2.Effectiveness of integrative medicine for preventing postoperative recurrence of primary hepatic carcinomas: a systematic review.

OBJECTIVETo systematically evaluate the effectiveness of integrative medicine (IM) for preventing postoperative recurrence of primary hepatic carcinomas (PHC).
METHODSSuch databases as The Cochrane Library, PubMed, EMbase, China National Knowledge Infrastructure (CNKI), VIP Database, Wan-fang Database were retrieved to collect randomized controlled trials (RCTs) on preventing postoperative recurrence of PHC by IM. The retrieval period started from database establishment to June 2013. Literature screening, data extraction, and quality evaluation were performed by two reviewers strictly according to inclusion criteria. Meta-analysis was conducted using RevMan 5.2 Software.
RESULTSA total of 5 RCTs involving 367 patient cases were included. Results of Meta-analysis showed that the IM group was superior to the Western medicine (WM) group in the 1-, 2-, and 3-year recurrence rates (P < 0.05).
CONCLUSIONIn the prevention of postoperative recurrence of PHC, the effectiveness of IM was more significant, when compared with treatment of Western medicine.

4.Effect of hepatocyte nuclear factor 1α on expression profile of hepatocellular carcinoma cells HuH7

Objective To investigate the regulation effect of hepatocyte nuclear factor (HNF) 1α on the gene expression profile and the signal pathways in HuH7 cells.Methods The expression of HNF1α was increased or decreased in HuH7 cells by Lenti-virus carrying HNF1α or shHNF1α.The expression profile of the cells after treated was examined by microarray technology.The difference expressed gene regulated by HNF1α were screened and the pathway was analyzed with DAVID software and related analysis system.The regulation effect of HNF1α on transforming growth factor (TGF)β signal pathway was detected by reporter gene test and the regulation role of HNF1α on related genes of TGFβ signal pathway was determined by real-time polymerase chain reaction (PCR) and Western blotting assay.Results The expression of HNF1α in HuH7 cells was significantly up-regulated by Lenti-virus carrying HNF1α gene (Lenti-HNF1α) and which was down regulated by Lenti-virus with shHNF1α gene (LentishHNF1 α).Expression profile analysis revealed that 339 genes were positively up regulated two times by HNF1α and 325 genes were negatively down regulated two times.Signal pathway analysis revealed that HNF1α regulated drug metabolism,biosynthesis of unsaturated fatty acids and glycolysis/gluconeogenesis metabolism signal pathways.Moreover,it also involved in the regulation of TGFβ、nuclear factor (NF)-κB and p53 tumor-related signal pathways.Furthermore,Luciferase reportor gene experiment indicated that up-regulated HNF1α could inhibit the activation of TGFβ signal pathway.And the results of real-time PCR and Western blotting verified that up-regulated HNF1α could inhibit TGFβ signal pathway related gene c-myc and TGFβ1 and then inhibited the activation of TGFβ signal pathway.Conclusion HNF1α broadly affects the gene expression profile and the tumor genesis and development related signal pathways in HuH7 cells,furthermore,HNF1α can inhibit the activation of TGFβ signal pathway.
5.Evaluation of the effectiveness of cough test during tension-free vaginal tape procedure in preventing post-operative voiding dysfunction

Objective To evaluate the value of cough test in the tension-free vaginal tape (TVT)procedure.Methods A cohort of 85 women with stress urinary incontinence underwent the TVT procedure with cough test (n =41) or without cough test (n =44).Patients in cough test group were performed according to the Ulmsten’s method strictly,with the stress of tape adjusted in light of cough test; whereas in other 44 operations,the tape was placed on the urethral tract without stress,and no cough test was performed.The urine catheter was removed after 48 hours postoperatively and follow-up evaluation was carried out at 12 month postoperatively.Results TVT procedure was carried out successfully in all patients by a single experienced surgeon.Four cases of urinary retention and 5 cases of voiding difficulty were observed in the cough test group.However,urinary retention or voiding difficulty was not detected in the nun-cough test group.Based on the twelve-month follow-up results,the cure rate was 92.6% (38/41) in the cough test group and 93.1% (41/44) in the non-cough test group.Flow-pressure study indicated that 11 cases in cough test group were in the obstruction zone,while only 3 cases in the obstruction zone were detected in the non-cough test group.Conclusions TVT is a safe as well as effective minimally invasive surgical procedure to treat female stress urinary incontinence.However,Adjusting stress of tape in accordance with cough test during the TVT may potentially increase the incidence of urinary dysfunction postoperatively.Therefore,no convincing evidence was gained to support the efficacy of cough test during TVT in terms of preventing postoperative voiding dysfunction.

7.The application of Kano model analysis in midwife outpatient service quality management

Objective To explore the Kano model analysis in the application of midwife outpatient service quality management. Methods Kano questionnaire was consisted of 24 items, including five parts, environment facilities, service evaluation, service technology, the team service, delivery service′quality. Using the Kano model analysis, the impact factors of the midwife outpatient service quality in five different quality attributes were determined. It included property and had the opposite answer to the question, charisma, essential properties and one property. Results By identifing the quality index classification of the pregnant women with satisfactory service quality to midwife outpatient service, attributable to a property were 1,2,3,4,8,9,10,14,20,21;attributable to mandatory attributes were 13, belonging to the charm of the property had 5,6,7,11,12,15,16,17,18,19,22,23,24. Midwives outpatient had most of charisma attribute, 54.2%(13/24) of the total;The second was one attribute, 41.7% (10/24) of the total. The technology level of midwife was essential attribute. Conclusions Kano mode analysis technology will be introduced to the midwife outpatient service quality management to provide decision reference for improving the quality of the hospital management level, improve maternal satisfaction, improve the obstetric operation efficiency.
8.Effect of Rehabilitation and Local Injection of Botulinum Aoxic A on Spastic Cerebral Palsy

ObjectiveTo evaluate the efficacy of rehabilitation associated with local intramuscular injection of botulinum toxic A (BTX-A) on spastic cerebral palsy (CP).Methods60 children with spastic CP were divided into experimental group and control group with 30 cases in each group. Cases of experimental group were treated with rehabilitation associated with local intramuscular injection of BTX-A. Cases of control group were treated only with rehabilitation treatment. The therapeutic efficacies of two groups were evaluated with physician rating scale (PRS) and activities of daily living (ADL) evaluation systems.ResultsImprovement of clinical evaluations index-PRS and ADL in experimental group was much more significant than that of control group (P<0.05). ConclusionRehabilitation associated with local intramuscular injection of BTX-A can improve the efficacy of spastic CP.

A new hasubanan alkaloid, hernsubanine E (1), as well as two known compounds p-hydroxybenzaldehyde (2) and (-)-syringaresinol (3) have been isolated from the whole plants of Stephania hernandifolia by various column chromatographic methods. Their structures were identified by physicochemical properties and spectral analyses. Compounds 2 and 3 were isolated from the genus of Stephania for the first time.

10.Underlying mechanism for prolongation of action potential duration in ventricular cardiomyocytes of rats suffered from thermal injury.

Severe thermal injury causes prolongation of action potential duration (APD) in cardiomyocytes, which results in cardiac dysfunction by inducing disturbance of calcium dyshomeostasis in cardiac myocytes. However, the underlying mechanism for APD prolongation remains unclear. In the present study, we examined the major action potential repolarization-related ion channel currents in rat ventricular cardiomyocytes, including transient outward potassium current (I(to)), inward rectifier potassium current (I(K1)) and L-type Ca(2+) current (I(Ca-L)) to investigate the alterations of these currents, which might account for the pathogenesis of APD prolongation induced by thermal injury. Twelve hours after approximately 40% of the total body surface area, full-thickness (third-degree) cutaneous thermal injury was produced in rats, ventricular cardiomyocytes were isolated from the hearts with systolic and diastolic dysfunction. APD was found to be markedly prolonged, while APD(50) and APD(90) in ventricular cardiomyocytes from rats with thermal injury were (46.02+/-3.78) ms and (123.24+/-12.48) ms (n=19), respectively, significantly longer than (23.28+/-4.85) ms and (72.12+/-3.57) ms (n=17, P<0.01) in ventricular cardiomyocytes from sham rats. Thermal injury remarkably suppressed Ito density in ventricular cardiomyocytes. Ito density at +60 mV was decreased from (34.15+/-3.78) pA/pF (n=20) in sham group to (20.39+/-1.98) pA/pF (n=25, P<0.01) in thermal injury group, and the decrease extended from -30 to +60 mV. Similarly, current densities of I(K1) from -120 to -80 mV in thermal injury group were also significantly lower than that in sham group. In contrast, we failed to detect any alterations in I(Ca-L) density, and voltage-dependence of activation and inactivation in thermal injury group, compared with that in sham group. Taken together, our data suggest that thermal injury results in function downregulation of transient outward potassium channels and inward rectifier potassium channels, which contributes, at least in part, to APD prolongation and subsequent cardiac dysfunction.
