1.The effects of hypoxia inducible factor-2? on the proliferation and invasion of the hepatocellular carcinoma cells

Objective To investigate the effects of hypoxia inducible factor-2?(HIF-2?) on the proliferation and invasion of hepatocellular carcinoma cell line SMMC-7721 under the anoxia condition.Methods Lipofectamine-mediated HIF-2? oligodeoxynucleotide(ODN) was transfected into SMMC-7721 cells and the expressions of HIF-2? mRNA and protein were detected to evaluate the transfection.The proliferative and adhesive activity was determined by methyl thiazolyl tetrazolium(MTT) method and the invasive activity was determined by a transwell cell culture chamber method.Results The expressions of HIF-2? mRNA and protein were inhibited significantly by HIF-2? antisense oligodeoxyribonucleotide(ASODN)(P0.05).Conclusion The blockage of HIF-2? expression inhibits the adhesive and invasive activity of SMMC-7721cells,and has no evident impact on their proliferation.
2.Using cardiac MRI to assess the myocardium necrosis of patients with acute coronary syndrome

Objective To evaluate the irreversible ischemia injury in patients with acute coronary syndrome using Cardiac MRI. Methods We have studied 35 patients with acute coronary syndromes who received standard medical therapy. Coronary angiogram confirmed that all the patients had suffered from multi-coronary artery stenosis without total occlusion. Cardiac MRI (CMRI) examinations were performed 6-12 weeks after the coronary angiography and revascularization. IR-GRE was used in the CMRI protocal to detect myocardial delayed enhancement (viability). The definition of hyperenhancement was that the signal of the region was two times that of the remote myocardium. The hyper-enhanced volume was presented as a percentage of LV mass (%LV), i.e. The total number of hyperenhanced pixels divided by the total number of pixels of the LV myocardium and multiplied by 100. Results CMRI revealed that all the patients had hyperenhanced region, of whom 30 (85.7%) had non-transmural enhancement and only 5 (14.3%) had transmural enhancement. The average size of the enhancement of non-transmural was significantly smaller than that of transmural hyperenhancement (45.7?7.3%). Conclusion The CMRI can demonstrate that infarction has occurred in patients with multi-coronary arteries stenosis.

5.Link of type 2 diabetes with insulin resistance and cytokines

Objective To study on the changes of the IL-6、IL-8、TNF-? in the type 2 diabetes with insulin resistance and the relation of plasma glucose or insulin resistance and cytokines. Methods (1) To assay the serum level of cytokines in 90 cases with type 2 diabetes mellitus (T2DM) and 30 normal controls. (2)To measure the level of fasting plasma glucose (FPG),hemoglohin A_1c (HBA_1c),fasting insulin(FINS),fasting C-peptide (FCP).(3) To analysis the relation of the level of FPG,HBA_1c,FINS,FCP and cytokines. Results (1) The serum levels of cytokines in T2DM group were significantly increase than in control groups,IL-6 and TNF-? in T2DM group with insulin resistance were significantly increase than in T2DM group without insulin resistance (2) There was significantly positively correlated between insulin resistance and the increasing of cytokine levels. Conclusion There is an excess activation of cytokines in T2DM,which is significantly correlated with insulin resistance. Cytokines play the important role in the occurrence and development of T2DM and insulin resistance.
6.Clinical efficacy of Sini decoction adjustment combined rabeprazole enteric capsules in gastroesophageal reflux cough and its effect on IL-8, SP, MCT levels

Objective To research the clinical efficacy of Sini decoction adjustment combined rabeprazole enteric capsules in treatment of gastroesophageal reflux cough and its effect on interleukin 8 (IL-8) and substance P (SP), the mast cell tryptase (MCT) levels.Methods 92 cases of gastroesophageal reflux cough were divided into control group and experimental group by drawing method , each had 46 cases.Control group were treated by rabeprazole enteric capsules, experimental group were treated with rabeprazole enteric capsules based on the control group.The curative effect, cough symptoms integral, IL-8, SP, MCT, gastric dynamic element (MOT) and the gastrin-releasing (GAS), pulmonary function and safety were compared between two groups.Results The total effective rate of experimental group(93.47%) was higher than the control group(76.08%), the difference was statistically significant (P<0.05).After treatment, the symptoms of experimental group(1.53 ±0.19) points was higher than the control groupthe difference was statistically significant(1.96 ±0.24)points,P<0.05.The IL-8, SP, MCT levels of experimental group were all lower than the control group ( P<0.05 ) .The MOT, GAS, and lung function of experimental group improved more significantly than the control group ( P <0.05 ) .The security between two groups has no significant difference .Conclusion The curative effect of Sini decoction adjustment combined Rabeprazole enteric capsules in treatment gastroesophageal reflux cough is obvious, can decrease IL-8, SP, MCT levels.
7.Tocetaxel combined with cisplatin in the treatment of advanced non-small cell lung cancer

Objective To evaluate the efficacy and side effects of the combination of tocetaxel and cisplatin in the treatment of advanced non-small cell lung cancer (NSCLC). Methods Thirty patients were treated with the combination for two courses. Every three weeks, tocetaxel was administrated on day 1 in a dose of 75mg/m~2 and cisplatin in dose of 30mg/m~2 from day 1 to day 3. Results Complete remission was found in two patients, partial remission in fourteen patients, in seven patients there was no change, and in seven patients there was an advance of the disease. The overall remission rate was 53.3%. The main side effects were suppression of myelopoiesis, gastrointestinal disturbance, alopecia, myalgia and arthralgia. Conclusion The combination of tocetaxel and cisplatin is effective and well-tolerated in the treatment of NSCLC.
8.Clinicopathological analysis of primary small intestinal tumors in 121 cases

Objective By analyzing the clinical and pathological characteristics of small intestinal neoplasms of patients presenting at our hospital,this study was to improve our cognition of this disease and the prognosis.Methods We collected and reviewed the medical records of 121 patients suffering from small intestinal neoplasms,who underwent surgery at Ruijin hospital from January 2003 to June 2009.Diagnosis was confirmed by pathological examination,and patients were followed-up.Results Intestinal hemorrhage,anemia and abdominal pain were the three main symptoms for all patients.CT,and gastrointestinal endoscopy were valuable for the diagnosis of small intestine neoplasms.Compared with open surgery,laparoscopic procedures can shorten the operation time and the postoperative length of hospital stay.Conclusions Surgical procedure is the key treatment for patients with small intestinal neoplasms.Long term follow-up plays important role in the detection of other synchronous or metachronous gastrointestinal tumors and improves the prognosis.
9.Mechanism of emodin enhancing chemosensitivity of pancreatic cancer cells

Objective To investigate whether emodin enhances chemosensitivity of pancreatic cancer cells and the mechanism. Methods Normal human skin fibroblasts and pancreatic BXPC-3 cells were divided into control group, cisplatin (5 mg/L) treatment group, gemcitabine (0.5 μmol/L) treatment group, emodin (50 μmol/L)+ cisplatin (5 mg/L) co-treatment group and emodin (50 μmol/L) + gemcitabine (0.5 μmol/L) co-treatmentgroup. The cell viability was detected by MTT assay, the cell apoptosis rate by flow cytometry, the expression of the multidrug resistance-associated protein 1 ( MRP1 ), MRP2, breast cancer resistance protein (BCRP) and P-glycoprotein (P-gp) mRNA in different cell lines was determined by reverse transcription polymerase chain reaction.All data were analyzed using the t test. Results The cell viability and apoptosis rate of pancreatic BXPC-3 cells after treatment were 62.44% ± 3.42% and 27. 10% ± 4.24% in the cisplatin treatment group, and they were 30.53% ±0.05% and 66.33% ±9.37% in the emodin + cisplatin co-treatment group. There were significant differences in cell viability and apoptosis rate between the two groups (t = 13.20, 5. 35, P < 0.05 ). The cell viability and apoptosis rate of pancreatic BXPC-3 cells after treatment were 79.82% ±2.83% and 13.48% ± 1.65%in the gemcitabine treatment group, and they were 45.65% ± 2.46% and 62.74% ± 10. 18% in the emodin +gemcitabine co-treatment group. There were significant differences in cell viability and apoptosis rate between the two groups (t = 12.89, 8.28, P < 0. 05 ). There was no significant difference in the viability of normal human skin fibroblasts between the cisplatin treatment group and the emodin + cisplatin co-treatment group, and also between the gemcitabine treatment group and the emodin + gemcitabine co-treatment group (t = 2. 08, 0. 64, P >0.05 ). Expression of MRP1 mRNA was detected in pancreatic BXPC-3 cells, whereas the expression of MRP2,BCRP and P-gp mRNA was undetectable. The expression of MRP1 mRNA in pancreatic BXPC-3 cells was significantly down-regulated in the cisplatin treatment group, and cisplatin + emodin co-treatment had an additive effect on down-regulating the expression of MRP1 mRNA. Conclusion Emodin may enhance chemosensitivity of pancreatic cancer cells to cisplatin by down-regulating the expression of MRP1 mRNA.
10.MELD score in the prediction of perioperative risks in patients who underwent partial hepatectomy for hepatocellular carcinoma

Objective To determine the perioperative risks of partial hepatectomy by determining the preoperative liver functional reserve in patients with hepatocellular carcinoma (HCC),and to compare the model for end-stage liver disease (MELD) score with the Child-Pugh classification in predicting prognosis.Methods We reviewed the clinical data of 202 patients with HCC who underwent partial hepatectomy.The MELD score and the Child-Pugh classification were determined preoperatively.Results The incidence of postoperative liver dysfunction happened in 44.0% of Child A patients,50% in Child B patients,41.6%in patients with a MELD score below 14,and 91.7% in patients with a MELD score of > 14.The difference between the rates of postoperative liver dysfunction in patients with a preoperative MELD score above 14 and below 14 was significant (P < 0.05),while that between patients with Child-Pugh A and B was insignificant (P > 0.05).The incidences of postoperative liver dysfunction in patient with a MELD < 8,8 ≤ MELD ≤ 14,MELD > 14 were 38.2%,57.6% and 91.7%,respectively,indicating that there was a positive co-relationship between the MELD score and the incidences of liver dysfunction.The Spearman rank correlation test showed the MELD score was significant correlated with the Child-Pugh score (r =0.404 ; P < 0.05).The areas under the ROC curves of the MELD score and the Child-Pugh score were 0.703 and 0.587 (P < 0.05).Conclusions The MELD score predicted postoperative liver dysfunction more accurately than the Child-Pugh classification.HCC patients undergoing partial hepatectomy with a preoperative MELD score > 14 had a high perioperative risk.To ensure the safety of partial hepatectomy,HCC patients with a preoperative MELD score > 14 requires active preoperative preparation,bringing the score near to or less than 14.
