1.New challenge of liver surgery: associating liver partition and portal vein ligation for staged hepatectomy

An innovative approach,named associating liver partition and portal vein ligation for staged hepatectomy (ALPPS),has made possible a marked increase in future liver remnant (FLR) over a short period of time,and has challenged the concept of unresectability.To date,ALPPS has been employed for a variety of primary and metastatic liver tumors from many centers across the world.Many technical variations of ALPPS have been described.However,much controversy has surrounded this procedure,and many problems remain to be solved,such as the reasons for rapid hypertrophy,the technical feasibility,safety and long-term oncologic outcomes,and the result compared with the conventional two-stage hepatectomy.These aspects are needed to be evaluated further in large-scale studies.We believe that the ALPPS procedure will has much room for further development.
2.Relationship between Ca, P and F concentration of plaque residues in the primary teeth and caries status, sugar exposure frequency.

OBJECTIVETo analyze the relationship between inorganic ingredients (Ca, P and F) concentration of plaque residues in the primary teeth and caries status, sugar exposure frequency.
METHODSSubjects were 93 of 3-5 years old children living in Beijing area. According to their caries status, they were classified into two groups: Severe early childhood caries(S-ECC) group and caries free(CF) group. Plaque samples were collected from subjects respectively. Ca, P and F concentration of plaque residues was measured by atomic absorption spectroscopy, spectrophotometer and fluoride electrode respectively. The information of daily sugar exposure frequency of children was obtained from a standardized questionnaire filled by their parents. The relationship between inorganic ingredients (Ca, P and F) concentration of plaque residues and daily sugar exposure frequency was analyzed and the relationship between inorganic ingredients (Ca, P and F) concentration of plaque residues and caries status in the S-ECC group was also analyzed.
RESULTSThe mean values for Ca, P and F concentration of plaque residues were lower in the S-ECC group, but the difference was not statistically significant (P > 0.05). The daily sugar exposure frequency in the S-ECC group was higher than that in the CF group. There were no relationships between Ca, P and F concentration of plaque residues and caries status (dmft, dmfs), sugar exposure frequency in the primary dentition.
CONCLUSIONPlaque residues from CF subjects did not contain a greater pool of calcium, inorganic phosphorus, and fluoride ions than that from S-ECC subjects.

A male patient, 67 years old, was admitted with the left auricle neoplasm over two months, increased rapidly for 10 days, on November 5, 2013. After admission biopsy, the pathological report: "spindle cell sarcoma", thin a total resection of the left auricle under local anesthesia was made, postoperative pathological report: tumor by short of spindle cells, arranged in bunchiness, a small number of seats is arrangeA striate;The nuclei are hyperchromatic and part of the visible nucleoli and empty bright cytoplasm, pathological nuclear fission was visible. 7.Prehospital delay and its influencing factors in acute ischemic stroke

8.Advances of associating liver partition and portal vein ligation for staged hepatectomy in the treatment of primarilly unresectable liver cancer

In recent years, the systematic treatment of liver cancer has made significant progress, but surgical resection remains as main treatment to obtain long-term survival for patients with liver cancer. With the rapid development of liver surgery, the complexity of liver anatomy and surgical operation are no longer the main obstacles to liver surgery. The insufficient future liver remnant (FLR) after hepatectomy has become the main limit of liver surgery. In clinical practice, most patients with liver cancer cannot undergo one-stage radical resection and can only receive non-surgical treatment, with poor long-term prognosis. The associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) provides hope for the treatment of liver cancer patients with insufficient FLR. ALPPS can induce significant hypotrophy of the FLR in a short period of time, and the radical tumor resection rate is as high as 90% to 100%, which significantly improves the prognosis of patients. However, in the early stages of the practice, there were considerable controversies on the safety of the operation. In recent years, with the continuous advancement of technology, the safety of ALPPS has been greatly improved. The authors synthesize the latest literatures and elaborate on the latest progress of ALPPS.
9.Activity of attentional and executive cortex and its relationship with cognitive impairment in subcortical ischemic vascular dementia

Objective To investigate the activity of attentional and executive cortex and its relationship with cognitive impairment in subcortical ischemic vascular dementia (SIVD).MethodsTwenty patients with SIVD and twenty normal control subjects who were matched by age,sex and education were enrolled.All of them underwent fMRI using SEMENTS 3.0T MR during Stroop task performance. The correlation between cognitive impairment and cortex activation in fMRI was analyzed.Results Cortical activation of bilateral dorsolateral prefrontal cortex,Nentrolateral prefrontal cortex,inferior parietal lobe have closed correlation with total score,visual trabecular spaces and execution,attention,verbalization,delayed memory and orientation score in MoCA test ( r =0.447-0.837,P < 0.05 ).ConclusionsCortex activation in fMRI can reflex the cognitive impairment of SIVD.
10.Comparative Study for Effects of Bare-handed Cardiopulmonary Resuscitation and Cardiopulmonary Resuscitator in Emergency Treatment

Objective: To compare the effects of bare-handed cardiopulmonary resuscitation (CPR) and cardiopulmonary resuscitator in emergency treatment. Methods: A total of 185 patients received CPR in our hospital from 2013-01 to 2015-10 were enrolled. The patients were divided into 2 groups: Bare-handed CPR (Bare-handed ) group, n=101 and Cardiopulmonary resuscitator (Mechanical) group, n=84;meanwhile, taking 30 min as border time, each group was further divided as short time CPR subgroup and long time CPR subgroup. High quality CPR was conducted in all patients. The success rates, time and blood levels of lactic acid after CPR were compared among short time subgroup, long time subgroup between Bare-handed group and Mechanical group respectively. Results: For short time CPR patients, the success rates, time and blood levels of lactic acid after CPR were similar between Bare-handed group and Mechanical group; for long term CPR patients, the success rates and time were similar between Bare-handed group and Mechanical group, while blood levels of lactic acid after CPR in Mechanical group (6.88 ± 1.82) mmol/L was lower than Bare-handed group (9.39 ± 2.39) mmol/L, P<0.05. Conclusion: The success rates of bare-handed CPR and mechanical CPR were similar for emergency treatment, both methods were effective; while mechanical CPR may save the manual labor, achieve better metabolic result which should be recommended in clinical practice.
