Pathologic pain is a common suffering and does great harm to people health. Therefore it is important to search some suitable targets for pain management. Recently it has been shown that activated inflammatory cells and glia are strongly implicated in the pathologic pain. Cyclic 3',5'-adenosine monophosphate,a universal intracellular second messenger,has been proven to inhibit the activity of inflammatory cells and glia by enhancing its activity,further to control pathologic pain. This review presents the current understanding of the underlying mechanisms for the inhibitive effect of cAMP signal pathway in inflammatory cells and glia,and the effect of phosphodiesterase inhibitors used for relieving pathologic pain.

ObjectiveTo observe the effect of Proprioceptive Neuromuscular Facilitation (PNF) technique on reaching ability of stroke patients in standing.MethodsThere were 9 stroke patients during recovery stage in the treatment group by using PNF technique for 3 months. The control group, other 9 stroke patients during recovery stage, received the general techniques for 3 months.ResultsReaching ability of stroke patients in standing in treatment group were significantly better than that of the control group(P<0.05-0.01).ConclusionThe reaching ability of stroke patients could be improved by PNF technique.

Objective To study the effect of continuous venovenous haemodiafiltration (CVVHDF) on the clearances of various solutes in multiple organ dysfunction syndrome (MODS)dogs.Methods Dogs were subjected to hemorrhagic shock plus resuscitation and endotoxemia to set up MODS model,then they were randomly divided into 2 groups: MODS group (M group)and MODS+CVVHDF group (M+C group).A PRISMA pre-dilution system with AN-69 filter was applied.Solute clearance of nitric oxide(NO),urea nitrogen(UN),creatinine (Cr),interleukin-6 (IL-6),interleukin-10 (IL-10),tumor necrosis factor-α (TNF-α) and endotoxin (LP) was determined with same dialysate and ultrafiltrate volume during CVVHDF.The clearance of various solutes (Kd) was calculated by the concentration of the cleared solutes in effluents (E) based on the formula Kd=(ExQE)/P.Results Clearances of NO,UN,Cr were greater than those of IL-6,IL-10,TNF-α,LP during the treatment of CVVHDF.Conclusion CVVHDF could effectively reduce the levels of various solutes in the development of MODS by convection and absorption.

Objective To explore the best procurement technology for cadaveric donor nephrectomy. Methods Clinical data of 1 439 donors nephrectomy in the past 24 years were retrospected.Bilateral nephrectomy,en bloc removal of both kidneys and en bloc removal with in situ cooling both kidneys were adopted in different periods of time.Operation time(OT),hot ischemia time(HIT),cold ischemia time(CIT),total ischemia time(TIT), renal graft beginning to produce urine time after revascularization(UT),renal function recovery time(RRT), damage rate(DR),quantity of preservation solution(PS), rate of acute tubular necrosis(ATN),primary non function(PNF) and poor perfusion of donor kidney(PPK) were observed and comparad among the 3 techniques. Results 216 cases were accomplished by bilateral nephrectomy and 432 kidneys were procured.1 134 cases were accomplished by en bloc removal and 2 264 kidneys were procured(4 cases had only one kidney each).Eighty nine cases were accomplished by en bloc removal with in situ cooling and 178 kidneys were procured. Objective To observe effects of Xinfeng capsule (XFC) on platelet parameters,ultrastructure,p-selectin and therapeutic effect on rheumatoid arthritis (RA) patients in active phase.Methods Platelet parameters,p-selectin and reactive indexes of 60 RA patients in active phase were detected.RA patients were divided into XFC treated group (35 examples) and Zhengqing Fengtongning (ZQF) control group (25 examples) randomly.After a course of treatment,the therapeutic effect and changes of platelet parameters,p-selectin,reactive indexes of two groups were observed.Setting up a normal control (20 examples) and detecting above-mentioned indexes.Observing platelet ultrastructure of two examples with transmission electron microscope of per groups.Results Compared with normal control group,PLT,PCT,MPV,CD62P of RA patients in active phase was obviously increased.Platelet ultrastructure was obviously damaged.The excellence rate of XFC exceeded ZQF control group.After being treated,platelet ultrastructure of the two groups were obviously improved,and XFC group was better than ZQF group.Compared with ZQF group,PLT,PCT,CD62P were obviously decreased in XFC group.PLT,PCT were positively correlated with CD62P,IgG,ESR,CRP,RF,arthrpain,arthrocele.And PLT was positively correlated with arthrtenderness,morning stiffness time.Conclusion PLT and PCT of RA patients in active phase were increased,and have correlation with reactive indexes.It suggests that PLT,PCT can reflect reactiveness of RA as objective indexes.XFC can not only treat inflammation and delay the aggravation of RA patient by improving platelet ultrastructure.

Objective To optimize the preparation of high-efficiency galactocylated poly-L-lysine (Gal-PLL) ligand of the asialoglycoprotein receptor in liver, providing premise and foundation for upper preparation of ultrasound contrast agent of liver targeted nanoscale perfluorocarbon microballoon and the liver targeted molecular imaging. Methods Chemical reactions of reductive amination were carried out on group A and group B according to different proportions of reaction component. Each group was subdivided into three subgroups. In group A, three different molar ratios of D-galactose and poly-L-lysine (PLL) were compounded respectively with equivalent and sufficient reductant borohydride. In group B, identical molar ratios of D-galactose and PLL were compounded respectively with three unequal reductants borohydride. Products of each group were separated and purified by sephadex column to acquire different molecular weight distributions and the results were analyzed. Results In the condition of identical reductant, the peak curve of compound's molecular weight appeared earlier when D-galactose decreased properly. In the condition of identical molar ratio of D-galactose and PLL,the peak curve of compound's molecular weight appeared also earlier when reductant decreased properly. When the molar ratio of D-galactose and reductant was 1∶1, the peak curve of compound Gal-PLL and free components was more obvious, and the quantity of compound Gal-PLL increased to maximum. Conclusions In the condition of identical reductant, coupling effect of D-galactose and PLL increased when D-galactose decreased properly. In the condition of identical molar ratio of D-galactose and PLL, coupling effect was better when reductant decreased properly. When the molar ratio of D-galactose and reductant was 1∶1, coupling effect of them was the best. The coupling of D-galactose and PLL was related to not only the proportion of D-galactose and PLL, but also the proportion of D-galactose and reductant.

Objective T o investigate the serum IgE w ith various postm ortem intervals (PMI) in guinea pigs due to sudden death from anaphylactic shock and to explore the effect of refrigeration of corpse on serum IgE level and its application value in forensic m edicine. Methods T he anim al death m odels of anaphylactic shock w ere established. T he corpses w ere preserved at room tem perature (20℃) for 6 h and then refrigerated at 4℃. T he serum w as sam pled at 6, 12, 24, 36 and 48 hours after death. T he IgE level of serum w as detected w ith ELISA . The control group w as also established. Results The serum IgE level had significant difference betw een the experim ental group and the control group (P<0.05). T here w as no significant difference am ong the experim ental groups at 6, 12, 24, 36 and 48 hours post-m ortem (P>0.05). Conclusion If the corpses w ere placed in 4℃ conditions 6 hours after anaphylactic death, the serum IgE still show s a good m arker w ithin 48 h for forensic investigation.

Diabetic peripheral neuropathy (DPN) is the most common complication of diabetes mellitus in nervous system. However, the pathogenesis is still unkown. Therapeutic options are mainly pathogenetically oriented therapy and symptomatic therapy. With an increasing understanding of the pathogenesis of DPN, many new therapies have emerged. Comprehensive therapy targeting different pathogenesis will be frequently applied in the future. In this article, the common therapy and advance in treatment of DPN are reviewed.

Objective To compare the graft function of 2 methods of kidney harvesting,the modified method of combined liver and kidney procurement and rapid en bloc kidney procurement. Methods The clinical data of 220 cadaveric renal transplantation recipients were collected(12 months follow-up),174 cases from en bloc kidney procurement and 46 from combined liver and kidney procurement.The average harvesting time,the incidence of renal vein injury,1 year kidney survival and incidence of acute rejection were compared between the two methods.Results In average harvesting time and incidence of renal vein injury,the en bloc kidney procurement were better than the method of combined liver and kidney procurement.However,the method of combined liver and kidney procurement was better than the en bloc kidney procurement in 1 year kidney survival,1 year incidence of acute rejection and average warm ischemia time.There was no difference in 1 year survival of patients and 24 h,1 week and 1 year graft function after transplantation. Conclusion Although the method of combined liver and kidney procurement is better than the en bloc kidney procurement in 1 year kidney survival and 1 year incidence of acute rejection,there is no difference between the 2 methods in 1 year survival of patients and graft function after transplantation.
