancer tissues

Background and purpose: Short tandem repeats (STR) multiplex PCR fluorescence detection technology is the most widely used DNA technology in individual identity and genetic identification. It's the most direct method to obtain accurate conclusions. However, some studies have indicated that the rate of STR mutations in tumor tissue is significantly higher than that in normal tissues or blood. This study aimed to investigate the tendency of genetic instability in 20 STR loci on autosomal and Amel loci in tumor tissue samples from lung cancer. Methods: This study, collected 75 cases of human lung cancer tissues and the adjacent normal tissues. DNA samples were extracted by tissue DNA extraction kit, amplified using MicroreaderTM 21 Direct ID System PCR amplification kit. Capillary electrophoresis was performed using API 3130 analyzer, and results were analyzed by genetic analysis software (Gene Mapper ID V3.2). Results: STR alterations were detected in 24 specimens from 75 lung cancer tissues (32%). Fifty-five alterations were detected in the frequently used 21 STR loci in total, including additional alleles 10 times, loss of heterozygosity 10 times, partial loss of heterozygosity 35 times. Partial loss of heterozygosity was the most common genetic alteration types accounting for 63.64% of the total alteration frequency. And multiple genetic alteration types could occur in the same lung cancer tissue. Among them, the highest alteration frequency occurred on D5S818 (7 times), secondly on D3S1358 and D12S391 (both 5 times), and no alterations on D2S441 and Penta E. Combining the experimental results and analysis on clinical data, this study found the statistical differences between the staging of lung cancer and the age of the patients with the STR loci alterations (P<0.05). However, the alterations did have much relationship with the classification of lung cancer and the patient's gender (P>0.05). Conclusion: STR loci of the lung cancer tissue were not stable, and the alteration occurred in the aged or high malignant degree lung cancer tissue more frequently. Meanwhile, no alteration was detected on D2S441 and Penta E. In the future research the two STR loci should be verified to determine whether they can be used as the stable STR loci in such cases by increasing the sample size.
