1.The latest research progress of torpor occurrence mechanism

Torpor refers to a state in which the metabolic activity in the body of the living animal is greatly reduced during the period of reduced food supply, which is manifested as a substantial decrease in body temperature, metabolic level, and exercise level. Mammals have a strict body temperature regulation system to maintain a constant body temperature. When the energy supply is insufficient for a long time, some mammals will enter a hibernation state. Torpor is very similar to the hibernation state. The research on the mechanism of torpor state is of great significance in aerospace, military medicine and other fields. This review summarizes the specific mechanisms regulating the occurrence of torpor from four aspects: adenylate cyclase activating polypeptide (adcyap) neurons, leptin, pyroglutamylated RFamide peptide (QRFP) neurons, and sympathetic nervous system, aiming to provide ideas for further research on the mechanism of torpor.
2.Curative efficacy of longxuejie capsule combinated with tinidazole in treatment of cervicitis and its effects on levels of immunoglobulin

Objective To investigate the clinical effect of longxuejie capsule in combination with tinidazole in the treatment of chronic cervicitis and its effects on levels of immunoglobulin.Methods 90 cases in our hospital from January 2013 to June 2014, with chronic cervicitis meeting the inclusion critera were randomly divided into 2 groups by method of stratified randomization equally.Control group (n=45) was treated with tinidazole and conventional treatments, while observation group ( n =45 ) was treated with tinidazole and longxuejie capsule.Then, the curative efficacy, scale of symptoms and levels of immunoglobulin were observed and compared.Results The therapeutic efficiency ratio of observation group was 95.6%, which was significantly higher than that of control group ( 80.0%, P<0.05 ).After the treatment, in comparison with the control group, scale of cervical secretion and severity of cervical lesion was statistically lower respectively than observation group ( P<0.05 ).As to immunological function, concentrations of IgG, IgM, IgA in observation group were statistically higher than that in control group after the treatment (P<0.05).The control group and observation group respectively in 2 cases and 3 cases of abdominal pain, alleviated by themselves without treatment.Conclusion longxuejie capsule combined with tinidazole is effective and reliable for patients with chronic cervicitis, which significantly improves symptoms and elevates levels of immunoglobulin.
4.Follow-up of graduates of general practice residency training working in community health service centers in Shanghai

Telephone interview was conducted to follow up the 151 graduates of general practice residency training in Zhongshan Hospital who are working in community health service centers in Shanghai up to June 2014;the results were compared with those of 130 graduates in 2011.Based on investigation of 2014,94 out of 151 graduates worked in downtown and 57 worked in suburbs of Shanghai,while in interview of 2011,72 out of 130 in downtown and 58 in suburbs.Among 151 participants in 2014 interview 13 had senior professional tide,138 (91.39%) had intermediate or primary professional titles,while 4 in 2011 interview had senior professional title,126 (96.92%) had intermediate or primary professional title.Among 151 participants in 2014 interview,40 (26.49%) were heads of GP teams and 45 (29.80%) held administrative positions,while in 2011 interview 33 (25.83%) were heads of GP teams and 30 (23.08%) held administrative positions.2014 interview showed that 211 first-author papers were published,59 research projects were obtained with 4.34 million Yuan funds,303 prizes was awarded in 151graduates,all of which were increased compared to those of 2011 group.The resign rates in 2011 and 2014 groups were 6.43% (9/140)and 10.06% (17/169),respectively;the job-moving rates in 2011 and 2014 groups were 5.00% (7/140) and 11.95% (19/159),respectively.The resign rate of 2011-2014 was 5.03% (8/159) and the job-moving rate of 2011-2014 was 11.95% (19/159).Conclusion The follow-up study shows that the graduates of general practice residency training of Zhongshan Hospital perform well in community health service centers in Shanghai.
5.Establishment of evaluation index system for urban general practice service team in Shanghai

Objective To establish an evaluation index system for Shanghai urban general practice service team to improve standardized and scientific management. Methods The evaluation index system for Shanghai urban general practice service team was established using two rounds of Delphi consultation. Results The established system consisted of 5 fast-grade indicators and 56 second-grade indicators. The 5 first-grade indicators were staffing, infrastructure, quality and quantity of service, continuing education and satisfaction degree of suppliers and demanders, with weight of 0.2153, 0.1978, 0.2087, 0.1869 and 0.1923 respectively. The active coefficient for two rounds of Delphi consultation was both 100%. The authoritative coefficient of the two rounds was 0.852 and 0.883 respectively. The harmonious coefficient of importance of the evaluation index was 0.251 and 0.411 respectively. Conclusion Through using Delphi method scientifically and reasonably, we set up the evaluation index system for Shanghai urban general practice service team, with integral contents, strong operability and easy application.
6.The modified posterolateral curved incision with double intermuscular approch in treating posterolateral tibial plateau fractures

Objective To investigate the efficiency of the modified posterolateral curved incision with double intermuscular approach in the treatment of posterolateral tibial plateau fractures.Methods From August 2009 to March 2013,15 men and 17 women aged 28 to 77 (mean,46.25) years with posterolateral spitting or collapse tibial plateau fractures were treated to undergo a modified posterolateral approach.All types of fractures were closed,without nerve or artery injury.The duration from injury to fixation was 3-18 (mean,7.13) days.The modified posterolateral approach was preformed to expose tibial anterolateral condyle and posterolateral condyle through intermuscular space of anterior tibial muscle,popliteus muscle and soleus muscle.After open reduction,anterolateral fractures and posterolateral fractures were fixed by the T-shaped plate and Golf shaped plate respectively.Results All of the 32 patients were followed up from 6 months to 36 months (in average of 15.3 months).Postoperative X-ray showed satisfied fractures without obvious collapse of the articular surface.At the last follow-up,the Rasmussen scores were range from 6 to 18 (in average of 14.06),namely 14 cases excellent,15 cases good,3 cases fair.The knee joint functions were evaluated according to HSS.The HSS results showed that there were 12 cases excellent,16 cases good,3 cases fair and 1 case bad.None of the cases suffered from injuries of crucial blood vessel or nerves.No incision necrosis,wound infection or fixation failure occurred during follow-up.Conclusion Open reduction and internal fixation via modified posterolateral approach can expose posterolateral and anterolateral tibial plateau fracture in one incision.Fibular head osteotomy can be avoided.The present approach could also reduce surgical injury,minimize the possibilities of incision necrosis or plate exposure.The modified posterolateral approach can be recommended in clinical practice.
7.Study on the mechanism of carbapenem resistance in Pseudomonas aeruginosa

Objective To investigate if the 23 strains of highly-resistant P.aeruginosa isolated from different patients in the in- tensive care unit (ICU) have the same origin;and explore the related mechanisms of carbapenem resistance in these multidrug- resistant P.aeruginosa.MethOds Antimicrobial susceptibility testing was performed using disk-diffusion technique.The strains were genotyped by rep-PCR with the primer ERIC2 followed by electrophoresis in agarose gel.We used a previously described imipenem-EDTA double-disk test for screening MBL-producing P.aeruginosa.Polymerase chain reaction for amplification of blaOprD, blaIMP, and bla VIM were performed to detect corresponding mutants.Results The result of antimicrobial suscepti- bility testing showed that 20 of the 23 P.aeruginosa isolates were muhidrug-resistant and highly resistant to imipenem and meropenem, and at least 5 antimicrobial agents tested in this study.The analysis of the rep-PCR products indicated that all the 19 carbapenem-resistant strains had an identical band pattern, which was different from that seen in the sensitive strains.Al- though imipenem-EDTA double-disk test identified 5 MBL-producing strains, PCR found that all the 23 strains were negative for bla VIM and blaIMP.Only one OprD-deficient mutant was identified.Conclusions The 19 highly-resistant strains of P. aeruginosa derive from a common origin.More researches are needed to clarify their mechanism of carbapenem resistance.
8.Recent advances in the study of mechanism of APOBEC3G against virus.

APOBEC3 is a class of cytidine deaminase, which is considered as a new member of the innate immune system, and APOBEC3G belongs to this family. The research about APOBEC3G is a new direction of innate immune defense mechanism against virus. APOBEC3G has the restrictive activity on many viral replications, which deaminates dC to dU in the viral genome and then induces extensive hypermutation. APOBEC3G can also interrupt viral replication at several phases such as reverse transcription, replication, nucleocapsid and so on by non-deamination mechanisms. However, virus can encode viral proteins to counteract the restriction activity of APOBEC3G. Elucidation of the antagonistic interaction between APOBEC3G and the virus will be contributed to development of new antiviral drugs in the future.

9.Analysis and study of intraocular pressure, visual field and P-ERG on patients with big cup/disk

?AlM: To analyze and study the changes of intraocular pressure ( lOP) , visual field and P-ERG on patients with big cup/disk (C/D) in 24h.
?METHODS: A total of 120 cases ( 240 eyes ) diagnosed with big C/D (C/D>0. 3) were divided into group A (C/D<0. 6, 67 cases, 114 eyes ) and group B ( C/D≥0. 6, 73 cases, 126 eyes). Forty cases (80 eyes) with small C/D (C/D≤0. 3) were chosen as control group. All cases underwent 24h lOP examination, vision examination of 30-2 SlTA - standard static threshold and pattern electroretinogram ( P-ERG) examination. The differences between the examination indexes of the three groups were analyzed.
?RESULTS:There was no significant statistical difference in the 24h average lOP (P>0. 05) among the three groups, while the amplitudes had significant statistical differences (P<0. 05). Compared with the control group, the lOP amplitudes of group B were obviously higher, and the difference was of statistical significance (P<0. 05). ln terms of static threshold visual fields, the mean deviations (MD) and pattern standard deviations (PSD) of the three groups had significant statistical differences ( P < 0. 05 ). Comparison between every two groups:compared with the control group, the MD and the PSD were significantly increased in group B, and the difference was of statistical significance (P<0. 05); while there was no significant statistical difference in group A (P>0. 05);Compared with group A, the MD and the PSD were significantly higher in group B, and the difference was of statistical significance (P<0. 05). ln terms of P-ERG examination, there was no significant statistical difference in P50’s latent periods among the three groups (P>0. 05), but there were significant statistical differences in amplitudes (P<0. 05). Comparison between every two groups: compared with the control group, the P50’s amplitude was significantly decreased in group B, and the difference was of statistical significance (P<0. 05), while there was no significant statistical difference in group A (P>0. 05); Compared with group A, the P50’s amplitude of group B was significantly lower, and the difference was of statistical significance (P<0. 05).
?CONCLUSlON: When the patients’ C/D is no less than 0. 6, there are obvious changes of 24h lOP, static threshold visual field and P-ERG’s P50 wave. For patients with big C/D, a single lOP examination is far from sufficient, so an in-depth visual function examination should be performed. Meanwhile, it’s feasible to regard C/D 0. 6 as a screening criterion for suspected glaucoma.
10.Study on mental health level and coping style of the families with chronic diseases patients

Objective To investigate the incidience of anxiety or depression in the families of chronic diseases and to discuss the relationship between anxiety or depression and coping style. Methods Using SAS, SDS,SCSQ as main tools,questionnaire investigation was made in 232 families of chronic diseases. Results The incidence of anxiety was 25.9%, and that of depression was 43.1%, and the incidence of concurrence was 25.9%. The mental health level was asscociated with the sex, active response of families, the age, disease course of the patients, hospitalization costs, economic conditions. Conclusions The families of chronic diseases patients have different degrees of negative mood, which affects the patients. Therefore, enchancing the mental intervation of families of chronic diseases will do good to the recovery of diseases and improvement of life quality.
