Objective To evaluate the clinical value of transrectal ultrasound guided systematic 13 cores prostate biopsy. Methods A total of 160 patients referred for abnormal digital rectal examination and (or) prostate specific antigen 4ng/ml or greater underwent transrectal ultrasound guided systematic 13 cores prostate biopsy. That was,in addition to the standard sextant biopsies, cores were taken from the far lateral and middle regions of the gland as described by Eskew.Pathological findings of the additional regions were compared with those of the sextant regions. Results Of the patients 35 % had cancer on biopsy(56 /160).Of the 56 patients with prostate cancer 12(21%)had carcinoma only in the additional regions, which would be undetected on the sextant biopsy technique (P

Objective To investigate the effect of repeated hyperbaric oxygen preconditioning (HOP) on the expression of hypoxia-inducible factor-1 alpha (HIF-1a) and erythropoietin (EPO) following spinal ischemiareperfusion (I/R) in rats. Methods Forty-eight male SD rats weighing 250-300 g were randomly divided into 3 groups (n = 16 each): sham operation group (S group); I/R group and repeated HOP group (HOP group). The animals in HOP group were pretreated with O2 ( > 98 % ) at 2.5 ATA I h/d for 5 consecutive days at 24 h before spinal cord I/R. The animals were anesthetized with intraperitoneal pentobarbital sodium 35 mg/kg. Spinal cord ischemia was produced by cross-clamping of abdominal aorta distal to renal artery for 20 min in I/R and HOP groups. Hind-limb motor function was assessed and scored st 48 h of reperfusion. The animals were then sacrificed and the L5-7 segment of the spinal cord was removed for determination of the expression of HIF-1 α and EPO mRNA and protein and microscopic examination. Results Compared with group S, the hind-limb motor function scores and the number of normal motor neurons in the anterior horn of the spinal cord were significantly decreased, and the expression of HIF-1α and EPO mRNA and protein was up-regulated in I/R and HOP groups ( P < 0.05). Compared with group I/R, the hind-limb motor function scores and the number of normal motor neurons in the anterior horn of the spinal cord were significantly increased, and the expression of HIF-1α and EPO mRNA and protein was up-regulated in group HOP ( P < 0.05). The spinal cord injury was attenuated in group HOP compared with group I/R. Conclusion Repeated HOP can reduce the spinal cord I/R injury though up-regulating the expression of HIF1α and EPO in rats.

Objective To optimize the enamel electron paramagnetic resonance (EPR) spectral processing by using the EPR spectral simulation method to improve the accuracy of enamel EPR dosimetry and reduce artificial error. Methods The multi-component superimposed EPR powder spectral simulation software was developed to simulate EPR spectrum models of the background signal(BS) and the radiation- induced signal (RS) of irradiated enamel respectively.RS was extracted from the multi-component superimposed spectrum of irradiated enamel and its amplitude was calculated. The dose-response curve was then established for calculating the doses of a group of enamel samples. The result of estimated dose was compared with that calculated by traditional method. Results BS was simulated as a powder spectrum of gaussian line shape with the following spectrum parameters:g =2.00 35 and Hpp =0.65 - 1.1 mT,RS signal was also simulated as a powder spectrum but with axi-symmetric spectrum characteristics. The spectrum parameters of RS were:g(1) =2.0018,g (11) =1.996 5,Hpp =0.335 - 0.4 mT.The amplitude of RS had a linear response to radiation dose with the regression equation as y =240.74x + 76 724 ( R2 =0.9947 ).The expectation of relative error of dose estimation was 0.13.Conclusions EPR simulation method has improved somehow the accuracy and reliability of enamel EPR dose estimation.

Objective To know about the post-traumatic stress disorders (PTSD) of women with termination of pregnancy for fetal anomalies (TOPFA),and investigate the relationship of PTSD and the big five personality traits. Methods The general questionnaire, the Impact of Event Scale-Revised (IES-R) and the Big Five Questionnaire were administered to the women with TOPFA in hospital from February 2014 to February 2015.Results The total score of IES-R of 84 cases was 26.20 ± 16.38.The scores of 49 cases(58.3%) were more than 19 and that of 20 cases (23.8%) more than 35.Women who had the first pregnancy had higher level of arousal symptoms,12.84±7.41 vs.9.65±5.53 (t =2.25,P < 0.05).Women whose scores were above 19 had lower adaptation, sociality, altruism and conscience compared with those scored ≤ 19,14.96±3.29,16.84±3.00,18.55±3.49,17.57±2.99 vs.13.57±2.82,18.31±2.65,20.03±2.32,18.86±2.21,t=2.02-2.33,P < 0.05.The IES-R score had no linear correlation with that of the big five personalities, and intrusion subscale had linear correlation with the adaptation.Avoidance symptoms had low positive relationship with sociality in women whose IES-R were above 19,r =0.23,P < 0.05.In populations whose IES-R scored above 19,avoidance subscale had positive weak correlation with sociality,r=0.31,P <0.05.Conclusions The women with TOPFA had high level of PTSD before termination.Women with sever posttraumatic stress symptoms had lower sociality and altruism. Moreover,of the women who had sever posttraumatic stress symptoms, one with higher sociality always had higher avoidance symptoms.

Objective To evaluate the significance of surgical exploration for the refractory arterial crisis daring the hypersensitive period (48 h to 96 h) after replantation of severed fingers. Methods One hundred and seventy-one patients experienced refractory arterial crisis during the hypersensitive period after replantation of the proximal thumb from February 1995 to February,2005 in our department.Eighty-seven of them were managed with surgical exploration, including incision injury (n=6),saw injury (n=17),rotation and avulsion injury (n=30), and crush injury (n=34).Eighty-four cases received conservative treatment, including incision injury (n=6),saw injury (n=16).rotation and avulsion injury (n=29),and crush injury (n=33).In the surgery group, the e- mergent explorations were performed as soon as the refractory arterial crisis arose,If arterial spasm or/and thrombosis were found, the involved parts were resected before the artery ends were anastomosed or the finger artery was repaired by cubital vein graft. In the other group, conservative managements were carried out by using intramuscular injection of 30 mg Papaverine and intravenous injection of 20,000-unit Urokinase in 20 mL normal saline. If symptums were not alleviated after half an hour, the procedures were repeated. The conservative managements also included abirritative antipsychotics and analgesia of anodyne.Meanwhile, the survival state of all the digital replants was observed. Results In the surgery group,78 fingers survived, the surviving rate being 89.7%.In the conservative group,41 fingers survived with a surviving rate of 48.8%.The difference was statistically significant (P<0.01).No obvious complications happened in the two groups. Conclusion Since surgical exploration is crucial to management of refractory arterial crisis during the hypersensitive period after replantation of severed fingers,it should not be readily abandoned.
