1.Correlation of the quality of life and lower urinary tract symptoms in patients under intravesical chemotherapy

Objective To evaluate the quality of life (QoL) in patients undergone intravesical chemotherapy for non-muscle invasive bladder cancer and analyze the correlation of the quality of life and low urinary tract symptoms in this group of patients.Methods 31 male patients and 15 female patients were enrolled from Jan.2012 to Mar.2013.The average age was 54 (35-71) years.Questionnaires of QoL and core lower urinary tract symptom score (CLSS) were given to 46 patients before intravesical chemotherapy and the 1 st,6th month after starting the instillation.Before the instillation,the scores of QoL and CLSS were 55.7±6.2 and 6.5±2.1,respectively,the change of QoL and CLSS were compared to baseline,and then the correlation of the low urinary tract symptoms and quality of life was analyzed.Results At the end of 1st and 6th month,the QoL scores were 74.7±8.1 and 78.5± 10.6.The CLSS scores were 8.9±2.0 and 9.1 ± 1.8,respectively.The differences were significant (P <0.05) when compared to the baselines.The areas of working activity and free time were affected much more than other areas,and the scores increased from 8.3±2.1 and 19.2±5.7 to 14.3±5.6 and 23.7±4.2,respectively.The area of free time was found to be associated with local symptoms (r=0.61).Conclusions Intravesical treatment can impair the QoL of patients.Low urinary tract symptom is an important factor and is associated with impairment of QoL,suggesting the relief of local symptoms may improve the overall quality of life.
2.Analysis of the brain tumor operation in cerebello-pontine angle

Objective To discuss the microsurgery approaches and the prevention and treatment of complication about the tumors in cerebellopontine angle.Methods The 53 cases with cerebellopontine angle brain tumor were retrospectively analyzed.Results In all 53 patients,the tumor of 23 cases were full-resected,29 cases' tumor were resected more then 3/4,only one cases' tumor was partly treated.Following operation,one case died,5 cases had hearing decreasing,3 cases had severely facial nerve weakness and non hearing,10 cases had facial nerve anaesthesia,futhermore there was one case with long-period coma because of the injury of brain stem.Conclusion The important factors that decrease the complications of cerebellopontine angle tumor surgery include surgery approachs,operating experience and mechanism of complication.
3.Operative treatment of hypertensive intracerebral hemorrhage

Objective To explore the effect of operative treatment of hypertensive intracerebral hemorrhage.Methods 386 cases with hypertensive intracerebral hemorrhage were retrospectively analysed,hematoma was removed through big bone flap and small skull window,or drainage and urokinase infusion with small hole puncture.Results 39 cases died(10.1%),according to ADI grade system:Ⅰ 106 cases(27.4%),Ⅱ 136 cases(35.2%),Ⅲ 50 cases(13%),Ⅳ 43 cases(11.1%),Ⅴ 12 cases(3.2%).Conclusion To increase the cure rate and life quality,and obtain a lower mortality,it is a key to do surgical operation such as microsurgery or drainage and urokinase infusion.
4.Effects of three procedures for benign prostatic hyperplasia on stress responses:A comparison study

Objective To investigate the effects of three procedures for benign prostatic hyperplasia (BPH) on neuroendocrine and immune responses and its clinical significance. Methods Patients were divided into three groups with 20 patients in each group: the Group Ⅰreceived the transuretheral resection of prostate (TURP), the Group Ⅱ received the suprapubic transvesical prostatectomy (SPP) and the Group Ⅲ underwent the retropubic prevesical prostatectomy (RPP). The pre- and post- operative serum concentrations of Interleukin-6 (IL-6) and cortisol as well as changes of T-cell subgroups in the three groups were analyzed , respectively. Results Serum IL-6 concentrations remarkably increased at the third postoperative hour in patients of all the three groups, but those in Group Ⅱ and Group Ⅲ were significantly higher than those in Group Ⅰ( P 0 05). 5.Clinical value of miRNA-27a-3p expressed in peripheral blood and cancer tissues of colorectal cancers

Objective To investigate clinical value of miRNA-27-3p expressed in colorectal cancers,liver metastasis,and the peripheral blood,and analyze its target genes.Methods Among 78 cases of colorectal cancer patients,sera,colorectal cancers,and liver metastases were used to detect miRNA-27a-3p expressions with real-time quantitative polymerase chain reaction (RT-PCR) and analyze its clinical significance.We predicted miRNA-27-3p target genes,and confirmed the target genes and their correlation between miRNA-27a-3p and target genes.Results The expression level of miRNA-27a-3p in 78 cases of colorectal cancers was 3.13 ± 1.72,which was significantly higher than that in matched adjacent cancer tissues (1.06 ± 0.42) and control group (0.68 ± 0.27) (P < 0.05).The expression level of miRNA-27a-3p in 27 cases of liver metastases was 3.48 ± 1.15,which was significantly higher than that in paired colorectal cancer tissues (2.34 ± 1.03) (P <0.05) and adjacent tissues (0.81 ± 1.14) (P <0.05).Expression levels of miRNA-27a-3p in patients with colorectal cancer tissues and peripheral blood had no significant difference in age,sex,serum carcinoembryonic antigen (CEA) levels,tumor location,and pathological types (P > 0.05);however,it had positive significant difference in lymph node metastasis,liver metastasis,tumor node metastasis (TNM) staging (P < 0.05).The expression of miRNA-27a-3p was negatively correlated with the expression of target gene GP73.Conclusions The higher expression was miRNA-27a-3p in peripheral blood and colorectal cancer tissues,higher risk of liver metastasis occurs.MiRNA-27a-3p might participate in the occurrence and development of colorectal cancer by regulating target gene expression of GP73.In peripheral blood of colorectal cancer patients,miRNA-27a-3p might be used for potential auxiliary diagnosis and predict liver metastasis.
6.The influence factors of maintenance dialysis patients with carotid plaque and its relationship with cardio-vascular disease

Objective To analyze the influence factors of maintenance dialysis patients with carotid plaque and explore its relationship with cardiovascular disease(CVD). Methods 52 patients on peritoneal dialy-sis and 46 patients on maintenance hemodialysis were involved in this study. Bilateral carotid intima-media thick-ness(IMT)and plaque,the related clinical indicators and CVD were recorded. According to the mode of dialysis, carotid plaque,CVD respectively,we divided our population into two groups,and statistical analyses were made between the two groups. Results Age,percentage of male,the number of hypertension and diabetes,blood uric acid ,blood glucose ,the incidence of cardiovascular disease are higher ,but the level of albumin is lower in the carotid plaque group. Binary Logistic Regression analysis showed that hypertension ,albumin and uric acid signifi-cantly influenced carotid plaque formation (P < 0.05). CVD is closely related to the carotid plaque (r = 0.415 , P = 0.000). Left and right IMT value are higher in the patients with CVD (P < 0.01);carotid plaque mainly distributed in the common carotid artery(CCA)and carotid artery bifurcation(BIF),and with the larger number in the patients with CVD(P<0.01). Conclusions Carotid plaque formation is closely related with the occurrence of CVD in maintenance dialysis patients. Hypertension ,albumin and uric acid are risk factors for the formation of carotid plaque,so dialysis patients in addition to maintain the blood pressure and improve nutrition,are also very important to control the level of uric acid.
7.CT and pathological analysis of peripheral primitive neuroectodermal tumors in abdomen-pelvis region

Objective To analyze the correlations of clinical characteristics,CT and pathology features of peripheral primitive neuroectrodermal tumors (pPNET)in abdomen-pelvis region,and to improve the understanding of the disease.Methods The clinical information,CT and pathological features of 23 confirmed pPNET patients in abdomen-pelvis region were analyzed retrospectively.Results (1)All patients had preoperative clinical and imaging data integrallty.(2)23 cases showed soft tissue masses with fuzzy boundary and the average diameter were 10.72 cm.Among all,9 cases showed with varying degrees of sac necrosis,while 2 cases showed with platelet hemorrhage and 4 cases with speckled calcifications.Most showed slight-to-moderate heterogeneous enhancement and the necrosis and cystic are-as showed more obvious.12 cases showed with different degree of recurrence,invasion or distant metastasis.5 cases combined with tumor emboli and 4 cases with tumor feeding vessels.(3)Immunohistochemical examination showed CD99 expressed in 1 9 patients, CD56 in 11,vimentin(Vim)in 12,synaptophysin(Syn)in 11,neuron specific enolase(NSE)in 4,FLI-1 in 4 and epithelial membrane antigen (EMA)in 4 patients.Conclusion pPNET is common among young patients with a high degree of malignancy.CT features include large mass with invasive growth,commonly combined heterogenous density,necrosis and cystic changes.Persistent enhancement is observed during nephrographic phase.
8.CT features of heterotopic pancreas and its correlation with pathologic changes

Objective To analyze the CT findings of heterotopic pancreas and the correlation with pathologic changes.Methods CT findings in 1 9 patients with heterotopic pancreas proven by surgery were reviewed retrospectively,14 of whom were scanned with enhanced CT.The correlation of CT findings and pathologic results were also analyzed.Results All patients had a single lesion with sized range from 0.8 to 4.7 cm and mean diameter of (2.2±0.6)cm.All lesions with homogeneous density were showed on plain scans including 7 lesions in stomach,8 in duodenum,2 in jejunum,1 in ileum and 1 beside the cardiac lymph nodes.The border of the lesions in 5 was unclear with exudative changes,mimicking acute pancreatitis.A sign of central umbilical concave was found in 2.Pathological results confirmed 14 lesions in submucosa,3 in muscular layer,1 in serosal layer and 1 in abdominal cavity.Hetero-topic pancreas presented early obvious enhancement in 9,mimicking normal pancreas,which was composed chiefly of pancreatic aci-ni.The enhanced degree of 4 lesions was lower than normal pancreas,which were composed of pancreas ductal epithelium and few pancreatic acini.No significantly enhancement was seen in 1,mimicing liomyoma,which was composed chiefly of pancreas ductal epithelium and smooth muscle hyperplasia.Conclusion Heterotopic pancreas in most cases presents characteristic submucosal disea-ses-like pattern,and different enhanced patterns of heterotopic pancreas are related with the pathologic changes.
9.Relationship of CT features and pathological grades of malignancy in gastric stromal tumors

Objective To study the relationship between the CT features and pathological grade of malignancy of the gastric stro-mal tumors (GST).Methods A retrospective review was performed based on the database of 80 consecutive patients with GST.Age and gender of the patients were recorded,and the CT findings of GST including growth pattern,morphology,tumor site,boundary, mucosa,serosa,ulcer,volume and maximum diameter of the tumor were also measured.The patients were divided into three groups:very low and low risk group,intermediate risk one and high risk one according to the pathology.All parameters were ana-lyzed by one-way ANOVA or Fisher’s exact test.Results The patients in 3 groups (from low risk to high one)were 30,30 and 20 respectively.There were no significant differences in age,gender,location and mucosa among 3 groups(P>0.05).However,the growth pattern,morphology,boundary,serosa,ulcer,volume and maximum diameter of the tumor were significantly different (P<0.05).Conclusion The CT features including growth pattern,morphology,boundary,serosa,ulcer,volume and maximum diameter of the tumor can be used to evaluate the grades of malignancy in GST.
10.Clinical research on utility of fluorescence in situ hybridization assay in the diagnosis of urothelial cancer

Background and purpose: Urothelial cancer is the most common malignant neoplasm in the urinary system. Urine cytology is the standard morality to diagnose urothelial cancer. Although cytology has been shown to have a high specificity, the sensitivity is unacceptably low. The count of copies of chromosomes in interphase cells by fluorescence in situ hybridization (FISH) assays has been successfully used as a screening tool in genetic and cancer studies. In this study, we investigated the value of FISH assay for diagnosis of urothelial cancer. Methods:Voided urine samples from 100 patients with haematuria were analyzed by FISH and cytology; labeled probes for chromosomes 3, 7, 9 and 17 were used to assess chromosomal abnormalities. The gold standard was pathology diagnosis. The overall sensitivity and specificity of FISH were evaluated and compared with cytology. Results: The sensitivity of FISH and cytology in detection urothelial cancer were 74.7% and 46.0% respectively; the specificity was 92.3% and 100% respectively (different not significant). There was a significant difference between the sensitivity of FISH and cytology in detection. Conclusion. The FISH assay has a higher sensitivity than cytology and a similar specificity in the detection of urothelial cancer, and could be used as a new method for diagnosis of urothelial cancer.
