1.The clinical pathological analysis of 23 patients with renal damage caused by microscopic polyangiitis

Objective To study the clinical features and pathology of renal damage in patients with MPA. Method The clinical pathological changes of 23 MPA patients were analyzed and the patients with positive ANCA were compared with those with negative ANCA. Results Most MPA patients were senile and male with the symptoms of lung damage, pleuritis, arthritis and myalgia as well as extrarenal symptoms such as fever, weight-loss, and anorexia. 65.2% of the patients were ANCA (+). Symptoms of renal function damage were hematuria and proteinuria, which could be found in all the patients. Different degree of renal damage could be detected. Glomerular cresent formation, which were mostly fibrous, could be found in all of the 23 patients. Half of the patients have tuft necrosis and interstitial vessel vasculitis. Conclusion MPA patients often have extrarenal symptoms besides renal function damage. Patients with positive ANCA differs from patients with negative ANCA in both clinical manifestation and kidney pathology.

Objective: Try to get the information about the civil will of donation of cadaver in Wuhan. Method: Make stochastic visit to the dwellers in Wuchang district of Wuhan and then study the statistical data. Content: Legal system, sensibility, traditional ideas and public opinions etc. Result: The most notable factors are legal system and sensibility. Conclusion: In order to implement the new law,we should strengthen the dissemination of donation of cadaver.
3.Cardiac differentiation of human embryonic stem cells by coculturing with human fetal cardiomyocytes

Objective To explore the cardiac differentiation of human embryonic stem cells(ESCs)in coculturing with human fetal cardiomyocytes.Methods Fetal cardiomyocytes and fibroblasts were obtained from the aborted fetus,and human ESCs were from the inner cell mass(ICM)of the surplus embryos for tube babies,with the treated fibroblast as feeder layer.Human ESCs of passage 2 to 5 were cocultured with fetal cardiomyocytes at the ratio of 1∶2,and the XY chromosome types of these two kinds of human cells were different to each other in order to be marked.At the same time,human ESCs without coculture were used as negative control.The cocultured cells were harvested 5 days later and were double-stained with human XY chromosome probe and cardiac specific antibody(desmin or cardiac troponin I,cTnI).Results On coculture day 5,40%-50% human ESCs expressed cardiac specific antigens,and human ESCs without coculture did not express those antigens.Conclusion Human ESCs could be induced into cardiac differentiation by the coculture with fetal cardiomyocytes,and hopefully they will be the candidate for cell transplantation of heart.
4.THE HEALTH, PROTEIN AND AMINO ACIDS STATUS OF THE PEOPLE CONSUMING PREDOMINATELY CEREALS IN DIET

Objective: To evaluate the status of protein and amino acids in farmers who consumed cereals predominately in Henan province, China. Method: Socioeconomic information of 101 selected families was obtained by interviewing; food consumption data were gathered by 12 separated 24 h recalls, and FFQ method. Results: The protein intake was 74 g/capita/day in average, 87% from plant sources. Lysine is the first limiting amino acid . By anthropometric measurements of children aged 5-12 years, 26% of them were stunted and 40% underweight. Conclusion: The total amount of protein matched adult requirements, but was insufficient for children growth. Lysine fortification and other measures may be suggested for relieving child undernutrition.
5.Preparation and Physicochemical Property of Epirubicin Hydrochloride Solid Lipid Nanoparticles

OBJECTIVE:To prepare epirubicin hydrochloride solid lipid nanoparticle(EPI-SLN) and investigate its physicochemical property. METHODS: EPI-SLN was prepared by ultrasonic dispersion technique with glyceryl behenate as matrix,and the appearance,particle size,? electric potential,and entrapment efficiency of the SLN were evaluated and its stability at 3 month storing at 4 ℃ was investigated. RESULTS: EPI-SLN assumed spherical shape with a particle diameter of (212.8?6.2) nm,? electric potential of (-24.7?0.3) mV and entrapment efficiency of 82 %. The EPI-SLN at 4 ℃ was stable after storing for 3 months,showing no marked change in mean diameter,? electric potential or entrapment efficiency. CONCLUSION: The prepared EPI-SLN is up to the standard.
6.SURVEY ON DIETARY COMPOSITION OF THE INTELLECTUALS IN THREE CITIES

Objective: To investigate the dietary nutritional status of the intellectuals in three cities in China. Methods: The intellectuals aged over 45, with senior academic title, working in universities or research institutes in Tianjin, Guangzhou and Chengdu, were selected as volunteers. Food frequency questionair was used to conduct the diretary survey. And the results were compared between the intellectuals of nutrition and non-nutrition speciality. Results: Compared with Dietary Guideline and Balance Diet Pagoda for Chinese Residents, the intakes of cereals, milk, fruits,and egg of the vulunteers were adequate, but the intakes of meat and oils were more, while those of fish and shrimp, legume,and vegetables were less than the suggested levels in Pagoda. The food pattern of the intellectuals in nutrition speciality was more advantageous than that in non-nutrition. Conclusion: The dietary pattern of the intellectuals should be improved, according to the Dietary Guideline and Balanced Diet Pagoda for Chinese Residents.
7.Modified Arytenoid Adduction for Unilateral Vocal Fold Paralysis

Objective To evaluate the efficacy of modified arytenoid adduction in the management of patients with unilateral vocal fold paralysis(UVFP).Methods A retrospective review was performed on 22 patients who underwent modified arytenoid adduction for UVFP between February 2001 and December 2007.Pre-,and 3 months postoperative aspiration,perceptual(GRBAS),acoustic data(fundamental frequency,F_0,fundamental frequency perturbation,jitter,amplitude perturbation,shimmer,normalized noise energy,NNE)and aerodynamic(maximal phonatory time,MPT,mean airflow rate,MFR)were analyzed statistically.Results The ratings of postoperative aspiration were significantly decreased than that of the preoperation(P<0.0001).There was a significant decrease in GRBAS scales postoperatively versus preoperatively(P<0.0001).The mean values of voice acoustics parameters (F_0,jitter,shimmer,NNE)were significantly decreased,the maximum phonation time were significantly longer,and the mean airflow rate were significantly decreased after operation than that of the preoperation(P<0.001).Conclusion Modified arytenoid adduction is an effective medialization technique that can restore satisfactory speech and prevent aspiration in patients with UVFP.
8.Follow-up for vascular structure and function in children with successfully repaired coarctation of aorta

Objective Even after successful surgical repair,patients with coarctation of the aorta (CoA) are at high risk of long-term morbidity and mortality due to cardiovascular events,which is probably related to persistent arterial disfunction during long-terr follow-up after operation,The aim of the study was to explore the alterations of vascular structure and function in children with successfully repaired CoA in the short-and mid-term follow-up.Methods A cohort of 20 children who underwent CoA repair between January 2010 and October 2010 in Guangzhou women and children's Medical Center was studied.There were 14 males and 6 females in CoA group,which comprised 6 patients with isolated CoA,14 patients with CoA associated with intracardia anomalies,whose median age of operation was 4 months (rang from lmonth to 10.0 years).And 20 patients with isolated ventricular septal defect (VSD) were included as VSD group during the same time,with 12 males and 8 females,whose median age of operation was 5 months (rang from 1 month to 12.0 years).Resting blood pressure,flow-mediated dilation (FMD) of the brachial artery,carotid intima-media thickness (IMT) were compared in CoA group and VSD group,including preoperative media data and follow-up of 1 month,6 months and 1 year.In addition,as comparison to the operation group,20 health children with normal echocardiographic findings,whose median age was 5 months (rang from 3 month to 10.0 years),were selected as health group for the 1-year following up.None of them had obesity,hyperlipidemia,diabetes mellitus,metabolic diseases or systemic inflammatory disease.Results As a result of the datas before operation and those I month,6 months and 1 year after operation,all children were normotensive at rest.In the same period,Carotid IMT in CoA group[(0.47 ± 0.10)mm,(0.49 ±0.10) mm,(0.57 ±0.07)mm,(0.61 ± 0.07) mm]was significantly thicker than that in VSD group[(0.41 ±0.11) mm,(0.43 ±0.11)mm,(0.51 ±0.08) mm,(0.55 ±0.08) mm](P<0.05) and health group[(0.40 ±0.09) mm,(0.42 ±0.11)mm,(0.50 ±0.08) mm,(0.57 ±0.08) mm](P <0.05),Brachial artery FMI in children with CoA[(5.4,6 ±1.51)%,(5.71 ±1.88)%,(5.42±1.69)%,(5.27±1.02)%]was significantly lower than that in the VSD control group[(6.69±1.45) %,(6.66±1.21)%,(6.81 ±1.03)%,(6.43±1.34)%](P<0.05) and health group[(6.59 ±1.84)%,(6.84±1.41)%,(6.91 ±1.31)%,(6.56±1.62)%](P<0.05).Significant difference could not be found in neither the IMT nor the FMI between the VSI control group and health group in 4 period respectively,P > 0.05.Conclusion Children after successful coarctation repair have abnomal structural and functional properties of the aorta above the place of coarctation even their blood pressure at rest is normal.These results confirm that the alterations in mechanical properties of carotid arteries as well as the generalized endothelial dysfunction in children with coarctation of the aorta are persistent,which can not be prevented or reversed by surgical repair,and which may partly explain the high incidence of cardiovascular disease observed in their adulthood and reduced life expectancy,furtherly supporting the claim that coartation of the aorta is a systemic vascular disorder which needs long-term follow-up of vascular function.
9.Genetic analysis of two fetuses with congenital heart defects and 3q microdeletion.

OBJECTIVETo determine the nature of genomic copy number variations (CNVs) in two fetuses with congenital heart defects (CHD) and explore the correlation between 3q microdeletions and CHD.
METHODSGenomic DNA was extracted from fetal umbilical cord tissue, and chromosome copy number variations were detected by low coverage whole genome sequencing.
RESULTSBoth fetuses had microdeletions of the long arm of chromosome 3. Fetus 1 had ventricular septal defect, cleft lip and palate, and a 1.66 Mb deletion on 3q29. The deleted region encompassed all of the critical genes for 3q29 microdeletion syndrome. Fetus 2 had overriding aorta, ventricular septal defect, and a novel 240 kb deletion on 3q28.
CONCLUSION3q29 microdeletion may result in CHD in combination with cleft lip and palate. Genomic CNVs can be detected by low coverage whole genome sequencing.

10.Retrospective analysis of the predictive value of immunoglobulin and complement combined leukocyte levels on the outcome of severe COVID-19

Objective:To retrospectively analyze the blood leukocytes (WBC), lymphocytes (LYM), lymphocyte% (LYM%), and serum total immunoglobulin (IGA, IGG, IGM) and complement (C3, C4) index levels to explore its predictive value for the outcome of COVID-19 severe pneumonia.Methods:Eighty-five COVID-19 patients with severe pneumonia diagnosed in our hospital were randomly selected and were divided into good outcome group (50 cases) and poor outcome group (35 cases). WBC, LYM, LYM%, IGA, IGG, IGM, and C3, C4 level data, and analyze the differences between the two groups, the correlation of each indicator, and ROC curves of single and joint detection to explore relationship between indicators and outcomes, and the predictive efficacy of indicators on outcomes.Results:Differences in WBC, LYM, LYM%, IGG, and IGA levels were significant between the two groups ( P=0.000, 0.015, 0.000, 0.000, 0.001), among them with significant differences, LYM and LYM% were significantly positively correlated ( r=0.669, P=0.000), while WBC and LYM% levels were significantly negatively correlated ( r=-0.600, P=0.000), WBC and IGA levels were significantly positively correlated ( r=0.283, P=0.009) and IGG and IGA levels were also significantly positively correlated ( r=0.0.442, P=0.000); After logistic regression analysis, WBC, LYM, LYM%, IGG, and IGA are all important influencing factors ( P=0.001, 0.022, 0.000, 0.000, 0.003); but only the levels of WBC, IGG, and LYM% are Independent risk factors ( P=0.034, 0.004, 0.001), the ROC curve of the single detection and joint detection of their predicted outcome performance, respectively, and the max AUC (AUC=0.890, P=0.000) at the time of joint testing of WBC, LYM% and IGG, index YI=0.657, it has the greatest predictive power for adverse outcomes, with a sensitivity of 77.10% and a specificity of 88.00%. IGM, C3, C4, IGG/IGM, and C3/C4 levels were not significantly different( P=0.066, 0.204, 0.076, 0.310, 0.156). Conclusions:The levels of WBC, LYM, LYM%, IGG, and IGA in the early admission of COVID-19 infected patients with severe pneumonia have important predictive value for the outcome of them. WBC, LYM% and IGG levels are independent risks and joint detection of the three indexes have the best predictive performance.
