1.Effect of electroacupuncture at Lower He-Sea points including Yanglingquan (GB 34) on nuclear factor-κB and interleukin-1β in guinea pigs with acute cholecystitis

Objective: To observe the effect of electroacupuncture (EA) at Lower He-Sea points on the expression levels of interleukin-1β (IL-1β) in the serum and gallbladder tissues, and nuclear factor-κB (NF-κB) in gallbladder tissues of the guinea pigs with acute cholecystitis (AC), and to explore whether Yanglingquan (GB 34), the Lower He-Sea point pertaining to Dan Fu (gallbladder), is relatively specific for the Dan Fu (gallbladder) disorders. Methods: Eighty-two healthy guinea pigs were randomly divided into 6 groups according to the random number table method, a blank group, a model group, a Yanglingquan (GB 34) group, a Zusanli (ST 36) group, a Shangjuxu (ST 37) group, and a Xiajuxu (ST 39) group, with 12 guinea pigs in the blank group while 14 in the other groups, respectively, half males and half females in each group. Except for the blank group, guinea pigs in the other groups were injected with E. coli into the gallbladder to establish AC models. Guinea pigs in the blank group were fed routinely without special treatment; those in the model group were daily tied up for 30 min without EA treatment; those in the 4 groups receiving EA treatment were acupunctured at the corresponding Lower He-Sea points after daily binding and stimulated with the SDZ-V EA instrument. After successful modeling and treatment for 5 d, blood was collected from the abdominal aorta of the guinea pigs, and the gallbladder tissues in each group were isolated for hematoxylin-eosin (HE) staining to observe the morphological changes. Enzyme-linked immunosorbent assay (ELISA) was used to detect the serum IL-1β level, and immunohistochemistry (IHC) was used to detect the expression levels of NF-κB and IL-1β in gallbladder. Results: On the 3rd day after modeling, the guinea pigs in the five groups with modeling were mentally depressed with decreased appetite, significantly reduced activities, slouch, lassitude, slack and matted fur, and loose stools; two guinea pigs were selected from each group (one male and one female, not included in the final statistics) to isolate the gallbladder after sacrifice; macroscopic observation showed that the gallbladder wall was differently thickened; the bile color was dark green and opaque with particles suspended or accumulated; light microscope observation showed that the submucosal blood vessels of the gallbladder were congested, along with mucosal edema, ulceration, necrosis, shedding, and a large number of inflammatory cells infiltrating in the lamina propria, indicating that the AC model was successfully prepared. Compared with the model group, the gallbladder tissue injuries of the four groups receiving EA treatment were all differently repaired, the serum IL-1β levels were reduced (P<0.01 or P<0.05), and the IL-1β levels in the gallbladder tissues were reduced (P<0.01 or P<0.05). Compared with the model group, the NF-κB expression level in the Yanglingquan (GB 34) group was significantly reduced (P<0.01), but was not statistical different in the Zusanli (ST 36) group, Shangjuxu (ST 37) group and Xiajuxu (ST 39) group (all P>0.05). Compared with the Yanglingquan (GB 34) group, the gallbladder tissues of the Zusanli (ST 36) group, Shangjuxu (ST 37) group and Xiajuxu (ST 39) group were more severely damaged, and the expression levels of serum IL-1β, the NF-κB and IL-1β in the gallbladder tissues were increased (P<0.01 or P<0.05). Intervention effect of Yanglingquan (GB 34) on AC guinea pigs was superior to that of Zusanli (ST 36), Shangjuxu (ST 37) and Xiajuxu (ST 39). Conclusion: EA at the Lower He-Sea points of the stomach, large intestine, small intestine and gallbladder can produce curative effects on AC guinea pigs and reduced the inflammatory symptoms. Intervention effect of Yanglingquan (GB 34) on AC guinea pigs is superior to that of Zusanli (ST 36), Shangjuxu (ST 37) and Xiajuxu (ST 39). The mechanism of EA at Yanglingquan (GB 34) in treating AC may be regulating IL-1β and NF-κB to control the inflammatory response and improve the gallbladder tissue damage.

3.Inheritance on and innovation of traditional Chinese medicine (TCM) flavor theory and TCM flavor standardization principle flavor theory in Compendium of Materia Medica.

All previous literatures about Chinese herbal medicines show distinctive traditional Chinese medicine (TCM) flavors. Compendium of Materia Medica is an influential book in TCM history. The TCM flavor theory and flavor standardization principle in this book has important significance for modern TCM flavor standardization. Compendium of Materia Medica pays attention to the flavor theory, explain the relations between the flavor of medicine and its therapeutic effects by means of Neo-Confucianism of the Song and Ming Dynasties. However,the book has not reflected and further developed the systemic theory, which originated in the Jin and Yuan dynasty. In Compendium of Materia Medica , flavor are standardized just by tasting medicines, instead of deducing flavors. Therefore, medicine tasting should be adopted as the major method to standardize the flavor of medicine.

4.An investigation of risk factors of wound complications after operative fixation for calcaneal fractures

Objective To explore the risk factors contributing to wound complications following operative fixation for fractured calcanea.Methods Fitly-six patients with 61 fractured calcanea who had been treated with operative fixation from March 2000 to March 2003 were enrolled in investigation.General information of the pa- tients,ages,injury causes,classification of fracture,fall heights,smoking,delayed operation time,operation time and tourniquet time were documented preoperatively and postoperatively to be analyzed statistically.Results We report an overall wound complication rate of 14.8%(9/61).Wound infection,haematoma,dehiscence,heel necrosis and osteomyelitis were noted in our series.We identified the following as risk factors of postoperative wound compli- cations:1)fall from more than 3.4 m high(P<0.05);2)delayed operation time less than seven days(P<0.05); 3)operating time beyond two hours(P<0.05);4)tourniquet time beyond 1.5 hours(P<0.05).Conclusion In order to decrease wound complications following operative fixation tor fractured calcanea,surgeons should pertorm the operation 7 to 10 days after the injury,and reduce the operation time and tourniquet time.
5.THE EFFECT OF OXYGEN FREE RADICAL ON DNA ,MATRIX CMPONENT SYNTHESIS AND ULTRASTRUCTUREOF HUMAN EMBRYO CHONDROCYTES

By using the method of chondrocytes in monolayer culture in vitro we observed the effect of free radicals generated by the xanthine oxidase and xanthine on DNA, matrix proteoglycan (PG), collagen synthesis and ultrastructure of human embryo chondrocytes. Our results showed that the synthesis of DNA, PG and collagen of the chondrocytes were obviously inhibited and the membranes and subcellular organelles of the chondrocytes were damaged strongly. On the bases of these results we suggest that oxygen free radicals from poymorphonuclear leukocytes and phagocytes would be the important factors of cartilage injury in arthritis.
6.The comparative study of the treatment to intertrochanteric fractures and subtrochanteric fractures of the aged patients

Objective To compare and to assess the treatment of interchanteric fractures and subtrochanteric fractures by proximal femoral nail(PFN) and dynamic hip screw(DHS). Methods The patients with intertrochanteric or subtrochanteric fractures were divided into PFN and DHS group randomly. The common conditions, ASA score, operative procedures and postoperative conditions were evaluated. Results During the course of operation, the mean operation time was 62.8 min in DHS group, 45.4 min in PFN group. The mean bleeding was 230 ml in DHS group, 164 ml in PFN group. The mean length of incision was 13.7 cm in DHS group, 8.7 cm in PFN group. The mean drainage was 120 ml in DHS group, 106 ml in PFN group, there was no significant difference between two groups. 84 (94.4%) patients of DHS group were followed-up 12-25 months with a mean of 18.6 months. 74(94.9%)patients of PFN group were followed-up 12-21 months with a mean of 18.2 months. In DHS group, the average fracture healing time was 4.6 months, 2 patients with deep hematoma, the reduction was unsatisfactory in 6 patients, while in PFN group, the average fracture healing time was 4.3 months, deep hematoma occurred in 1 patient, the reduction was unsatisfactory in 5 patients, but there was no significant difference between two groups. The leg length measured at 12 months post-operatively revealed that the diseased site in DHS group was averagely 1.4 cm shorter than the healthy site; that the diseased site in PFN group was averagely 0.8 cm shorter than the healthy site. Conclusion There is no significant difference between two operative methods concerning to bony healing time, complications and recovery of the function. PFN is less invasive, easier to perform and with shorter operation time.
7.Complications of locked nailing in humeral shaft fractures

Objective There are many methods to treat adult humeral shaft fracture. Plates fixation and intramedullary locking nail are two important methods. Though plate fixation are "gold criterion", there were many defects limited its development. With humeral intramedullary nail widely used, this study is to analysis and investigates the complications of locked nailing in humeral shaft fractures. Methods Between January 2000 and January 2004, 90 humeral shaft fractures (74 acute fractures and 16 delayed unions or nonunions) in 89 patients were treated with humeral intramedullary locking nails and followed up for an average of 25.4 months(12-34 months). There were 56 males and 33 females, with an average age of 39.3 years. Acute fractures included 62 closed, 7 Gustilo type Ⅰ, 3 type Ⅱ and 2 type Ⅲa open fractures. 12 nonunions had previous operations. 8 patients had radial nerve palsy. In general, acute fractures were treated with closed nailing and nonunions were treated with open nailing with bone grafting. 53 proximal humeral fracture were treated by antegrade nail, the others with distal humeral fracture were treated by retrograde nail. Results In total, 17 patients had 18 significant complications. 5 of them were persistent nonunions. There was no statistics difference between antegrade nail and retrograde nail. The others were protruded screws, fracture gap, shoulder impairment, elbow impairment, angular malunion, deep infection and postnailing radial nerve palsy. Conclusion The risk of operative comminution was significantly higher in retrograde nailing, and operative comminution resulted in a significantly higher risk of nonunion. Many complications of humeral locked nailing can be prevented by improving the implant design or surgical techniques.
8.Application of the nerve network model in the analysis of factors affecting hospitalization expenses

Objective To analyze factors affecting the expenses of inpatients and their comprehensive force of influence so as to check irrational increases of medical expenses. Methods Data on hospitalization expenses for disease entities ranking in the first ten places in a third tier hospital in 1997 were taken as samples and a statistical analysis was conducted of the medical expenses incurred in the discharged cases by means of the nerve network model. Results Major factors affecting hospitalization expenses were days of hospitalization, pre hospitalization conditions, pre operation length of stay and age, the comprehensive force of influence of which was respectively 0 4879 , 0 2124 , 0 1847 and 0 0612 . Conclusion To check the irrational increases of medical expenses, we must focus our efforts on the reduction of expenses for medicine and shorten days of hospitalization and pre operation length of stay.
9.Expression and significance of NgR mRNA in adult rats′ optic nerve

Objective To observe and evaluate the expression and significance of Nogo-66 receptor (NgR) mRNA in adult rats′ optic nerve. Methods Optic and sciatic nerves of 8 adult rats were used to make the sections, which were divided into 3 groups: optic-nerve experimental group, sciatic-nerve control group, and optic-nerve negative control group. In situ hybridization was used to observe the expression of NgR mRNA in optic nerve and sciatic nerve. Results The expression of NgR mRNA in the 8 rats was positive in optic nerve and negative in sciatic nerve. The positive signals were arranged along the long axis of optic nerve. Conclusion The expression of NgR mRNA is positive in optic nerve while negative in sciatic nerve in adult rats, which suggests that the positive expression and distribution of NgR may be related to the poor regenerate ability of optic nerves.
